2.Telomere, Telomerase and Tumor

Telomere is the end structure of chromosome and it will be shortened during replication. Telomerase is a reverse transcripatse consisting of both RNA and protein components and synthesizes telomeric DNA by copying the template sequence of its own RNA components to maintain telomere length for function.Telomerase activity in germline cells,immortal and neoplastic cells was detected,but not in mostly normal cells.The telomere-telomerase hypothesis was brung out to explain this phenomenon.According to this hypothesis,re-actived telomerase will maintain telomere's length for protecting chromosome to make the cell immortal.The aging procedure will be explained by this hypothesis too.This hypothesis provides a new concepts of cancer and cancer's treatment.
3.Clinical study on platinum-based chemotherapy In elderly patients with advanced non-small cell lung cancer

Objective To evaluate the efficacy and safety of platinum-based chemotherapy in elderly patients with advanced non-small cell lung cancer(NSCLC). Methods Clinical data of forty-one elderly patients (≥70 years) with advanced NSCLC who were treated by platinum-based chemotherapy were analyzed retrospectively. Results Among the 41 patients, 18 (43.9%) patientswere treated with vinorelbine, 9 (22.0%) patients with gemcitabine, 7 (17.1%) patients with paclitaxel and docetaxel respectively. The overall response rate was 19.5%. The median time to progression (MTTP) was 5.8 months, and 1-year survival rate was 65.8% with a median survival time (MST) of 14.2 months. No significant statistical differences were found among the four regimens. The major side effect was bone marrow suppression. Three patients withdrew chemotherapy after one cycle due to grade Ⅲ-Ⅳ myelosuppression. There was no death associated with chemotherapy. Conclusions The platinum-based combined chemotherapy is effective and well-tolerated in elderly patients with advanced NSCLC.
4.Evaluation of efficacy and safety of ZD1839 as monotherapy in Chinese patients with advanced non-small cell lung cancer

Objective To report the outcome of gefitinib for Chinese patients with advanced nonsmall cell lung cancer(NSCLC) at Peking Union Medical College Hospital. Methods From Oct. 2002 to Apr. 2006,204 patients with advanced NSCLC received oral ZD1839(250 mg/d)treatment. The were 110(59.9%)men and 94(40.1%)women aged between 25 and 85 years. Thirty-two patients had squamous cell carcinoma, 125 adenocarcinoma, 30 bronchoalveolar carcinoma or adenocarcinoma with partial bronchoalveolar carcinoma, 6 adenosquamous carcinoma. and 11 unspecified. Twenty-six patients had no history of chemotherapy, 62 had no disease progression after chemotherapy, and 111 failed to prior one or more regimens. Median survival was calculated using the Kaplan-Meier method and a Cox regression analysis was used to detect differences in median survival between strata. Results The median survival of all patients and of patients failed to prior chemotherapy were 16.3 months(95% confidential interval CI, 14.5-18.2)and 12.5 months(95% CI 9.3-15.7). The rate of 1-year survival was 57%. The obiective tumor response rate and stable disease rate were 31.4% and 41.7% respectively.The median survival were significantly related with ECOG scores,pathology types,disease progression after chemotherapy,objective efficacy of gefitinib and changes of short-breathing. Among 26 patients with no prior chemotherapy,the median survival was not statistically significant compared with that of other patients. Among the enrolled patients,111 had disease progression and 62 had stable disease after prior chemotherapy, and their median survivals was statistically different. At the time of this analysis. 142 patients had disease progression,58 of whom withdrew from taking gefitinib,and 84 continued gefitinib therapy until death. The median survivals for these subgroups were not significantly different. Among 142 patients with disease progression, 40 received other systemic treatment, the median survival was statistically significant compared with that of other patients. Objective response was significantly related with age,smoking status,pathological type,change of short-breathing and rashes induced by gefitinib. Adverse events were generally mild(grade 1 and 2)and reversible. The most frequent adverse events were rash 72.6%(138/190)and diarrhea 33.7%(64/190).Conclusion Our studv suggests that treatment with gefitinib may be well tolerate and beneficial for some Chinese patients after failure of prior chemotherapy.
5.The efficacv and safety of pemetrexed as monotherapy for Chinese patients with advanced non-small cell lung cancer

