1.Influence of preoperative biliary drainage on morbidity rate of severely obstructive jaundice patients after pancreaticoduodenectomy

Objective:To investigate the influence of preoperative biliary drainage (PBD) on morbidity of severely obstructive jaundice patients after pancreaticoduodenectomy (PD).Methods:A total of 98 severely obstructive jaundice(Serum total bilirubin>300 μ mool/L) patients underwent PD between February 2010 and October 2015 were enrolled in the study.The patients were divided into two groups based on undergoing PBD or not.The no-PBD group comprised 52 patients and the PBD group comprised another 46 patients.Perioperatives parameters,including operative time,intraoperative blood loss,postoperative mortality and morbidity and postoperative hospital stay were compared between the two groups.Results:The demographics,preoperative examinations and pathological results were similar between the two groups (P>0.05).Operative time of the no-PBD group was statistically longer than the PBD group (379.44 ± 88.57min vs 346.98 ± 57.17 min,P<0.05).Besides,intraoperative blood loss of the no-PBD group were much more than the PBD group (365.00 ± 187.07mL vs 297.83 ± 139.57 mL,P<0.05).There was no statistical difference of mortality rate between the no-PBD group and the PBD group(3.85% vs 2.17%,P>0.05).The overall morbidity rate of the 2 groups were similar (53.85% vs 43.48%,P>0.05),but the pancreatic fistula rate of no-PBD group was significantly higher than the PBD group (30.77% vs 13.04%,P<0.05).Conclusion:PBD could reduce operative time,intraoperative blood loss and pancreatic fistula rate after PD.Meanwhile,the mortality and overall morbidity rates were similar between the two groups.PBD should be considered for severely obstructive jaundice patients.
2.The influence of sublingual immunotherapy on quality of life in children with allergic rhinitis.

To analyze the impact of sublingual immunotherapy(SLIT)on the quality of life in children with allergic rhinitis.Fifty children with allergic rhinitis who have received sublingual immunotherapy were enrolled in this study.Quality of life was evaluated via measurement of VAS score and rhinoconjunctivitis quality of life questionnaire(RQLQ)before and after treatment.Twenty patients after treatment had complete remission,13 cases were partly alleviated and 17 cases had no response.The total effective rate was 66%.The Multi-VAS scores and Uni-VAS scores in each observation time point(at half a year,one year,two years after treatment)had statistically significant difference compared with that of pre-treatment with SLIT(<0.05).According to RQLQ scores,the quality of life,nasal symptoms,conjunctiva symptoms,non-nasal(ocular)symp-toms,behaviors and emotional responses were greatly improved in each time point compared with that of pretreatment(<0.05).Symptomatic treatment scores in each time point after treatment were significantly different and had a positive correlation with the scores of RQLQ(<0.05).SLIT can improve the nasal allergic symptoms,children's life quality and reduce the use of symptomatic treatment medicines.

3.Clinical evaluation of retention strength of two different adhesive systems on glass fiber-reinforced posts

Objective:
To evaluate the clinical effect of two different resin cements on the glass fiber-reinforced posts.
Methods :
One hundred and thirty-six teeth were randomly divided into two groups, with sixty-eight in each. Group A used Multilink N self-etch system and group B used RelyX Unicem self-adhesive system to bond fiber posts. Follow-up examinations took place at 6, 12 and 24 months after the placement.
Results :
The success ratios of two groups were all 97% above and there was no significant difference between two groups (P > 0.05).
Conclusion
Multilink N self-etch system and RelyX Unicem self-adhesive system all have good effect on glass fiber-reinforced posts bonding.
4.Effect and Mechanism of Zacopride on Vasodilatation of Isolated Coronary Arterial Rings in Experimental Rats

