OBJECTIVE:To study the factors affecting the compatible stability of amphotericin B(AmB)and 5%glucose injec-tion from different manufacturers, and to provide reliable evidence for the selection of solvent carrier of AmB. METHODS:After the combination of AmB and 5%glucose injection from different manufacturers(manafactarer A,B,C,D,E),it was shielded from light with room temperature(25℃)for 0,1,2,4,6 and 8 h. HPLC was conducted to determine the content changes of AmB in samples and observe the character, clarity and pH values. RESULTS:With the prolonging of time, insoluble impurities were detected in AmB and 5%glucose injection with the pH value less than 4.2 and the content of main drugs was decreased. Only 5%glucose injection from manufacturer A was in line with the requirements of AmB compatibility. CONCLUSIONS:The compatible stability of AmB and 5%glucose injection is closely related to pH values. It is suggested that manufacturers of infusions make clear descriptions of the pH value to select suitable infusions and safe drugs.

Objective To observe the safety of midazolam and fentanyl in coronary intervention and its effect on haemodynamics. Methods 150 cases undergoing coronary intervention were randomly divided into three groups(n=50 for each):the control group were given injection of 5 ml saline, midazolam group were given 0.04 mg/kg midazolam and combined fentanyl group were given injection of 0.02 mg/kg midazolam with 1.2μg/kg fent-anyl intravenously. Heart rate(HR),mean blood pressure(MAP),SpO<,2>,OAA/S and BIS were observed during the intervention and the patients' satisfaction and the incidence of complications were investigated. Results There was no significant difference among the three groups in MAP and HR (F=3.34,2.98,P>0.05). MAP increased from (95.7±14.5) mm Hg to (85.4±15.3) mm Hg after treatment (t=4.34,P<0.01) and HR increased from (83.3±23.4) times/min to (78.4±22.7) times/min in control group (t=3.37,P<0.01). BIS score was (90.5±7.2),(75.5±12.8) and (72.3±14.1) during intervention and 24 hVAS score was (53.5±25.4),(58.8±18.2) and (71.9±16.8) in control group, midazolam group and combined fentanyl group, with significant difference between groups (F=10.89,8.56,P<0.01). Conclusion Low dose of midazolam and fentanyl can make the patients calm, which relieves the tensity and anxiety and enhance the tolerance and safety of intervention but has no remarkable effect on bemodynamics.

OBJECTIVE:To observe the effects of different routes of administration of tranexamic acid on coagulation function and amount of bleeding in patients with one stage posterior surgery of thoracic tuberculosis. METHODS:40 patients suffered from thoracic tuberculosis in our hospital from Jan. 2011 to Dec. 2013 were randomly divided into intravenous group(5% Glucose injec-tion 100 ml+tranexamic acid 10 mg/kg,through an intravenous drip at 30 min before closing the wound) and topical application group(5% Glucose injection 10 ml and tranexamic acid 10 mg/kg,through soaking the wound before closing the wound)with 20 cases in each group. Other 15 cases suffered from the thoracic tuberculosis in our hospital from Jan. 2009 to Dec. 2010 were includ-ed in control group. 3 groups received one stage posterior surgery of thoracic tuberculosis, interbody fusion and internal fixation. The difference of hemoglobin, coagulation function and the amount of suction drainage were observed before and after surgery, and followed up. Bone graft fusion and therapeutic condition of tuberculosis were observed in the study. RESULTS:There was no statistical significance in postoperative suction drainage between intravenous group and topical application group (P>0.05),but their decrease was more significant than control group, with statistical significance(P<0.05). There was no statistically difference in fiber protease, prothrombin time or activated partial thromboplastin time among 3 groups(P>0.05). The difference value of he-moglobin in control group before and after operation was significantly higher than in intravenous group and topical application group, with statistical significance (P<0.05);there was no statistical significance between intravenous group and topical applica-tion group(P>0.05). 55 patients were all followed up and bone graft of all cases were fused, and all patients were cured and no case recurred. CONCLUSIONS:Tranexamic acid by intravenous application or topical application can reduce hemorrhage and ane-mia after operation of thoracic tuberculosis, and has no effect on blood coagulative system.
