1.Early diagnosis value in of serum lysophosphatidic acid and CA-125 detection ovarian cancer

Objective To explore the clinical value of early diagnosis of ovarian cancer by serum lysophosphatidic acid(LPA) and CA-125.Methods 50 patients with ovarian cancer from October 2005 to February 2008 were selected as ovarian cancer group,at the same period selected 44 patients with ovarian benign tumor(ovarian benign disease group),and 50 healthy women as the healthy control group.All patients were diagnosed and confirmed by preoperative blood and pathology.The serum LPA and CA-125 of two groups were detected.Results The serum LPA level and the positive rate in the ovarian cancer group was higher than that of the ovaries benign group or the control group(P<0.05).The CA-125 level in the ovarian cancer group was similar to that of the ovaries benign group(P>0.05),while the CA-125 level in the ovarian cancer group or the ovaries benign group was higher than that of the control group(P<0.05).The specificity of the LPA was better than that of the CA-125 detection.In the early diagnosis of ovarian cancer,the sensitivity of the combination(85.7%) was better than either of them(P<0.01);the plasma LPA level and positive rate of CA-125 of the phase Ⅱ~Ⅳ ovarian cancer patients were higher than that of phase Ⅰ (P<0.01);the CA-125 positive rate of the serious cystadenocarcinoma was higher than that of the cystadenocarcinoma(P<0.01).Conclusion LPA is a sensitive biomarker for the early diagnosis of ovarian cancer,especially combined with CA-125.It should be widely used in clinic.
2.The clinical significance of the serum estradiol-2,tumor necrosis factor-alpha and vascular endothelial growth factor in patients with endometriosis

Objective To detect the level of the serum estradiol-2 (E_2), tumor necrosis factor-alpha (TNF- α) and vascular endothelial growth factor (VEGF) in patients with endometriosis (EMS) and explore their clinical significance. Methods Fifty-nine EMS patients from January 2006 to January 2009 were selected as EMS group and 60 normal women were selected as control group. The serum E_2,TNF-αand VEGF in EMS group 24 h pre-operation, 7 d post-operation and 6 months after operation were detected, and compared with control group. Results The levels of the serum E_2[(216.5 ± 59.7) ng/L],TNF- α [(30.4 ± 17.5) μg/L]and VEGF [(250.7 ± 88.7) ng/L]in EMS group pre-operation were significantly higher than those in control group [(100.2 ± 33.2) ng/L, (11.2 ± 3.6) μg/L, (103.2 ± 49.2) ng/L]and post-operation [(121.3±44.6) ng/L, (13.4 ± 6.2) μg/L, (153.9 ± 58.7) ng/L](P < 0.01). But there was no significant difference between control group and post-operation of EMS group (P > 0.05). The levels of the serum E_2 [(316.5 ± 77.6) ng/L],TNF-α [(51.1 ± 12.3) μg/L]and VEGF [(305.1±69.7) ng/L]with stage Ⅲ-Ⅳ in EMS group were higher than those in control group or those with stage Ⅰ - Ⅱ [(170.7±48.2) ng/L, (25.8 ± 10.1) μ g/L, (169.2 ± 36.1) ng/L](P < 0.05 or < 0.01). The levels of the serum E_2,TNF- α and VEGF with stage Ⅰ - Ⅱ in EMS group were also higher than those in control group (P < 0.05). Nine patients recurred at 6 months after the operation. The levels of the serum E_2[(187.8 ± 46.7) ng/L],TNF- α [(23.9 ± 9.5) μg/L]and VEGF [(185.3 ± 57.4) ng/L]of the recurred EMS patients stepped up significantly higher than those of the non-recurred EMS patients [(112.7±30.3) ng/L, (13.2±4.7) μg/L, (116.4±30.3) ng/L](P < 0.01). While there was no significant difference between control group and non-recurred EMS patients (P >0.05). Conclusions The serum E_2,TNF-αand VEGF may play important roles in the development of the EMS. And the detection of the serum E_2,TNF-αand VEGF is useful to judge the patient's condition and the prognosis of the EMS.
3.Analysis on professor WANG Juyi's crevice theory from Tao Teh King.

