1.Impact of nursing intervention on the secondary infection in chemotherapy of elderly patients with lung cancer

Objective To investigate the effect of intensive nursing intervention on the secondary infection in chemotherapy of elderly patients with lung cancer.Methods 168 cases of elderly lung cancer patients whose neutrophil ≤ 1.0×109/L during chemotherapy were randomly divided into two groups,namely,the intensive care group and the control group,with 84 cases in each group.The control group was given routine nursing of infection prevention,while the intensive care group was given intensified nursing for infection prevention.A contrastive analysis was conducted in terms of the incidence,clinical characteristics and the duration of antibiotic treatment of the two groups of patients with secondary infection.Results The secondary infection rate of the intensive care group was lower than that of the control group during chemotherapy.What's more,the duration of fever and treatment of the intensive care group was shorter than that of the control group,hence the patients were safe during infection period.The difference was statistically significant.Conclusions Strengthening the nursing intervention can reduce the incidence rate of secondary infection in aged patients with lung cancer,increase the tolerance of patients to chemotherapy and ensure the successful completion of the chemotherapy cycle,prolong the life span of the patients with an improved quality of life.
2.Roles of depression and C-reaction protein in early development of cardiovascular disease in the elderly diabetes patients

Objective To investigate the role of depression and C-reaction protein (CRP)in the early development of vascular disease in the elderly patients with type 2 diabetes. Methods Totally 396 patients with type 2 diabetes were recruited in the study,including 196 cases with depression and 200 cases without depression.Depression was evaluated by Hamilton depression scale (HAMD),and CRP was measured using rate nephelometry.The related parameters of wave intensity (WI) such as accelerating wave intensity (W1),decelerating wave intensity (W2),arterial stiffness β,pressure-strain elasticity modulus (Ep),arterial compliance (AC),pulse wave velocity -wave intensity(PWVWI),pulse wave velocity (PWV) and argumentation index (AI) were detected and compared between the groups. Results The levels of CRP [(8.1±2.8)mg/L vs. (3.9±3.2) mg/L,t=6.411,P=0.000],W2 [(4.82±1.33) mm Hg· m-1 · s-3 vs.(2.27±1.28)mm Hg · m-1 · s-3,t=4.256,P=0.021],arterial stiffness β(12.58±3.23 vs.8.96±2.24,t=4.634,P=0.019),Ep (157.90±52.64 vs.118.24±53.08,t=5.512,P=0.007),PWVβ(7.53± 1.41 vs.6.68±1.56,t=3.982,P=0.024),AI(19.26± 13.21 vs.15.34±10.23,t=3.213,P=0.037)and PWV-WI (7.59±1.67vs.6.63± 1.45,t =2.613,P =0.046) in cases with depression were higher than the cases without depression,while the AC value was reduced (0.70±0.19 vs.0.78±0.21,t=4.676,P =0.017).There was no difference in W1 value between the groups.The CRP level and HAMD score were positively associated with W2 (r=0.470,P=0.000),stiffness β (r=0.582,P=0.000),Ep (r=0.352,P=0.004),PWV-WI (r=0.233,P=0.019),PWVβ (r=0.340,P=0.007) and AI (r=0.265,P=0.015),but negatively related with AC(r=-0.457,P=0.000).HAMD score was positively correlated with CRP level(r=0.328,P=0.003). Conclusions The carotid artery wave intensity W2,blood vessel elasticity parameter β,Ep,AC,PWV-WI,PWVβ and AI determination can evaluate the risk of cardiovascular diseases in elderly diabetes,and higher CRP level and HAMD score may deteriorate its process.
3.Studies on the criteria of two-way referral between hospital and community health-care service centerfor patients with cerebral hemorrhage: an analysis of 91 cases

This study aimed to study the criteria of two-way referral between tertiary-care hospital and communityhealth-care service center for patients with cerebralhemorrhage and a protocol of implementation to promote its treatment and rehabilitation.The criteria of two-way referral between tertiarycare hospital and community health-care service center for patients with cerebral hemorrhage after surgical operation were formulated on a trial basis with mutual consultation by the department of neurosurgery,Shenzhou hospital affiliated to Shenyang Medical College and Hunhewan Community Health-Care Service Center,Heping district,Shenyang. Totally,91 patients of cerebral hemorrhage admitted to the hospital during October 2007 to September 2010 were recruited for the study,46 in the intervention group with two-way referral and 45 in the control group by self-referral determined by their family members.All the patients of the two groups were followed-up for three years.During the follow-up in the intervention group,40 of 46 (95%) patients could comply with taking medication ordered by physicians,ability of daily living remarkably improved by Barthel index in 21 (50%),physical function recovered well in 33 (79%),34 (81%) could insist in self-monitoring and 32 (76%) could visit physicians for regular consultation,all these being significantly better than those in the control group (64%,31%,28%,25% and 25%,respectively) (P <0.01 ).Medical and rehabilitative cost for patients with cerebral hemorrhage accounted for 575 RMB yuan per month in intervention group,as compared to 2246 RMB yuan for those in control group.Two-way referral for patients with cerebral hemorrhage can reduce their medical cost and promote their better rehabilitation.
4.Research on Prevention and Treatment of Cardiovascular Disease by Translational Medicine Based Chinese Medicine.

