ObjectiveTo display the hot research points in stroke awareness, emergency treatment and evolution. Methods Papers on stroke awareness and emergency treatment were retrieved from Web of Science.Their knowledge atlas was plotted by key word frequency analysis and co-word analysis.Results The number of papers on stroke awareness and emergency treatment has been increasing year by year, and stroke awareness and emergency treat-ment have been concerned more and more since 1996 , indicating that the situation is still severe in stroke aware-ness and emergency treatment and no breakthrough has been made in this field.ConclusionStress should be laid on acute ischemic stroke, especially tPA, in study of stroke awareness and emergency treatment Research should be transformed into combined prevention and control from treatment .
2.Differentiation of human placenta mesenchymal stem cells into insulin-producing cells

Objective To explore the methods of inducing human placenta mesenchymal stem cells (MSCs) to differentiate into insulin-producing cells. Methods MSCs were obtained from human placenta and were induced to differentiate into insulin-producing cells by activin A, epidermal growth factor (EGF), basic fibroblast growth factor (bFGF) and other cytokines. The cells were identified by immunocytochemistry staining, Western Blot and animal experiment after differentiation. Results After induction, the cells possessed some characteristics of islet βcells, and could express pancreatic duodenal homeobox-1 (PDX-1), insulin and C peptide. The induced cells could also decrease blood glucose levels of diabetic mice. Conclusions These results showed that human placenta MSCs can be induced to differentiate into insulin-producing cells by cytokines.
3.Effects of dopamine alone or dopamine plus norepinephrine on hemodynamics, tissue oxygenation and renal function during orthotopic liver transplantation

Objective To evaluate the effects of dopamine (DA) alone or DA plus norepinephrine (NE) on hemodynamics, tissue oxygenation, acid-base balance and renal function during orthotopic liver transplantation (OLT).Methods Thirty ASA IE or IV patients undergoing OLT were randomly divided into 2 groups of 15 patients each. In group A DA was continuously infused during operation. The initial infusion rate was 1-3 ?g?kg-1 ?min-1 . The rate was then adjusted to MAP between 60-80 mmHg. While in group B DA and NE were continuously infused. The initial infusion rate of DA was 1-3 (?g?kg-1?min-1 and that of NE 0.03 ?g?kg-1?min-1 . Both rates were adjusted to MAP between 60-80 mm Hg but the maximal DA infusion rate was 5 ?g?kg-1?min-1 . Anesthesia was induced with midazolam, propofol, fentanyl and vecuronium and maintained with isoflurane inhalation, continuous propofol infusion and intermittent i.v. boluses of fentanyl, midazolam and pipecuronium. Radial artery was cannulated and S-G catheter was placed via right internal jugular vein. Hemodynamic variables such as MAP, CVP, PAP, PCWP, SVRI, PVRI, blood gases, mixed venous blood O2 saturation (SvO2), blood lactate, Cr and BUN were measured and O2 delivery (DO2 ) and O2 consumption (VO2 ) were calculated and recorded after induction and before surgery (T1 baseline), 60 min after operation was started (T2), at 60 min anhepatic phase (T3) and 60 min neohepatic phase (T4 ) and at the end of operation ( T5). Results There were no significant differences between the two groups with respect to age, sex, body weight, duration of operation and types of surgery. MAP and HR were quite stable in both groups. 4.Retrospective study on the treatment of severe asthma

Objective To evaluate the effect for the treatment of severe asthma. Methods The data of 47 patients with severe asthma who were admitted to emergency department were retrospectively anayzed. Results Of total 47 patients ,45 were rescued, with the survival rate of 95.7%. Arterial blood gas was improved after treatment (P < 0.01). Conclusion Appropriate commencement, mode, strategy, and early weaning of mechanical ventilation, combined with the administration of bronchodilators and eorticosteroids are the important way to rescue patients with severe asthma.
5.Effect of Glutamine in stress ulcer Prevention and treatment of acute cerebral infarction Patients

