1.Development of Korean Academy of Medical Sciences Guideline Rating the Physical Impairment: Lower Extremities.

Lower Extremities Committee of Korean Academy of Medical Sciences Guideline for Impairment Rating develops new guidelines which are based on McBride method, American Medical Association Guides, Disability evaluation by The Korean Orthopaedic Association, The Korean Neurosurgery Society, and Korean Academy of Rehabilitation Medicine. The committee analyzed and discussed to create an ideal method practical in Korea. Our committee endeavors to develop new methods which are easy to use, but are suitable for professional use and also independent from the examinee's intentions. The lower extremities are evaluated on the basis of anatomic change, functional change, and diagnosis based evaluation. Nine methods are used to assess the lower extremities. Anatomic assessment includes leg length discrepancy, ankylosis, amputation, skin loss, peripheral nerve injury, and vascular disease. In functional assessment, range of motion and muscle strength are included. Diagnosis-based assessments are used to evaluate impairment caused by specific fractures, deformities, ligament instability, meniscectomies, post-traumatic arthritis, fusion of the foot, and lower extremity joint replacements.

2.Limb ischemic preconditioning attenuates apoptosis of pyramidal neurons in the CA1 hippocampus induced by cerebral ischemia-reperfusion in rats.

The purpose of this study was to investigate the effects of limb ischemic preconditioning (LIP) on apoptosis of pyramidal neurons in the CA1 hippocampus induced by global cerebral ischemia-reperfusion in rats. Forty-six rats whose bilateral vertebral arteries were occluded permanently were assigned to one of four groups: sham group, limb ischemia group, cerebral ischemia group and LIP group. LIP was performed by occluding the bilateral femoral arteries for 10 min 3 times in an interval of 10 min. Global cerebral ischemia was underwent by occluding the bilateral common carotid arteries for 8 min immediately after LIP. Assays for apoptosis of the hippocampal neurons were biologically and morphologically performed using gel electrophoresis, TUNEL and AO/EB staining. Characteristic DNA ladder was clearly visualized with gel electrophoresis in the hippocampus in cerebral ischemia group, but not in LIP group. The number of TUNEL-positive cells in the CA1 hippocampus was significantly reduced by LIP from 69.8+/-12 (cerebral ischemia group) to 17.8+/-5.8 (P<0.01). AO/EB staining also showed a reduction of apoptosis in LIP group compared with cerebral ischemia group. These results suggest that LIP can inhibit hippocampal neuronal apoptosis induced by cerebral ischemia-reperfusion, which contributes to the protection against the delayed neuronal death induced by cerebral ischemic insult.

3.Clinical observation of lower extremity deep venous hrombosis (DVT) postoperative fracture treated by Maixuekang capsule and low molecular heparin calcium (Subilin).

OBJECTIVETo observe the clinical curative effects of lower extremity deep venous thrombosis (DVT) postoperative fracture treated by maixuekang capsule and low molecular heparin calcium (Subilin).
METHODFrom Feb 2008 to Apr 2010, 214 cases of lower extremity DVT postoperative fracture were randomly divided into experimental group and control group. The 107 cases of them were treated by maixuekang capsule based on the comprehensive treatment.
RESULTThe 89 cases were cured and improved in the observation group, but 18 cases were ineffectiveness. The 66 cases were cured and improved in the control group, but 41 cases were ineffectiveness. Maixuekang capsule group had 83. 18% (89/107) total effective rate and 61.68% (66/107) total effective rate in the control group. There was significant difference between the two groups (P < 0.05). After the treatment, the perimeter between the suffered limb and the healthy one was significantly reduced, the blood rheology examination had been improved significantly. There was also significant difference between the two groups (P < 0.05).
CONCLUSIONMaixuekang capsule for lower extremity deep vein thrombasis has good effect It has both thrombolytic and anticoagulant effects for lower extremity DVT postoperative fracture treated by maixuekang capsule and low molecular heparin calcium (Subilin). It's a recommended treatment.
