1.A cross mapping study on basie data of human resource management in the field of clinical nursing

Objective To develop a kind of clinical nursing human resource mamagement basic data set fits to national and international standard. Methods CNHRMBDS was developed according to refereces and related standard, and then compared with related data set of nursing management. Results There were 179 data elements in the CNHRMBDS. It included all data elements related to human resources for health in China Hospital Basic Data Set Standard 1.0Version, China Public Health Information Classification and Basic Data Set Standard1.0 Version, and Community Health Information Basic Data Set Standard. Twenty-four data elements in the World Health Organization Human Resources for Health Minimum Data Set and 155 data elements in the American Nursing Management Minimum Data Set were found comparable to those in the CNHRMBDS. Conclusions The CNHRMBDS was found complete and able to meet relevant national and in-ternational standards. It should be further developed to be a national standard for health profession.
2.Effect of Acupuncture plus Medication on Serum 5-HT, CGRP and ET in IBS-D

Objective To observe the efficacy of acupuncture plus medication in treating diarrhea-irritable bowel syndrome (IBS-D) due to liver depression and spleen deficiency, and its effect on serum 5-hydroxytryptamine (5-HT), calcitonin gene-related peptide (CGRP) and endothelin (ET). Method Sixty IBS-D (liver depression and spleen deficiency) were recruited and randomized into an observation group and a control group, 30 cases each. Patients in the observation group were intervened by acupuncture plus modified Bai Zhu Shao Yao powder, while the control group was intervened by acupuncture alone. The total effective rate, change of syndrome score of traditional Chinese medicine (TCM) and diarrhea-irritable bowel syndrome quality of life scale (IBS-QOL), as well as the levels of serum 5-HT, CGRP and ET, were observed. Result The total effective rate was 83.3% in the observation group, significantlyhigher than 63.3% in the control group (P<0.05). There were no significant differences in comparing the global and component TCM symptoms scores and IBS-QOL between the two groups before the treatment (P>0.05). The global and component TCM symptoms scores and IBS-QOL dropped significantly in both groups after the intervention (P<0.05). The global TCM symptoms score and IBS-QOL in the observation group were significantly lower than those in the control group after the intervention and 3 months after the intervention (P<0.05); the component TCM symptoms scores in the observation group were significantly lower than those in the control group after the intervention (P<0.05). There were no significant differences in comparing the serum 5-HT, CGRP and ET levels between the two groups (P>0.05) before the treatment. The levels of 5-HT, CGRP and ET decreased significantly after the treatment in both groups (P<0.05); after the intervention, the levels of 5-HT, CGRP and ET in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Acupuncture plus medication can effectively mitigate the symptoms in IBS-D (liver depression and spleen deficiency) and enhance the quality of life, which is possibly achieved by down-regulating the serum 5-HT, CGRP and ET levels.
3.The relationship between osteopontin plasma concentration and disease activity in systemic lupus erythematosus

Objective To investigate the relationship between plasma concentration of osteopontin (OPN) and disease activity in systemic lupus erythematosus (SLE).Methods The plasma concentration of OPN in 28 healthy controls and 38 SLE patients was measured by enzyme-linked immunosorbent assay. Results The plasma concentration of OPN was significantly higher in SLE patients compared to that in the healthy controls (420?203 ng/mL vs 73?14 ng/mL,P<0.001),in the patients with proteinuria com- pared to that in the patients without proteinuria (623?88 ng/mL vs 288?135 ng/mL,P<0.001),in the patients with active disease compared to that in those with inactive disease (529?143 ng/mL vs 185?66ng/mL,P<0.001),in the patients before treatment compared to that in the patients after treatment (556?130 ng/mL vs 142?22 ng/mL,P<0.001).The plasma concentration of OPN in patients with arthritis was not different from those without arthritis (489?153 ng/mL vs 375?222 ng/mL,P>0.05).Plasma concentration of OPN showed a significant positive correlation with systemic lupus erythematosus disease ac- tivity index (SLEDAI) (r=0.93,P<0.001),but a significant negative correlation with serum C3 level (r=-0.49,P<0.05).There was no correlation between plasma concentration of OPN and antinuclear an- tibody titre,erythrocyte sedimentation rate,or serum level of C4.Conclusions These data show that plas- ma OPN level has some correlation with the SLE activity,renal damage and progression or amelioration of SLE.OPN could be involved in the pathogenesis of SLE.
4.Vasodilation Effect and Underlying Mechanism of Active Fraction from Compound Prunella Vulgaris L. in I-solated Thoracic Aorta of Rats

