1.Study on Quality Control of Ethyl-acetate Parts ofFerula Sinkiangensis

Objective To establish the method for quality control of ethyl-acetate parts of Ferula sinkiangesis. Methods HPLC was used to detect the contents of ferulic acid, farnesiferol A and farnesiferol C. Waters XTerra RPC18 column (250 mm×4.6 mm, 5 μm) was used; acetonitrile-0.1% phosphoric acid was as mobile phase with gradient elution; flow rate was 1.0 mL/min; detective wavelength was 324 nm; temperature was 30℃; sample volume was 10 μL ResultsFerulic acid, farnesiferol A, and farnesiferol C showed a good linear relationship range from 0.05– 1.0 mg/mL, 0.132–2.64 mg/mL, and 0.118–2.36 mg/mL, respectively. The average recovery rates were 99.34%, 98.96% and 99.24% respectively. The contents of ferulic acid, farnesiferol A and farnesiferol C were 33.4, 76.5, 72.3 mg/g respectively.Conclusion The method was simple, accurate and repeatable, with good repeatability and stability, which can be used for the quality control of ethyl-acetate parts ofFerula sinkiangesis.
2.Study on Anti-oxidant Activity Parts of Ferula sinkiangensis K. M. Shen

Objective To establish a high-throughput detection of medicine on DPPH free radical scavenging activity;To analyze and preliminarily screen the anti-oxidant activity part of Ferula sinkiangensis. Methods Taking DPPH free radical scavenging rate of ascorbic acid as positive control, and IC50 as the evaluating criterion of DPPH free radical scavenging capability, oxidation resistance of different polar parts (petroleum ether part, ethyl acetate part, methyl alcohol part, and water part) of Ferula sinkiangensis was evaluated and screened. Results Different polarity parts of Ferula sinkiangensis DPPH radical scavenging of IC50 were:petroleum ether part 3252.22 μg/mL, ethyl acetate part 36.22 μg/mL, methyl alcohol part 32.22 μg/mL, water part 2643.38 μg/mL, and positive control group ascorbic acid 27.16 μg/mL. Conclusion The ethyl acetate and methyl alcohol parts of Ferula sinkiangensis were effective anti-oxidant activity site to eliminate DPPH free radicals. In this study, a simple, sensitive, and high-throughput detection method of DPPH radical scavenging assay was established to provide reference for the anti-oxidation medicine detection screening.
3.Application of Fuzzy Comprehensive Evaluation in Formulation Screening

OBJECTIVE:To investigate the application of fuzzy comprehensive evaluation in formulation screening. METH-ODS:Taking Compound paracetamol and chlorphenamine maleate granules as model drug,based on single factor experiment,L9(34) orthogonal test was used to screen the formulation with the amount of aspartame,hawthorn powder essence,milk powder essence and saccharose as factors,using dissolution rate of caffeine as index. The granules prepared by 9 kinds of formulation in orthogonal test were evaluated with fuzzy comprehensive evaluation in terms of taste,aroma,color and dissolubility. The optimized formula-tion was validated and compared with original formulation. RESULTS:The formulation 5 was the optimal choice in both orthogo-nal test and fuzzy comprehensive evaluation,and obtained same results. Compared with sensory comprehensive score (63.12) and dissolution rate of caffeine (91.3%) in original formulation,two indicators of optimized formulation in 3 tests were 84.00 and 99.07% in average (RSD<2.0%,n=3). CONCLUSIONS:Fuzzy comprehensive evaluation can be used for the formulation screening of sensory evaluation index. It is scientific and accurate.
4.Efficacy comparison between two-field and three-field lymphadenectomy for thoracic esophageal squamous cell carcinoma.

