1.Progress in diagnosis and treatment of extra-articular manifestations of juvenile idiopathic arthritis

Characterized by chronic arthritis, juvenile idiopathic arthritis (JIA) refers to a group of rheumatic diseases in children with unknown causes and strong clinical heterogeneity.Most patients have good prognosis, while some may be involved with important extra-articular organs and tissues, including cardiac system damage, pulmonary or pleural lesions, renal amyloid degeneration, uveitis, and so on.Delay in diagnosis and treatment may lead to short-term and/or long-term involvement in vital systems, thus resulting in the aggravation of the disease or poor prognosis, which may decrease their life quality and even threaten their life.Understanding the extra-articular manifestations of JIA is very important to manage disease.
2.Fungous Sinusitis and CT of Diagnosis

Objective To analyze the clinical symptoms and CT findings of fungus sinusitis in order to evaluate CT diagnosis. Methods To analyze retrospectively the collected data of 12 patients who underwent operations for fungos sinusitis which was proved pathologically. Results The main clinical feature of fungos sinusitis was bloody supperative rhinorrhea in all cases. In the CT scans, we found the density of the involved sinus cavities rised inhomogeneities in all patients. CT scans also showed spot-like calcification in 7 cases, bony proliferation in 5 cases and bony absorption in 2 cases. The operation revealed a lot of filthy or inflammatory polypoid substances in the involved sinus cavities in all patients. Pathological examination demonstrated clusters of fungus in the filthy substances. Conclusion The diagnosis of fungus sinusitis mainly derived from clinical symptoms and radiological examinations. The final diagnosis depended on pathology. It is of great value for reference that CT scans can show the anatomical structures of the sinuses and help to diagnose fungus sinusitis.
3.Effect of aspirin resistance on cardiovascular events in non-insulin-dependent diabetes mellitus patients

Objective To explore the effect of aspirin resistance on cardiovascular events in non-insulin-dependent diabetes melli-tus patients .Methods 496 patients with non-insulin-dependent diabetes mellitus received daily aspirin therapy (100mg/d) 4 weeks were recruited from Jun .2006 to Sept .2010 .Platelet aggregation was measured by light transmission aggregometry .The patients were divided into three groups :aspirin resistance group ,aspirin semi-resistance group and aspirin-sensitive group .The clinical bio-chemistry indications and the adverse cardiovascular events were collected .Results 11 .69% of patients were found to be resistant to aspirin therapy ,and 37 .90% of patients to be aspirin semi-responders .There were 67 .48% female in aspirin resistance group and aspirin semi-responders .The preprandial glucose of group AR and ASR was higher than that of group AS (P<0 .05) .The HbA1C and TG of group AR were higher than those of group AS(P<0 .05) .There was statistically significant difference of the rate of car-diovascular events .Conclusion The measure of platelet aggregation should be taken in the diabetic mellitus patients and pay atten-tion on the dose of aspirin recommended .Particular attention is should also devote to the incidence of adverse cardiovascular events .
4.Epstein-Barr virus and inflammatory bowel disease

Epstein-Barr Virus (EBV) is the first human virus related to oncogenesis.EBV infection is associated with inflammatory bowel disease (IBD) with unknown causality.The prevalence of EBV in intestinal tissue from patients with IBD is significantly higher and related to the exacerbation of the disease and refractory IBD.Immunosuppressive therapy has improved outcomes associated with IBD.However,it is also associated with an increased risk of opportunistic infection,and lymphoproliferative disorders (LDs) maybe due to EBV infection.Here we review our current understanding of the pathogenesis of EBV infection in colonic mucosal inflammation,EBV-induced disease exacerbation,lymphomagenesis in IBD,and clinical approaches therefrom.
5.Effects of Different Types of Ankle Brace on the Static Postural Stability in Patients with Functional Ankle Instability

Objective To study the effects of different types of ankle brace on human body static postural stability.Method Ten male subjects with unilateral functional ankle instability (FAI)were enrolled in the study.The FlexiForce pressure control system was used to control the pressure on the ankles when wearing the soft brace.The static postural stability without braces as well as with soft and semi-rigid braces was measured using WIN-POD system.Results (1)In eye-open testing,no significant difference was found in indexes of COP sway between wearing or not wearing braces (P>0.05).Compared with wearing semi-rigid brace,significant increase was observed in the average X dev when wearing soft braces (P<0.05).(2)In eye-close testing,no significant differences were found in COP sway either wearing braces or not,as well as wearing different braces.Conclusion Wearing soft and semi-rigid brace had no effects on the static postural stability of FAI patients.But the static balance ability of wearing semi-rigid braces is better than that of wearing soft braces.
6.Breast cancer in young women:a report of 59 cases