The crevice theory proposed by professor WANG Juyi is analyzed in this article. In the crevice theory, it is believed that the meridians are located among "skin, pulse, flesh, tendon and bone", and the crevice is the path for the transportation of qi-blood and body fluid in the body, which is the essential condition for vital movement. This is in agreement with the idea of "action through inaction" from Tao Teh King. Based on crevice theory, the abnormalities of meridians can reflex the deficiency and excess of diseases, and regulating meridians is a significant way to treat diseases, and maintaining smooth meridian is important for healthcare.

4.Analysis on tests of ESR,CRP and serum amyloid changes before and after total hip arthroplasty

Objective To analyze the changes of erythrocyte sedimentation rate (ESR) ,C‐reactive protein (CRP) and serum am‐yloid A (SAA) before and after total hip arthroplasty(THA) .Methods A total of 124 cases of unilateral THA in our hospital from April 2012 to April 2015 were selected as the research subjects .The CRP ,ESR and SAA levels were detected before operation and on postoperative 1 ,3 ,5 ,7 ,14 d ,and 1 ,3 months .The differences of above indexes between the patients with postoperative deep venous thrombosis (DVT ) obstruction and the patients with normal recovery were retrospectively analyzed .Results CRP was sharply increased after operation and reached the peak value on 3 d ,then rapidly faded away ,and recovered to the preoperative level at postoperative 1 month;ESR was continuously elevated until reaching the peak value ,then slowly declined ,and recovered to the normal level at postoperative 3 months;SAA was sharply elevated after operation ,reached the peak value on 3 d ,then slowly de‐clined and could recovered to the normal level at postoperative 3 months .The CRP ,ESR and SAA levels in the patients with post‐operative complications occurrence were significantly higher than those in the patients with postoperative normal recover at the same time ,moreover which could not recover to the preoperative levels .Conclusion Dynamically monitoring CRP ,ERS and SAA after THA could conduce to early discover postoperative lower limbs DVT blockage complication .

6.Expression and Phosphorylation of PKR and NF-κB p65 in Cervical Lesions

Objective To identify the significance of expression and phosphorylation of protein kinase R(PKR) and nuclear factor NF-κB p65 in cervical lesions, and the effect of high-risk human papilloma virus(hsHPV) on expression and phosphorylation of R(PKR) and NF-κB p65. Methods A total of 67 patients with cervical cancer, 149 patients with cervi-cal intraepithelial neoplasia (CINⅠ-Ⅲ) and 15 normal control were included in this study. The expression levels of PKR, phosphorylated PKR (p-PKR), NF-κB p65 and phosphorylated NF-κB p65 (p-NF-κB p65) were detected by immunohisto-chemical SP method in three groups. Results The positive expression rates of PKR and p-PKR in cytoplasm were signifi-cantly lower in hsHPV positive group than those in hsHPV negative group (27.2% and 11.0% vs 41.1% and 21.1%,χ2 =4.858 and 4.371,P<0.05). The positive expression rates of NF-κB p65 and p-NF-κB p65 in cytoplasm and nucleus were significantly higher in hsHPV positive group than those in hsHPV negative group (46.3%, 25.7%, 22.8% and 12.5% vs 32.6%, 14.7%, 11.6%and 4.2%,χ2=4.345,4.048,4.729 and 4.650 respectively,P<0.05). The positive expression rates of PKR in kytoplasm and karyon were significantly lower in NF-κB p65 (+) group than those in NF-κB p65 (-) group (25.5%vs 38.0%and 20.4%vs 36.3%,χ2=3.898 and 4.396 respectively, P<0.05). The positive expression rate of PKR in kyto-plasm was significantly lower in p-NF-κB p65 (+) group than those in p-NF-κB p65 (-) group (19.0%vs 36.0%,χ2=4.462, P<0.05). Conclusion hsHPV may inhibit the expression and phosphorylation of PKR but promote the expression and phosphorylation of NF-κB p65. The expression and phosphorylation of NF-κB p65 may inhibit the expression of PKR. Regu-lating effects of three may be associated with the generation and progression of cervical cancer.
7.Effect of p53 on Protein Kinase R and Biological Behavior of Cervical Cancer HeLa Cells

