1.Neuromyelitis optica-IgG detection in serum by indirect immunofluorescence assay

Objective To establish a method to detect neuromyelitis optiea (NMO)-IgG in patients serum using indirect immunofluorescence assay (IFA). Methods The normal tissues (cerebellum/ midbrain, kidney and stomach) from C57 mice were cryosectioned onto microscope slides as detective substrate. For NMO-IgG detection, isolated serum from patient with NMO, multiple sclerosis (MS), optic neuritis or myelitis was incubated with the tissue sections on the slide at 4℃ overnight and subsequently incubated with a fluorochrome-cojugated lgG specific for human. For double immunostaining with aquaporius-4 (AQP4), the slides were incubated with primary antibody of AQP4 and secondary antibody of IgG-TRITC. Detection of NMO-IgG and its co-localization with AQP4 was analyzed using fluorescence microscope. Results All 182 serum samples from patients were tested using IFA. Some samples revealed a characteristic immunohistochemical staining of NMO-IgG in mouse CNS tissues, predominately in pia and subpia, and capillaries in white and grey matter in the cerebellum, midbrain, and spinal cord. Double immunostaining with AQP4 demonstrated the co-localization of NMO-IgG with AQP4. Conclusions We established an IFA using a substrate from C57 mouse cerebellum/midbrain, kidney and stomach tissue to detect NMO-IgG in patient serum. This method is specific and efficient in detection and may be useful in diagnosis and differential diagnosis of neuromyelitis optica.
2.Assessment of cardiac intervals with tissue velocity imaging in normal subjects

Objective To quantitatively assess cardiac intervals with tissue Doppler imaging(TDI) and normal reference values. Methods Apical 4 chamber, 2 chamber and long axis views of two-dimensional TDI were acquired in 29 healthy individuals(using GE Vivid 7 commercially ultrasound machine). The basal and middle segments in left ventricle(anterior septal, anterior, lateral, posterior, inferior and septal wall) and in the free wall of right ventricle were analyzed with tissue velocity imaging(TVI) model.Cardiac intervals of each segment, which included the isovolumic contraction time, contraction to peak time, insovolumetric relaxation time, rapid filling time and artrial contraction time were measured using TVI in the left ventricle and free wall of the right ventricle. Results The successful rate of sample was 100% in basal segment of each wall. 4.Repair function of extracorporeal membrane oxygenation recirculation in vivo for swine liver after cardiac death

Objective To investigate the repair function of extracorporeal membrane oxygenation (ECMO) in vivo for the liver after cardiac death with warm ischemia injury for 30 min from cardiac death swinc.Method Ten landraces,30 to 40 kg,randomized to experimental group and control group,were used to make 30-min cardiac death models through clamping trachea after deep anesthesia.An intravenous cannula was placed through right iliac arteries and veins,and connected to ECMO extracorporeal circulation pipes in experimental group.The balloon catheter was placed to diaphragm plane through left femoral artery.The ECMO was performed to infuse abdominal organs,and pH and electrolyte were adjusted.The circulation flow rate,intraperitoneal organ perfusion pressure,venous blood gas,electrolyte,transaminase,and bile product,etc.were monitored and recorded.The livers of control group were retrieved after 30-min cardiac arrest and stored in cold UW for 4 h.Pathological tissue was sliced and stained by HE.Result After 30-min cardiac arrest,the liver showed obvious congestion appearance; pathologically,there were hepatic sinus expansion,blood cells clog,and erythrocyte aggregation.Circulating blood gas analysis revealed severe acidosis.After the ECMO recirculation started,circulation flow rate maintained to 1 L/min,the liver gradually restored bright red,pathological biopsy showed that hepatic sinus expansion disappeared,and clogged blood cells dispelled.AST was markedly increased to (226.0 ± 28.0) U/L after 30-min cardiac arrest and reduced to (150.0 ± 30.0) U/L 4 h after the ECMO recirculation.Average bile production was 7.75 ml/h.Conclusion ECMO recirculation in vivo can repair the injured livers from cardiac death donor with 30-min cardiac arrest.
5.Effects of vascular endothelial cell growth factor on apoptosis of rat intervertebral disc cells cultured in vitro

