1.Clinical observation of methyprednisolone combined with high dose immunoglobulin in treatment of severe bullous skin disease

Objective To explore the effect of methylprednisolone combined with high dose immunoglobulin (IVIg) in treatment of severe bullous skin disease. Methods 51 cases of severe bullous skin diseases were randomly divided into control group and observation group and control group received conventional methylprednisolone treatment on the basis of the observation group in this high-dose injection combined with IVIg,therapeutic effect were observed.Results The observation group hospitalization days ( 15.6 ± 6. 2) d, hormone usage ( 87. 6 ± 13.7 ) mg, hormone reduction time(8. 6 ±2. 4) d,were obviously lower than the control group(23. 1 ±6. 3)d,( 105.3 ± 1.64) mg, ( 13.8 ±4. 1 ) d ( t = 2. 771,3. 102,3. 219, P < 0. 05 ). The observation group total effective rate was significantly higher than the control group( x2 =6. 248,P <0. 05). The observation group therapy effect-acting period and obviously subsidise lesions time was(8. 9 ±2. 4)d, (17.6 ±3. 7)d respectively,were significantly lower than the control group( 13. 1 ±3. 8) d, (23. 6 ± 5. 1 ) d, the differences between the two groups were statistically significant ( t = 2. 893,3. 347, P <0. 05). The observation group complication occurred in 5 cases were lower than those of the control group of 9 cases (x2 = 5.245 ,P < 0. 05 ). Conclusion Methylprednisolone combined with high dose IVIg treatment of severe bullous skin disease could shorten the course and speed up the restoration of skin lesions, reduce and alleviate the glucocorticoid dosage and side effects, improve the cure rate , and it was worthy of promotion.
2.The effect of the severity of congenital pectus excavatum on the cardiac morphology in children: evaluation with multislice helical CT.

This paper is aimed to evaluate the CT manifestation of congenital pectus excavatum and its effect on the cardiac morphology. CT features of 34 children with pectus excavatum were retrospectively reviewed. The Haller index (HI), CT depression index (CTDI), cardiac rotation angle (CRA), pulmonary vein angle (PVA), cardiac compression index (CCI) and cardiac asymmetry index (CAI) were measured from the images on the PACS system. The relationships among these indexes were evaluated. The HI was 4.12 +/- 1.48, the CTDI was 2.39+/- 0.70, CRA was (53.52 +/- 7.68) degrees, PVA was (66.51 +/- 11.02) degrees, CCI was 2.43 +/- 0.95, and CAI was 1.55 +/- 0.56. There was statistical significance of CCI and CAI between children with severe or slight pectus excavatum. The HI and CTDI were positively related with CCI and CAI respectively, whereas there was no statistical significance of CRA and PVA between children with severe or slight pectus excavatum. There was no statistical significance of HI, CTDI, CRA, PVA, CCI and CAI among different age groups. The morphological change of the heart compressed in children with pectus excavatum can be accurately evaluated by multislice helical CT.

6.Mitochondrial combined oxidative phosphorylation deficiency type 1 in childrena case report and literature review

Objective To summarize the phenotypic and genotypic characteristics of mitochondrial combined oxidative phosphorylation deficiency type 1 (COXPD1), and to improve the clinicians' awareness of this mitochondrial encephalomyopathy. Methods The clinical characteristics, physical examination, laboratory examination and other data of a child with COXPD1 were analyzed retrospectively. The diagnosis was confirmed by clinical whole exon sequencing and high-precision mitochondrial genome full-length PLUS gene detection, and the phenotype and genotype were analyzed by reviewing relevant literature. Results A one-year and five-month-old boy mainly presented with hyperlactacidemia and abnormal liver function. Clinical whole exon sequencing showed that the child had homozygous variation of c. 688G>A(p.G230S) in the GFM1 gene. Sanger sequencing verified that the variation was respectively inherited from the parents of the child (both were heterozygous) with the autosomal recessive inheritance pattern. The high-precision mitochondrial genome full-length PLUS detection also did not find pathogenic mutations related to clinical phenotypes. The child was diagnosed with COXPD1. After "cocktail" therapy and liver protection therapy, the patient's condition improved. Conclusions The phenotype of COXPD1 is complicated and variable, mainly liver type and brain type. The mutation of GFM1 gene affects mitochondrial translation system function, and early gene detection is helpful for definite diagnosis.
7. A New Hasubanan Alkaloid from Stephania hernandifolia

Objective: To study the chemical constituents of Stephania hernandifolia. Methods: Various chromatographic techniques were used to isolate the constituents, and the structures were elucidated on the basis of HR-EI-MS, 1D-NMR and 2D-NMR spectral analyses. Results: A hasubanan alkaloid, hernsubanine D (1) was isolated from the whole plants of S. hernandifolia. The compound was screened for the cytotoxic activity against two human cancer cell lines in vitro. Conclusion: Compound 1 is a new compound without cytotoxicity against A549 and K562 cells. © 2014 Tianjin Press of Chinese Herbal Medicines.