Objective To investigate the effects of p53 on expression and activity of protein kinase R (PKR) as well as biological characters of HeLa cells from cervical carcinoma patients. Methods Recombinant plasmid vector pEGFP-C1/p53 was constructed to over-express p53 then it was transfected into HeLa cells. Transcription levels of p53 and PKR mRNA were detected by reverse transcriptase polymerase chain reaction (RT-PCR) among pEGFP-C1/p53 transfection group, pEGFP-C1 transfection group and blank control group(only transfection reagent was added);Protein expression lev?els of p53, PKR, phosphated PKR(p-PKR)and phosphatedαsubunit of eukaryotic initiation factor 2(p-eIF2α)which is the downstream substrate of PKR were detected by Western Blot among three groups;Proliferation of HeLa cell were deter?mined by methyl thiazolyl tetrazolium(MTT)assay;Invasion of HeLa cell were determined by Transwell cell assay. Re?sults Recombinant plasmid vector pEGFP-C1/p53 was successfully constructed to overexpress p53;Transcription level of p53 and PKR mRNA in pEGFP-C1/p53 transfection group were higher than those in pEGFP-C1 transfection group and in blank control group (P<0.05),and there were no significant difference between their levels in pEGFP-C1 transfection group and in blank control group;Protein expression levels of p53, PKR, p-PKR andp-eIF2α in pEGFP-C1/p53 transfection group were higher than those in pEGFP-C1 transfection group and in blank control group (P<0.05),and there were no sig?nificant difference between those expression levels in pEGFP-C1 transfection group and in blank control group;MTT and Transwell cell results showed that proliferation and invasion of HeLa cells in pEGFP-C1/p53 transfection group were weaker than those in pEGFP-C1 transfection group and in blank control group (P<0.05),and there were no significant difference between proliferation and invasion of HeLa cells in pEGFP-C1 transfection group and in blank control group. Conclu?sion p53 can up-regulate the expression and activity of PKR, promote activation of PKR/eIF2αsignal transduction pas?sage and restrain cell proliferation and invasion of HeLa cells.
8.Relationship between gallstones and acute pancreatitis

Objective To investigate the predictive factors of acute pancreatitis for patients with gallstone.Methods One hundred and eighteen patients with gallstones who were admitted to Department of Gastroenterology,People's Hospital of Wuhan University were selected,the gallbladder size,number and size of gallstones,common bile duct stones were determined by imaging study.Then the patients were grouped according to the complication of acute pancreatitis.Results Sixty-one cases of acute pancreatitis occurred out of 118 patients.The gallbladder size of 74 patients was normal,and among them 49(66.2%) developed acute pancreatitis; the gallbladder size of 44 patients was increased or decreased,and among them 12(27.3%)developed AP.Thirty-one patients had a single gallstone,and 11 (35.5%) developed AP; while 87 patients had multiple gallstones,and 50 (57.5%) was complicated with AP.Among patients with AP,11 patients had a single gallstone,and the size of the gallstone in the 8 patients (72.7%) was ≥10 mm; while 50 patients had multiple gallstones,and the size of the gallstone in the 41 patients (82.0%) was < 10 mm.Nineteen patients were complicated with common bile duct stones,and 17 (89.5%) developed AP; while 99 patients were not complicated with common bile duct stones,and 44 (44.4%) developed AP.The difference between the two groups was statistically significant (x2 =16.758,P =0.000 ; x2 =4.425,P =0.029 ; x2 =13.434,P =0.001 ; x2 =12.994,P =0.000).Conclusions Acute biliary pancreatitis is associated with gallstones.The size of gallbladder,size and number of gallstone,as well as the presence of common bile duct stones are related to the development of AP.
9.Promoting General Ward Round Actively and Improving Efficiency of Logistics Services

10.Hallux valgus treated by Reverdin operation

Objective To describe the Reverdin technique for treatment of hallux valgus (HV) based on correction of proximal articular set angle (PASA). Methods Thirty-one Reverdin osteotomies (in 31 feet of 17 female patients)were performed between 1992 and 1998. Reverdin operation is unique among the surgical procedures for its ability to correct large PASA with a medial wedge-type osteotomy of the metatarsal head. Results On the follow-up of 1 to 8 years, patients rated outcomes as excellent or good in 96% (30 feet). There was no complication of delayed union or avascular necrosis of the metatarsal head. Radiographical follow-up: PASA averaged 19 degrees(range 30-12 degrees), had a postoperative averaged value of 1.95 degrees, HV angles averaged 30 degrees(range 41-23 degrees), had a postoperative value of 14.3 degrees. Intermetatarsal angles averaged 10.9 degrees (range 16-4 degrees), had a postoperative value of 5.8 degrees. Conclusion The preoperative PASA value is the most important factor in hallux valgus evaluation, as well as the selection of operative techniques. The normalization of PASA safegurds more successes and less recurrences.
