1.Chemoembolization with MMC-Iipiodol or ADM-lipiodol mixture for the treatment of bronchogenic carcinoma

Objective To study the clinical value of transbronchial arterial chemoembolization with MMC-li piodol or ADM-lipiodol mixture in the treatment of lung cancer.Methods In this study 32 cases with lung cancer,bronchial arteriography were adopted to identify the tumor-feeding branch.After perfusing 500~1000mg 5-Fu and 60 ~80mg CDDP through microcatheter,embolization with the mixture of 3 ~10ml lipiodol and 20mg MMC or 40mg ADM was performed.Results Short-term effectiveness;complete response(CR)was achieved in 1 case,while partial response(PR)in 19 cases,steady disease(SD)in 11 cases and process disease(PD)in 1 case.The total effectiverate (CR plus PR)was up to 62.5%(20/32).Complications included S cases of the chest pain and 2 cases of the chest wall epidermis pain,which were relieved after symptomatic management.1 case produced spinal injury,which was partially recovered after symptomatic management.Conclusion Transbronchial arterial chemoembolization with MMC-li piodol or ADM-lipiodol mixture has good short-term effect in the treatment of lung cancer.The procedure should be carried out carefully to avoid severe complications.
2.Investigation and Analysis of the Coefficient of Variation of Internal Quality Control of Homocysteine

Objective To carry out the current status of internal quality control by statistical analysis of the Internal Quality Control (IQC)data of homocysteine in 2014 March.Methods Web-based External Quality Assessment (EQA)system was used to collect IQC data of homocysteine from 292 EQA participant laboratories nationwide.The data include thecoefficient of variation (CV)of IQC data under control in March 2014 and long-term cumulative data.Acceptable rates of CVs of two-lot internal quality controls in homocysteine were calculated according to 5 criteria,that were 1/3TEa,1/4TEa and the speci-fications based on biological variation including the minimal,appropriate and optimal allowable imprecision.The instrument the participant laboratories used were sorted into 6 groups and the passing rate of each group were calculated by the 5 crite-ria.Results 292 laboratories reported the data of level 1 IQC for homocysteine,106 of which reported the data of level 2 IQC.The passing rate was different according to different criteria.The passing rate had few difference by the criteria of 1/3TEa and the minimal allowable imprecision based on biological variation,which were from 63.36% to 76.42%.It was also true for 1/4TEa and the appropriate allowable imprecision based on biological variation,which were from 34.25% to 57.55. The passing rate was only 10.62%~16.98% by the criteria of appropriate allowable imprecision based on biological varia-tion.Statistical results showed that instruments the participant mainly used were HITACHI (77/292),Olympus (61/292), Roche (19/292),Beckman (14/292),Abbott (10/292)and Simens (10/292).The passing rates of all the instrument group had few difference except Simens group.Conclusion Most of the laboratories could meet the criteria of 1/3TEa and the min-imal allowable imprecision based on biological variation.But less than half of the laboratories could meet the criteria of 1/4TEa and the appropriate and optimal allowable imprecision based on biological variation.The precision performance of ho-mocysteine among laboratories needs further improvement.
3.Surgical treatment of acetabulum top compression fracture with sea gull sign

Objective To investigate the surgical method and clinical curative results of acetabulum top compression fractures with sea gull sign. Methods Data of 14 patients who had acetabulum top compression fractures with sea gull sign were retrospectively analyzed and were accepted surgical therapy and followed up. There were 5 females and 9 males, aged from 28 to 71 years (average, 49.9 years). The pre-operative time was from 4 to 14 days (average, 9 days). Letournel-Judet fracture classifica-tion:eight cases for the anterior column+posterior semi-transverse, six cases for a simple anterior column. Three patients with lum-bar compression fracture, two patients with tibial plateau fracture, one patient with multiple rib fractures. All patients were accept-ed open reduction and bone graft and internal fixation through ilioinguinal approach or ilioinguinal approach+Kocher-Langenbeck approach. The quality of fracture reduction was assessed according to the Matta reduction criterion after operation. The hip func-tion was evaluated according to Matta. Results 14 patients were followed up from 6 to 60 months (average, 36 months). All pa-tients reached bone healing;healing time was from 3 months to 4 months (average, 3.4 months). According to the Matta reset stan-dard after operation, there were 6 excellent cases, 5 good cases, 2 fair cases, 1 poor case, the excellent and good rate was 78.5%(11/14). The Matta was from 10 to 18 scores (average, 16.4 scores), 5 excellent cases, 5 good cases, 3 fair cases, 1 poor case, the ex-cellent and good rate was 71.4%(10/14). Traumatic arthritis occurred in 3 patients. Pain was serious in two patients and disap-peared after total hip joint replacement. Pain was mild for one patient and disappeared after accepted non steroidal painkiller. One patient had heterotopic ossification after operation, which was not treatmented because of no obvious symptom. Conclusion The appropriate operation time of acetabular roof compression fracture with sea gull sign was from 5 to 10 days after fracture, which was no more than two weeks had best. Through ilioinguinal approach or ilioinguinal approach+Kocher-Langenbeck approach, the articular surface could be reduced excellent and got sufficient bone graft. The clinical efficacy was satisfactory after operation.
4.Clinical Observation of Mailuoning Injection in Adjunctive Treatment of Acute Cerebral Infarction

