1.Association of fluid overload with acute kidney injury and prognosis in critically ill children

Objectives To investigate the association of fluid overload (FO) with the development and mortality of acute kidney injury (AKI) and to evaluate the predictive value of FO in mortality of critically ill children. Method A prospective study was conducted among critically ill children who were admitted to the children's intensive care unit (PICU). FO levels were assessed during the course of the disease and PRISM Ⅲ scores were evaluated within 24 hours of admission. Binary logistic regression analyses were conducted to evaluate the association of FO with the development and mortality of AKI after adjusting for confounding factors. The area under the receiver operating characteristic curve (AUC) was calculated to assess the predictive value of FO for mortality. Results In 362 children included, there were 26 children (7.18%) having average FO≥5%, and AKI in 24 children (6.63%) and 18 children (5.0%) died. The mean FO (OR=1.26, 95%CI: 1.10~1.43, P=0.001) and the maximum FO (OR=1.12, 95%CI: 1.02~1.23, P=0.018) were significantly correlated with the development of AKI in critically ill children within 7 days of admission to PICU. However, after adjusting for age and PRISM Ⅲ, both factors had no association with AKI (all P>0.05). After adjusting for the potential confounders such as AKI and the severity of disease, the average FO was significantly associated with mortality (AOR=1.34, 95%CI: 1.12~1.60, P=0.002). The AUC of mean FO that predicted mortality risk was 0.801 (P<0.001). Conclusion Fluid overload is associated with the development and the prognosis of AKI in critically ill children, and has important predictive value for mortality.
2.Predicting value of ischemia grade on arrhythmia in patients with acute ST-segment elevation myocardial infarction

Objective To evaluate clinical significance of ischemia grade to predict severe arrhythmia in patients with acute ST-segment elevation myocardial infarction(STEMI).Methods A total of 225 patients with STEMI admitting to emergency department were enrolled.All enrolled patients were divided into two groups based on the QRS complex electrocardiogram on admission:grade 1Ⅱ ischemia group(135 cases) and grade Ⅲ ischemia group (90 cases).All patients received thrombolytic therapy.The incidence rate of ST segment resolution (STR) and severe arrhythmia in hospital stay was observed.Results The ST segment elevation (Σ.ST) on admission and 2 h after thrombolysis in grade Ⅲ ischemia group was significantly higher than that in grade Ⅱ ischemia group [(0.84 ± 0.57) mV vs.(0.44 ± 0.35) mV,(0.50 ± 0.23) mV vs.(0.11 ± 0.06) mV] (P < 0.01).The backing rate of ST segment ≥ 50% 2 h after thrombolysis in grade Ⅲ ischemia group was significantly lower than that in grade Ⅱ ischemia group [56.7% (51/90) vs.83.7% (113/135)] (P <0.01).The creatine kinase MB (CK-MB) peak value in grade Ⅲ ischemia group was significantly higher than that in grade Ⅱ ischemia group [(363 ± 105) U/L vs.(212 ± 97) U/L] (P < 0.01).There was no significant difference in the incidence of severe arrhythmia between two groups (P > 0.05).Multiple Logistic regression analysis demonstrated that the independent predictors of severe arrhythmia were duration from symptom to thrombolysis and initial ΣST,whereas grade Ⅲ ischemia remained a strong predictor of severe arrhythmia.Conclusion Grade m ischemia on admission is associated with lower incidence of STR in patients with STEMI after thrombolysis and a strong predictor of severe arrhythmia.
3.Development of a rapid assay for human cardiac troponin I

Objective:To develop a sensitive,rapid,simple gold immunochromatography assay for human cardiac troponin I(cTnI).Methods:The cTnI was used to immunize Balb/c mice by route method.The spenocytes were fused with SP2/0 myeloma cell lines.Six hybridoma cell lines,secreting stably McAbs to cTnI antigen have been obtained and screened.The 3F10 was coupled with colloidal golds,the 3A7 biotinylated.Streptavidin was coupled with the nitrocellose strip to prepare an test apparatu.According to red line in nitrocellose,thus can analysis the results.Results:The sensitivity of cTnI measured by this apparatus was 0.4 ng/ml.The 30 patients with acute myocardial infarction(AMI)were detected for cTnI by this apparatus and by the product of PBM Co USA as standard.The coincidence rate of both two assays was 93.6%. Conclusion:This method is a sensitive,specific simple and rapid assay.
4.Effects of Desflurane and Sevoflurane on Somatosensory Evoked Potentials in Patients Undergoing Neurosurgery