Objective To assess the outcome of pemetrexed as monotherapy for Chinese patients with advanced non-small cell lung cancer ( NSCLC) at the Peking Union Medical College Hospital.Methods From February 2006 to August 2009, 69 patients with advanced NSCLC, including 36 (52.2% ) men and 33 (47.8%) women, received pemetrexed monotherapy.Six patients had squamous cell carcinoma, 57 adenocarcinoma, 6 unspecified.Median survival (MS) and progress-free survival (PFS) were calculated using the Kaplan-Meier method.Results Adverse events (AEs) were generally mild (grade 1 and 2) and reversible. The most frequent AEs were gastrointestinal adverse events, skin rash, fever and fatigue. Total 5 cases with 3/4 grade AEs were reported, including fever, fatigue, rash, decreased platelets and elevated ALT/AST. The objective tumour response rate and stable disease rate were 10.1% (7/69) and 47.8% (33/69) respectively. Median PFS of all patients was 4.7 months (95% CI 3.0 -6.4) and mean overall survival was 14.7 months (95% CI 11.5 - 17.9).Conclusions Our study suggests that treatment with pemetrexed may be well-tolerated and beneficial for some Chinese patients after failure of prior chemotherapy. The rate of disease control is high and the frequency of 3/4 grade AEs is very low.
6.Comparative study of different dosages of dexmedetomidine combined with fentanyl for monitored anesthesia care during endoscopic variceal ligation

Objective To investigate the ideal dosage of dexmedetomidine( DEX) with 1?0 μg/kg fentanyl for monitored anesthesia care ( MAC) during endoscopic variceal ligation ( EVL) . Methods A total of 60 patients scheduled for elective EVL were randomly divided into 3 groups ( n=20) . After fentanyl was infused intravenously at the dosage of 1?0 μg/kg, the loading dosage of DEX at 1?0 μg/kg ( group D1 ) , 1?5 μg/kg ( group D2 ) , or 2?0μg/kg ( group D3 ) was continuously infused in 10 min, respectively. When the modified OAA/S score reaching≥3 point, EVL was carried out. The change in modified OAA/S score, the operation duration time, recovery time, satisfaction rates of patient and doctor, and complications were recorded. Results There were no significant differences in regarding of general status, operation duration time and satisfaction score of patients between the 3 groups ( P>0?05 ) . Before endoscope insertion, the OAA/S score in group D3(4?4±0?2)was higher than that in D1(3?4±0?5)or D2 groups(3?8±0?3)(P<0?05), and there was no significant difference between the scores of D1 and D2(P>0?05).At the time?point of 5 mins after endoscope insertion, the OAA/S score in group D3(4?5±0?3)was significantly higher than that in D1(3?5±0?6)or D2 groups(3?7±0?4)(P<0?05),there were no significant differences between D1 and D2( P>0?05) . At the end of the procedure,there was no significant difference in OAA/S score between the 3 groups(P>0?05).Compared with group D1(3?1±0?9)min and D2(3?8±0?8)min, the recovery time in group D3(6?6±1?2)min was significantly longer (P<0?05), while there was no significant difference between D1 and D2(P>0?05). The satisfaction score of endoscopist in group D1(8?0±0?8) was significantly lower than that in group D2(9?4±0?6)or D3(9?5±0?5)(P<0?05), there was no significant difference between groups D2 and D3 ( P>0?05 ) . No tachycardia, hypertension or hypoxemia occurred during the procedure. There was no significant difference in rate of hypotension among the three groups ( P>0?05) . The incidences of nausea(30%) and body movement(15%) in group D1 were significantly higher than those in group D2 and D3(P<0?05).There were no differences between D2 and D3(P>0?05). The incidence of bradycardia in group D3(40%) was significantly higher than those in group D1(0) and D2(10%)(P<0?05).There were no differences between those in D1 and D2(P>0?05). Conclusion Combined with 1?0 μg/kg fentanyl, 1?5 μg/kg DEX is more efficient and safer for EVL in the status of MAC.
7.Determination of five compounds in Scrophularia ningpoensis by HPLC-UV-ELSD.

