1.Therapeutic effect of modified S-P approach without disconnecting the rectus femoris in patients with Pipkin typeⅠandⅡfractures

Objective To investigate the therapeutic effect of modified S-P approach without disconnecting the rectus femoris in patients with Pipkin typeⅠandⅡfractures. Methods The clinical data of 58 patients with Pipkin type Ⅰ and Ⅱfractures were retrospectively analyzed. Among the patients, 29 patients were treated with conventional S-P approach (conventional group), and 29 patients were treated with modified S-P approach (modified group). The incision length, operation time, bleeding, visual analogue score (VAS), postoperative complications and hip joint function at the final follow-up were compared between 2 groups. Results The patients of 2 groups had not incision infection, and showed primary. X-ray of 3 months after surgery showed fracture healing. There was no statistical difference in the time of follow-up between conventional group and modified group:(16.1 ± 5.2) months vs. (15.8 ± 5.6) months, P>0.05. The incision length, bleeding, VAS 3 d and 4 weeks after surgery in modified group were significantly lower than that in conventional group:(8.1 ± 0.8) cm vs. (12.3 ± 1.2) cm, (144.3 ± 16.5) ml vs. (172.4 ± 21.6) ml, (4.5 ± 1.1) scores vs. (5.9 ± 1.5) scores and (1.6 ± 0.7) scores vs. (2.4 ± 0.8) scores, and there were statistical differences (P<0.05). There were no statistical differences in operation time, VAS 3 months after surgery, incidence of postoperative complications and excellent and good rate of hip joint function at the final follow-up between 2 groups (P>0.05). Conclusions The modified S-P approach without disconnecting the rectus femoris is a better method in patients with Pipkin type ⅠandⅡ fractures, and it has the advantages of revealing more clearly, minimal invasion, lower bleeding and slight postoperative pain.
3. Application of ultrasmall superparamagnetic iron oxide in MRI diagnosis of central nervous system: Recent progress

Ultrasmall superparamagnetic iron oxide (USPIO), as the contrast agent of MRI, possesses two major properties: long half time in the plasma and specific binding with macrophages. Compared with gadolinium, widely-used in clinic presently, USPIO has its unique advantages in diagnosis of central nervous system diseases, though which still need further clinical verification. This article reviews the application of USPIO in MRI diagnosis of some central nervous system diseases.
4.Brief analysis on liver depression and herpes zoster

According to years of treatment experience on herpes zoster,combined with its current onset characteristics as well as modem medicine,the authors probed into the relation between liver depression and herpes zoster on the following two aspects:one was liver depression and healthy qi (immunity) ; the other was liver depression and the incidence of herpes zoster.We considered that long-term liver depression impairs healthy qi.Then herpes zoster appears because detriment of healthy qi leading to decreased immunity,reactivation and reproduction of virus.Liver depression is a significant factor for the onset of herpes zoster.
5.Dopaminergic neuroprotection of Poacynum hendersonni in MPTP mouse

Aim To study the dopaminergic neuroprotection of quercetin-3-O-sophoroside and total flavonoids derived from Poacynum hendersonni in MPTP-induced mice model of Parkinson's disease.Methods To inject MPTP intraperitoneal to C57/bl6 mouse at 30 mg·kg~(-1)·d~(-1) 5 days for PD model preparation, qercetin-3-O-sophoroside(25 mg·kg~(-1))and total flavonoids(250 mg·kg~(-1))were treated through intragastric administration 7 days before.Watching the mice behavior change through autonomic activities and pole test after final treatment, dopamine content in striatum was determined with fluorescence photometer.The neurons pathological change in substantia nigra(SN)was observed by pafaffin slice and immunohistochemistry staining to analyze the dopaminergic neuroprotective effects of Poacynum hendersonni.Results Treatment of quercetin-3-O-sophoroside for 7 days in advance, can improve PD mice motivation and dopamine concentration in striatum, tyrosine hydroxylase immune-reactive neurons in SN increased as well.Conclusion Quercetin-3-O-sophoroside can attenuate the impairment of MPTP in mice model of PD.