Objective To compare the effects of desflurane and sevoflurane on somatosensory evoked potentials (SEP) in patients undergoing spine neurosurgery. Methods Thirty patients with ASA grades I-Ⅱand undergoing spinal deformity orthopaedic surgery were selected and divided into two groups: desflurane group and sevoflurane group (n=30) . Bispectral index (BIS),the state of muscle relaxation, SEP and hemodynamics were monitored during operation. By maintaining the BIS in the range of 35-45,the state of muscle relaxation disappeared and hemodynamic became stable. Adjust the concentration of anesthetic step by step from 0 to 0.3, 0.6 and 0.9 maximum alveolar concentrations (MAC), then inhaled different concentrations of drugs. Record the incubation period and amplitude of SEP before inhaling anesthetics (based on the value), in the moment of achieving each inhaled concentration ( T1, T2 and T3) , and the moment that the inhaled anesthetics blows completely (regression value) . The percentages of T1, T2, T3, and the regression value compared with basic value were calculated.Results Compared with the basic value, T1, T2 and T3 of left arm, left leg and right leg in desflurane group, T2 and T3 of right arm in desflurane group, T2 and T3 of left arm, left leg and right arm in sevoflurane group,and T2 and T3 of right leg in sevoflurane group showed incubation period delaying ( <0.05) . And the incubation period extended along with the increase of inhaled anesthetics dosage. Compared with the basic value,T1,T2 and T1 of left arm and left leg in desflurane group,T1 of right arm in desflurane group, T1 and T2 of right leg in desflurane, T3 of left arm in sevoflurane group, T1 and T3 in left leg in sevoflurane group had decreased amplitude ( < 0.05), especially T1. Compared with desflurane, sevoflurane showed a weaker inhibition effect on the incubation period and amplitude of T1,T2,T3 of left arm and left leg,T2 and T3 of right arm and right leg, and the amplitude of T1 of right leg ( < 0.05) . Conclusion Both of desflurane and sevoflurane have inhibition effect on SEP. The inhibition effect gradually strengthens along with the increasing of dose. The inhibition effect of desflurane is stronger than sevoflurane.
5.Identification of Gardenia Based on cpDNA Barcoding

Objective To test and eva1uate the abi1ity of three potential chloroplast DNA (cpDNA) barcoding sequences;To find new methods to identify the species of gardenia. Methods Three cpDNA sequences were amplified and sequenced by universal primers of matK, rbcL and psbA. By comparing PCR amplification efficiency, length, intra-and inter-specific divergence, and barcoding gap, BLAST and DNA MAN were used to evaluate these loci. Results The amplification efficiency of 5 samples from 3 gardenia species was 100%. Analysis of the intra-and inter-specific divergence of matK among the sequences showed that barcoding gap was superior to psbA and rbcL, with higher identification efficiency. Conclusion Gardenia jasminoides Ellis can be better identified by matK sequence.

Objective To retrospectively analyze the cardiac systolic synchrony and cardiac function in patients with chronic heart failure (CHF) by gated SPECT myocardial perfusion imaging (GSMPI). Methods (1) From January 2010 to February 2015, 123 cases including 82 CHF patients (57 males, 25 females, age (59.5±11.0) years) and 41 healthy people (control group; 27 males, 14 females, age (33.8±5.2) years) were enrolled in this retrospective study. According to the New York Heart Association (NYHA) classification, the CHF patients were classified into grade Ⅰ-Ⅳ groups. The systolic synchrony and cardiac functional parameters including PHB, PSD, LVEF, EDV, summed rest scores (SRS) were acquired by Emory Cardiac Toolbox software. Differences of PHB, PSD and LVEF were compared between the CHF group and the control group using two-sample t test. The difference among the four CHF groups was compared by one-way analysis of variance. The difference of some clinical factors was compared between the two groups with and without damage of systolic synchrony. The relationship between the cardiac synchrony and myocardial perfusion was analyzed by Pearson correlation analysis. Results There was no obvious difference of PHB and PSD between the grade Ⅰ CHF patients and the control group (t=-1.502 and -0.448, both P>0.05), while LVEF was significant different (t=10.419, P<0.05). Significant difference of PHB, PSD and LVEF existed between the grade Ⅱ-Ⅳ CHF patients and the control group (t values: from -27.250 to 32.723, all P<0.05). There were significant differences of PHB, PSD and LVEF among the 4 CHF groups (F=118.05, 4.13 and 154.37; all P<0.05). The differences of LVEF, EDV and SRS were significant between the patients with and without damage of systolic synchrony (t=9.57, 10.85, 18.87, all P<0.05). The ratios of damage in systolic synchrony in grade Ⅰ-Ⅳ CHF patients were 8.7% (2/23), 60.0%(12/20), 15/18 and100% (21/21), respectively. PHB and PSD were both positively correlated with SRS (r=0.808 and 0.773, both P<0.05). Conclusions The damage of systolic synchrony are getting severer from patients with NYHA grade Ⅱ to patients with NYHA grade Ⅳ. The damage could be accompanied by the heart failure progression. Diabetes mellitus, LVEF, EDV, ESV, and SRS are related to the damage. The myocardial perfusion damage is positively correlated with the damage of cardiac systolic synchrony. GSMPI is useful to early diagnosis and treatment of heart failure.
8.Applicative value of acoustic radiation force impulse imaging in differential diagnosis of small hypoechoic thyroid nodules

Objective To investigate the applicative value of acoustic radiation force impulse imaging (ARFI) in the differential diagnosis of small hypoechoic thyroid nodules.Methods 31 patients were pathologically confirmed as having a total of 44 small hypoechoic thyroid nodules ≤ 1 cm in diameter,followed by analyses of the characteristics of their ARFI images,including virtual touch tissue imaging (VTI) and virtual touch tissue quantification (VTQ).Based on VTI images of the nodules,the hardness of small hypoechoic thyroid nodules was respectively scored,and the ratios of VTI images to gray scale image areas were calculated.With pathological diagnosis as the gold standard,VTI scores,area ratios and VTQ values of the ROC curve in diagnosis of thyroid malignant nodules were sketched so as to explore the optimal cut-off points in VTI scores,area ratios and VTQ values in the diagnosis of malignant nodules.Results By the ROC curve test,VTI score of 3 was determined as the cut-off point of areas ratio,with the sensitivity and specificity of diagnosing thyroid cancer being 96.0 % and 94.7%,respectively.There were 34 nodules with VTI score greater than 3 (including 9 benign nodules and 25 malignant nodules) and 10 nodules with VTI score of 3 or less (all benign nodules).Malignant thyroid nodules had higher ratio of VTI images to gray scale image area than that of the benign nodules (1.58 ± 0.18 vs 1.30 ± 0.10,P <0.05).When 1.47 was set as the cut-off point of areas ratio,the sensitivity and specificity of diagnosing thyroid cancer were 80.0% and 94.7%,respectively.The VTQ value of 2.76 m/s in diagnosis of thyroid malignant nodules was designated as the cut-off point,with a sensitivity of 88.0 %,specificity of 94.7 %.Conclusions The ARFI technique is able to provide information of thyroid nodule hardness,which guarantees its high applicative value in the differential diagnosis of small hypoechoic thyroid nodules.
9.Efficacy and safety of nebulized hypertonic saline in different concentration in infants with moderate to severe bronchiolitis

Objective To evaluate the efficacy and safety of frequently nebulized hypertonic saline(HS) in different concentration with Terbutaline in infants with moderate to severe bronchiolitis.Methods A total of 195 infants were randomly enrolled to either Group A(5% HS 4ml+Terbutaline 2.5mg,n=65),Group B(3% HS 4ml + Terbutaline 2.5mg,n=65) or Group C[0.9% normal saline(NS) 4ml + Terbutaline 2.5mg,n=65].All the infants were given inhalation treatment q6h after admission for 4 times,and then q8h till the remission of wheezing.The primary outcome was the hospital length of stay(LOS),and the secondary outcomes were the clinical severity score(CSS),remission time of wheezing,cost and side effects.
10.Effects of disease diagnosis and operative procedure on grouping of diagnosis related groups(DRGs)

Objective To learn the impacts of major diagnosis, other diagnoses, major surgery and other surgeries on the grouping of DRGs, and to optimize the DRGs data grouping quality by analyzing the main influencing factors of DRGs.Methods Based on regrouping results of the 1 940 questionable cases which have been corrected, using SPSS 13.0 to study the impacts of the questions found on the grouping of DRGs.Results 438 Patient records affecting DRGs grouping were regrouped according to major diagnosis, other diagnoses, major surgery and other surgeries.Influences of the above four groups on the grouping vary in general For comparison between two groups, P>0.007 1 between main surgery and other surgeries;while P<0.007 1 for comparison between other groups.Conclusion The highest influence of grouping was found in questionable major diagnosis, much higher than such other factors as other diagnoses, major surgery and other surgeries.This conclusion conforms to the steps of basic DRGs grouping logic-sorting the eases according to major diagnosis first of all Correct naming of surgery influences DRGs grouping, but the influence extent of major surgery and other surgeries is close.