Objective To Apply appropriate dose of Glutamine(Gln)(300mG/d)for patients with acute cerebral infarction,and observe its preventinG and curinG function to the stress ulcer. Methods Sixty cases patients of acute cerebral infarction were observed and divided into the conventional treatment Group( 30 cases),Glutamine treatment Group( GLU,30 cases). Gastric pH value of patients was tested before treatment and after one week treatment. The stool or vomit occult blood positive( OB+)number of patients within one week, OB( +) duration and the number of patients need blood filterinG were observed. Results There was no siGnificant difference between the conventional treatment Group and the GLU Group in the Gastric juice PH value before treatment(t=0. 647,P>0. 05). After one week treatment,the Gastric juice PH value of the GLU Group was siGnificant hiGher than the conventional treatment Group(t=5. 25,P<0. 05);the number of OB( +)of GLU Group were less than the conventional treatment Group,but there was no statistical siGnificant difference(χ2=0. 577,P>0. 05);the OB( +)duration of GLU Group was less than the conventional treatemnt Group,and there was satistical siGnificant difference(t =4. 768,P <0. 05);the number of blood filterinG of GLU to treat stress ulcer was less than conventional treatment Group,and there was satistical siGnificant difference(χ2=l. 0l6,P>0. 05). Conclusion SupplementinG appropriate dose of Glutamine in patients with acute cerebral infarction can reduce the incidence of stress ulcer,decrease medical expenses.
6.Solubility and Apparent Oil/Water Partition Coefficient of Sulfamethazine

OBJECTIVE:To determine the equilibrium solubility and the apparent partition coefficient of sulfamethazine(SM2) in a series of phosphate buffer solutions of different pH so as to provide a basic study for the exploitation of SM2 preparation.METHODS:A series of buffer solutions of different pH were prepared.The apparent solubility was determined by saturation method;Ko/w was calculated with concentration ratio of SM2 in n-octanol and water phase after partition equilibrium.RESULTS:The maximum equilibrium solubility was 1.916 g?L-1 at pH 2 and 1.375 g?L-1 at pH 9,and the maximum of apparent partition coefficient was 3.9070 at pH 8.CONCLUSION:The equilibrium solubility and apparent partition coefficient of SM2 are correlated to pH of the medium.SM2 dissolved preferably when pH6.8,but SM2 was more distributed in the lipid phase and easier to be absorbed by body when pH=3~8.
7.Adsorptive property of novel chitosan microspheres for plasma bilirubin and cytokines: A preliminary screening in vitro of novel biomaterials

BACKGROUND: The effective therapy of artificial liver for severe hepatitis needs an absorbing material which possesses strong adsorptive property, high adsorption rate and good blood compatibility.OBJECTIVE: To study the adsorptive property of eight novel adsorbents for plasma bilirubin and cytokines in severe hepatitis patients. DESIGN: A controlled observation.SETTINGS: Graduate School of Tianjin Medical University, Tianjin Haihe Hospital, Tianjin Third Central Hospital and the Institute of Polymer Chemistry in Nankai University.PARTICIPANTS: All plasma was collected from 30 severe hepatitis patients hospitalized in Tianjin Third Central Hospital from November 2004 to November 2005. Informed consent was obtained from each patients. This experiment was approved by the hospital ethical committee. All the patients were divided into two groups at random: group 1 (n=10) and group 2 (n=20). The level of total bilirubin (TBiL) before therapy in two groups was (377.3±147.5) μmol/L and (327.6±140.1) μmol/L, respectively.METHODS: ①Adsorbents: Chitosan (Qingdao Lizhong Chitosan Factory, Shandong) with relative molecular weight 97 000 and de-acetyl grade 85%; Adsorbents No.1-3 were prepared by using 1%, 3%, 5% polyethyleneglycol (relative molecular weight 600) as porogenic agent. Adsorbent No.4 was aminated crosslinked chitosan microspheres; Adsorbent No.5 was divinyl-benzene crosslinked macroporous polystyrene microspheres; Adsorbent No.6 was post-crosslinked macroporous divinyl-benzene styrene copolymer microspheres; Adsorbents No.7 and 8 were chitosan wrapped adsorbent No.5 and 6.②Detection: Step 1: 3 mL plasma collected from each severe hepatitis patient in group 1 was absorbed with 1 mL of 8 kinds of adsorbents. Levels of plasma TBiL, direct bilirubin (DBiL) and indirect bilirubin (IBiL) before and after adsorption were determined by using the vanadate oxidation method to analyze the average adsorption capacity so as to screen the adsorbents with the better adsorptive properties. Step 2: 3 mL plasma collected from each severe hepatitis patient in group 2 was absorbed with 1 mL of two adsorbents selected from the step 1. The levels of bilirubin, interleukin-6 and tumor necrosis factor-α before and after the adsorption were analyzed by ELISA method.MAIN OUTCOME MEASURES: The levels of bilirubin and cytokines before and after the adsorption were determined.RESULTS: ①The data in the first step experiment showed that after No.4 and No.5 adsorbents were used, the level of plasma TBiL, DBiL and IBiL were significantly decreased (P < 0.01); no differences were found for other six kinds of adsorbents (P > 0.05).②The data in the second step experiment showed that the average levels of plasma TBiL, DBiL, IBiL, interleukin-6 and tumor necrosis factor-α were remarkably reduced after using adsorbents No.4 and No.5 (P < 0.01). Compared with adsorbent No.5, there were significant decrements for adsorbents No.4 (P < 0.01).CONCLUSION: Aminated chitosan microsphere has significant effects on adsorbing bilirubin and cytokines in plasma of severe hepatitis patients in vitro.
8.Therapeutic effects of Hanbi Formula on adjuvant-induced arthritis rats