Objective: To investigate the effect with its possible mechanisms of zacopride on vasodilatation of isolated coronary arterial rings in experimental rats.
Methods: The tension of vasodilatation of isolated coronary arterial rings of male SD rats was recorded by Powerlab and DMT system. The rats were divided into 4 groups: +Endo (vehicle) group, +Endo (zacopride) group and -Endo (vehicle) group, –Endo (zacopride) group.n=6 in each group. The vasodilatation effects of zacopride on KCl (60 mmol/L) and U46619 (10-6 mol/L) pre-constricted arterial ring were recorded; the effects of different agents on zacopride caused vasodilatation were studied.
Results: In both +Endo (zacopride) and –Endo (zacopride) groups, zacopride showed a dose dependent vasodilatation effect on coronary ring pre-constricted by KCl and U46619. The maximum vasodilatation effect of zacopride in KCl treated+Endo (zacopride) group was (90.15 ± 6.38) %, in U46619 treated-Endo (zacopride) group was (81.67 ± 4.97 ) %; the maximum vasodilatation effect of zacopride in KCl treated-Endo (zacopride) group was (85.48±5.04) %, in U46619 treated–Endo (zacopride) group was (79.65 ± 3.51) %, compared to each corresponding vehicle group, allP<0.05. The inhibitor of IK1 channel, BaCl2 could signiifcantly reduce the vasodilatation effect of zacopride in KCl and U46619 pre-constricted coronary ring,P<0.05. However, the inhibitor of eNOS (L-NAME), the blocker of KCa channel (TEA), blocker of Kv channel (4-AP) and blocker of KATP channel (Glib) had no such signiifcant effects, allP>0.05.
Conclusion: Zacopride had vasodilatation effect on coronary arterial ring which was pre-constricted by KCl and U46619, which might be related to the channel of IK1.
5.Effects of MS23 on KCl~-,?-adrenoceptor agonists-and U46619-induced contractions of arteries isolated from rat different organs

Aim To study the vasodilation of MS23,a brand new phosphodiesteras inhibitor,on contractions induced by various stimuli in rings of arteries isolated from rat different organs.Method Tension of aortic and microvessel rings were recorded isometrically by PowerLab and DMT system respectively.Results MS23 concentration-dependently shifted the noradrenaline(NA)-induced concentration-contraction curves rightward in a non-parallel manner with the maximal contraction depressed by 74.7%.MS23 and aminophylline(Ami) produced concentration-dependent relaxation on KCl or NA-induced precontraction.Endothelium deprivation and NO synthesis inhibition induced by L-NAME failed to affect the relaxation.MS23 and Ami relaxed KCl-induced precontraction of rat coronary,middle cerebral,renal and mesenteric arterial rings in a concentration-dependent manner,and showed no organ preference in this respect.Conclusion MS23 antagonizes and relaxes contractions induced by various stimuli in the rings of arteries isolated from different organs of rat without marked preference among the organ origin of artery and stimuli.The vasorelaxation induced by MS23 is related neither to endothelium nor to nitric oxide synthesis.

7.Clinical analysis of the treatment of maxillary odontogenic cyst by nasal endoscope fenestration through nasal base.

Objective:To investigate the feasibility and clinical effect of the surgical approach and method of transnasal fenestration under nasal endoscope for the treatment of maxillary odontogenic cyst. Methods:The clinical data of 23 cases with maxillary odontogenic cysts treated by nasal endoscopy through nasal fenestration were retrospectively analyzed. All cases underwent nasal endoscopy and CT examination before the operation. The mucosal membrane of the parietal wall of the cyst was excised through fenestration of the nasal base. The cyst fluid was removed by decompression, and the bony opening of the nasal base was trimmed and enlarged to the edge of the cyst. The intraoperative and postoperative effects were observed. Results:All cases were well exposed under the direct vision of nasal endoscope. The top wall of the cyst was removed to maximize the communication between the cyst cavity and the nasal floor. There were no complications such as nasolacrimal duct injury, turbinate atrophy, necrosis, and facial numbness. All patients were followed up for 6-12 months, and the clinical symptoms gradually disappeared after surgery. The inferior turbinate was in good shape, the cyst cavity was smooth, the cyst wall was determined, and no cyst recurrence was observed. Conclusion:The treatment of odontogenic cyst of maxilla under nasal endoscope through nasal fenestration is convenient. It has less trauma, fewer complications and a satisfactory curative effect, which is worthy of clinical promotion.

8.Clinical analysis of craniocerebral trauma complicated with thoracoabdominal injuries in 2165 cases.

