1.Splenectomy for recurrent hypersplenism after splenic arterial embolization

ObjectiveTo study the Resultsof splenectomy for recurrent hypersplenism after splenic arterial embolization. MethodsA retrospective analysis was carried on 12 p1atients who underwent splenectomy for recurrent hypersplenism after splenic arterial embolization which had been carried out in 51 patients at this hospital from Jan. 2001 to Dec. 2008. ResultsSplenectomy was successfully carried out in all the 12 patients. The operative time ranged from 90 to 240 minutes. The mean blood transfusion was 810 ml. There was 1 case of pancreatic leakage after the operation, which healed with drainage for 4 weeks. One patient developed upper gastrointestinal hemorrhage 14 months after the operation, and he received a meso-caval shunt. Conclusion We should select patients carefully for splenic arterial embolization as splenectomy after splenic arterial embolization is difficult.
2.Determination of the Contents of Puerarin in Infant Fuxieling Effervescent Granules With HPLC

OBJECTIVE: To establish a method for determination of the contents of puerarin in infant Fuxieling effervescent granules with HPLC.METHODS:The puerarin was extracted by ultrasonics, using methanol as solvate.The stratographic column was Shim - Pack C1s with acetonitrile - water( 10: 90) as mobile phase. The flow rate was 2.0ml/min and the detecting wavelenghth was 250nm.The temperature of column was 25℃ .RESULTS:The resolution between the puerarin and the adjacent peaks was >1.5 .The number of theoretical plates was 18 550 .The standard curve was linear in the range of 0.964μg~7. 712μg .The average recovery was 100.2% (RSD = 1.76%, n = 6).CONCLUSION:The method is simple, rapid and accurate. It can be used for quality control of the preparation.
3.The research of the effective needs of continuing medical education

Objective To investigate and analyze the data of continuing medical education in order to understand the real need of the learners.Methods Reviewing and making statistic analysis of the data of Renji hospital's continuing medical education courses from 2007 to 2012,including the amount,title,discipline,geographical distribution,training content and the questionnaire survey.Results It shows that the learners from coastal and developed areas have a higher demand for cuttingedge technology and the new progress in medicine(49.1%),while the learners from inland and western regions prefer to learn the basic medical theory and technology (41.9%).Senior doctors tend to study cutting-edge technology and new progress medical courses (51.7%) while the junior persons prefer the basic medical knowledge (46.6%).These mean that the needs of content of continuing medical education is associated with the regions and the level of the doctors (P<0.05).Conclusion The conclusions prompt that medical development between different regions of China may be unbalanced.Chinese government should pay more attention in terms of allocation of health resources and personnel training.It also provides advice for future courses so that it may improve the efficiency of the medical education resources.

Microsoft Access 2003 is an excellent and easy-to-use development tool of applied programs. The use of it not only can lead to the development of practical application systems but is also advantageous to real-time maintenance and redevelopment. Very good results in actual work have been obtained through the use of an integrated personnel information administrative system developed by the authors with Microsoft Access 2003.

6.Action mechanism and time of administration of indomethacin in SMAO shock rabbits

To study action mechanism and time of administration of indomethacin in SMAO shock rabbits. Method:Thirty-two SMAO shock rabbits were randomly divided into four groups: control group A,indomethacin 5mg/kg I.V. at the occlusion of superior mesenteric artery (group B), indomethacin 5mg/kg L V. immediately after clamp being removed(group C) and the same dose 30min after clamp being removed(group D). Survival rate,hemodynamics, plasma calcium, myocardial calcium, TXB_2 and 6-keto-PGF_1. levels were observed in 4h shock period. Myocardial apecmen were examined with electronic microscope. Result: After indomethacin I.V,the survival rate was higher and bemodynamie better than those of controll. The plasma calcium level remained stable in indomethacin groups. Myocardial calcium content of group B and C was lower than that of group D and A. The TXB_2 and 6-ketoPGF_1. contents were similar in all groups. Under electronic microscope, the damages were slightest in group C and most serious in group D and A among the four groups. Conclusion:Indomethacin has a beneficial effect on the SMAO shock rabbits ,the mechanism of which can be related to improving myocardium contractivity as well as to blocking calcium channel,rather than the action through prostaglandins. The time of administration should be early as possible after shock.
7.Efficacy of transanal local resection for early low rectal cancer

