2.Surgical treatment in degenerative scoliosis.

Degenerative scoliosis is one of the important reasons of low back pain in middle-aged and elderly. The surgical treatment has been gradually paid more attention to in recent years. Pain and the quality of life are the key points of the surgical treatment. In addition to neurogenic compression,clinical symptoms and health-related quality of life are also closely related to the three-dimensional deformity of spine. Researchers try to employ different classifications of degenerative scoliosis to guide personalizedly surgical treatment. In the future,osteoporosis and complications may be the research highlights in the surgical treatment of degenerative scoliosis.

3.A monitoring study of electromyography median frequency on fatigue of erector spinalis in drivers working at high altitude.

OBJECTIVETo evaluate the role of muscle fatigue in the pathogenesis of high prevalence of low back pain(LBP) in drivers who work at high altitude, in order to seek the effective measures for prevention and treatment of LBP.
METHODThe spectral electromyographic(EMG) changes during isometric contraction of erector spinalis to fatigue in 26 drivers in the field were consecutively recorded after daily driving for 5 days. The relationship between the decline of median frequency and the development of the back extensor fatigue were analyzed.
RESULTSThe decline rates of EMG median frequency of the erector spinalis on 1-5 working day [(-0.258 +/- 0.087), (-0.248 +/- 0.070), (-0.256 +/- 0.097), (-0.267 +/- 0.093), (-0.274 +/- 0.081)%/s respectively] were significantly different from that of baseline measured before work [(-0.175 +/- 0.114)%/s, P < 0.05 or P < 0.01]. A positive correlation was also found between the decline rate and the working time(r = -0.809, P < 0.05).
CONCLUSIONThe high prevalence of LBP in the drivers working at high altitude is due to acumulative fatigue of back extensor resulting from long-distance driving, lack of proper rest, decrease of muscle endurance and increase of liability to fatigue, consequently leading to the muscle dysfunction and finally LBP.

5.Effect of thunder-fire moxibustion combined with vibration training on low back pain of primary osteoporosis.

OBJECTIVE:
To compare the therapeutic effect of thunder-fire moxibustion combined with vibration training and simple vibration training on low back pain of primary osteoporosis by 's modulus of ultrasonic wave, and seek an objective evaluating method.
METHODS:
A total of 60 patients were randomized into an observation group and a control group, 30 cases in each one. The patients in the two groups were treated with vibration training using whole body vibration treatment bed. In the observation group, thunder-fire moxibustion was applied at Yaoyangguan (GV 3), Mingmen (GV 4), Ganshu (BL 18), Shenshu (BL 23) and Dachangshu (BL 25). The treatment was given once every other day, 3 times a week for 4 weeks. The visual analogue score (VAS), real-time shear wave elastography (RTSWE) and medical outcomes study 36-item short-form hearth survey (SF-36) were used to evaluate pain intensity, multifidus muscle tone ('s modulus) and quality of life before treatment, after 4-week treatment and 1 month after treatment.
RESULTS:
Compared before treatment, the VAS scores, 's modulus of multifidus muscle and 5 dimensions of SF-36 (physical condition, body pain, general health, social function and mental health) after 4-week treatment and 1 month after treatment were significantly improved in the two groups (all <0.05), the physiological role in the observation group after 4-week treatment and 1 month after treatment were improved (both <0.05). In the observation group, the VAS scores, 's modulus of multifidus muscle and 3 dimensions of SF-36 (physiological role, body pain and general health) after 4-week treatment and 1 month after treatment were superior to the control group (all <0.05).
CONCLUSION
The therapeutic effect of thunder-fire moxibustion combined with vibration training is superior to simple vibration training in relieving low back pain intensity and multifidus muscle tone, and improving quality of life for primary osteoporosis. RTSWE technique can be an objective examination method to evaluate pain.

6.Scoping review of acupuncture-moxibustion treatment for non-specific low back pain.

