1.Reconstruction of slope bone defects of proximal tibia in total knee arthroplasty

Objective Bone defect in proximal tibia was common problem needed to manage in serious genu varum during total knee arthroplasty. In order to introduce measures and experiences to reconstruct slope bone defects in proximal tibia with autograft of tibial plateau in total knee arthroplasty, 29 knees of 22 cases treated in our hospital were reviewed retrospectively. Methods From November 1992 to September 2000, 22 cases with slope bone defects in proximal tibia underwent total knee arthroplasty, there were 5 males with 5 knees and 17 females with 24 knees, the average age of the patients was 61.2 years ranging from 43 to 78 years. The diagnosis of the patients was osteoarthritis in 13 cases (14 knees) which were associated with genu varum in 12 cases (13 knees) and genu valgum in 1 case (1 knee), rheumatoid arthritis in 7 cases (12 knees), synovial chondromatosis in 1 case (1 knee), and Kaschin-Beck disease in 1 case (1 knee). All of the knee prostheses were fixed by antibiotic cement. During operation, the slope bone defect of the proximal tibia was repaired into steping bone defect by osteotomy, and then the defect was restored with the resected tibial plateau autograft. Results Three days after operation, functional excise of the knee was begun, the wound healed in one stage in all the patients. The patients were followed up 1 to 9 years with the average of 4.2 years. Knee pain and deformity of the patients disappeared. The HSS knee score improved from preoperative 10-26 to postoperative 76-94. One patient was revised with ⅠB-Ⅱwedge prosthesis because of late infection, absorption of the autograft bone and loosening of the prosthesis. Nonunion, displacement, bone absorption of the autograft or loosening of the prostheses were not found in the other cases at final follow-up. Conclusion In total knee arthroplasty with slope bone defects in proximal tibia, the autograft of resected tibial plateau may reconstruct its integrity and prevent collapse of the tibia plateau. These procedures will markedly provide the initial stability of the prostheses and improve the success rate of the surgery.
2.Determination of Icariine in Qianlietong Tablets by HPLC

OBJECTIVE:To establish an HPLC method for the determination of Icariine in Qianlietong tablets.METHODS:Hypersil ODS(200 mm?4.6 mm,5 ?m) column was used for the HPLC determination,and the mobile phase consisted of acetonitrile-water(30∶70) at a flow rate of 1.0 mL?min-1.The detection wavelength was 270 nm.RESULTS:Icariine has good linear relationship in the range of 0.050 6~0.506 0 ?g(r=0.999 9).The average recovery was 102.67%(RSD=0.99%,n=6).CONCLUSION:The established method is simple,reproducible,specific and suitable for the quality control of Qianlietong tablets.
3.Toll-like receptor 4 signal transduction pathway and its role in ischemic stroke

Patients with ischemic stroke is often accompanied by inflammtory response. Studies have suggested that toll-like receptor 4 (TLR4) has induced the occurrence,development and of ischemic stroke and secondary brain injury. This article reviews the toil-like receptor 4 signal transduction pathway and its roles in ischemic stroke in order to provide certain basis for the development of TLR4-targeted medication.
5.Comprehensive evaluation and technical experience of total laparoscopic splenectomy and esophagogastric devascularization on the treatment of patients with portal hypertension

Total laparoscopic splenectomy combined with pericardial devascularization has become an important surgical treatment for portal hypertension patients. Compared with the traditional open surgery, laparoscopic surgery has multiple advantages including precise operation, small trauma and quick recovery. Accurate preoperative evaluation, precise surgical operation and delicate perioperative management are still important parts in perioperative management of laparoscopic surgery. Preoperative reasonable and accurate individualized assessment, strict control surgical indications and contraindications are the important basis to ensure the perioperative surgery to be safe and rational; standardized and individualized surgical operation is the key for accurate vascular disconnection, reducing intraoperative bleeding and perioperative complications; the concept of fast-track surgery is conducive to the rapid recovery of patients with portal hypertension and reduce the operation related complications. Anticoagulant therapy is a safe and effective treatment to reduce portal vein thrombosis. It is an important task to strengthen the cooperation among different disciplines for the treatment of portal hypertension in the future.
6.Separation of phospholipid in bear gall by Micellar electrokinetic capillary chromatography

AIM: To develop micellar electrokinetic capilitary chromatography. A method MECC for the separation of major phospholipid from bear gall. METHODS: Following the optimization experiments the MECC was applied to the separation with the 35 m MSDC, 6 mM sodium tetraborate, 10 mM disodium hydrogen phosphate, 30% (v/v) 1-propanol, and pH 8.5 as buffer system. The voltage was 30 kV. the temperature was set at 40 (?C). The capillary length was 570 mm with the effective one of 500 mm. Injection pressure was 50 mbar?4 s and UV detection wavelength was at 200 nm. RESULTS: The separation of phospholipid in bear gall was achieved in the method developed. CONCLUSION: The method is fast, effective and complementary to the HPLC.
7.Inflammatory reaction characteristics of perihematoma brain tissue in patients with acute hypertensive intracerebral hemorrhage

48 hours.The expressions of leukocyte common antigen(LCA) and intercellular adhesion molecule-1(ICAM-1) in the brain tissues collected during the surgery were detected by immunohistochemistry staining method.Results LCA and ICAM-1 immunopositive cells and microvessels were verified in all 29 cases of ICH groups,and the expressions were more than control group(all P48 hours group were higher than that in 6~24 h group(both P48 hours group.Conclusions Acute inflammatory reaction occurs in acute period of ICH.Expression of ICAM-1 reinforces gradually during early 48 hours since the onset of ICH.

10.Research progress of natural-derived compounds in the treatment of non-alcoholic fatty liver disease

Non-alcoholic fatty liver disease (NAFLD) is the most common chronic liver disease. The prevalence of NAFLD is increasing year by year in the world, which seriously threaten the public health. The pathogenesis of NAFLD is complex, and there is no specific treatment for NAFLD. Natural-derived compounds have the characteristics of multi-target and multi-mechanism, which can improve the curative effect and reduce the toxic and side effects by regulating multiple factors of the disease. They are ideal drugs for treating complex diseases and have unique advantages in improving NAFLD. However, low intestinal absorption, poor bioavailability, and single medicine efficiency limit the utilization of many compounds, and further drug development and clinical application are challenging. This paper reviews the research progress of natural-derived compounds in the prevention and treatment of NAFLD in recent years, analyzes the existing problems, and discusses the improvement strategies, so as to provide reference for related research.