Objective:To investigate the vasorelaxant effect of active fraction from compound Prunella vulgaris L. ( AFCP) on the i-solated thoracic aorta of rats and underlying mechanism. Methods:The study was performed with the tension experiment of the isolate rat thoracic aorta. The changes of vascular ring tension were measured by biological signal acquisition and analysis system, and the vasodila-tor effect of AFCP was observed. Results:AFCP(100-500μg·ml-1)could induce significant relaxation in aorta rings pre-contracted by phenylephrine (PE,1 μmol·L-1), and the relaxation effect was significant(75% ±8%) in endothelium-intact aortic and endothelium-denuded aortic. The vasodilatation effect of AFCP was not significantly affected by nitric oxide synthase ( NOS) inhibitor NG-nitro-L-argi-nine( L-NNA) , guanylate cyclase inhibitor MB,potassium channel blocker TEA and glibenclamide. In Ca2+-free bath solutions, AFCP (300 μg·ml-1 ) could shift downward dose-response curve of CaCl2 and significantly reduce the maximum contraction amplitude of PE. Conclusion:AFCP can relax rat aorta rings in a dose-dependent manner, which is endothelium-independent. The mechanism may be re-lated to the inhibition of intracellular calcium release and extracellular calcium flow, and has nothing to do with NO pathway, prostacyclin generation and calcium-activated potassium channels.
5.Nursing care and clinical observation of Levosimendan in decompensated heart failure

Objective To observe levosimendan-induced hemodynamic improvement of chronic decompensated heart failure patients and adverse effects,and sum up the nursing experience of Levosimendan.Methods Sixty-three patients with decompensated heart failure were treated with Levosimendan,based on optimized conventional treatment for heart failure.Aminoterminal pro-brain natriuretic peptide (NT-proBNP) and left ventricle ejective fraction (LVEF) were measured at baseline and one week after Levosimendan administration.In addition,we summarized the clinical observation,adverse effects and nursing experiences of treatment with Levosimendan in decompensated heart failure patients.Results Levosimendan increased 24-hour urine output in patients at the first day after treatment.This effect of levosimendan persisted for 7 days at least.We observed an improvement in LVEF and a reduction of NT-pro-BNP at 7 days after the initiation of levosimendan treatment.Clinical efficiency of levosimendan in patients was 84.1% in this study.Twenty two patients(34.9%) experienced different adverse events.Conclusions Levosimendan therapy can improve cardiac function and reduce BNP in patients with decompensated heart failure.The rate of adverse effects of Levosimendan is high.It is important to enhance therapy effect by careful nursing and observing variety state during treatment with Levosimendan.These data will be useful for the nursing care of Levosimendan in clinical practice.
6.Blood oxygenation level dependent functional MRI study on the changes of motor cortex in patients with amyotrophic lateral sclerosis

