2.Antidiabetic and insulin-sensitizing effects of neoglitazone in different animal models of type 2 diabetes

AIM To assess antidiabetic and insulin sensitizing effect of a novel thiazolidinedione,neoglitazone, in different animal models of type 2 diabetes. METHODS Neoglitazone combined with low-dose insulin(0.2U·d-1 per mouse,sc)were given in streptozotiocin (STZ)-induced diabetic mice for 7 d to inspect its insulin-sensitive improvement. The antidiabetic effect of chronic oral treatment (8 wk) with neoglitazone on spontaneous diabetes mice (DB/DB mice) was examined. The levels of blood glucose were measured by a One-Touch blood glucose meter and pathology of heart, kidney, and pancreas tissues were observed under a light microscope. The hyperinsulinaemic-euglycaemic clamp technique was applied to measure the increase of glucose infusion rate (GIR) of neoglitazone on the immune insulin-resistant rats induced by Bacillus Calmette-Guerin therapy decreased ( 14±s4)%, (55±24)%,and (28±7)% of blood glucose levels compared with CMC-Na+insulin group, respectively (P < 0.01) . In DB/DB mice, neoglitazone showed better reduction in blood glucose levels than those of model animals (P < 0.01) , and pathological studies indicated that neoglitazone attenuated the diabetic kidney and pancreas lesions. During a hyperinsulinaemic-euglycaemic clamp,neoglitaxone(10,30,and 100mg·kg-1·d-1,ig for 2 wk )-treated groups required significantly higher GIR to maintain basal glucose concentrations than model group (P < 0.01 ). CONCLUSION Neoglitazone could directly enhance insulin sensitivity and ameliorate insulin resistance in different diabetic animal models.
3.Current applications of dexamethasone for cancer treatment.

Dexamethasone is a synthetic glucocorticoid that is widely used in clinical due to its multiple pharmacological effects. Recently, dexamethasone is increasingly utilized in anti-cancer therapy. It is frequently used to prevent side effects of chemotherapy such as nausea, vomiting and pain, as well as to increase the anti-tumor activity of the cancer chemotherapeutic agents as a chemosensitizer and to inhibit tumor growth as an anti-cancer agent in some certain cancers. Dexamethasone produces the effects in anti-inflammation, anti-angiogenesis, control of estrogen activity and so on, by binding to glucocorticoid receptor to regulate gene expression of some important bio-signal molecules. Those signal pathways could interfere with the transcription of various factors which can regulate proliferation, invasion and metastasis of tumors.
4.Management of liver injury combined with posthepatic vein injuries

Objective To explore the management methods for liver injury combined with posthepatic vein injuries in order to make an improvement in corresponding treatment. Methods Twenty-three cases with liver injury combined with posthepatic vein injuries were treated with the following 5 ways: repairing the hepatic vascular blockage step-by-step, pressing directly to exhibit the injured vessels for repair, ligating the injured hepatic veins, suturing the hepatic tissues and packing with gauze. Results Of all, 18 cases (78.3%) were cured and 5 (21.7%) died. Conclusions Rational preoperative emergent resuscitation and correct complex intraoperative management are key to the liver injury combined with posthepatic vein injuries.
5.Clinical Observation of Pregabalin Assisting Anti-histamine Drugs in the Treatment of Secondary Skin Pru-ritus in Uremic Patients Underwent Maintenance Hemodialysis

OBJECTIVE:To observe the clinical efficacy and safety of pregabalin assisting anti-histamine drugs in the treat-ment of secondary skin pruritus in uremic patients underwent maintenance hemodialysis. METHODS:140 uremic patients with sec-ondary skin pruritus underwent maintenance hemodialysis were randomly divided into control group and treatment group,with 70 cases in each group. Control group was given loratadine orally,10 mg,qd. Treatment group was additionally given pregabalin on the basis of control group,75 mg,po,at night of hemodialysis end,increasing by 75 mg every week if no obvious relief was found,maximal dose of 300 mg. Both group received 3 months of treatment. Clinical efficacy,VAG itching score,modified Duo score,PSQI score before and after treatment and the incidence of ADR were compared between 2 groups. RESULTS:Total effec-tive rate of treatment group (90.00%) was significantly higher than that of control group (71.43%),with statistical significance (P<0.05);VAG itching score,modified Duo score and PSQI score of treatment group after treatment were significantly lower than control group and before treatment,with statistical significance(P<0.05). There was no significant difference in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Pregabalin assisting anti-histamine drugs in the treatment of secondary skin pruritus in uremic patients with maintenance hemodialysis can efficiently relieve the itching symptoms,improve sleep quality,and not increase the risk of ADR.

