Sepsis is a common desease with a high mortality in pediatric intensive care unit. The patho-genesis of sepsis remains unclear. Recent studies have found that extracellular histones play an important role in the incidence of sepsis. This review provides new ideas for the diagnosis and treatment of sepsis.

Mesenchymal stem cells (MSCs) can specifically migrate to multiple kinds of tumors and their metastases in a systemic manner(to all parts of the body). Therefore genetically modified MSCs can serve as vehicle of anticancer factors to inhibit growth of tumor. But many questions remain to be answered and the precise mechanisms remain unclear. This paper reviews the mechanism of MSC homing, MSCs as delivery vectors for cancer therapy and its advantages over traditional drug vehicle. Objective To investigate the efficacy and safety of laparoscopic repair in children with congenital esophageal hiatal hernia. Methods From September 2001 to December 2008,seven children with congenital esophageal hiatal hernia,including 4 boys and 3 girls,were treated by laparoscopic repair and Nissen's fundoplication in our hospital. One of them received laparoscopic pyloroplasty in the meanwhile because of delayed gastric emptying (confirmed by preoperative gastrointestinal angiography); one child underwent laparoscopic high ligation due to right oblique inguinal hernia; spleneolus was found in one patient,no further treatment was carried out. Results The procedures were completed in all of the patients without conversion to open surgery. The mean operation time was 94.2 min (75-150 min) in this series,and the average blood loss was 5 ml (2-10 ml). No patient received blood transfusion after the surgery. The children began to intake milk or regular diet in 24-48 hours after the operation. They were discharged from hospital in a mean of 4.5 days (ranged from 3 to 7 days). Then,the children were followed up for 6 to 20 months (mean,12.6 months). One of the patients suffered from vomiting on the 10th day after the operation,1 patient showed recurrence in 1 year postoperation and then was cured by a second operation. The other five patients were healthy during the follow-up. Conclusions Laparoscopic repair of congenital esophageal hiatal hernia is a safe,effective,and minimally invasive procedure. It is feasible for patients complicated with other diseases. The indications for the surgery must be strictly selected.

Objective To explore the value of laparoscopic partial nephrectomy for children with duplex kidney.Methods Between June 2006 and February 2009,we performed laparoscopic partial nephrectomy on totally 21 children with duplex kidney.Three or four trocars were used for the operation.Through transabdominal approach the peri-renal tissues were freed and the vessels supplying the duplicated kidney were ligated.Afterwards,we resected the duplicated kidney,as well as the ureter,and a subperitoneal drainage tube was indwelt.Results The operation was completed in all of the 21 cases without conversion to open surgery.The mean operation time for laparoscopy was 190 min(range,130 to 210 min).The intraoperative blood loss ranged from 5 to 10 ml.No patient received blood transfusion.Liquid diet was given in 8 to 10 hours after the operation.In this series,the postoperative hospital stay ranged from 5 to 7 days with a mean of 6.2 days.Follow-up was available for 3 to 28 months in the cases(mean,20.2 months).During the period,the clinical symptoms disappeared and B ultrasonography showed no abnormalities.Conclusions Providing larger operation space and better exposed surgical field,laparoscopic partial nephrectomy for children with duplex kidney is feasible and safe with quicker recovery.

Objective To explore the effects of different processes on the contents of three alkaloid ingredients in Coridalis Rhizoma.Methods The contents of three alkaloid ingredients (protopine, dehydrocorydaline, tetrahydropalmatine) were determined by HPLC. Agilent HC-C18(2) column (4.6 mm × 250 mm, 5μm) was set at 35℃; acetonitrile-0.1% phosphoric acid solution (triethylamine adjusted pH 5.3) (26:74) was set as mobile phase; the flow rate was 1.0 mL/min and detection wavelength was set at 280 nm; the injection volume was 10μL. The three components were separated well under these chromatographic conditions. The linear relationship between the mass of each component and the peak area was good (r2≥0.9996). The average recoveries were 100.31%, 99.34%, and 99.34%, respectively (RSD≤2.70%).Results The contents of protopine, dehydrocorydaline, tetrahydropalmatine and the total contents of three alkaloids were higher than other groups in large size samples (diameter of fresh products was 2.0–3.0 cm) which were boiled 5–6 minutes, the medium size samples (diameter of fresh products was 1.5–1.9 cm) which were boiled 2–4 minutes and the small size samples (diameter of fresh products was 0.7–1.4 cm) which were boiled 1–2 minutes. The contents of alkaloid in boiled process were lower than steamed process.Conclusion The Results of study provide an experimental basis for origin processing of Coridalis Rhizoma.

Objective: To improve the treatment order and improve the work efficiency of the doctors, our hospital has run the triage calling system in out-patient department. Methods:Using VC++ language and the middleware technology, we realize the function of voice calls, large screen display control, background management and data synchronization. HIS system realizes the automatic call patient registration by transfer the registration information to the triage system. Results: The trial operation of outpatient triage calling system in our hospital has completely changed the previous traditional triage mode of“nurse as orientation”. The system reduces not only the work pressure of health care, but also significantly improve the doctors' work efficiency and service quality. Conclusion:Guide triage system can effectively improve the environment and outpatient treatment order, greatly enhance the patient’s satisfaction.

Objective: To discuss the purposes and methods of determining and employing operational intervention levels in preparedness and response for a nuclear emergency. Methods:According to the safety standards and technical reports issued by the International Atomic Energy Agency (IAEA) and combined with specific conditions in China, the operational intervention levels suitable for applying in China are analyzed. Results: The newly published IAEA technical report provided special operational intervention levels for emergency at a light water reactor. The recommended values are practical that our country can use for reference. Conclusion: It is suggested that the related branches for response to nuclear emergency in China establish feasible operational intervention levels by referring to the IAEA reports and taking into account the special conditions of nuclear facilities in China, in order to improve the capability of medical response to nuclear emergency.
