The middle thyroid vein has been noted to be frequently absent during thyroidectomies in Papua New Guinea (PNG). To verify this and other known inconsistencies in the neck a total of 103 bodies were dissected at the Port Moresby General Hospital in 2002. The middle thyroid vein was absent in 59% of the cases. It would be to the advantage of surgeons in PNG to be aware of this.

3.The Use of Docosahexaenoic Acid Supplementation to Ameliorate the Hyperactivity of Rat Pups Induced by in utero Ethanol Exposure

It has been demonstrated that in utero ethanol(EtOH) exposure induces hyperactive behavior and learning disturbances in offspring. In order to investigate the effects of docosahexaenoic acid(DHA) on these neurobehavioral dysfunctions of rat pups induced by in utero EtOH exposure, pregnant Wistar rats were divided into four treatment groups depending on the type of oil added to the diet and drinking water as follows; (a)5% safflower oil with tap water(TW/n−6), (b)3% safflower oil and 2% DHA with tap water(TW/n−3), (c)5% safflower oil with 10%−EtOH(ET/n−6), (d)3% safflower oil and 2% DHA with 10%−EtOH(ET/n−3) at gestational day (GD)7. 10%−EtOH was administered to dams in ET/n−6 and ET/n−3 groups from GD 7 to the pups’ weaning(postnatal week 4), and all pups were fed with the same diet that was given to their dams during the entire examination period. The open−field test and the water E−maze test were conducted for all pups, and a spontaneous motor activity test and the Sidman electric shock avoidance test were performed for some of male pups. Amounts of monoamine metabolites in striatum were then determined, and fatty acid analyses of total brain lipids were performed. The male pups in the ET/n−6 group showed significantly more rearing and square−crossing movements in the open−field test, and significanrly higher spontaneous motor activity during the dark period in the daily cycle compared to the males in the TW/n−6 group. The male pups in the ET/n−3 group showed fewer of these behaviors in the open−field test compared to the ET/n−6 group males, and a normal pattern of spontaneous motor activity. Learning disturbance induced by in utero EtOH exposure was not observed in the E−shaped water maze, but was observed in the avoidance rates in the Sidman electric shock avoidance test. However, there was no significant modifying effect of DHA on the avoidance rates in EtOH exposed pups. The analysis of the fatty acid composition of total lipids in the brains of the pups revealed high levels of DHA in the diet reflected an increased level of brain DHA and caused a decreased level of the brain arachidonic acid. Retroconversion from DHA to eicosapentaenoic acid was also observed. However, there was no significant effect of DHA on the levels of monoamine metabolites. These results support the hypothesis that DHA can counteract the attention deficit hyperactivity disorder.

4.A Butter Diet Induces Higher Levels of n-3 PUFA and of n-3/n-6 PUFA Ratio in Rat Serum and Hearts than a Safflower Oil Diet

The effects of a 47-week diet of butter of safflower oil as fat in combination with casein or soy protein as protein were observed for the serum concentrations of lipids and fatty acid compositions in rat serum and heart. Serum total cholesterol (Chol) did not differ among the four experimental diet groups. In the butter groups, significantly higher low-density lipoprotein (LDL)-Chol and lower high-density lipoprotein (HDL)-Chol were observed than in the safflower oil groups (p<0.005, respectively). Higher levels of α-tocopherol were found in the butter groups than in the safflower oil groups (p<0.05) and in the casein groups than in the soy protein groups (p<0.01). In comparison with the safflower oil groups, the butter groups showed higher n-3 polyunsaturated fatty acids (PUFA) contents and lower n-6 PUFA contents in serum and the hearts (p<0.005).The ratios of n-3/n-6 PUFA in the butter groups in serum, 0.26 and 0.18, and in the hearts, 0.37 and 0.36, (butter-casein diet and butter-soy protein diet, respectively) were higher than those of the safflower oil groups of under 0.01 in serum and 0.02 and 0.03 in the hearts (safflower oil-casein diet and safflower oil-soy protein diet, respectively) (p<0.005). In the soy protein groups, higher n-3 PUFA contents in the hearts were found than those of the casein groups (p<0.05). This study suggested that the butter diet induces higher levels of n-3 PUFA and a higher n-3/n-6 PUFA ratio than the safflower oil diet in rat serum and hearts over a long feeding period.