7.Impact of patients with esophageal cancer knowing diagnosis on the mental health of their children

Objective To explore the impact of patients with esophageal cancer knowing diagnosis on the mental health of their children and investigate the factors.Methods 107 family carriers whose parent diagnosed esophageal cancer in General Hospital of Jinan Command were interviewed.Respondents were categorized into two groups:one group was those whose parents knew their diagnosis(Group A,n=45)and the other group was those whose parents did not know(Group B,n=62).The Chinese version of Self-reporting Inventory(SCL-90) was used.Results The scores of 107 children of esophageal cancer patients in depression(1.62±0.30),anxiety(1.78±0.34) and hostility (1.93±0.47) were higher than national norms (P<0.01).Compared with group A,group B showed a significant higher degree of anxiety((1.89±0.33) vs (1.62±0.33),P<0.01).In multiple regression analysis,age and the knowledge of esophageal cancer diagnosis had statistically significant difference (P< 0.05).Conclusion Patients' knowledge of the esophageal cancer diagnosis contribute to relieving family carers' symptoms of anxiety.

9.Application of PDCA circulation method to shorten the time of emergency inspection report in return

Objective Application of PDCA method to improve the coincidence rate of emergency inspection report time.Methods 472 cases of emergency inspection report returns the fraction defective of the time from September to December 2014 (blood routine,blood biochemistry,blood coagulation,cerebrospinal fluid,urine analysis).Analyzing the reason and take measures to improve the emergency inspection report time coincidence rate.At the same time,counted 654 cases of emergency inspection report returns the fraction defective of the time after the improvement from July to November 2015,and compared and analyzed the coincidence rate before and after the improvement measures.Results Emergency inspection report of the total fraction defective return time decreased from 13.9 % to 4.9 % by taking measures.Blood coagulation,blood biochemistry,urine analysis,routine blood,cerebrospinal fluid and urine analysis of emergency return time unqualified rate decreased from 18.2 %,16.2 %,10.3 %,6.75 % and 9.1% to 5.2 %,5.0 %,6.4%,5.0 % and 3.9 % respectively.Conclusion The application of PDCA could improve the time coincidence rate of emergency department,and to finish the emergency detection and report detection results in the shortest possible time.
10.Changes of electroencephalographic activities during fully automated simultaneous peripheral arteriovenous exchange transfusion in neonates with hyperbilirubinemia

Objective To analyze the changes of electroencephalographic activities during fully automated simultaneous peripheral arteriovenous exchange transfusion(ET) in neonates with hyperbilirubinemia.Methods A total of 45 neonates who suffered from severe hyperbilirubinemia and underwent fully automated simultaneous peripheral arteriovenous exchange transfusion were studied from March 2009 to March 2016 in Neonatal Intensive Care Unit of Guangzhou Women and Children's Medical Center,and 46 ETs were performed in 45 babies who were divided into 2 groups according to the severity of hyperbilirubinemia:the encephalopathy group and the none-encephalopathy group.Nineteen patients were in the encephalopathy group,in which 11 were male and 8 were female.The other 26 patients were in the none-encephalopathy group,in which 15 were male and 11 were female.Changes in amplitude integrated electroencephalogram(aEEG) during ETs were analyzed,including background activities,sleep-wake cycle (SWC)and seizures.Results Forty-five patients with hyperbilirubinemia underwent 46 fully automated simultaneous peripheral arteriovenous ETs.As a result,total bilirubin dropped from (524.90 ± 110.96)μmol/L before ETs to (245.62 ±78.97) μmol/L after ETs,with clearance rate of 53.2%.And indirect bilirubin dropped from(486.16 ±90.39) μmol/L before ETs to(222.19 ± 79.49) μmoL/L after ETs,with clearance rate of 54.3%.On the other hand,there was no significant difference in the changes of electroencephalographic activities during ETs,including background activities (x2 =0.16,P > 0.05),SWC (x2 =0.71,P > 0.05) and seizures (x2 =0.30,P > 0.05).However,there were significant difference in suppressions on background activities between the encephalopathy group and the none-encephalopathy group(Fisher's exact test P =0.042),though there were no significant statistical differences in SWC or seizures between the 2 groups (x2 =0.65,P > 0.05;x2 =2.07,P > 0.05,respectively).Conclusions In neonatal hyperbilirubinemia,fully automated simultaneous peripheral arteriovenous ET is safe and efficient without significant influence on electroencephalographic activities as a whole.However,background activities are more significantly depressed in infants of bilirubin encephalopathy than that of non-encephalopathy during ET.
