1.Study of characteristic differences using CT and MRI for intracranial hemangiopericytoma and meningiomas of different pathological type

Objective To study the characteristic differences using CTs and MRIs between intracranial hemangiopericytomas (HPCs)and meningiomas of different pathological type.Methods 8 cases of HPC and 5 1 cases of meningioma approved by patholo-gy were selected.All cases underwent MRIs,CTs and enhancements before operation.The imaging characteristics of these tumors were analyzed with chi-square test.Results Compared with atypical meningioma,tumor bleeding(χ2 = 1 1.484),blood vessel flow void phenomenon(χ2 =8.571),skull damage(χ2 =5.529)showed remarkably significant statistical differences(P <0.05);compared with malignant meningioma,tumor bleeding(χ2 =1 1.484),blood vessel flow void phenomenon(χ2 = 1 1.429)showed remarkably significant statistical differences(P <0.05).Conclusion There are differences between HPC and meningiomas of different pathologi-cal type.It is helpful to improve the diagnosis level through comparative analysis of the MRI and CT findings.
2.Minimal invasion and closed reduction with external fixation for elderly femoral intertrochanteric fracture.

OBJECTIVETo evaluate the clinical effects of minimal invasion and closed reduction with external fixation in treating elderly femoral intertrochanteric fracture.
METHODSFrom August 2007 and September 2013, 43 patients with elderly femoral intertrochanteric fracture were treated by minimal invasion and closed reduction with external fixation. There were 26 males and 17 females with an average age of 78 years old ranging from 68 to 92 years. Durations from injury to operation ranged from 1 to 8 days with an average of 3 days. According to AO classification, there were 22 cases with type A1, 15 with type A2, 6 with type A3. The time of bone healing were observed after operation and the clinical effects were evaluated by Harris scoring standard.
RESULTSAll patients were followed up from 10 to 18 months with an average of 13 months. The incisions obtained one-stage healing after removal of external fixation. All the fractures were healed and the mean healing time was 16 weeks (ranged, 12 to 18 weeks). Two patiens complicated with the thread needle loosening in the femoral trochanteric, 2 cases with the hip varus deformity and 1 case with the delayed union. No bone nonunion, external fixer breakage, blood vessel or nerve damage were found. According to the Harris scoring standard,the average Harris score of hip joint function was 85.89±7.36, and 18 cases got excellent results, 19 good.
CONCLUSIONMinimal invasion and closed reduction with external fixation can obtain satisfactory results in treating elderly femoral intertrochanteric fractures. It is a simple and effective method for elderly patients who cannot tolerate anesthesia and surgery trauma.

3.Effect of miRNA-218 on renal cell carcinoma in nude mice

Objective To explore the effects of miRNA-218 on renal cell carcinoma of nude mice in vivo.Methods The pcDNA3.1-miR-218 and its control negative plasmids were stably transfected into renal cell carcinoma cell line A498 and 769-P.These cells were inoculated into nude mice in different groups to observe the changes of body and tumor and to detect the expression of miR-218 in the tissues of nude mice.Results In the A498 cells + pcDNA3.1-miR-218 transfected group,the weight loss of tumor bearing nude mice after 25 days was lower than that in the control group,and the tumor volume was smaller than that in the control group after 10 days (P < 0.05).In the 769-P cells + pcDNA3.1-miR-218 transfected group,the weight loss of tumor bearing nude mice was lower than that in the control group after 19 days,and the tumor volume was smaller than that in control group after 10 days (P < 0.05).The expression of miRNA-218 in bearing nude mice with A498 cells or 769-P cells transfected by miRNA-218 was significantly higher than that in the control group,and the difference was statistically significant (P < 0.05).Conclusion Up-regulation of miRNA-218 expression can inhibit the growth of renal cell carcinoma of nude mice in vivo.
4.Recent progress in study onneuroprotection of phosphodiesterase 5 inhibitors

In recent years,the role of phosphodiesterase 5(PDE5)has been highlighted in the development and progression of neurological disease.PDE5 inhibitors show significant effect of neruoprotection,which may be related with some effects such as resistance to stroke,anti-oxidation,inhibition of neuroinflammation and amelioration of cognitive deficits.Based on the domestic and overseas researches about PDE5,this review systematically summarized the neuroprotection of PDE5 and their related mechanisms.
5.Construction and application in renal cell carcinoma 769-P cells of miR-218 expression plasmids

Objective To construct a stable expression plasmids miR-218,to lay the experimental foundation for the expression and mechanism of miR-218 in human renal cell carcinoma.Methods The primers were designed by Has-miR-218 and the miR-218 fragment were amplified by PCR,and then which was connected with the carrier pcDNA3.1 (+) to build a stable expression plasmids.The recombinant plasmids were trasfected into renal cell carcinoma cell line 769-P,and the expression of miR-218 in these cells were detected.Results The plasmids were constructat successfully,which was determind by enzyme digestion and sequencing results.The constructed expression plasmids pcDNA3.1-miR-218 transfection of human renal cell carcinoma cell line 769-P,miR-218 cxpression level of cells was significantly increased after trasfected by recombinant plasmids carrying miR-218 gene (relative expression quantity was 1.64).Conclusion The miR-218 expression plasmids pcDNA3.1-miR-218 were successfully constructed,the plasmids can be applied to the study of miR-218 function and mechanism in renal cell carcinoma.
6.Effect of electric vagal stimulation on myocardial ischemia-reperfusion injury in rats

