1.Analysis of Helicobacter pylori infection in different groups of population in Yangzhou

Objective To investigate the infection status and distribution characteristic of Helicobacter pylori (Hp)in different groups of population in Yangzhou.Methods 2 917 individuals from outpatients,hospitalized patients,and physical examination pop-ulation were recruited in the study.Serum samples from the subjects were rapidly detected using Helicobacter pylori urease antibody detection kits(Colloidal Gold method).The results were statistically analyzed.Results The totally Hp-detection rate of the 2 917 individuals was 32.9% (959/2 917).The Hp-detection rates of outpatients,hospitalized patients,and physical examination popula-tion were 33.1%,34.5% and 30.0% respectively.There were no significant difference in the positive rates between the male and female individuals(P >0.05).Overall,The Hp-positive rates increased with age.The minors(juveniles and children)had the lowest Hp-positive rate 16.0%(285/1 529);The Hp-positive rate of young adults was 34.0%(221/650),which was at the middle;elderly group had the highest Hp-positive rate 61.38%(453/758).59.8% of the patients diagnosed with gastrointestinal diseases were Hp positive.Among which,patients with peptic ulcer(74.4%)and gastric cancer(70.6%)have the highest detection rates.Some pa-tients with blood diseases,cardiovascular disease and some other diseases were also found with high Hp-detection rates.Conclusion The distribution of the Hp infection in population is positively correlated with age,but not with gender.Gastrointestinal diseases, especially peptic ulcers and gastric cancer,have high Hp infection rates,the relationships of Hp infections with some other diseases cannot be ignored and need to be further explored.
2.Effect of laminin and type Ⅳ collagen on proliferation of Schwann cells

BACKGROUND: Schwann cells play an important role in regeneration of peripheral nerves and the extracellular matrix(ECM) is also important to growth of Schwann cells.OBJECTIVE: To explore the effects of laminin and type Ⅳ collagen on the adhesion and proliferation of Schwann cells.DESIGN: A controlled trial in groups with Schwann cell as subject.SETTING: Plastic Surgery Laboratory of the Fourth Military Medical University of Chinese PLA.MATERIALS: The trial was conducted in the Plastic Surgery Laboratory of Xijing Hospital Affiliated to the Fourth Military Medical University of Chinese PLA from January through April in 1995. The Schwann cells were extracted from newborn rabbits.METHODS: The culture plates were coated with laminin and type Ⅳ collagen. While in the control group, the antibody of laminin and collagen Ⅳ,collagen Ⅰ and polylysine were added respectively. The Schwann cells of the same concentration were cultured in plates of each group. The cells attachment rate in each group ,was determined after 2, 6, 24 hours respectively. After 72 hours 3H-TdR were incorporated into culture matrix fluid and double channel liquid scintillation counter was used for measurement of radiactivity count per minute.MAIN OUTCOME MEASURES: ① Attachment rate of Schwann cell. ②3H-TdR counting.RESULTS: The attachment rates in laminin and collagen Ⅳ groups (66%and 59% ) were higher than those of the type Ⅰ collagen and polylysine groups(45% and 43% ) after 2-hour culturing. Those in laminin antibody and collagen Ⅳ antibody groups were lower. The incorporation value were (10.0±2.7)×103, (1.3±0.3)×103, (10.4±2.4)×103, (1.4±0.5) ×103, (5.8±2.7)×103, (3.3±1.0)×103 per minute respectively in laminin, laminin antibody, collagen Ⅳ, collagen Ⅳ antibody, collagen Ⅰ and polylysine groups. The incorporation values in laminin and collagen Ⅳgroups were higher than controls.CONCLUSION: Laminin and type Ⅳ collagen promote the attachment and proliferation of Schwann cells in vitro. This study provides experimental basis for applying the action of Schwann cell to the nerve tissue engineering.
3.Effect of mental toughness level on positive and negative emotion and mental health in children with epilepsy