5.A cost-effective modified micromethod for measuring urine iodine.

A modified micromethod for measuring urine iodine was successfully established and validated. The micromethod showed good correlation with the method used by several World Health Organization (WHO) collaborative laboratories (y = 0.9342x + 4.6213; r = 0.962; p = 0.01; n = 50). The micromethod also showed good agreement when compared to the reference WHO method. The sensitivity of the assay was 13.809 ug/L (n = 8) and mean recoveries were 114, 103 and 106% at concentrations of 30, 40 and 50 ug/L (n = 3) respectively. At iodine concentrations of 51 +/- 15.5, 108 +/- 32.4 and 149 +/- 38.6 ug/L, intra-assay coefficient of variations (CVs) were 13%, 7% and 5% respectively (n = 20), and inter-assay CVs were 10%, 15% and 7% respectively (n = 10). The assay showed good linearity plot (y = 1.0407x + 60.451; r = 0.993; n = 3).

6.Evaluation of Quality of Life in Patients with Breast Cancer Treated with Chemotherapy

Recently the evaluation of health-related quality of life (QOL) in cancer patients has become important in carrying out a treatment strategy. During a period between June and August 2004, we studied the QOL in 16 breast cancer patients with or without chemotherapy. The regimens of chemotherapy were EC (n=6), AT (n=1) and CMF (n=1). We made the Functional Assessment of Cancer Therapy Scale-General (FACT-G) in all patients. The patients treated without chemotherapy were superior to those with chemotherapy with respect to physical and emotional well-being (p<0.05). There were no differences in social well-being and relationship to families between the two groups. The patients treated without chemotherapy made significantly higher scores in the basic activities of daily living than those with chemotherapy (p<0.05). Compared with patients receiving chemotherapy, patients treated without chemotherapy had better quality of life (p<0.05). We concluded that it was important for us to assist in decision making about treatment and supportive care needs.

7.Lower Eicosapentaenoic Acid and Higher Arachidonic Acid Levels in Sera of Young Adults in the Netherlands than in Japan

To survey risk factors in coronary heart disease, we compared serum fatty acid composition and lipids for university students in Japan (33 males and 29 females) and in the Netherlands (20 males and 19 females). No significant differences were found between the mean levels of cholesterol (Chol) and triglycerides (TG) between the subjects in the two countries. The mean levels of polyunsaturated fatty acid (PUFA), monounsaturated fatty acid (MUFA) and saturated fatty acid (SFA) of Japanese students were similar to those of the Dutch students. In both countries, the levels of Chol showed a positive correlation with the levels of PUFA, n−6 PUFA, linoleic acid (C18: 2n−6), and arachidonic acid (AA, C20: 4n−6) but no correlation with the percentages of PUFA and the ratio of PUFA/SFA. On the other hand, the TG levels correlated inversely with the percentage of PUFA and the ratios of PUFA/SFA in both countries. When compared to those of Japanese students, low eicosapentaenoic acid (EPA, C20: 5n−3) and high AA were found in the Dutch students (p < 0.001, respectively). The total amounts of n−3 PUFA in the Dutch were significantly lower than those in the Japanese (p < 0.001) but no differences among those of n−6 PUFA. The ratios of EPA/AA and n−3/n−6 PUFA of the Dutch students were lower than those of the Japanese students (p < 0.001, respectively). The ratio of EPA/AA showed a positive correlation with EPA but not with AA in both countries. The levels of Toc which will decrease the risk of coronary vascular disease (CVD) were lower in Japan than those in the Dutch in both sexes (p < 0.01, respectively). These results suggest that the low EPA and high AA levels and the low n−3/n−6 PUFA ratio may lead to greater incidence of CVD.