Objective To study the chemical constituents in the seeds of Euphorbia lathyris.MethodsCompounds were isolated by methods of column chromatography(silica gel,including reversed phase),Sephadex,and recrystallization.On the basis of spectroscopic methods including IR,MS,NMR,and X-ray,structures of compounds were confirmed.Results Twenty-two multi-type compounds were isolated from ethanol extract in the seeds of E.lathyris.Their structures were identified as 5,15-O-diacetyl-3-O-phenyl-6(17)-epoxylathyrol(1),5,15-O-diacetyl-3,7-O-dibenzoyl-7-hydroxylathyrol(2),5,15-O-diacetyl-3-O-benzoyl-lathyrol(3),20-O-hexadecanoyl-ingenol(4),3-O-hexadecanoyl-ingenol(5),15,17-O-diacetyl-3-O-cinnamoyl-17-hydroxyjolkinol(6),5,15,17-O-triacetyl-3-O-benzoyl-17-hydroxyisolathyrol(7),5,15-O-diacetyl-3-O-nicotinoyl-lathyrol(8),5,15-O-diacetyl-3-O-benzoyl-7-O-nicotinoyl-7-hydroxy-lathyrol(9),ingenol(10),lathyrol(11),esculetin(12),?-sitosterol(13),benzene-1,2,3-triol(14),palmiticacid(15),2,3-dihydroxypropyl icosanoate(16), 2,3-dihydroxypropyl oleate(17),2,3,4-trihydroxybutyl hexadec-3-enoate(18),aurantianide acetate(19),benzoic acid(20),p-hydroxybenzoic acid(21),oleic acid(22).Conclusion Among these,compounds 10,11,1419 are obtained from this plant for the first time and compounds 1-3 are the main diterpenes.
9.Protective effect of active component of Eclipta on hepatic injury induced by ConA in mice

Objecave To explore the protective effect of active component of Eclipta on hepatic injury induced by Concanavalin A(ConA)in mice. Methods Mouse hepatic injury models were established by injection of ConA(15 mg/kg)via tail vein.The effect of Eclipta extracts on serum alanine aminotransferase (ALT) and liver histology in both normal mice and hepatic injury mice was examined.A pure compound was obtained by means of bioactivity-guided isolation,and IR,~(13)C-NMR, ~1H-NMR,~1H-~(13)C HMBC and ~1H-~(13)C HMQC were used for structural identiffcation. Results Eclipta extracts decreased serum ALT in hepatic injury mice induced by ConA.and demonstrated anti-apoptosis effect induced by ConA in hepatoeytes.Wedelolaetone,a eoumarin,was isolated from Eclipta,and its structure was identified on the basis of spectroscopic analysis.Wedelolactone inhibited ConA-induced T cell proliferation selectively,and completely antagonized the effect of ConA at the concentration of 10μg/mL. Conclusion Eclipta herb has hepatoprotective effect.Wedelolactone,the anti-inflammatory and anti-apoptosis active component of Eclipta,might serve as a lead compound for developing new antiinflammatory drugs.
10.Studies on strengthening and restoring effects and hepatoprotective effect of Eclipta

Objective To evaluate the strengthening and restoring effects and hepatoprotective effect of Eclipta extracts. Methods Sixty Kunming mice were randomly divided into control group, low-dose Eclipta group, high-dose Eclipta group, cyclopho9phamide(CY) group, low-dose Eclipta CY group and high-dose Eclipta CY group(n = 10). Eclipta 8 g·kg~(-1)d~(-1) or 4 g·kg~(-1) · d~(-1) were administered by oral gavage for 7 d. Immunosuppression mouse models were established by intraperitoneal injection of 0. 05 g/kg CY on the fourth day of administration. The effects of Eclipta extracts on thymus index, spleen index and carbon clearance index in both normal mice and immunosuppression mice induced by CY were tested, and those on proliferation of lymphocyte and T cells induced by ConA were examined. Based on these data, strengthening and restoring effects of Eclipta were observed. Besides, 70 Kunming mice were randomly divided into control group, low-dose Eclipta group, high-dose Eclipta group, CCl_4 group, DDB group, low-dose Eclipta CCl_4 group and high-dose Eclipta CCl_4 group. 20 g·kg~(-1)·d~(-1) and 10 g·kg~(-1) ·d~(-1) Eclipta were administered by oral gavage for 7 d. Hepatic injury mouse models were established by intraperitoneal injection of 10 mL/kg CCl_4 2 h after the last administration. The effects of Eclipta extracts on serum ALT, MDA, TP and T-Bil levels in both normal mice and hepatic injury mice induced by CCl_4 was tested, and those on the viability of the primary culture hepatocytes were determined. Based on these data, hepatoprotective effect of Eclipta was observed. Results Eclipta extracts increased thymus index and carbon clearance index in both normal mice and immunosuppression mice induced by CY, promoted proliferation of lymphocyte and proliferation of T cells induced by ConA, reduced serum ALT and MDA levels in hepatic injury mice, and improved the viability of the primary culture hepatocytes. Conclusion Eclipta herb has strengthening and restoring effects and hepatoprotective effect.