OBJECTIVETo compare the safety and efficacy between three-field lymphadenectomy and normative Ivor-Lewis two-field lymphadenectomy for thoracic esophageal squamous cell carcinoma METHODS: Clinical data of 375 patients with thoracic esophageal squamous cell carcinoma who underwent three-field lymphadenectomy(3FL) or Ivor-Lewis two-field lymphadenectomy(2FL, Ivor-Lewis) in Fudan University Shanghai Cancer Center during 2013 were retrieved and collected from electronic medical record system. Ninety-one patients received three-field lymphadenectomy (3FL group), including 16 cases of intra-cervical gastro-esophageal anastomosis and 75 cases of intra-thoracic gastro-esophageal anastomosis, while 284 patients received Ivor-Lewis two-field lymphadenectomy (2FL group) with all intra-thoracic gastro-esophageal anastomosis. Short-term outcomes were compared between two groups, including postoperative anastomotic leakage, pneumonia and respiratory failure, chylothorax, reoperation and 90-day death. Total harvested lymph nodes and positive lymph nodes in each group were also compared. A total of 338 patients were enrolled into survival analysis. Survival curve was presented by Kaplan-Meier method.
RESULTSAs compared to 2FL group, the 3FL group had significantly higher ratio of N3 patients [19.8% (18/91) vs. 5.3% (15/284), P=0.000], stageIII( patients [58.2%(53/91) vs. 43.0%(122/284), P=0.007], and upper thoracic cancer patients [12.1%(11/91) vs. 3.5%(10/284), P=0.027]; also the 3FL group had more harvested lymph nodes (40.1±14.6 vs. 25.3±9.4, P=0.000) and more positive lymph nodes (3.3±4.0 vs. 1.7±3.2, P=0.000). With respect to pneumonia and respiratory failure, chylothorax, reoperation and 90-day death, no significant differences were found between the group (P=0.447, P=0.751, P=0.678, P=0.685). The 3FL group had a significantly higher incidence of anastomotic leakage than 2FL group [7.7% (7/91) vs. 1.8% (5/284), P=0.011], while its incidence of intrathoracic anastomosis leakage was 4.0% (3/75), which was not significantly different with 1.8%(5/284) of 2FL group (P=0.372). Median follow-up was 33 months. Overall 1-, 2-, 3-year survival rates were 94%, 81% and 70%, while 1-, 2-, 3-year survival rates of 3FL group were 90%, 73% and 66%, of 2FL group were 95%, 84% and 72%, respectively, without significant differences between the two group(P=0.135). Further subgroup analysis showed that no significant differences of postoperative survival in stage I(, II( and III( patients were observed between the two groups (P=0.541, P=0.511, P=0.402), meanwhile no significant differences of postoperative survival in patients with metastasis and without metastasis were found between the two groups as well (P=0.985, P=0.233).
CONCLUSIONSThree-field lymphadenectomy can be performed with acceptable perioperative morbidity and mortality. The prognosis value of three field lymphadenectomy needs further investigation. Patients with thoracic esophageal squamous cell carcinoma may have favorable survival through normative Ivor-Lewis two-field lymphadenectomy.

5.Sacral neuro modulation for chronic transit constipation: a clinical analysis of 10 cases

Objective:To assess the clinical effects of sacral nerve regulation therapy for patients with chronic transit constipation.Methods:A total of 10 patients were treated with sacral nerve regulation therapy at our department from Jul 2017 to May 2018. The efficacy was assessed by bowel habit diary, Wexner constipation scores, constipation patients' quality of life scale and gastrointestinal quality of life index.Results:Nine patients were implanted with permanent sacral nerve stimulator with the analysis of constipation patients' quality of life scale, the postoperative symptoms were significantly improved compared with those before operation ( P<0.01), and the symptoms of physical discomfort, psychological discomfort, worry and anxiety were also significantly improved after operation, with statistical significance (all P<0.01).The Wexner constipation score was significantly higher after operation ( P<0.01). Conclusion:SNM is a clinically safe, efficacious, simple, minimally invasive and reversible new technique for the treatment of chronic transit constipation.
6.Analysis on adverse event in acupotomy therapy based on literature research.

OBJECTIVE:
To explore the current domestic situation of the adverse event in acupotomy so as to promote the standardization of acupotomy and improve the clinical effect and safety of acupotomy therapy.
METHODS:
The method of retrospective journal researching was used. The case report about the adverse event in acupotomy from CNKI, WANFANG and VIP databases was retrieved and the types, the features, the causes, the numbers of the papers and the cases were analyzed.
RESULTS:
Fifty-eight papers were included, with total 343 cases. It was indicated that the adverse events in acupotomy were mainly postoperative local pain, hematoma, swelling, nerve damage, tendon rupture, adhesion, injury, infection, syncope, anesthesia accident, undeserved hormone use, etc. The lower level of standardization of acupotomy in the clinical use may lead to adverse events.
CONCLUSION
Safe acupotomy treatment for some complicated diseases and dangerous parts remains to be improved, and the standardization of the acupotomy is an important approach to improve its safety. The related work needs to be carried out.