Translational medicine is inevitable in the development of modern medicine, and the uprising concept of translational medicine provides an opportunity for the development of Chinese medicine (CM). Their ideas are well communicated. There are two patterns of researching on CM based on translational medicine: 'literature to bench to bedside' and 'bench to bedside to bench'. CM has her advantages in preventing and treating cardiovascular disease. Effective methods for preventing and treating cardiovascular disease by CM should be further studied based on translational medicine concepts.

5.Efficacy and Pharmacoeconomic Analysis of Four Proton-pump Inhibitors in the Treatment of Duodenal Ulcer with Hemorrhage

Objective:To evaluate the therapeutic effects and pharmacoeconomic results of four proton-pump inhibitors in the duo-denal ulcer patients with hemorrhage. Methods:Using a retrospective analysis, 90 duodenal ulcer patients with hemorrhage were in-volved. According to the proton pump inhibitors, there was no statistic difference among those groups(P>0. 05). The mean time to stop bleading,hemostalsis rate,and the incidance of adverse drug reactions of those four groups,there were no statistic differece too( P>0. 05). The cost of pantprazole group was much lower than the other three groups by cost-minimication analysis. Results:The effective rate of the four groups was 80. 0%, 86. 7%, 90. 0% and 90. 0%, respectively;there was no statistic difference among those groups(P>0. 05). The mean time to stop bleeding, hemostasis rate, and the incidence of adverse drug reactions of those four groups, there were no statistic difference too(P>0. 05). The cost of pantoprazole group was much lower than the other three groups by cost-minimization alanysis. Conclusion:The effect and safety of the four groups were basically equivalent, while the coot of pantoprazole is lowest, it is the best treatment plan by pharmacoeceonomics.
6.Perspective on the Implementation of Prescription Administrative Policy in Ward Area Pharmacy

OBJECTIVE: To faciliate the implementation of the new Prescription Administrative Policy in ward area pharmacy. METHODS: Based on our practical working experience, the Prescription Administrative Policy was studied from perspective of quality of personnel, sense of responsibility and drug standards etc. And the difficulty encountered in the implementation of the prescription administrative policy was explained. RESULTS & CONCLUSIONS: Solving such problems as including medical orders in prescription management, clarifying the responsibility of irrational medication without sticking to instrcutions of drug package inserts, the demanding requirements on pharmacists’s quality and the enforcement of precription evaluation contributes to the implementation of the new Prescription Administrative Policy in ward area pharmacy.

9.The relationship of serum placental growth factor and carotid atherosclerosis in patients with type 2 diabetes mellitus

Objective To explore the relationship between serum placental growth factor (PLGF) and carotid atherosclerosis (CAS) in patients with type 2 diabetes mellitus (T2DM ). Methods Serum PLGF were determined by ELISA in 53 patients with T2DM ,who were divided into T2DM without carotid plague (T2DM group ,n= 27) and T2DM with carotid plague (T2DM + CAS group ,n= 26). Fasting plasma glucose (FPG) ,glycosylated hemoglobin A1c (HbA1c) and some other metabolic variables were also measured. The results were compared to those of 27 healthy controls (NC group). Results Serum PLGF levels were significantly higher in T2DM group and T2DM + CAS group than in NC group (P<0.01). T2DM+CAS group showed significantly higher serum levels of PLGF compared to that of T 2DM group(P<0.01).Correlation analysis showed that serum PLGF was positively correlated with TC ,LDL‐C , hsC‐RP and IL‐6 in T2DM patients (P<0.01). Similarly ,multiple regression analysis showed that LDL‐C and IL‐6 were the independent factors of serum PLGF level (P<0.05). Furthermore ,Serum PLGF was positively associated with carotid plague. Conclusion PLGF is positively related with chronic inflammation factors in T2DM patients. Serum PLGF may play an important role in the occurrence and development of carotid atherosclerosis and other macroangiopathy in T 2DM.
10.Analysis on causes of dysfunction of bioprosthetic valves

Objective To analyze the causative of damaged bioprosthetic valves implanted in 41 patients (with total 47 valves) over one year. Methods From June 1993 to June 2004,re-replacement of valves was performed in 41 patients with dysfunctional bioprosthetic valves. The interval between the two operations was 1~16 years [mean (9.7?4.1) years]. The 47 dysfunctional bioprosthetic valves were carefully observed and pathologically examined. Results Of the 47 valves, no valve frame was damaged; tear or perforation existed in 42 valves (89.3%); calcification was found in 35 valves (74.5%), with fibrosis; excrescence and thrombus coexisted in 2. Heavily damaged ones were sent for pathological examination, and infiltration of abundant phagocytes and monocytes was found. Conclusion Early damages were mainly tear or rupture, while late damages were compound outcomes of multiple damaging factors such as calcification, rupture, and tear.