OBJECTIVETo explore the optimal treatment for craniocerebral trauma complicated with thoraco-abdominal injuries.
METHODSA total of 2165 cases of craniocerebral trauma complicated with thoraco-abdominal injuries admitted to our hospital between July 1993 and June 2003 were retrospectively studied. Among them, 382 cases sustained severe craniocerebral trauma (in which 167 were complicated with shock), 733 thoracic injuries, 645 abdominal injuries and 787 thoraco-abdominal injuries. On admittance, 294 cases had developed shock. With the prime goal of saving life, respiratory and circulatory systems and encephalothilipsis were especially treated and monitored. Priority in management was directed to severe or open injures rather than to moderate or closed injures. For cases with cerebral hernia due to intracranial hematoma and severe shock due to blood loss, cerebral hernia and shock were treated concurrently.
RESULTSAfter treatment, 2024 (93.49%) cases survived and the other 141 (6.51%) died. Among patients who had severe craniocerebral injury with shock and those without, 78 (46.71%) and 53 (24.56%) died, respectively. For patients who had underwent craniocerebral and thoraco-abdominal operations concurrently and those who had not, the death rates were 58.49%-65.96% and 28.57% respectively, indicating a significant difference (P<0.05).
CONCLUSIONSTreatment for hematoma hernia, shock and disturbed respiration is the key in the management of multiple trauma of craniocerebral, thoracic or abdominal injuries, especially when two or three conditions occurred simultaneously. Unless it is necessary, operations at two different parts at the same time is not recommended. It is preferred to start two concurrent operations at different time.

9. Pollution characteristics and risk assessment of carcinogenicity or non-carcinogenicityonheavy metalsin Particulate Matter 2.5 in Shenzhen

Objective:
To assess the pollution characteristics and risk assessment of carcinogenicity or non-carcinogenicity on heavy metals in PM2.5 in Shenzhen.
Methods:
PM2.5 samples were collected monthly from the year of 2014 to 2015, and analyzed by seasons. 12 heavy metal elements (Pb, Hg, Mn, Sb, Al, As, Be, Cd, Cr, Ni, Se, Tl) in PM2.5 were detected by ICP-MS spectrometry. Health risk assessment was conducted using the recommended United States Environmental Protection Agency (USA EPA) model.
Results:
The median of PM2.5 concentration was 45.10 μg/m3 in Longgang district of Shenzhen. The non-carcinogenecity risks of the metals in PM2.5 existed in spring, autumn and winter (HQ>1). Three metal elements including As, Mn and Cd have higher HQ levels. The carcinogenecity risk levels in four seasons were winter, autumn, spring and summer, respectively. The carcinogenecity risks in four seasons were between 10-6 to 10-4. As, Cr and Cd have higher carcinogenicityrisks.
Conclusion
The heavy metals in PM2.5 have both carcinogenecity risk and non-carcinogenecity risk to residents in Longgang district of Shenzhen, the occupational health management must be continuously strengthened, the further research and the measures for prevention and control should be considered.

Objective:
To investigate the association between single nucleotide polymorphisms (SNP) in cytokine IL6, IL10 genes and the susceptibility to primary hepatic carcinoma(PHC) of Shunde district in Guangdong Province.
Methods:
Patients from two hospitals in Shunde District of Foshan City were selected from October 2010 to October 2012. Case group inclusion criteria includedprimary liver cancer; local residents of Shunde or living in Shunde for more than 10 years. The control group inclusion criteria included: patients visited ENT, general surgery and physical examination department in the same hospital during the same period; Local residents or living in Shunde for 10 years and above. The control group was matched 1∶1 by gender, and age (±3 years old) with case group. A total of 306 subjects were collected. Questionnaires were used to investigate the information including demographic characteristics, PHC status survey and so on. The venous blood was collected from each subject to extract DNA, and to detect label SNP site and genotype. Hardy-Weinberg equilibrium was detected in the control group by the goodness-of-fit χ2 test. Multivariate conditional logistic regression was used to estimate the relationship between IL6 (rs1800796), IL10 (rs1800871, rs1800872)genes polymorphisms and susceptibility to PHC.
Results:
