1.Clinical characteristics and therapeutic strategy of severe craniocerebral injury in children

The chance of craniocerebral injury in children is high.Severe craniocerebral injury is still the difficult point in current clinical treatment.Children severe head trauma is different from adults.It is characteristic of severe primary injury,rapid progressing secondary injury,confusing vital signs,less contrecoup injury,high incidence of epilepsy,cerebral ischemia in familiar,and good prognosis.This article focused on 2 hot issues in recent therapeutic strategy of severe craniocerebral injury in children——intracranial pressure monitoring and hypothermia therapy.The former is simple,convenient,safe and practical,is worth popularizing widely.
2.Relationship of statistics and data management in clinical trials.

A perfect clinical trial must nave a solid study design, strict conduction, complete quality control, non-interference of statistical result, and acceptable risk-benefit ratio. To reach the target, the quality control (QC) should be performed from the study design to conduction, from the analysis to conclusion. We discuss the relationship between data management and biostatistics from the statistical point of view, and emphasize the importance of the statistical concept and methods in the improvement of data quality in clinical data management.

According to the recommended diagnostic criteria of obese adults in Asia-Pacific region,8 284 cases of middle-aged and elderly people in Uygur and Han populations were collected randomly to study the status and distribution characteristics of two nationalities.Standardized prevalences of obesity were 20.1 2%in Uygurs and 23.077%in Hans.Woman in Hans reached the highest prevalence.Alcohol-drinking,abnormal giycometabolism,and serum cholesterol levels were the most relevant risk factors.

Objective:Based on the survey of qualitative and quantitative on rural doctors in 6 provinces and cities in China , this paper aimed to compare the present situation of construction of rural doctors under the new health care reform and explore the impact of the new health care reform as well as to propose some suggestions . Methods:Based on the survey of questionnaire and in -depth interviews on rural doctors in 6 provinces and cities in China .Results:The problems existed for rural doctors in China such as insufficient number of personnel , ag-ing, lack of young rural doctors supplement , low level the requirements and situation of professionalism , etc.Con-clusion:It should accelerate the process of the cultivation of rural doctors , promote the education level of rural doc-tors, gradually ease the current situation of the rural doctor shortage .

6.An immunohistochemical study on the distribution in organs in cases with morphine poisoning.

OBJECTIVE:
To investigate the distribution of mophine in organs in cases with morphine poisioning to select ideal organs for immunohistochemical derection.
METHODS:
Localization and half quantitation of morphine in the brain, the kidney, the heart, and the liver were studied in 8 cases with morphine poisoning by immunohistochemical SP method.
RESULTS:
Morphine was mainly detected in the cytoplasm of certain parenchymal cells of the organs. The distribution varied greatly with different cases and organs. In the brain and kidney, morphine-positive cells could be easily found.
CONCLUSION
The kidney and brain may be the ideal organs for sampling in suspected morphine poisoning cases with.

7.Construction of T vectors based on Xcm I recognition site and optimization of PCR fragments for ligation.

OBJECTIVETo construct T vectors based on Xcm I recognition site and optimize the PCR fragments for its ligation.
METHODSWe firstly cloned the human histone H4 cDNA containing one Xcm I recognition site at both its 5' and 3' end into pCDNA 3.0 vector and then generated T vector with pCDNA 3.0 backbone by cutting the recombinant plasmid with Xcm I. To increase the ligation efficiency, the primers were firstly phosphorylated before DNA fragments amplification and then the PCR products were treated with Taq DNA polymerase and dATP after PCR amplification. Two DNA fragments with the length of 312 bp and 1 329 bp were ligated to it and the ligation mixture was transformed into E. coli DH5α competent cells and the positive rates of the transformants were evaluated by PCR and DNA agarose gel electrophoresis.
RESULTSOur results showed that the T vector produced by our method could ligate to the target DNA fragments with high efficiency. Besides, the phosphorylation state of the primers used for PCR amplification is also an important factor determining the cloning efficiency. What's more, as for longer DNA fragments, retreatment with Taq DNA polymerase and dATP after PCR amplification and purification could improve the ligation efficiency significantly.
CONCLUSIONOur protocol may overcome the dependence on blue/white screening to get positive clones and provide a potent way to generate T vectors and ligate them to the target PCR fragment.

8.Predictive factors of hepatitis B virus DNA negative conversion among patients with chronic hepatitis B during lamivudine therapy.