This study systematically searched and sorted out randomized controlled trial(RCT) of acupuncture-moxibustion treatment for non-specific low back pain by scoping review, so as to demonstrate the current state of the research evidence and provide a reference point for future clinical research and healthcare decision-making. Eight commonly used Chinese and English databases were searched, and the search time was from the establishment of the databases to July 7, 2023, so as to analyze the characteristics of the current status of the current research through visualization methods. A total of 50 studies were included, including 23 studies in Chinese and 27 studies in English. The overall number of studies showed an increasing trend. The percentage of studies published in Chinese non-core journals was 42.0%. The disease subtypes of interest were mainly chronic non-specific low back pain, accounting for 68.0% of the studies. The sample sizes of the studies were mainly concentrated in the range of 50-100 cases. A total of 15 types of interventions were categorized, with acupuncture interventions being the most studied. Duration of treatment did not exceed one month in 80.0% of the studies. Only 8.0% of the studies used minimal clinical important difference(MCID) as a basis for judgment. The follow-up period was set within 3 months in 28.0% of the studies, and 82.0% of the studies concluded that acupuncture-moxibustion was effective in the treatment of non-specific lower back pain. Adverse events were reported in 20.0% of the studies. The risk of bias in the included studies was dominated by low risk of bias and uncertain risk of bias, with fewer studies focusing on high risks of bias. In most of the studies, acupuncture-moxibustion was significantly more effective than the control group. The research on acupuncture-moxibustion treatment for non-specific low back pain is developing rapidly, but there are still insufficient studies on psychological state, safety, and other indicators, and there are still some studies with uncertain risks of bias, which is not conducive to the generalization and application of the findings. Therefore, future studies should improve and refine these shortcomings.

7.Structural equation model analysis of risk factors for low back pain among greenhouse vegetable-planting farmers.

OBJECTIVETo explore the risk factors of low back pain (LBP) among greenhouse vegetable planting farmers and estimate the level of the effects.
METHODSA self-made questionnaire based on the Dutch Musculoskeletal Questionnaire and the Nordic Questionnaire was conducted to 639 greenhouse vegetable planting farmers and then structural equation model was used to analyze the risk factors of LBP in SmartPLS software.
RESULTSThe coefficient of determination of the model was 0.827, and the structural coefficients of dynamic loads, static loads, force exertion, ergonomic environment and repetitive loads on LBP were 0.21, 0.43,0.27, 0.045 and 0.034 respectively, and the total effects of the above latent variables on LBP were 0.21, 0.43,0.27, 0.33 and 0.034 respectively.
CONCLUSIONThe main risk factors of LBP among greenhouse vegetable planting farmers were static loads, ergonomic environment, force exertion and dynamic loads.

8.Low back pain prevalence of female workers in flat-grained veneer wood industry.

OBJECTIVETo study the low back pain(LBP) and its cause on female workers in flat-grained veneer wood industry.
METHODSBending posture was analyzed by observation and the prevalence of low back pain was investigated by physical examination and questionnaire among 299 female workers.
RESULTSThe prevalence of fatigue compliant in selecting, remending and sticking workers was 68.8%, 66.7% and 59.0%, respectively, which mainly involved in the part of low back. The prevalence of low back pain in selection (53.8%) and remending (58.7%) workers was higher than that in sticking workers (30.1%), which was in accordance with the tenderness between L4/L5 or L5/L6 and on the psoas major. Posture analysis indicated that the biggest bending range of selecting and remending workers (80 degrees ) was larger than that of sticking workers (60 degrees ), as well as the daily bending times[(4396+/-817), (1696+/-286), (1094+/-476)] and the time they kept bending[(6.5+/-0.6), (6.2+/-1.3), 4.5+/-0.9) h].
CONCLUSIONBending posture is common among female workers especially those who work in selecting and remending and might be the major causes for the high prevalence of LBP in flat-grained veneer wood industry.

9.Lower back pain in truck drivers working in plateau areas and its prevention.

OBJECTIVETo investigate the epidemiological characteristics of lower back pain of truck drivers in plateau areas with an analysis of the causes and evaluated the effects of the protective measures.
METHODSThe self-reported lower back symptom was investigated by questionnaire in 1,132 male drivers in plateau areas. Vibration of the driver lumbar back was measured real time with or without wearing corset. Study the effect of the special corset for drivers and the massage on back by self-control design of big sample intervention experiments.
RESULTSThe prevalence of lower back pain of drivers in plateau areas was 86.1%. High level of its prevalence is due to many factors. Special corset for drivers and massage on back by themselves can reduce the degree of the pain.
CONCLUSIONDriving in plateau areas is a special occupational work with high level of prevalence of lower back pain which can be prevented.