Objective To study the changes of motor cortex in patients with amyotrophic lateral sclerosis(ALS)while executing sequential finger tapping movement by using blood oxygenation level dependent(BOLD)functional MRI.Methods Fifteen patients with definite or probable ALS and 15 age and gender matched normal controls were enrolled in the BOLD study,and all the subjects were right-handed with no other diseases or any recent medication history.A 3.0 T MR scanner was employed and gradient echo EPI(GRE-EPI)sequence was used to acquire the functional images.Subjects executed sequential finger tapping movement at a frequency of 1-2 Hz during a block design task.fMRI data were analyzed by using statistical parametric mapping(SPM)2.Volume of activated brain areas was compared with the use of a Student's t-test.Results Bilateral primary sensorimotor cortex(PSM),bilateral posterior aspect of premotor area(PA),bilateral supplementary motor area(SMA),contralateral inferior lateral premotor area (ILPA),bilateral parietal region(PAR),and ipsilateral cerebellum showed activation in both ALS patients and normal controls when executing the same motor task.The activation areas in bilateral PSM and bilateral posterior aspect of PA(right hand ipsilateral activation:ALS(924.5±141.1)mm3,control(829.9±98.4)mm3,P=0.05;right hand contralateral activation:ALS(9143.8±702.8)mm3,control(8638.8±506.4)mm3,P<0.05;left hand ipsilateral activation:ALS(1162.5±357.4)mm3,control(902.5±184.2)mm3,P<0.05;left hand contralateral activation:ALS(8255.2±870.2)mm3,control (5934.6±616.4)mm3,P<0.05),bilateral SMA(right hand bilateral activation:ALS(6564.3±720.6)mm3,control(4710.7±416.3)mm3,P<0.05;left hand bilateral activation:ALS(6970.5±961.8)mm3,control(3688.9±672.3)mm3,P<0.05),and ipsilateral cerebellum(right hand ipsilateral activation:ALS(2720.0±1154.2)mm3,control(254.3±84.4)mm3,P<0.05;left hand ipsilateral activation:ALS(4794.4±1237.0)mm3,control(1689.0±719.6)mm3,P<0.05)were significantly larger in ALS patients than in normal controls.Extra activation areas including ipsilateral ILPA,contralateral cerebellum and bilateral posterior limb of internal capsule were only detected in ALS patients.Conclusions Similar activation areas were seen in both groups while executing the same motor task,but the activated areas were more prominent in ALS group.The increased activation areas in ALS patients may represent neural reorganization.while the extra activation areas in ALS patients may indicate functional compensation.
7.Career identity and satisfaction of family doctors in Pudong New Area of Shanghai municipality

Objective To survey the career identity and satisfaction of family doctors in Pndong New Area of Shanghai municipality.Methods Two hundred and thirty four family doctors were selected with random sampling method from 6 community health centers in Pudong New Area of Shanghai.The survey on career identity and satisfaction was conducted using a self-designed questionnaire.Results The results showed that 37.2% of participants were very satisfied (8.1%,19/234) or satisfied (29.1%,68/234) with their current career situation.The three lowest satisfaction factors were salary (14.1%,33/234),benefit and social insurance (27.3 %,64/234),professional reputation and social status (20.9% 49/234).Meanwhile,the three highest factors were quality of their leadeship (79.5%,186/234),interpersonal relationship(74.4%,174/234) and the institutional management (63.3%,148/234).The survey also showed that 44.5% and 30.6% of participants expected their monthly pay as 8 001-10 000 and 5 000-8 000 RMB; 39.1% thought the current performance evaluation mechanism as effective,while 25.7% thought the opposite.Conclusion The social status and salary level of family doctors need to be improved; there is also an urgent need to improve the performance evaluation mechanism and other supporting measures.
8.Effect of Hyperbaric Oxygen Combined with Acupuncture and Moxibustion on Traumatic Peripheral Facial Nerve Injury:A Electromyography Study

Objective To explore the effect of hyperbaric oxygen combined with acupuncture and moxibustion on traumatic peripheral facial nerve injury, and the results of electromyography. Methods From January, 2007 to January, 2013, a total of 160 patients with traumat-ic peripheral facial nerve injury were divided into treatment group (n=80) and control group (n=80). Both groups received routine drug treat-ment. Hyperbaric oxygen combined with acupuncture and moxibustion was applied to treatment group in addition. The clinical efficacy and the results of electromyography were analyzed before and 36 days after treatment. Results After treatment, the cure rate and total efficiency was significantly higher (χ2>7.657, P<0.001), the motor latency of facial nerve was shorter (t>2.214, P<0.05), the M wave amplitude of fa-cial nerve was significantly higher (t>3.116, P<0.01), the number of recruitment potential was more (χ2=9.76, P<0.05), and the motor unit potential voltage was significantly higher (t>5.45, P<0.001) in the treatment group than in the control group. Conclusion Hyperbaric oxy-gen combined with acupuncture and moxibustion is more effective on traumatic peripheral facial nerve injury than routine.
