1.Neoadjuvant chemotherapy and preoperative radiation for stage Ⅰb-Ⅱb cervical cancer

Purpose:To evaluate the effect of preoperativ e chemotherapy or brachtherapy and radical hysterectomy on stage Ⅰb-Ⅱb locally advanced cervical cancer with high risk factors. Methods:preoperative chemotherapy or radiotherapy followed by r adical hysterectomy compared to radical hysterectomy alone in stage Ⅰb-Ⅱb cer vical cancer with higher risk factors(bulky,lower grade,small cell cancer or ade nocarcinoma). Results:①Pelvic lymphnode metastases rate was significantly decreased in the neoadjuvant chemotherapy arm compared with surgery alone(P 0.05). Conclusions:Preoperative chemotherapy followed by surgery is e ffective and superior to surgery alone in stage Ⅰb-Ⅱb cervical cancer with high risk factors.It might be able to reduce tumor volume and eliminate effectiv ely disease in lymphnodes and possibly micrometastases.It may be beneficial in improving quality of life of patients by decreasing the need for postoperative r adiotherapy. Response to chemotherapy may be a prognostic factor.Preoperative br achytherapy showed no significant advantage in eliminating lymphnodes matastases as compared to surgery alone.
2.The prevalence, awareness, and control of hypertension in Xianghe county of Hebei province

Objective To investigate the prevalence,awareness,control status and associated risk factors of hypertension in a rural population in Xianghe county in North China.Methods A total of 830 adults (aged≥35) from Xianghe county were examined during July to August,2011.Blood pressure was obtained using a standardized sphygmomanometer after a 5-minute sitting rest.Information on gender,age,education level,marital status,smoking,drinking,income,family history of hypertension and use of antihypertensive medications was obtained.Results A total of 42.4% of all subjects had hypertension.Among those with hypertension,the awareness rate was 54.8% and 50.0% of the patients were taking antihypertension medication with the control rate of 11.9% [BP < 140/90 mm Hg (1 mm Hg =0.133 kPa)].Lower age and education level,lower body mass index,negative family history of hypertension were associated with poor awareness of hypertension and worse compliance with the treatment.Older age,positive family history and alcohol consumption were associated with poor blood pressure control.Conclusions Hypertension is highly prevalent in Xianghe rural area.The awareness,treatment and control rate are all low.There is an urgent need for comprehensive strategies to improve prevention,screening,and treatment of hypertension in rural China.
3.Relationship of Resistance to Trimethoprim-sulfamethoxazole and Genes sul1 and sul2 in Stenotrophomonas maltophilia

OBJECTIVE To detect genes sul1 and sul2 in Stenotrophomonas maltophilia,and their relationship to drug resistance to trimethoprim-sulfamethoxazole(SXT).METHODS K-B was carried out to detect the drug resistance to SXT of S.maltophilia;minimal inhibitory concentration(MIC) was measured with micro broth dilution method.Genes sul1 and sul2 were amplified by PCR.RESULTS Eight isolates(7.8%) showed resistance to trimethoprim-sulfamethoxazole,sul1 Was positive in four isolates in which one contained sul2 also.Four isolates showed high MIC to SXT.CONCLUSIONS Genes sul1 and sul2 of S.maltophilia are associated with the high drug resistance to SXT.
5.Nursing care of a child with Beckwith-Wiedemann Syndrome frequent hypoglycemia

Objective:To summarize the experience of blood glucose management and nursing for a newborn with repeated hypoglycemia in Beckwith-Wiedemann syndrome.Method:A multidisciplinary team was formed, and formulated an individualized care plan for a large infant with Beckwith-Wiedemann syndrome. A dynamic blood glucose monitoring system was used to closely monitor blood glucose fluctuations for this children, dynamically adjusted the amount of infusion and milk according to the blood sugar situation, detected and dealed with hypoglycemia in time, strengthened the skin care of child and implemented psychological care for the family.Results:After refined treatment and care, the child′s vital signs were stable, and his blood sugar could maintain within a normal range before being discharged from the hospital.Conclusions:The use of dynamic blood glucose monitoring system under the guidance of a multidisciplinary team can effectively monitor and control the blood glucose fluctuations of children with BWS syndrome, which can provide a basis for further treatment of children.