Objective To evaluate the effect of electric vagal stimulation on myocardial ischemia-reperfu-sion (I/R) injury in rats.Methods Forty-five male Sprague-Dawley rats,aged 4-5 months,weighing 180-220 g,were randomly divided into 3 groups (n =15 each):sham operation group (S group),I/R group,electric vagal stimulation group (EVS group).Myocardial I/R was induced by occlusion of left anterior descending branch of coronary artery for 30 min followed by 120 min reperfusion in I/R and EVS groups.In group EVS,the left cervical vagus nerve trank was stimulated for 30 min with continuous electric pulses (5 V,2 ms,10 Hz) starting from 10 min before reperfusion until 10 min of reperfusion.The animals were sacrificed at the end of 120 min reperfusion and the hearts were removed for determination of the infarct size,TNF-α and IL-6 contents and myeloperoxidase (MPO) activity in myocardial tissues.Results Compared with group S,the infarct size was significantly enlarged,and TNF-α and IL-6 contents and MPO activity were increased in I/R and EVS groups (P < 0.05).The infarct size was significantly smaller,TNF-α and IL-6 contents and MPO activity were lower in EVS group than in I/R group (P < 0.05).Conclusion Electric vagal stimulation can reduce myocardial I/R injury in rats and activation of cholinergic anti-inflammatory pathway is involved in the mechanism.
7.Analysis of the death causes of 187 cases of kidney transplant recipients

Objective To analyze the death causes of cadaveric kidney transplantation recipients. Methods The clinical data of 1400 cases of cadaveric kidney tansplantation between 1978 and 2003 were analyzed retrospectivvely. The death rate, causes and period were analyzed among the died recipients after kidney transplantation. Results 187 patients were died after renal transplantation in this stage with the ratio being 13.36% (187/1400). The death causes contained infection, cardiocerebra l vascular diseases and hepatic failure by turns, and the ratio was 37.97% , 31.56% and 14.97% , respectively. There were 87 ( 46.5% ) death cases w ith normal transplanted-kidney function. Conclusions The leading causes of patients' death were infection, cardiocerebral vascular d iseases, and hepatic failure. About half of these patients were died with normal transplanted-kidney function.
8.Management of 240 cases of non-functioning renal graft

Objective To explore the methods for treating no-functioning renal graft.Methods The management of 240 cases of non-functioning renal graft between 1978 and December 2003 was retrospectively analyzed.Results Twenty-nine recipients with renal failure one month after renal transplantation were subjected to removal of the renal graft. One year after renal transplantation, the recipients with renal failure received resection of the renal graft, 4 were treated with renal artery embolism, and 114 did not receive special treatment.Conclusion The non-functioning renal graft required removal at early stage of renal transplantation. At later stage, no symptomatic cases of post-transplanted renal failure required no management after stopping immunosuppressant. The appropriate management is reserved for the symptomatic cases.

10.Efficacy evaluation of 104-week telbivudine therapy in HBeAg-positive chronic hepatitis B

Objective To evaluate the efficacy of telbivudine in HBeAg-positive chronic hepatitis B (CHB) patients by comparing the efficacy of initial telbivudine therapy in treatment-naive patients with sequential telbivudine therapy in patients with poor response to adefovir.Methods A total of 90 HBeAg-positive CHB patients were assigned to receive sequential telbivudine therapy following poor response to adefovir dipivoxil (n=45),or initial telbivudine therapy in antiviral treatment-naive patients (n=45).All patients were treated with telbivudine 600 mg daily for 104 weeks.The efficacy was evaluated in terms of liver function tests,serum HBV markers,HBV DNA and antiviral drug resistance.Results Telbivudine showed good overall efficacy after treatment for 104 weeks in terms of alanine aminotransferase normalization rate (91.1%),HBV DNA negative conversion rate (80.0%),HBeAg loss rate (57.8%),and HBeAg/HBeAb seroconversion rate (30.0%).The HBV DNA negative conversion rate in initial treatment group was significantly higher than that in sequential treatment group (P<0.05).However,among the patients with early response,the efficacy did not show significant difference between groups (P>0.05).The patients with early response showed significantly better efficacy than those without early response,in terms of higher HBV DNA negative conversion rate,higher HBeAg loss rate and HBeAg/ HBeAb seroconversion rate (P<0.000 1 or P<0.05),but lower virological breakthrough rate (P<0.05).Conclusions Telbivudine has shown reliable efficacy in CHB patients.Initial telbivudine therapy is better than sequential therapy in CHB patients with poor response to adefovir.However,for patients with early response to telbivudine,no statistical difference is found between initial and sequential therapy in long-term treatment efficacy (104 weeks).The patients receiving sequential telbivudine therapy should be monitored closely for early antiviral response to optimize treatment.