AIM To research the therapeutic effects of Hanbi Formula (Astragali Radix,Aconiti Radix cocta,Scorpio,Scolopendrap and Pheretima) on adjuvant-induced arthritis rats (RA) and its mechanism of action.METHODS RA rat models were established by using Freund's adjuvant,and then the rats were divided into six groups,namely control group,model group,dexamethasone positive group,Baoguang Fengshi Liquid (Notopterygii Rhizoma et Radix,Radix angelicae pubescentis,Chuanxiong Rhizoma,etc.) positive group,and low,high doses of Hanbi Formula groups.The volume and swelling of toes were measured.The interleukin-1 β (IL-1β) and tumor necrosis factor-α (TNF-α) of serum were detected by ELISA;the proliferative capacity of lymphocytes was tested by methyl thiazolyl tetrazolium (MTI) method;synovial tissue was histopathologically examined with HE staining.Finally,the expressions of interleukin-17 (IL-17) and TNF-α in synovial tissue were determined by immunohistochemical assays.RESULTS Hanbi Formula could significantly relieve toe swelling of RA rats.Compared with the model group,Hanbi Formula could significantly alleviate synovitis in rats with RA,down-regulate the expressins of IL-1 β and TNF-α in serum and synovial tissue,and inhibit lymphocyte proliferation.There were no significant differences in above indices between low-dose and high-dose Hanbi Formula groups,which was quite with Baoguang Fengshi Liquid,but less than dexamethasone.CONCLUSION Hanbi Formula possesses an obvious function of anti-RA,and its mechanism may be related to the inhibition of lymphocyte proliferation and reducing secretion of inflammatory cytokines.
9.The survey about health knowledge condition in HBV-positive pregnant women

Objective To investigate the HBV- positive pregnant women about the knowledge of the route of hepatitis B transmission, hepatitis B hazards, health care during pregnancy, announcements of child- care to explore targeted and effective health education model with self- designed question-naires. Methods Nurses in obstetrics department carried out this questionnaire investigation in the outpa-tients of HBV- positive pregnant women, after that the results underwent statistic analysis. Results The overall knowledge of HBV- positive pregnant women in announcements of child- care, health care during pregnancy and hepatitis B hazards was not very well, and the announcements of child- care was the lowest, followed by health care during pregnancy and hepatitis B hazards. The route of hepatitis B transmission was better, but still not satisfactory. The education should highlight these areas. Well- educated women were better than those who lack education. The age between 20 to 30 years old was better. Conclusions The HBV-positive pregnant women is lack of heath knowledge, so we should gave health intervention in a multi- level, multi- stage standardized and a variety of ways.