OBJECTIVETo study the occurrence and predictive factors of hepatitis B virus DAN negative conversion among patients with chronic hepatitis B and liver cirrhosis during lamivudine therapy.
METHODSClinical data of 202 patients with chronic hepatitis B and 55 patients with post-hepatitis B cirrhosis treated with lamivudine therapy were analyzed and PCR as well as mispairing PCR-restricted fragment length polymorphism (RFLP) assay were applied. The data including ALT, AST, total bilirubin (TBIL), albumin, etc. were analyzed by SPSS software.
RESULTSThe HBV DNA negative conversion occurred in 87.62%, 95.04%, 86.63%, 82.65%, 75.43%, and 77.27% of the chronic hepatitis B patients and in 94.54%, 98.18%, 81.81%, 74.35%, 73.91%, and 63.63% of liver cirrhosis patients, respectively, after 3, 6, 12, 18, 24, 30 months. The results of log-rank test and Cox proportional hazard model analysis indicated that combination of lamivudine and interferon, high ALT level,low HBV DNA level, and low albumin level at baseline were significantly related to HBV DNA negative conversion.
CONCLUSIONCombination treatment with lamivudine and interferon, high ALT level, low HBV DNA level, and low albumin level at baseline were significant predicting predicting factors for HBV DNA negative conversion in chronic hepatitis B patients.

9.Investigation of optimum concentrations of betaine for improving the resolution of sequencing G-C rich DNA with trinucleotide repeats.

OBJECTIVETo develop an optimal sequencing system which can improve the resolution of sequencing G-C rich DNA with abundant trinucleotide repeats by applying concentration gradients of betaine to the Sanger sequencing system.
METHODSConcentration gradients of betaine were introduced into the sequencing system by taking the 5' terminal of Nogo-B cDNA (Am-Nogo-B) (G-C%=72%, without trinucleotide repeats) and 5' terminal of Huntingtin cDNA (Am-HTT) (G-C%=74%, with abundant CAG and CCG repeats) the results of sequencing were compared.
RESULTSThe optimum concentration of betaine for sequencing Am-Nogo-B has differed from that for Am-HTT. Result of sequencing Am-Nogo-B has achieved the best quality when the concentration of betaine was at 0.8-1.2 mol/L, whereas the result of sequencing Am-HTT obtained the best quality when the concentration of betaine was at 1.6 -2.4 mol/L. The results were reproducible.
CONCLUSIONG-C rich DNA with similar G-C% required different concentrations of betaine in the sequencing system due to base pair compositions. The sequencing system developed for improving the resolution of sequencing of G-C rich DNA with abundant trinucleotide repeats can be used as a reference for similar studies.

10.Study on the biceps brachii microcirculation blood flow reserve capacity of the Chinese rowers.

OBJECTIVETo investigate the effect of chronic endurance exercise on microcirculatory reserve capacity of biceps brachii in Chinese rowers and provide a certain basis for the date standard foundation of monitoring of functional status and the foundation of database of reserve capacity of blood of Chinese rowers.
METHODSEmpty stomach in the morning, 77 rowers from different groups and 24 common health people were noninvasive tested by using PeriFlux System 5000, the test indexes include the microcirculatory reserve capacity and other related indexes of biceps brachii. The test sites of all athletes were the same space in biceps brachii of the right side of body, there was no space differences of all athletes . All athletes were tested in the relatively stable functional status, common people were healthy. The test value included basic values and heating values, put the before and after heating of microcirculatory blood perfusion (MBP) as the microcirculatory reserve capacity.
RESULTSHeavyweight female (198. 97 ± 98. 81) > heavyweight male (183. 45 ± 64. 31) > lightweight male (151. 01 ± 65. 96) > lightweight female(140.53 ± 43.22) > common male people(127.21 ± 56.38) > common female people(103.54 ± 33.41), the microcirculatory reserve capacity of each group athletes were higher than common people, except the comparison between lightweight female and common male people, and there was no significant difference among the different group athletes.
CONCLUSIONChronic endurance exercise can improve the microcirculatory reserve capacity of rowers, especially the heavyweight rowers; the normal value of microcirculatory reserve capacity of heavy weight rowers should be more than 160, and lightweight rowers should be more than 120. There was no significant difference among different sex athletes, if the value of microcirculatory reserve capacity is significant lower than normal, it shows that athletes are in the state of fatigue.