8.Body mass index and nutritional status of adults in two rural villages in Northern Malaysia.

There has been a change in the lifestyles of populations, including reduced physical activity and consumption of foods high in calories. Overweight and obesity are now replacing the more traditional public health concerns such as under-nutrition and infectious diseases as some of the most significant contributors to ill health. Determination of the body mass index (BMI) profile and nutritional status of adults of two rural coastal villages in Northern Malaysia was part of a community diagnosis in a community survey. Height and weight were measured and BMI calculated. Blood pressure was measured using a manual sphygmomanometer according to WHO guidelines. A standardized questionnaire was used to interview the villagers concerning their health. Out of the total population, 504 were above 20 years of age. Data was available for 441 persons for analysis. There were 210 (47.6%) males and 231 (52.4%) females. The prevalence of underweight was 9.8% (n=43), overweight 25.9% (n=114) and obesity 17% (n=75). The problem of over-nutrition was significantly higher among females, especially housewives. (p< 0.05). Those in ages 41-70 years were the majority with problems of over-nutrition (p=<0.05). More than half (52.9%; n=39) of those who were obese had hypertension (p=<0.05). Results show that a higher number of women especially housewives were obese and more than half of those obese subjects had hypertension. A more thorough nutritional profile using waist, hip and body fat measurement as well as an assessment of the dietary intake and activity regime of these villagers is needed. Interventions need to be carried out before more serious complications of obesity become rooted in this community.

9.Enriched n-3 polyunsaturated fatty acids of dense LDL subfraction in Japanese women with small, dense LDL categorized by non-denaturing gradient gel electrophoresis

;Small, dense low density lipoprotein (LDL) has been suggested to be highly atherogenic as a result of their low-binding affinity to LDL receptors, their prolonged plasma half-life and low resistance to oxidative stress. Although marine n-3 polyunsaturated fatty acids (PUFAs) have beneficial effects for the prevention of cardiovascular diseases, there is little information on detailed fatty acid composition in LDL particles. In the present study, LDL subfractions were isolated from the plasma of 45 clinically healthy Japanese women by density gradient ultracentrifugation using a light fraction of 1.025-1.034 g/ml (LDL-I), an intermediate fraction of 1.034-1.044 g/ml (LDL-II), and a dense fraction of 1.044-1.060 g/ml (LDL-III). A clear relationship between the frequency of fish intake and increases in plasma n-3 PUFAs was apparent. Although plasma n-3 PUFAs in women with small-sized LDL using non-denaturing gradient gel electrophoresis (GGE) did not show a significant difference, n-3 PUFAs and n-3 PUFAs/n-6 PUFAs in dense LDL-III subfraction increased in the small-sized LDL group using GGE, compared to those with buoyant LDL particles. The result of enriched n-3 PUFAs in dense LDL subfraction suggests that n-3 PUFAs may help prevent atherosclerosis in the arteries of Japanese women with small-sized LDL.

10.A retrospective review comparing the use of Gonal-F and Metrodin-HP for in-vitro fertilisation (IVF).

All cycles of IVF with pituitary down-regulation (n = 57) done at the Damansara Fertility Centre in the year 2000 were studied. All the 57 patients had controlled ovarian hyperstimulation, either using Metrodin HP (n = 27) or Gonal-F (n = 30). Of these, 53 patients reached oocyte pick-up, 26 patients in Metrodin HP group and 27 patients in Gonal-F group. Gonal-F resulted in a higher clinical pregnancy rate of 66.6% compared to Metrodin HP 38.5% (p < 0.05). The live birth rate tends to be higher in Gonal-F group (40.7%) compared to Metrodin HP (30.8%), (p > 0.05).