Objective To evaluate the efficacy of transanal local resection for early low rectal cancer. Methods The clinical data of 72 patients with early low rectal cancer who had been admitted to Union Hospital of Fujian Medical University from January 1997 to April 2009 were retrospectively analyzed. Patients were divided into 2 groups, and they received transanal local resection (group A, n = 31) and radical resection (group B, n =41), respectively. Postoperative local recurrence and distal metastasis were analyzed by Fisher exact test, the survival was analyzed by life table method. The difference in survival between the 2 groups was analyzed by Wilcoxon test. Results Three patients were found to have surgical complications in group B, while no complication was detected in group A. The local recurrence was 6% (2/31) in group A and 0 (0/41) in group B (P < 0.05); the distal metastasis was 6% (2/31) in group A and 2% (1/41) in group B, with no significance difference between the 2 groups (χ~2 =0. 062, P >0.05). The 3- and 5-year survival rates were 93%, 87% in group A, and 95%, 91% in group B, with no significant difference between the 2 groups (χ~2 =0.099, 0.144, P >0.05). Conclusions The prognosis of patients with early low rectal cancer who received transanal local resection is similar to those who received radical resection. Transanal local resection is safe and feasible when the indications are strictly followed.
8.The effects of transcranial magnetic stimulation on motor function in patients with Parkinson's disease: A meta-analysis

Objective To explore the effects of transcranial magnetic stimulation (TMS) on motor function in patients with Parkinson's disease (PD) using meta-analysis. Methods Eight comparative studies of the effects of TMS were meta-analyzed. Results The combined studies confirmed a significant difference before and after TMS treatment. Between the experimental and control groups the effect was also highly significant. Conclusion TMS may play an active role in the rehabilitation of motor function for patients with Parkinson's disease.
9.Construction of eukaryotic expression vector of human antimicrobial peptide LL-37

Objective To construct eukaryotic expression vector of human antimicrobial peptide LL-37 pPIC9-LL-37, and transform the plasmid pPIC9-LL-37 into P.pastoris GS115 to obtain the recombinant P.pastoris strains.Methods The full-length of antimicsobial peptide LL-37 gene was artificially synthesized by overlap extension method and was fused to pPIC9 and then the fused plasmid was transformed into E.coli DH5?.After analysis by PCR and sequencing,the plasmid pPIC9-LL-37 was transformed into P.pastoris.The colonies exhibiting the phenotype of His+Mut+ or His+Mut-were screened by means of MM and MD plates and the insertion was confirmed by PCR.Results The results of PCR and sequencing confirmed that the LL-37 gene was correctly inserted into pPIC9. The colonies of 10 His+Mut+ and 9 His+Mut-were obtained by means of MM and MD plates screening and were confirmed by PCR.Conclsion The recombinant P.pastoris strains containing LL-37 were successfully obtained.
10.The effect of different ablation lines in left atrium on modified cardiac Cox maze procedure

Objective Background and objective The Cox maze Ⅲ procedure has been considered the gold standard for the surgical treatment of atrial fibrillation (AF) and the modified Cox maze use bipolar radiofrequency ablation instead of the cut-and-sew technique,while also reducing the connecting lines in left atrium.This study was to understand whether completely isolation the posterior left atrium by increasing left atrium ablation lines can enhance the survival after modified Cox maze procedure.Methods From Jan 2009 to Dec 2009,all the patients underwent the Cox maze procedure,following the same examination,were divided into two groups (case-control): box lesion group (n =60),which is means the right and lefi pulmonary vein lesions were connected inferiorly by adding ablation lines,thereby completely isolating the posterior left atrium and the non-box lesion group (n =60).Similar interventions were given during the perioperative periods.Comparing the risk factors before surgeries,then follow-up was 100% complete,and the mean follow-up was 9 ± 8.4 months.Results Except the box lesion had long time of extracorporeal circulation,the characteristics of the 2 groups were similar because there were no differences in age,AF duration,left atrial diameter,left ventricular ejection fraction or homochronous operation between the groups and there was no operative death.No patient was lost to follow-up.In the mean follow-up duration,a patient was died of intracerebral hemorrhage at 2 months postoperationly in the box lesion group and a patient suffered from stroke at 12 months postoperationly in the non-box lesion.The overall freedom from AF recurrence was higher in the box lesion group at 1 (70% vs 51.2%,P =0.039) and 3 (78.3% vs 60%,P =0.030) months.While it was 80% vs 71.7% (P =0.286) at 6 months.Eight patients wrere readmitted because of the recurrence of AF,4 and 2 patients underwent catheter ablation in box and non-box group respectively,one patient in each group underwent electrical conversion.After treatment,all of the patients were recovered to sinus rhythm and continued to take cordarone.Conclusion Compared to the single connecting between right and left pulmonary vein,isolating the entire posterior left atrium by creating a box lesion showed higher freedom from AF in the earlier months (1-3months).However,half a year after the surgery,there were no significant differences between the two groups.