OBJECTIVE:To observe clinical efficacy and safety of Mailuoning injection in adjunctive treatment of acute cere-bral infarction. METHODS:A total of 65 patients with acute cerebral infarction selected from neurology department of our hospital were divided into control group(32 cases)and observation group(33 cases)according to random number table. Control group was given conventional treatment. Observation group was additionally given Mailuoning injection 10 mL added into 0.9% sodium chlo-ride injection 250 mL intrnrenously,ivgtt,qd,on the basis of control group. Both group were treated for 15 d. Clinical efficacy as well as serum levels of ox-LDL,BNP and MMP-9,NIHSS score before and after treatment,the occurrence of ADR were com-pared between 2 groups. RESULTS:The response rate of observation group was 90.91%,which was significantly higher than 65.63%,with statistical significance(P<0.05). Before treatment,there was no statistical significance in serum levels of ox-LDL, BNP or MMP-9,NIHSS score between 2 groups (P>0.05). After treatment,serum levels of ox-LDL,BNP and MMP-9,NIHSS score in 2 groups were all decreased significantly,and the observation group was significantly lower than the control group,with statis-tical significance (P<0.05). There was no statistical significance in incidence of ADR between 2 groups (P>0.05). CONCLU-SIONS:Mailuoning injection has significant therapeutic efficacy for acute cerebral infarction,can significantly reduce serum levels of ox-LDL,BNP and MMP-9,promotes neurological function and the recovery of patients with cerebral infarction with good safety.
5.Preliminary investigation of diagnostic value of ultra-high b-value based diffusion-weighted imaging ;in prostate central gland diagnosis

Objective To explore the value of ultra?high b?value DWI in diagnosis of prostate cancer in central gland. Methods Seventy?one consecutive patients, who were scheduled for prostate biopsy, were prospectively screened. T2WI, conventional DWI with b?value of 1 000 s/mm2 and ultra?high b?value DWI with b?value of 2 000 s/mm2 and 3 000 s/mm2 were performed in each examination. Twelve?core ultrasound guided prostate systematic biopsy was operated within 3 weeks after MRI examination. Images were interpreted based on prostate MR guidelines (PI?RADS) and were corresponding to histological results conducted by ultrasound guided prostate systematic biopsy. Using biopsy as the gold standard, the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated for different imaging methods. Sensitivity and specificity differences between ultra?high b?value DWI and conventional DWI were analyzed using the McNemar test. The areas under the curves (AUCs) between ultra?high b?value DWI and other modalities were compared by using the Z test. Results Forty lesions were identified in the prostate central glands from the 33 sample patients in 71 examinees. Twenty two lesions were identified as prostate cancer in 15 patients and 18 lesions were identified as benign prostatic hyperplasia in 18 patients. MRI analysis of lesions in central gland, 27 (67.5%), 20 (50.0%), 32 (80.0%) and 35 (87.5%) were diagnosed accurately with the T2WI, conventional DWI and ultra?high b?value DWI (b=2 000, 3 000 s/mm2) respectively. The sensitivity and specificity for ultra?high b?value DWI was 90.9%and 83.3% with a b?value of 3 000 s/mm2 and was 86.4% and 72.2% for 2 000 s/mm2. These values were significantly higher than conventional DWI with a b?value of 1 000 s/mm2 (59.1%and 38.9%, P<0.05). The detection of lesions was comparable with ultra?high b?value DWI at 2 000 s/mm2 and 3 000 s/mm2 (P>0.05). The AUCs were 0.674, 0.510, 0.793 and 0.871 in T2WI, conventional DWI and ultra?high b?value DWI at 2 000 s/mm2 and 3 000 s/mm2 respectively. ROC analysis showed greater AUCs for the ultra?high b value DWI, than for the T2WI and conventional DWI (P<0.05). Conclusion The ultra?high b?value DWI is a valuable MRI modality in the diagnosis of prostate cancer in central gland.
6.Effect of Guishen Pill on expression levels of Oct-4, MVH, and Egr-1 in mice with diminished ovarian reserve.