7.In vitro effects of propranolol and isoproterenol on the expression of beta-2 adrenergic receptor on infantile hemangioma endothelial cells

Objective To explore the role of beta-2 adrenergic receptor in the pathogenesis of infantile hemangioma.Methods In vitro cultured infantile hemangioma endothelial cells were divided into propranolol and isoproterenol groups.The propranolol groups were further classified into 5 groups to be treated with propranolol solutions at concentrations of 10,15,20 μg/ml,EGM-2 medium (blank control group 1),and EGM-2 medium containing 0.16% DMSO (DMSO group) respectively,while the isoproterenol groups were classified into 4 groups to be treated with isoproterenol solutions at concentrations of 5,10,20 μg/ml and EGM-2 medium (blank control group 2) respectively.After 24-and 48-hour treatment,enzyme-linked immunosorbent assay (ELISA) was performed to measure the expression of beta-2 adrenergic receptor in cell culture supernatants in the above groups.Results After 24-hour treatment,15-μg/ml and 20-μg/ml propranolol groups showed significantly decreased expression of beta-2 adrenergic receptor compared with blank control group 1 and DMSO group (all P < 0.05).After 48-hour treatment,all the propranolol groups showed significantly decreased expression of beta-2 adrenergic receptor compared with the blank control group 1 (all P < 0.05).However,the expression of beta-2 adrenergic receptor was significantly higher in the 10-and 20-μg/ml isoproterenol groups than in the blank control group 2 after 24-hour treatment (all P < 0.05),and higher in the 20-μg/ml isoproterenol group than in the blank control group 2 after 48-hour treatment (P < 0.05).Conclusion Propranolol can down-regulate the expression of beta-2 adrenergic receptor on the surface of vascular endothelial cells,while isoproterenol can up-regulate its expression.
8.Preparation of Cuiru Oral Liquid

Cuiru oral liquid (COL ) for the promotion of lactation is prepared from an aqueous extract of Angelica sinensis, Astragalus membranaceus (Fisch.) Bge., Rehmbnnia glutinosa Libosch.,et al. Its formulatiou and processing were briefly desctibed, and the quality standard and stability of the finished product were studied. Results showed that the formu lation processing and stability of COL were suitable for clinical trial. Moreover, its pharmacology was briefly described.
9.Gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture in 30 children

Objective To explore the efficacy of gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture. Methods A total of 30 children with esophageal stricture underwent gastrofiberscope-assisted balloon dilatation between October 1996 and November 2005 in this hospital.The procedure was conducted using the type JIF-SP gastrofiberscope(Olympus Company,Japan) and a balloon catheter with pressure monitor(Boston Company,USA).The stricture was 0.2~0.6 cm in diameter and 0.6~2.5 cm in length(mean,1.7 cm).Under the direct vision of gastrofiberscopy,the balloon catheter was perorally inserted to the stricture.Then the balloon was insufflated with air while monitoring the pressure.The balloon dilatation lasted for 5 min. Results A total of 113 times of balloon dilation were performed in 29 children,while a conversion to traditional surgery was required in 1 child with serious esophageal stricture due to difficulty to dilatation.In the 29 children,the dilatation was conducted for 2~3 times every month until the strictured esophagus reached 0.6~0.7 cm in diameter after 4~5 times of balloon dilatation.Follow-up surveys for 3 months ~ 5 years in the 29 children showed no dysphagia or recurrence of stricture.Conclusions Gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture is a feasible and safe technique.
10.Two-port laparoscopy versus open high ligation for indirect inguinal hernia in children

Objective To compare curative effects between laparoscopic and open surgery in the treatment of pediatric indirect inguinal hernia.Methods A group of 410 children with indirect inguinal hernia underwent micro-laparoscopic operation(Laparoscopic Group) from April 2001 to January 2006.This group was compared with another group of 408 children with indirect inguinal hernia,treated with open high ligation of the hernial sac(Open Group) from September 2004 to January 2006.In the Laparoscopic Group,a 5-mm trocar was inserted at the umbilicus for the exploration of bilateral internal rings.Another 5-mm trocar was inserted at the site slightly superior to the McBurney's point for introducing a needle holder.At the level of the neck of the hernial sac,the orifice of the vaginal process was circumferentially sutured and ligated.In the Open Group,high ligation of the hernial sac was conducted through a transversal incision at the external ring.Results The operations in the Laparoscopic Group were smoothly completed.The operative time for one side of hernia was significantly shorter in the Laparoscopic Group(33.4?9.6 min) than in the Open Group(38.0?7.6 min)(t=-7.596,P=0.000).In the Laparoscopic Group,no edema of the scrotum,subcutaneous hydrops,or infection occurred.The patients were followed for 1~55 months(mean,23.2 months).Short-term recurrence was observed in 3 children,who required a re-operation of laparoscopic repair.No testicular atrophy,iatrogenic retained testis,intestinal adhesion,or other long-term complications happened.In the Open Group,follow-up observations for 1~18 months(mean,10 months) revealed recurrence in 18 children,8 of whom were then cured with laparoscopic surgery and 10 of whom with open surgery.Iatrogenic retained testis happened in 9 children and a re-operation of open surgery was carried out.Conclusions Two-port laparoscopic high ligation of hernial sac for pediatric indirect inguinal hernia is minimally invasive,safe,and effective,but not suitable for children with cardiac insufficiency or newborn incarcerated hernia.
