2.A long term follow-up study on the nonsurgical therapy of abdominal aortic aneurysm in senile patients

Objective To study the course of development of aneurysm of abdominal aorta (AAA) with nonsurgical treatment in senile patients. Methods 37 subjects were included in the study, and they were divided into 3 groups according to the size of aneurysm observed at the first visit. The diameter of aneurysm was measured, and the measurement was repeated at periodic follow up. The yearly increase of the diameter was calculated and analyzed. Results For the 37 patients, the AAA mean diameter was 4.68cm at the first visit. The period of follow-up was 0.5 to 11 years, with a mean of 6.1 years. The average yearly increase in tumor diameter was 0.47cm/year. 14 patients with aneurysm diameter of 6.0cm had an average increase of 0.67cm/year. 5 patients died during the follow-up period, 2 of them died of rupture of the aneurysm, the diameter of which was 8.35cm and 8.91cm, respectively. Endoaneurysmorrhaphy with stent was performed in 7 patients. Conclusion The size of AAA in the senile patients increased slowly if the aneurysm diameter is 6.0 cm, the tumor size might increase quickly, and active intervention is recommended.
4.The relationships between Ultrasonographic Features and the Expression of estrogen receptor and progesterone receptor in Postmenopausal Women with Infiltrating Ductal Carcinoma of Breast

Objective To investigate the relationships between of 2-dimensional (2D) and color Doppler ultrasonographic features and the expressions of both estrogen receptor (ER) and progesterone receptor (PR) in postmenopausal women with infiltrating ductal carcinoma (IDC) in breast, and to evaluate the feasibility of predicting the expressions of ER and PR in IDC by means of ultrasonographic features. Methods The clinical data of 96 patients with breast infiltrating ductal carcinoma diagnosed by pathology during Jun. 2002 to Jun. 2007 were retrospectively analyzed. The correlation between ultrasonographic features and both ER and PR expressions in the mastectomy specimens determined by immunohistochemical technique was statistically analyzed. Results Of the 96 cases, the positive expression rates of ER and PR were 54.17% and 50%, respectively, the bi-positive and bi-negative expression rates of ER and PR were 41.67% and 37.5%, respectively. 5.Influence of sufentanil preconditioning on early cognitive function and myocardial protection in patients undergoing on-pump CABG

Objective:To observe influence of preoperative sufentanil delayed preconditioning (SPC) on perioperative cogni‐tive function and myocardial protection effect in patients undergoing on -pump coronary artery bypass grafting (OP‐CABG) .Methods:A total of 60 patients undergoing selective OPCABG were randomly and equally divided into sufentanil group (SPC group) and routine treatment control group (control group) ,blood sample was collected before operation ,1h , 6h ,12h ,24h and 48h after aorta declamping to measure level of cardiac troponin I (cTnT );and all patients received neu‐ropsychological assessment before and 7d after operation .Results:Compared with control group ,there was significant re‐ductions in serum cTnI levels [1h:(23.66 ± 3.15) ng/ml vs .(14.96 ± 2.13) ng/ml] in sufentanil group from 1h after aor‐ta declamping to 48h after CABG ,P<0.05 all .The inotropic agent usage score on 12h ,24h and 48h after entering ICU of sufentanil group was significantly lower than that of control group [12h:(6.4 ± 0.6) scores vs .(8.4 ± 0.6) scores] , P<0.05 all .Incidence rate of postoperative cognitive dysfunction (POCD) in sufentanil group was significantly lower than that of control group (13.3% vs .30.0% ,P=0.041) .Conclusion:Preoperative sufentanil delayed preconditioning posses‐ses myocardial protection effect , and reduce incidence rate of cognitive dysfunction so also possesses certain brain protection in patients undergoing on-pump coronary artery bypass grafting .
6.Investigation on outbreaks of acute respiratory tract infection caused by respiratory syncytial virus in kindergartens in Hangzhou

Objective:
In order to analyze the characteristics of the outbreak of acute respiratory tract infection in children caused by respiratory syncytial virus(RSV).
Methods:
The field epidemiological investigations were conducted for the two outbreaks in kindergartens in Hangzhou. Data were analyzed by descriptive method. Samples with positive respiratory syncytial virus nucleic acid were sequenced using PCR.
Results:
The two outbreaks occurred in kindergartens. There were 21 cases in kindergarten A, lasting 11 days, and 43 cases in kindergarten B, lasting 33 days. The epidemic curve showed a proliferation pattern. The cases were concentrated in nurseries and K1 classes, primarily among children aged 2-4 years. The most common symptoms were fever and cough, mainly upper respiratory tract infection, and no severe cases were found. Upper respiratory tract samples were collected and detected as positive for RSV. Four samples were sequenced and identified as subgroup B.
Conclusion
During the outbreak of acute respiratory infection in kindergartens, respiratory syncytial virus should be given primary consideration in the process of identification of the outbreak caused by other respiratory infections, and strictly control measures should be taken to reduce the long term impact of the epidemic.