ObjectiveTo explore the effect of mental toughness level on positive and negative emotion and mental health in children with epilepsy.MethodsIn accordance with the International League against epilepsy and epilepsy syndrome in 1989 International Classification of young children with epilepsy,160 cases were sampled.Based on the Adolescent Resilience Scale ( HKRA ) score,according to the 27% principles of delimitation,the patients were divided into high HKRA score group and low HKRA score group,43 cases in each group.Then positive emotions and negative affect scale (PANAS),mental health test (MHT) were applied to two groups.Results,Except family support dimension,the others HKRA dimension and total score were higher in female children with epilepsy than those in male patients,there was significant difference (P< 0.01 ).The positive affect score were higher in high HKRA score group than that of low HKRA group ; and negative emotion was significantly lower than that in low HKRA group (P< 0.01 ).Except physical symptom dimension,other dimensions and total score of HKRA in high HKRA score group were higher than those in low HKRA group,there was significant difference (P< 0.01 ).Children with epilepsy HKRA score was positive correlation with positive emotion score but negative correlation with negative affect scores (P < 0.01 ) ; in addition to physical symptom,HKRA score and MHT score,and all the other dimensions were negatively related (P<0.01 or 0.015).ConclusionThe mental toughness level are significantly related to children's epilepsy patients with positive and negative emotion and mental health status.A high level of mental toughness can increase positive emotional experience,and reduce the negative emotional experience of children with epilepsy.
4.Changes of cortical-limbic pathways in male recurrent unipolar major depressive disorder during attention control dysfunction

ObjectiveTo study information transmission circumstances within the neural circuits related to the attentional controls of the mate depression patients.MethodsTwelve unipolar depression patients and twelve age- and education- matched healthy controls were involved in the Stroop word-color interference test to detect the variation of brain inhibitory function by Magnetoencephalography scanning.ResultsCompared with the healthy controls,the abnormally activities in depressed patients during in chronological order were separately as follow:decreased activation in the left postcentral gyrus ( t =3.10 ),the right superior frontal gyrus ( t =2.16 ),the left middle frontal gyrus (t=2.64),the left parietal lobe (t=2.89),the right middle temporal grus( t=1.92),the right superior temporal gyrus(t=2.84),the right insula(t=3.03),the right precuneus(t=2.98),the bilateral middle frontal gyros ( t =3.64,t =2.25 ),the right postcentral gyrus ( t =2.20),the right cingulate gyrus ( t =4.04 ),the bilateral precuneus ( t =2.79,t =1.87 ) ;increased activation in the left caudate body ( t =1.91 ),the left pulvinar ( t =2.56),the left middle temporal gyrus( t =1.89),the left precentral gyrus( t =2.02) (P < 0.05,uncorrected).ConclusionThe abnormal activation in the brain regions related with attentional control processing is detected in the male recurrent unipolar recurrent major depression,performances of limbic system increased and.cortex declined more in the recurrent depressed.
5.Clinical study of raise ultrasonic display rate for calcification in breast lesions

ObjectiveTo evaluate the value of MicroPure software system in the display of calcification and diagnostic accuracy of breast carcinoma.MethodsSixty-three patients with 68 breast lesions were checked by conventional ultrasound,X-ray and MicroPure software technique.The calcification was described and the lesions were classified with BI-RADS.All cases were pathologically confirmed by biopsy or operation.Results ①Compared to the detection rate for calcification of X-ray,the display rate of conventional ultrasound was 80.6% ,and the display rate of MicroPure software technique was 100%.②The area under the curve of diagnosing benign and malignant of breast lesions was 0.916,0.857 and 0.970 respectively with ultrasound,X-ray and MicroPure software technique.③Three lesions was classified Ⅲ in BI-RADS with conventional ultrasound,and Ⅳ in BI-RADS with MicroPure software.The pathologic results were malignant.ConclusionsMicroPure software system obviously raised the ultrasonic display rate of calcification,meanwhile,it raised the diagnostic accurate rate of breast cancer.It especially raised the tendency rate for the patients who were classified between Ⅲ and Ⅳ in BI-RADS.
6.Clinical application of fluorescent probe PCR for detection of mycoplasma pneumoniae

Objective To explore the clinical application of FP-PCR method to detect MP-DNA.Methods Five hundred and sixty-three children suspected of MP infection were enrolled in experimental group. FP-PCR was adopted to detect MP-DNA. MP-DNA was re-detected later in 60 children. At the same time,MP-Ab (MP antibody) was detected by means of particle agglutination. MP-Ab was re-detected one or two weeks later. Also 20 healthy children were selected as the control group. Results The positive rate of MP-DNA and MP-Ab were 34. 99% and 35.52% respectively,which showed no significant difference (x2 =0. 31, P > 0. 05). The coincidence of the two methods was 97. 69%. But the positive rate of MP-DNA was significantly higher than that of MP-Ab in the early stage(30. 48% vs 10. 16%) (x2 = 74. 46, P < 0. 05).The sensitivity and specificity of FP-PCR were 96. 00% and 98.62% respectively. The result of reviewed MP-DNA was consistent with the clinical diagnosis. Conclusion FP-PCR method is very sensitive, convenient and stable. It is fit for the clinical application ,especially the diagnosis of early MP infection. It helps to identify those who had been infected with MP before.
7.The fronto-cingulate executive function in recurrent depression revealed by the magnetic source imaging

9.Influence factors of long-term outcomes of mitral valve repair for moderate and severe mitral regurgitation due to myxomatous degeneration

Objective To explore and conclude the influence factors of long-term outcomes of mitral valve repair for moderate and severe mitral regurgitation due to myxomatous degeneration.Methods To review the in-patient data and followup outcomes of 261 patients after mitral valve repair for moderate and severe mitral regurgitation due to myxomatous degeneration from Jan 1993 to Jan 2008 in Changhai Hospital of Second Military Medical University.Results There were 7 perioperative deaths and 254 survivors who obtained satisfactory perioperative outcomes.During the follow-up,24 patients were lost and 230 patients were followed up from 36 months to 174 months (77.3 ±30.3) months and follow-up rate was 90.6%.Multivariate Cox regression shows age ≥ 60 years old,left ventricular ejection fraction < 0.50,undergoing combined coronary artery bypass grafting were the independent risk factors for long-term death after operations and left ventricular ejection fraction < 0.50,New York Heart Association functional classification Ⅲ-Ⅳ,anterior leaflet prolapse were the independent risk factors for long-term recurrent moderate or severe mitral regurgitation after operations and prosthetic ring or band annulopasty was a protective factor.Conclusion The age ≥60 years old,left ventricular ejection fraction < 0.50,undergoing combined coronary artery bypass grafting,New York Heart Association functional classification Ⅲ - Ⅳ,anterior leaflet prolapse,and prosthetic ring or band annulopasty were closely related with long-term adverse events after operations.
10.Surgical treatment on aortic valve disease combined with non-specific aortitis

Objective To summary the methods and results of first and second operations on patients with aortic valve disease and non-specific aortitis.Methods The total 34 patients including 23 cases with aortitis and 11 cases with Behcet disease were studied from Jan 2000 to Dec 2010.The first operation was Bentall procedure in 18 cases and AVR in 16 cases.Fourteen of 16 cases who had AVR developed severe paravalvular leakage,and undewent the second operation including 10 aortic root replacement (8 valve-conduit and 2 homograft) and 4 non-anatomic AVR.Results Eighteen patients who had first operation of Bentall procedure all survive without aortic pseudoaneurysm after the follow-up of 6 months to 11 years.Fourteen redo cases all survive except for one case died of repture of aortic pseudoaneurysm 1.2 years postoperatively.Conclusion Preoperative diagnosis in these patients is very difficulty.The first operation of root replacement is of choice.The second operation is very difficulty to handle,root replacement can achieve satisfactory results.Non-anatomic AVR is easy to perform,and good hemo stasis intraoperatively,and is a satisfied alternative method with good results.