OBJECTIVETo study the effect of Guishen Pill (GSP) on expression levels of Oct-4, MVH, and Egr-1 in mice with diminished ovarian reserve (DOR).
METHODSTotally 40 female C57BL/6J mice were randomly divided into 4 groups, the normal control group, the model group, the GSP group, and the dehydroepiandrosterone (DHEA) group, 10 in each group. Pregnant mare serum gonadotropin (PMSG), human chorionic gonadotropin (HCG), and prostaglandin F2α (PGF2α) were sequentially administrated to produce superovulation. The DOR model was established by exposing to ozone inhalation. Mice in the GSP group were intragastrically administered with GSP at 0.3 mL. Those in the DHEA group were intragastrically administered with DHEA at 0.3 mL. Equal volume of normal saline was intragastrically administered to mice in the normal control group and the model group. All mice wer treated for 21 days. Serum levels of estrogen (E2), progestogen (P), and anti-Müllerian hormone (AMH) were measured by ELISA. Changes of Oct-4, anti-AMH, and early growth response gene-1 (Egr-1) mRNA in ovaries were dtected by Real-time PCR.
RESULTSCompared with the model group, serum levels of E2, P, and AMH, as well as contents of estrogen receptor (ER), progestogen receptor (PR), MVH, and Oct-4 mRNA significantly increased in the GSP group and the DHEA group (P < 0.05).
CONCLUSIONGSP could improve expression levels of Oct-4, MVH, and Egr-1 mRNA in DOR mice and their ovarian function.

7.Expression of lipoprotein related genes in subchondral bone of early experimental osteoarthritis.

OBJECTIVETo study the expression of lipoprotein related genes in subchondral bone of early experimental os-teoarthritis, which may play an important role in the pathogenesis of osteoarthritis.
METHODSAnimals are equally divided into two groups: experimental group and control group, both of which contain fifteen rats of similar weight. The right knee joints of experimental group underwent surgery,which involved in both medial collateral ligament(MCL) transaction and medial meniscectomy, while the control group was only carried out with a sham operation. Rats were killed at 1, 2 and 4 weeks postsurgery to obtain the right knee joints. Total RNA of the subchondral bone was extracted,and then hybridized to Agilent Whole Rat Genome Microarray. Differentially expressed genes analysis was used to study the chemokine signaling pathway.
RESULTSApoa5 expression was down-regulated at 1, 2 weeks post-surgery, Apoc2 expression was up-regulated at 1 week after surgery, Apol3 expression was up-regulated at 1 and down-regulated at 4 weeks post-surgery, Lrp1 expression was down-regulated at 1, 2 weeks after surgery. Lrp5 was down-regulated at 2 weeks after surgery. Gpihbp1, Lpl, Tfpi and Vldlr were up-regulated at 1 weeks after surgery. Lrpap1 and RGD1309808 were down-regulated at 4 weeks after surgery.
CONCLUSIONDysregulation of lipoprotein related genes plays an important role in pathogenesis of early experimental osteoarthritis.

8.A meta-analysis of the prevalence and risk factors of irritable bowel syndrome in Chinese community

Objective To estimate the prevalence and risk factors for irritable bowel syndrome (IBS) in China.Methods Cross-sectional studies relevant to IBS conducted among Chinese were identified through the databases including PubMed,Web of Science,the Cochrane Library,CBM,CNKI,Wanfang data and VIP.Quality of studies was assessed according to the criteria for cross-sectional studies recommended by Agency for Healthcare Research and Quality(AHRQ).Analysis of data,publication bias and sensitivity were performed with Stata (Version 12.0).Results A total of twenty-three studies were extracted.No obvious publication bias was detected in all analysis except the effect of depression on IBS prevalence.Pooled prevalence of IBS in China was 6.5%.IBS was more common in women than in men (8.1% vs 6.8%;OR=1.23,95%CI 1.09-1.38) and high rate in age group between 30 to 59 years (6.9% ; OR =1.22,95% CI 1.12-1.32).Intestinal infection history(OR =2.39,95% CI 1.69-3.38),anxiety (OR =2.95,95 % CI 1.94-4.49),depression (OR =1.85,95 % CI 1.11-3.09),food allergy (OR =2.80,95% CI 2.12-3.67) and alcohol consumption (OR =1.15,95% CI 1.07-1.24) might increase the risk for IBS.There were no significant difference of IBS prevalence between urban and rural areas (OR =0.97,95% CI 0.72-1.29),neither in different education classes (OR =0.85,95% CI 0.70-1.03).Sub-group analysis showed IBS prevalence varied apparently with different diagnostic criteria:prevalence defined by Manning was 11.8% and by Rome Ⅱ and Rome Ⅲ prevalence values were 4.4% and 8.9% respectively.Conclusions Pooled prevalence of IBS in China was 6.5%.IBS is more common in age group between 30 to 59 years.Female,history of intestinal infection,anxiety,depression,food allergy and alcohol consumption were risk factors for IBS in Chinese population.
9.Arthrography-assisted reduction in minimally invasive treatment of tibial plateau fractures