OBJECTIVETo develop an HPLC-UV-ELSD method for the determination of aucubi, harpagide, harpagoside, angoroside C and cinnamic acid in Scrophularia ningpoensis.
METHODThe analytical column was SHIMADZU C18 (4.6 mm x 250 mm, 5 microm). The mobile phase was acetonirile-0.4% acetic acid in a gradient elution. Initial conditions was 5% A; 0-20 min, changed to 10% A; 20-50 min, to 55% A. The flow rate was 0.8 mL x min(-1) and the column temperature was 30 degrees C. The UV detector wavelength was set at 280 nm for harpagoside, angoroside C and cinnamic acid, and the evaporative light-scattering detector (ELSD) drift tube temperature was 105 degrees C, the flow rate of nebulizer gas was 1.2 L x min(-1) for aucubi and harpagide.
RESULTAucubi, harpagide, harpagoside, angoroside C and cinnamic acid was separated well. The linear calibration curves were obtained over of 0.752-13.536 microg for aucubi (r=0.9993, n=6), 0.8280-14.90 microg for harpagide (r=0.9994, n=6), 0.6360-11.45 microg for harpagoside (r=0.9997, n=6), 0.5440- 9.792 microg for angoroside C, (r=0.9997, n=6) and 0.0108-0.1939 microg for cinnamic acid (r=0.9999, n=6). The mean recovers of five compounds were 98.12%, 99.14%, 100.21%, 98.17% and 100.35% with RSD of 2.3%, 1.5%, 1.9%, 1.7% and 0.5%.
CONCLUSIONThis method could simultaneously determinate the content of the five compounds in the S. ningpoensis.

8.Risk factors for positive resection margins after endoscopic submucosal dissection of early esophageal squamous carcinomas and precancerous lesions

Objective To identify the risk factors for positive resection residues after endoscopic submucosal dissection ( ESD ) of early esophageal squamous carcinomas and precancerous lesions. Methods A retrospective analysis was performed in 315 patients with early esophageal squamous cancer and precancerous lesion who underwent ESD. The pathological features of all resection margins in the specimen and the follow?up outcome of the patients with positive resection margin were evaluated. Univariate and multi?variate analysis were used to determine the risk factors for resection margin residues after ESD. Results In 315 lesions,there were 290 lesions with negative resection margins and 25 with positive resection margins.The number of lesions with positive lateral, basal, or both resection margins was 13, 8, and 4, respectively. Multivariate analysis showed that the depth of invasion( submucosal layer invasion, P=0?048) was the only independent risk factor for positive basal resection margin. The proportion of circumferential extension (≥3/4,P=0?014) and the depth of invasion( exceeding muscularis mucosa, P=0?007) were independent risk factors for positive lateral resection margin. Conclusion The diameter of the lesions and the depth of tumor invasion are independent risk factors for esophageal ESD positive resection margins. Accurate evaluation of lesion extension and invasive depth is critical to avoid residual or recurrent tumor after esophageal ESD.
9.Performance optimization of property-improved biodiesel manufacturing process coupled with butanol extractive fermentation.

The products concentrations in traditional acetone-butanol (AB) fermentation are too low that large amount of energy has to be consumed in the distillation and product recovery process. Aiming at direct utilization of the fermentation products, in this study, optimization of property-improved biodiesel manufacturing process coupled with AB extractive fermentation was conducted, under the condition of using the biodiesel originated from waste cooking oil as the extractant and high concentrated corn flour medium. The effect of biodiesel/broth volume ratio, waste supernatant recycle ratio, and electronic carrier addition on the major process performance index was carefully investigated. Under the optimized condition, the biodiesel quality was improved with the cetane value increased from 51.4 to 54.4; "actual butanol yield" reached to a level of 18%, and waste supernatant recycle ratio exceeded 50%. In this way, elimination of energy-consuming product recovery process and realization of "energy-saving & waste minimization" industrial production target advocated by the state government, could be potentially expected.

10.Granulocyte colony-stimulating factor mobilized bone marrow stem cells treat the acute myocardial infarction

AIM: To investigate the effects of granulocyte colony-stimulating factor (G-CSF)-mobilized bone marrow stem cells on treatment of the myocardial infarction in experimental rats. METHODS: Three hours after injected with isoprenaline(ISO) interaperitoneally to develop acute ischemic model, rats' bone marrow stem cells were mobilized by G-CSF and migrated to the site of myocardial infarction. The hearts were harvested from 24 hours to 2 weeks after administration of ISO for histopathological examination. RESULTS: 24 hours after administration of ISO , myocardial infarct zones scattered in the pallium of the control group ,there were a large amoumt of inflammatory cells infiltration around the infarct zones and majority of them were neutrophils. The infarction in the G-CSF treatment group was milder, majority of the infiltrative cells were monocytoid; 48 hours after administration of ISO, infarct zones expanded greatly in control group, while that of the G-CSF treatment group increased just mildly; 2 weeks after administration of ISO, there was no significant scar in the G-CSF treatment group. We also found the regeneration of myocytes in the pallium. CONCLUSION: G-CSF treatment protected the ischemic myocardium and it may be used to treat the acute myocardial infarction.
