1.Clinical analysis of 8 cases of neonatal intestinal muscle defect with perforation

Objective To study the clinical characteristics,treatment and prognosis of neonatal intestinal muscle defect with perforation.Methods The clinical data of 8 cases of newborns who had intestinal muscle defect with perforation in Department of Surgical Intensive Care Unit,Nanjing Children's Hospital Affiliated to Nanjing Medical University,were analyzed retrospectively.Results These 8 cases included 1 premature infant,1 term low birth weight infant,6 cases of term infants with normal birth weight,and all had no history of perinatal asphyxia.The ages of onset of all cases were < 7 d.Clinical manifestations were crying and sudden onset of obvious abdominal distention.Bile or fecal vomiting occurred in 5 cases.These patients included 5 cases with congenital anorectal malformations,of which 3 cases had low deformity,1 case with middle deformity and 1 case with high deformity,with no other complications of gastrointestinal malformation.Surgical treatment was used in 8 cases,in which 7 cases of rectal perforation and 1 case of ileal perforation were confirmed.All cases were isolated perforation with a diameter of about 0.8-2.0 cm.The perforated periphery of the intestine was membranous.Intestinal repair was performed after total removal of the diseased tissue,and the patients with rectal perforation were performed at the same time for the proximal colostomy.The histopathologic examinations showed that the periphery of perforation had intact mucosa,muscularis propria was absent,myenteric plexus was absent in the region of muscle loss.Two cases were discharged within 1 d after colostomy.Except for 1 case with high anorectal malformation in the control of poor defecation functions,there were no other postoperative gastrointestinal complications in these 6 cases.Conclusions Rectal perforation is the most common in neonatal intestinal muscle defects,and it is an isolated and large diameter perforation.Most cases are term infants with normal birth weight.These patients are often associated with congenital anorectal malformations.High,middle and low deformity can be accompanied by intestinal muscle defect.Surgical excision of diseased tissue should be complete,in principle,and intestinal stoma should be performed proximal to the perforation.In case of congenital anorectal malformations complicated with gastrointestinal perforation,the rectum should be explored as an important region.Neonatal intestinal muscle defect maybe a primary lesion according to clinical characteristics of early sudden onset after birth and pathological examinations.Except for complications caused by associated malformations,the prognosis is satisfactory.

Depression is the general symptom of gloomy mind,which could lead to suicide.The objective of this research is to explore the relation among different psychosocial factors of depressed countryside women and the roles of economy,family and health.Methods: Use SAS,SDS,Living Affair and individual status to ask 608 women in the countryside of Shanxi.Results: 33.9% women have depressed symptom,the analysis of logistic shows that the factors which will lead to depress are anxiety,minus life events,and so on.Conclusion: There is consanguineous connection between the depression of women and their living conditions,health,economy,and so on.

5.Identification for medically important yeast-like fungal species by sequence analysis of 18S rRNA gene

Objective To compare sequence analysis of the yeast-like fungal isolates with traditional methods and analyze the feasibility of identification of common yeast-like fungal by sequence analysis of gene. Methods 115 yeast-like fungal isolates were collected in the clinical laboratory of Beijing Tongren Hospital. DNA of yeast-like fungal was extracted and then amplified with universal primers of part of 18S rRNA genes followed by sequencing directly. The sequences obtained were submitted to the GenBank (NCBI) to identify the fungi. At the same time, the CHROMagar Candida and Vitek 32 YBC were used to identify the fungi. The identification accuracy with three methods was compared to explore the feasibility of the identification of sequence analysis. Results 18S rRNA gene sequence analysis was compared with traditional method. There were some differences in the identification results of 13 strains. The coincidence rate between CHROMagar Candida and sequence analysis was 89. 2% (91/102) and the coincidence rate between Vitek 32 YBC and sequence analysis was 91.3% (105/115). The positivity rate of species-level identification by CHROMagar Candida , Vitek 32 YBC and the 18S rRNA gene sequence analysis were 88. 7 % ( 102/115 ), 100% ( 115/115 ), 100% ( 115/115 ). Conclusion Identification of medically important yeast-like fungal by sequence analysis of the 18S rRNA gene is reliability.
6.Effects of MCP-1 in isometric exercise training promotes the collateral artery formation in mice with myocardial ischemia

