1.Therapeutic strategies and relevant clinical problems in non-small cell lung cancer with brain metastases

Brain metastasis is among the most common complications of non-small celllung cancer (NSCLC) and may obviously influence the survival and quality of life in the NSCLC patients. Recently, the development in traditional treatments, the widely-used micro-molecular targeted drugs, and the combined treatment have improved the overall survival and the quality of life of these patients. In addition, the exploration of new therapeutic targets and cognitive function protective methods for current treatments also deserves at-tention. This review emphasizes the relevant clinical and basic problems and the therapeutic strategies in NSCLC with brain metastases.
2.Study on the gap junction protein Connexin26 gene of neonatal congenital cytomegalovirus infection

[ABSTRACT]OBJECTIVETo investigate the cytomegalovirus infection in neonates, characteristics of gap junction protein Connexin26 gene mutation and the hearing follow-up results, and to analyze their correlations. METHODS60 CMV-DNA positive and 40 CMV-DNA negative neonatal newborn from The Second Affiliated Hospital of Wenzhou Medical University and The first people's Hospital of Yongkang were screened, the blood biochemistry was analyzed, and the umbilical cord blood was reserved to detect the Connexin26 gene expression of mRNA with RT-PCR.PCR results was sequenced to track the newborn hearing, and analyze the correlations between neonatal cytomegalovirus types, the mutation of Connexin26 gene and hearing test results.RESULTS 26 cases from 60 CMV-DNA positive newborns were found with blood biochemical abnormalities. In all of the newborns, a total of 41 cases had 235delC mutation, 11 cases in the mutations for the development of hearing impairment. The results of correlation analysis showed that there were correlations between cytomegalovirus infection, gene mutation and hearing impairment.CONCLUSION Cytomegalovirus infection in neonates can lead to mutations in the Connexin26 gene, and may further lead to hearing impairment, and the probability of the mutation of Connexin26 gene and sensorineural hearing loss were higher in symptomatic cytomegalovirus infection neonates.
3.Detection and identification of first human bocavirus infection in Guangdong

Objective To study human bocavirus (HBoV) induced respiratory tract infection in Guangdong Province in China. Methods HBoV was deteced by using polymerase chain reaction (PCR) technology and was identified by DNA sequences. Results One strain of HBoV was detected and identified from 50 samples collected from children with acute respiratory tract infections. This was the first clinical case of HBoV infection reported in Guangdong and was named as GD-1 strain. The HBoV capsid protein (VP) gene amplified from the specimen by PCR was identified by sequencing and was compared with gene sequences in GenBank. Phylogenetic trees were constructed for sequence homology analysis. The nucleotides similarities between GD-1 and Beijing strains, France strains and Canada strains were over 98%, while the simlilarity was over 36% compared with Korea KNIH-2K6GJ2713 strain and over 77% compared with US NH4549 strain. Conclusion HBoV infection does exist in Guangdong Province. It is valuable to start systemic study on it.
4.Influence factors analysis of nurse self-compassion in a psychiatric hospital

Objective To understand psychiatry hospital nurse' s self-compassion situation, explore the influencing factors, for nursing managers to know about the clinical nurses psychological health and to provide a reference data of intervention to improve the level of self-compassion. Methods A total of 381 clinical nurses from the Affiliated Brain Hospital of Guangzhou Medical University (Guangzhou Huiai Hospital) completed the survey using the questionnaire including the Self-Compassion Scale (SCS) and general information questionnaire. The influence factors were analyzed by chi-square test and Logistic regression analysis. Results The total score of SCS was (85.43 ± 10.23) points in 381 clinical nurses with the medium level, which was less than that of other nurse group (109.21±9.76) points, and there was significant difference(t=-45.388, P < 0.01).The Logistic regression analysis showed that female and working in the psychiatric ward were the risk factors of self compassion(OR=1.772, 1.995, P<0.05 or 0.01). While on the night shift was the protective factor(OR=0.536, P < 0.01). Conclusions Psychiatric hospital nurse' s self-compassion is at medium level. When the nurses cope with the negative events may lack adjustment method. Nursing managers should pay attention to train the ability of the nurse individual self-compassion, targeted to carry out active intervention measures.

7.Application of problem-based learning teaching in resident standardized training in depart-ment of pediatrics

Problem-based learning (PBL)was used in resident standardized training in depart-ment of pediatrics of Changhai Hospital. Attending doctors with authority were taken as leaders teach-ing group and 3-5 resident doctors as team members. Cases were set up according to the targets of resident standardization training and common clinical diseases in each system. According to the results of the questionnaire after the teaching , both teachers and students were satisfied with the teaching effect and expected targets were reached. Residents made great progress in handling clinical problems.
8.Advances in the Pathogenic Research of Female Carriers with Symptomatic Duchenne Muscular Dystrophy

DMD/BMD is a X-linked recessive hereditary disease.It predominantly affects males.While female carriers do not have symptoms,due to their inactive X chromosome make it present mosaic.Recently,more and more papers reported that a clinically significant proportion of DMD/BMD female carriers have symptoms.They presented variable degrees of symptoms.But the mechanism of the pathogencity is still not clear.Most of the research considered that the dominating reason is the skewed X inactivation.It means that the predominant expression of the DMD mutant allele make the normal one have weak expression,thus no function dystrophin proteins could be generate,manifested as DMD/BMD.In this paper,we mainly summarized the relationship between skewed X inactivation and pathogenicity of the symptomatic DMD female carriers.
9.Role of pathogens distribution and urine pH changes in urinary tract infection for clinical prevention and treatment

Objective To explore the distribution of pathogens and the characteristics of the urine pH values in urinary tract infection (UTI),and provide a new idea for clinical prevention and treatment of UTI.Methods The data of 606 strains of pathogens and the infected urine pH values from urinary samples of patients with UTI from January 2007 to November 2009 in our hospital were obtained.The distribution of pathogens was detected,and these pathogens were divided into 3 groups according to the genus.Urine pH value of different pathogens was analyzed.Results Among 606 strains from UTI,the group of escherichia ranked the first with 186 strains (30.7%),and the infected urine pH by this bacteria ranged from 6.21 to 6.33.A total of 152 strains of candida were detected (25.1%),and the infected urine pH value was from 5.81 to 5.95.There were 86 strains of enterococcus detected (14.2%),and the infected urine pH was from 6.10 to 6.28.By contrasting the infected urine pH of these 3 groups,the infected urine pH by candida was acidic significantly (P
10.Correlation of serum acute amyloid A level and activity of Behcet's disease

Objective·To investigate correlation of serum acute amyloid A (A-SAA) level and activity of Behcet's disease (BD).Methods·Blood samples from patients with active BD (n=40),remission BD (n=15),Takayasu disease (TA,n=12),rheumatoid arthritis (RA,n=25),systemic lupus erythematosus (SLE,n=25) and healthy donor (HD,n=25) were collected.Serum A-SAA,ESR,and CRP levels were detected and compared among groups.The correlation between A-SAA and its downstream IL-8 or International Society for Behcet's disease (ISBD) disease activity scores was analyzed as well.Results·Serum A-SAA level was significantly increased in the patients with active BD [(115.70±87.78) mg/L],as well as patients with RA and TA,which levels were (68.72±61.50) mg/L and (96.25±87.41) mg/L respectively,but remained unchanged in SLE patients.Moreover,serum A-SAA level was found correlated with ISBD score as well as IL-8 level.Conclusion·A-SAA can work as a detection index for evaluating BD activity.
