1.To evaluate dynamic three-dimensional echocardiography using sequential segmental approach with STIC in diagnosis of feta ECD

Objective To evaluate the application of STIC with segmented analytical method in the diagnosis of fetal atrial septal defect. Method Review the 6600 high-risk pregnant women who accepted PHILIPS iE33 Echocardio graphy examination in our hospital , twelve cases by two-dimensional echocardiographicas atrial septal defect then using real-time STIC method to collect data and offline playback compared with sonography after birth. Results Compared with two-dimensional gray-scale ultrasonography , in the diagnosis of fetal atrial septal defect the coincidence rate of STIC with segmented analytical method was 96.5% and 96.7% respectively , no statistically difference between them (P > 0.03). However, the operating time in STIC with segmented analytical method is shorter, and with the statistically significant (P < 0.03). Conclusion Compared with gray-scale ultrasound , STIC technique combined with segmented analytical method can obtain more valuable information and improve the diagnosis coincidence rate , which is a valuable new technology.

3.Effect of oxycodone on hemodynamics and stress response in general anesthesia induction

Objective To investigate the effect of oxycodone and sufentanil with equivalent dose on hemo-dynamics and stress response in general anesthesia induction. Methods One hundred and twenty ASA I orⅡpa-tients with abdominal surgery were randomly divided into oxycodone group(group O,n=60)and sufentanil group (group S,n = 60). Anesthesia was induced with iv oxycodone 0.20 mg/kg(group O)or sufentanil 0.25 μg/kg (group S)respectively,together with iv propofol 2.0 ~ 2.5 mg/kg and cisatracurium 0.2 mg/kg. The patients were tracheally intubated using a single-lumen endotracheal tube. Mean arterial pressure(MAP),heart rate(HR),plas-ma levels of epinephrine(E)and norepinephrine(NE),cortisol(Cor)and blood sugar(Glu)and the occurrence of bucking before anesthesia induction(T0),immediately before intubation(T1),at the moment of intubation(T2), 1 min(T3)and 5 min(T4)after intubation were observed. Results Compared with these at T0,MAP and HR in 2 groups were lower at T1(P<0.05)and no significant difference was found in 2 groups(P>0.05). MAP and HR were significantly higher at T2 and T3(P < 0.05)and compared with those in group O,MAP and HR in group S were increased more significantly at T2 and T3(P<0.05). MAP and HR increased slightly at T4 in 2 groups but no significant difference was found in 2 groups(P>0.05). Plasma levels of E ,NE,Cor and Glu increased obviously at T2 and T3 in 2 groups(P < 0.05)and they were even higher in group S(P < 0.05). Plasma levels of E ,NE , Cor and Glu increased slightly at T4 in 2 groups but no significant difference was found in 2 groups(P > 0.05). Conclusion Compared with sufentanil ,oxycodone of equivalent dose used for general anesthesia induction can effectively keep smooth on hemodynamics and decrease stress response.
4.Non-surgical treatment of locoregionally advanced squamous cell carcinoma of the head and neck region

Deifnitive treatment for locoregionally advanced squamous cell carcinoma of the head and neck region (HNSCC) is challenging, and usually require multidisciplinary efforts involving surgery, radiotherapy, and chemotherapy. Although surgery followed by radiation or chemoradiation therapy remains the standard treatment for resectable disease, combined chemoradiation therapy provides an effective option with organ spearing potential. In addition, combined chemoradiation therapy is the only treatment option for non-metastatic advanced HNSCC. Recently published results from TAX323 and TAX324, two important randomized clinical trials on the efficacy of induction chemotherapy using docetaxel based regimen, showed that induction chemotherapy using TPF can signiifcantly improve patients’ survival as compared to the conventional PF regimen. However, whether TP or TPF induction chemotherapy should be combined with concurrent chemoradiation and considered as part of the standard treatment regimen remains controversial, and requires support from the results of well-designed randomized clinical trial.
5.Preparation of chitosan-5-fluorouracil nanometer particle and its drug-release characters