Objective:To investigate the role of monocyte chemoattractant protein-1 in the progress that isometric exercise training improves arteriogenesis.Method:Twenty-four male Sprague-Dawley rats were used,weighing (200±20)g.The rats were randomized into control group (CG),myocardial ischemia group (MI),exercise training group (ET),MCP-1 inhibitor group (LG).There were 6 rats in each group.Rats were continuously administered 10mg/kg subcutaneously isoproterenol for successive 2 weeks to establish the myocardial ischemia model.Successfully modeled rats were in groups MI,ET and LG.Isometric exercise training were performed in group ET and LG.The rats in group LG were given MCP-1 inhibitor leflunomide by gavage.After 8 weeks of training,the left ventricular myocardium was extracted and relative collateral blood flow (RCBF) was measured by microspheres.Artery density (AD) and monocytes were measured by immunohistochemistry analysis.Western blot analysis and real-time quantitative PCR were performed to assay protein and mRNA of MCP-1.Result:RCBF and AD increased significantly in group ET as compared to the rest groups.RCBF and AD in group LG showed higher than that in group MI but not significant.The number of monocytes,MCP-1 mRNA and MCP-1 expression were significantly elevated in ischemic myocardium of group ET.Interestingly,the number of monocytes,MCP-1 mRNA and MCP-1 expression showed lower than that in group MI but also not significant.Conclusion:Eight weeks of isometric exercise training can increase the expression of MCP-1 in ischemic myocardium and promote the arteriogenesis.
7.Association of serum levels of matrix metalloproteinases and their tissue inhibitors with mortality in patients with malignant middle cerebral artery infarction

Objective To investigate the relationship between serum levels of matrix metalloproteinases (MMPs) and tissue inhibitors of metalloproteinases (TIMP) and the mortality in patients with malignant middle cerebral artery infarction.Methods A total of 74 patients with malignant middle cerebral artery infarction (MMCAI) was assigned to research group,74 healthy volunteers were recruited to serve as healthy controls,and 242 patients with other cerebral infarction were recruited to serve as case controls.Quality-of-life questionnaire and clinical examination were used to collect the information of participants.Endpoint was 30-day mortality.Results The research group showed higher serum levels of MMP-9,MMP-10,and TIMP-1 relative to the case control group,and the case control group showed higher serum levels of MMP-9,MMP-10,and TIMP-1 relative to the healthy control group (P < 0.05).Compared to survivor ones,non-surviving MMCAI patients showed lower Glasgow Coma Scale score (P < 0.05),higher serum levels of tumor necrosis factorα (TNF-α),MMP-9,MMP-10,and TIMP-1 (P < 0.05).Multiple logistic regression analysis showed that serum levels of TNF-α,MMP-9,MMP-10,and TIMP-1 were associated with 30-day mortality,and the OR were 1.09,1.18,1.13,and 1.32,respectively.Conclusions Serum levels of MMP-9,MMP-10,and TIMP-1 levels in MMCAI patients were associated with mortality,and could be used as an auxiliary index of mortality in MMCAI patients.
8.Doppler ultrasound in evaluating the curative effect of normobaric oxygen therapy on fetal atrial premature contractions

Objective To evaluate the curative effect of normobaric oxygen therapy on fetal atrial premature contractions(PACs) with Doppler ultrasound. Methods Maternal normobaric oxygen therapy was performed in one hundred and seventy three fetus with PACs but without congenital heart disease diagnosed by Doppler echocardiography. Another 63 cases with isolated PACs receiving no therapy served as controls. 9.EFFECT OF LBNP ON REGULATIVE MECHANISMS OF CARDIOVASCULAR SYSTEM OF MALE AND FEMALE PILOT-STUDENTS

To analyze the change of physiological indices in male and female pilot students during lower body negative pressure (LBNP) exposure, one female group and one male group participated in the experiment. They were subjected to LBNP up to 50 mmHg for 8 min. Electrocardiogram (ECG), heart rate (HR), blood pressure (BP), cerebral blood flow (CBF) and time of exposure were monitored during LBNP. The results showed that during LBNP, heart rate (HR) increased while systolic blood pressure (SBP) and cerebral blood flow (CBF) decreased with the prolongation of exposure. It is concluded that LBNP exposure could produce similar physiological changes in both male and femal pilot students.

Objective To analyze the impact of health service and some other factors on the improvement of health. Methods The production function model introduced by Auster was employed. With the age adjusted mortality taken to be the output of health and factors like health service, life style, living environment, economy, and education taken to be the input, the two-stage least square was used to analyze the impact as well as the degree of the impact of the various factors. Results The impact of health service, the contribution of which to the age adjusted mortality was around 0.37, was greater than that of education but smaller than that of economy. Conclusion The result indicates that health service is an important influencing factor in the improvement of health. Full attention ought to be paid to it so that investment into health, especially public health, will be expanded and the health of the people better protected.