Objective To observe the effect of vascular endothelial cell growth factor (VEGF)on apoptosis and matrix metabolism of rat intervertebral.disc cells cultured in vitro. Methods Culture system of rat intervertebral disc cells was established to culture the monolayer intervertebral disc cells in vitro.The well-grown intervertebral disc cells of the second generation were chosen and anti-Fas antibody was applied to induce their apoptosis.Then,VEGF at different concentrations ( 10,100,1 000 μg/L)were used to affect their apoptosis and metabolism.The apoptosis of the cells stained with Propidium Iodide (PI) was detected by using flow cytometry.The levels of hydroxyproline and proteoglycan in the culture medium were detected by using chloramines T method and DMB chromometry method,respectively.Results ( 1 ) The apoptotic ratio of the intervertebral disc cells affected by different concentrations of VEGF (10,100,1 000 μg/L) was (87.62 ±11.06)%,(53.30 ±9.23)% and (16.75 ±4.21)% respectively,with significant differences in comparison with the control group (P < 0.01 ).(2)The contents of hydroxyproline [ (6.71 ±0.33) μg/L,(9.12 ±0.41 ) μg/L,( 11.58 ±0.12) μg/L] and proteoglycan [ (23.21 ± 2.87) μg/L,( 32.45 ± 5.23 ) μg/L,(37.18 ± 3.22) μg/] in the culture medium raised with the increase of VEGF concentration ( 10,100,1 000 μg/L),with statistical differences compared with those of the control group (P <0.01 ).The contents of hydroxyproline and proteoglycan were positively correlated with the concentration of VEGF (ra=0.972,P<0.01;rb =0.907,P<0.01). Conclusion VEGF can inhibit the apoptosis of the rat intervertebral disc cells cultured in vitro and promote collagen and proteoglycan synthesis of the cell matrix.
6.Effects of pioglitazone on the expression of genes relative to differentiation and function of primary brown adipose tissue cells in mice with diet-induced obesity

The effect of pioglitazone on the expression of genes relative to differentiation and function of primary brown adipose tissue cells was detected for the new treatment of obesity and type 2 diabetes.The results showed that pioglitazone promoted the differentiation and function of brown adipocytes( P<0.05 ).

8.Diagnosis of X-Linked creatine transporter deficiency in a patient from Northeast China

Background: Creatine transporter (CRTR) deficiency is the most common creatine deficiency syndrome,
of which the final diagnosis relies on mutation in the X-linked CRTR gene. To date, more than 90
mutations in the SLC6A8 gene have been reported. This paper discusses a novel mutation detected via
the thorough sequencing of all the X-chromosome-specific exons investigated in a four and a half year
old boy with an intellectual disability, speech and language delay and motor disturbance. Methods:
A brain magnetic resonance imaging (MRI) and a proton magnetic resonance spectroscopy (MRS)
were carried out, the creatine and creatinine concentrations in the urine were checked and all exons
were sequenced. Results: A detailed clinical investigation revealed a reduction in the cerebral creatine
levels in the brain by the MRS, elevated creatine and creatinine concentrations in the urine and signal
abnormalities in the left frontal cortex of the brain by the MRI. A novel change was identified in the
heterozygosity of the exon 10: c.1395-c.1401 deletion.
Conclusion: The use of a combination of powerful new technologies, such as thorough exome-nextgeneration
sequencing and a brain MRS, should be considered, in order to determine any neurometabolic
diseases, especially when the signal abnormalities in the brain MRI cannot be explained by any other
factors. This mutation results most likely in a dysfunction of the creatine transport and synthesis,
hence causing central nervous system symptoms.

10.Effects of standardized three-stage rehabilitation treatment on neurological deficit scores and ADL performance of ischemic stroke patients

Objective To observe the effects of standardized three stage rehabilitation treatment on the neu- rological deficit scores (NDS) and ADL performance of ischemic stroke patients.Methods A total of 164 ischemic stroke patients were recruited and randomly divided into a rehabilitation group and a control group.The neurological function and ADL performance of the patients were assessed by using NDS and Modified Barthcl Index (MBI) at the admission,at the end of 1st,3rd and 6th months post stroke.Results No significant differences were found be- tween the rehabilitative and the control groups with regard to NDS and MBI at admission.The NDS demonstrated a decreasing tendency,while the MBI score an increasing tendency in both groups.In the control group,significant difference of NDS was found between admission and the end of 1st month as well as between the end of the 1 st and the 3rd months.In rehabilitation group,significant difference was revealed between all the time points with regard to NDS and MBI scores.At the end of the 1st,3rd and 6th months,the MBI scores of the rehabilitation group were signifi- cantly higher than those of the control group,indicating that the ADL performance of those treated with standardized three-stage rehabilitation protocol was improved quicker than those without the protocol.Conclusion Standardized three-stage rehabilitation treatment could improve the neurological function and ADL performance of the ischemic stroke patients.