Objective:To evaluate arthrography-assisted reduction in minimally invasive treatment of tibial plateau fractures.Methods:From January to May 2019, 9 patients with tibial plateau fracture were treated by arthrography-assisted minimally invasive surgery at Department of Orthopaedics and Traumatology, Hong-Hui Hospital. They were 6 males and 3 females with an average age of 45.5 years (from 27 to 63 years). According to the Schatzker classification, there were 6 cases of type Ⅱ, one of type Ⅲ and 2 cases of type Ⅳ. Arthrography was used to determine the lowest filling point of contrast medium after intra-articular injection and to ensure satisfactory reduction of articular surface after fracture reduction and bone grafting, followed by minimally invasive percutaneous plate implantation. Comparisons were made between preoperation and 6 months postoperation in the range of knee motion and knee scores of The Hospital for Special Surgery (HSS). Knee X-ray films were evaluated by the Rasmussen imaging system at 6 months postoperation. Collapse of articular surface was recorded pre- and post-operation as well as postoperative complications.Results:All the 9 patients were followed up for 6 to 9 months (average, 7 months). All patients achieved bony union within 12 weeks postoperation. The preoperative knee flexion (60.7°±13.1°) and HSS (51.9±5.7) were significantly improved to 122.4°±10.8° and 84.9±5.3 at 6 months postoperation ( P<0.05), but there was no significant difference between preoperation and 6 months postoperation in knee extension (4.4°±2.5° versus 4.6°±2.9°) ( P>0.05). The collapse of articular surface was improved from preoperative 9.5 mm (from 5 to 15 mm) to postoperative 1.3 mm (from 0 to 3 mm). The Rasmussen imaging scores at 6 months postoperation showed 6 excellent and 3 good. No such postoperative complications occurred as incision infection, joint stiffness, loosening or breakage of implants. Conclusion:Arthrography helps minimally invasive treatment of tibial plateau fracture because it can better display fracture collapse during operation and accurately judge indirect reduction of articular surface.
10.Sodium iodide symporter gene expression in thyroid of mice with different iodine intakes

Objective To investigate the sodium iodide symporter (NIS) gene expression during different iodine intakes and its function in thyroid autoregulation. Methods BabL/c mice were randomly divided into five groups according to their different iodine intake levels : low iodine (LI), normal iodine (NI), five-fold iodine (5HI) ,ten-fold iodine (10 HI) and fifty-fold iodine (50 HI). After three months and six months administration, they were sacrificed and thyroids were excised. The mRNA and protein expression level were determined by real time quantitative PCR and immunohistochemistry respectively. Iodine content in thyroid tissue was measured with spectrophotometry and thyroid hormone level was measured with radioimmunoassay. Results Compared with NI group, NIS mRNA and protein expression was greatly increased in LI groups. Immunohistochemistry analysis indicated that NIS was mostly located at the basolateral membrane of thyrocyte,suggesting the improved activity in transporting iodine. But when faced with long-term and severe iodine deficiency, the iodine content in thyroid was finally decreased ,and hormone level lowered. On the other hand, in HI groups NIS mRNA and protein expression was greatly down-regulated. There was a tendency of decreasing NIS gene expression level with increasing doses of iodine intakes. In addition, NIS protein was found mainly in intracellular vesicles, rather than at the cell membrane,suggesting the loss of its activity. The iodine content in thyroid tissue was only slightly increased and was not consistent with the iodine intake levels. Conclusion These findings indicate that NIS may be. regulated at transcription, translation and post-translation levels. This phenomenon constitutes a highly specialized intrinsic autoregulatory system that protects thyroid from high doses of iodine, but at the same time ensures adequate iodine uptake for hormone biosynthesis. NIS plays a critical role in thyroid autoregulation mechanism.