Objective To prepare the nanometer particles (NPs) of chitosan-5-fluorouracil (5-FU), and to study its characters of drug release in vivo. Methods Cross-linking polymer technique was employed to prepare the chitosan-5-FU NPs, and their characters were detected by scanning electron microscope and photon correlation spectroscope (PCS); the loaded capacity was measured by ultraviolet spectrophotometry; high-performance liquid chromatography was used to detect the characters of drug release in vivo. Results The chitosan-5-FU-NPs were found to be round or elliptic in shape, with 120 to 150nm in diameter and having a good dispersive ability. The loaded capacity was 31.000%?0.001%. The blood concentration of 5-FU was controlled at 76?g/L, and it was maintained for 48h. Conclusion As a vehicle, chitosan-5-FU NPs can change the behavior of pharmacokinetics and prolong the cycle time of 5-FU in vivo. Chitosan-5-FU NPs ran reduce the initial burst and prolong the releasing time.
6.Practice of proton and heavy ion radiotherapies for head and neck malignancies

Radiotherapy is an important therapeutic method in the management of head and neck cancer.However,since the head and neck region has many important functions of the human body,radiotherapy may cause serious sequelae.Proton and heavy ion radiotherapies have better radiophysical and biological properties compared with conventional proton beam and are beneficial to tumor control and protection of normal tissue.The advantages of proton and heavy ion radiotherapies have been demonstrated in soft tissue tumors (especially skull base chordoma and chondrosarcoma),malignant melanoma,and adenoid cystic carcinoma resistant to conventional protons in the head and neck region,and the safety of proton and heavy ion radiotherapies have been confirmed in reirradiation for recurrent tumors.
7.Influence of anxiety or depression on prethrombotic state in patients with essential hypertension

Objective: To observe influence of anxiety or depression on prethrombotic state in patients with essential hypertension (EH). Methods: A total of 112 EH patients were equally divided into EH + A/D group and pure EH group. Levels of prethrombotic state indicators, including serum CD62P, von Willebrand factor (vWF), tissue type plasminogen activator (t-PA), plasminogen activator inhibitor (PAI)-1 and endothelin (ET)-1 were measured in two groups. Results: Compared with pure EH group, there were significant increase in levels of CD62P [(4.52±2.01) % vs. (8.38±1.96) %], vWF [(155.28±23.11) % vs. (185.23±22.21) %], PAI-1 [(54.35±13.21) ng/L vs. (79.88±14.53) ng/L] and ET-1 [(121.56±33.32) ng/L vs. (152.78±30.23) ng/L], P<0.05 all; and significant decrease in t-PA level [(19.37±11.46) ng/L vs. (9.56±8.32) ng/L, P<0.05] in EH + A/D group. Conclusion: Anxiety or depression can aggravate prethrombotic state of patients with essential hypertension through activation of platelets, influencing vascular endothelial function and fibrinolytic system imbalance.

Objective To study lentinan’s promotion on XELOX regimen’s curative effects on advanced gastric cancer. Method Cases with advanced gastric cancer were divided into observation group and control group according their therapy method. The curative effects, side effects, WBC, lymphocyte subsets, NK cells and quality of life were compared. Results The difference of disease control rates in two groups were not significant (P=0.091). The incidences of side effects in observation group were significantlly lower than in control group (P<0.05). The observation group’s WBC, Lym, CD 3+, CD 4+, CD 8+, NK cells and quality of life were significantly higher than in control group(P<0.05). Conclusion Lentinan could significantly promote gastric cancer patients’immunity and lessen side effects. It can promote XELOX regimen’s curative effects on gastric cancer and improve quality of life.
10.Drug Resistance of Pathogens in Pediatric Intensive Care Unit

OBJECTIVE To investigate the drug resistance of pathogens in pediatric ICU and discuss how to treat infections caused by these resistant strains.METHODS After genus identification,bacterial susceptibility test was carried out using Kirby-Bauer disk method.RESULTS A total of 270 clinical isolates were analyzed including 194(72%) strains of Gram-negative bacilli and 76(28%) strains of Gram-positive cocci.From them 182(67%) strains were isolated from sputum,78(29%) strains from blood and 10(4%) strains from secretion.The most common bacilli were Klebsiella spp,coagulase negative Staphylococcus,Escherichia coli,Enterococcus spp and Pseudomonas aeruginosa.About 66.7% of E.coli and 91.1% of Klebsiella spp isolates produced ESBLs,the isolating rate of MRCNS was 83.9%.CONCLUSIONS The characteristics of pathogen distribution and drug resistance surveillance must be understood in order to use antibiotics rationally,to control the increasing trend of drug resistance pathogens and to forbid the break out of nosocomial infections.