Objective To study the clinical diagnosis,treatment and prognosis of and the factors influencing prognosis of breast cancer(BC) in young women. Methods A restrospective analysis of the clinical data of 59 cases treated from Jan .1990 to Dec. 1998was carried out in the Second Xiangya Hospital. Results All patients underwent operation, including 31 radical mastectomy , 25 modified radical mastectomy and 3 simple mastectomy. 34 cases had axillary lymph node metastasis. The overall 3,5 and 7 year survival rates were 67.2% , 61.0% and 57.1%respectively. The 3,5 year survival rates in patients with axillary-lymph node metastasis were lower than those in patients without axillary lymph node metastasis (61.8%,45.5%vs 86.4% ,83.3%). 7.Leukotrienes and leukotriene receptor antagonists in food allergy gastrointestinal disorder

Food allergy( FA) is a growing health problem which affects young children′s growth. Re-cent studies have showed that leukotrienes can contract gastrointestinal smooth muscle,increase intestinal perm-bility,and it is obviously related to the development of FA gastrointestinal disorder. Leukotriene receptor antago-nists have provided a good therapeutic option and showed clinical benefits in the management of the FA gastroin-testinal disorder.
8.Advances on neurally adjusted ventilatory assist in newborn infant

Neurally ajd usted ventilatory assist( NAVA) uses the electrical activity of the diaphragm as a neural trigger to synchronize mechanical ventilatory breaths with the patient′s neural respiratory drive. Some studies have shown that compared with pressure-support ventilation,NAVA can improve patient-venti-lator synchrony while maintaining spontaneous breathing,unload respiratory musculaturei n term adn prte erm i nfants.Further studies are needed to determ ine whether NA VA will have significant impact on the overall outcomes of neonates.
9.Differential profeil proteomic of donor liver subject to ischemia and reperfusion injury during liver transplantation

ObjectiveTo identify the proteins presented variation during ischemia/reperfusion injury (I/RI) in liver transplantation (LT).MethodsAt Mar.2009,a total of nine liver sample tissues,three samples obtained from each donor liver at three different time points,were studied.The time points were as follows:(1) the time point immediately after donor liver was harvested (T1) ; (2) the time point just before the anastomosis of hepatic artery and potal vein (T2),during this time period the donor liver was kept in ice cold and transported,then back-table prepared,during time from T1 to T2,ischemia injury occurred; (3) the time after donor reconnected to the recipient (T3),during which the donor liver was subjected to reperfusion injury.The three time points represent normal control,ischemia injury,and reperfusion injury respectively.By using method of 2DE-MALDI-TOF and mass-spectrum,the differential expression of these protein at T1,T2,and T3 time point were compared.ResultsFrom the 1580 proteins that were isolated,19 ischemia/reperfusion injury (I/RI)-related proteins that varied markedly from time point T1 to T2 and from time point T2 to T3 were identified.They were metabolic enzyme,molecular chaperone,redoxase,cytoskeleton protein,signal transduction protein,and binding protein,respectively.The character of each protein was further analized based on their functions.ConclusionsFor the first time,we detected the protein related to I/RI during LT.We obtained independent proteomics information of ischemia injury,and reperfusion injury,respectively.Further functional analyses of these proteins may improve the understanding of the critical biological processes during I/RI.Medicine intervention on the I/RI-related proteins may result in improved survival of the recipients.
10.Clinical Study of Internal Radiotherapy with Hepatic Intra-ar te rial Iodine-131 Labeled Material for Hepatocellular Carcinoma

Objective To discuss the clinical val ue of internal radiation therapy with hepatic intra-arterial iodine-131 labele d material for the treatment of hepatocellular carcinoma (HCC). Methods This summarized paper was made on literatu re review. Results Iodine-131-lipiodol and several reported iodine-131-labeled antibodies to HCC associated antigens were concentrated in the foci of HCC with a high tumor-to-normal-tissue absorbed dose ratios. No s evere side effects occurred. It was used in various kinds of HCC patients, and m ostly showed a significant tumor response. Survival rate of HCC patients was rai sed in several clinical trials.Conclusion Internal radiotherapy with hepatic intr a-arterial iodine-131 labeled material may be considered as an effective metho d to treat HCC.
