1.Cardiovascular safety perceptions of sulfonylureas based on evidence

The relationship between Sulfonylureas(SUs) and cardiovascular (CV) risks has attracted great attention.Previous studies have illustrated that some SUs increase CV risk in type 2 diabetes (T2DM) patients;while others do not.Clinical studies have found that the CV risks of different SUs may be different.Compared with other SUs;glimepiride and gliclazide may have lower CV risks.Here we reviewed evidence-based CV safety of SUs and the recent research progress.
2.A retrospective study of ultrasonographic characteristics and clinical factors affecting the BI-RADS classification (probably benign breast lesions)

Objective To investigate how many probably benign lesions on ultrasound(US) fulfilled the published criteria and to evaluate how clinical and personal factors influenced the categorization of breast lesions.Methods A total of 487 lesions in 487 women with more than 12 months follow-up after the initial category 3 assessment on US were included.The initial US images were retrospectively reviewed according to previously published criteria,and evaluated several factors that could influence the characterization of breast lesions in clinical practice such as age,multiplicity,palpability,radiologist 's experience,and lesion size.Results Of 487 lesions,479 (98.36%) were benign and 8 (1.64%) were malignant.Of 487 lesions,203(41.68%) including 8 malignancies were reassessed as category 4.If strict criteria had been applied at initial US examination,the positive biopsy rate would have been only 3.94% (8/203) and 96.06% (195 of 203) biopsies would have been unnecessary.Lesions in women 40 years or older and multiple lesions were more frequently reassessed as category 4 (P = 0.008 and P = 0.006,respectively).The presence of palpability,lesion size,and the radiologist 's experience did not significantly influence the categorization of breast lesions on US.Of 487 probably benign lesions,41.68 % could be classified as category 4 lesions when strict criteria were applied in initial practice.Conclusions The multiplicity of the lesion and the patient 's age were found to have a significant influence on the classifcation of probably benign solid masses.
3.Posterior arch length of individual normal occlusion of adults in Lanzhou

BACKGROUND:Posterior arch length deficiency is closely related to the formation and development of malocclusion.
OBJECTIVE:To measure the posterior arch length in individual normal occlusions of adults in Lanzhou.
METHODS:A total of 102 current col ege students with individual normal occlusions, with a mean age of (19.33±2.67) years (range 18-24 years), including 47 males and 55 females from Gansu Province, were chosen to take lateral cephalograms. Al research objects were divided into three groups according to mandibular plane angle:high angle group (Frankfort mandibular-plane angle>32°), low angle group (Frankfort mandibular-plane angle<22°) and control group (22°≤Frankfort mandibular-plane angle≤ 32°). Posterior arch length of both maxil ary and mandibular arches was measured, and the data were compared between genders and groups and in groups.
RESULTS AND CONCLUSION:Posterior arch length of maxil ary arch was slightly longer in male group than that in female group, and the difference was not significant (P>0.05). The posterior arch length of mandibular arch was slightly longer in male group than that in female group, but the difference was no significant (P>0.05). Comparison between groups revealed that the posterior arch length of low angle mandibular arch was significantly longer than that of high angle mandibular arch in male group, and the difference was significant (P<0.05). The results indicate that the gender wil not influence the development of posterior arch length. Different vertical facial types and different genders have different posterior arch length in individual normal occlusions of adults
4.Effect of taurine on changes of nuclear and cytosolic [Ca~(2+) ] elicited by H_2O_2 in rat vascular smooth muscle cells

AIM As one of the reactive oxygen species, toxic dose of H 2O 2 leads to the necrosis or apoptosis of many kinds of cells. It is nuclear whether these processes are related with the changes of nuclear Ca 2+ elicited by H 2O 2. This study on the effect of taurine on changes of nuclear and cytosolic (Ca 2+ ) elicited by H 2O 2 in rat vascular smooth muscle cells. METHODS The techniques of Fluo 3 and laser scanning confocal microscopy were used. RESULTS Rapid and sustaining rise of nuclear [Ca 2+ ] ([Ca 2+ ] n) and cytosolic [Ca 2+ ]([Ca 2+ ] c) were found in cultured rat vascular smooth muscle cells upon adding of 0 5% H 2O 2. Pretreatment of the cells with 20 mmol?L -1 taurine, a regulator of cellular Ca 2+ homestasis, significantly decreased the rise amplitude of [Ca 2+ ] n ( P 0 05) elicited by H 2O 2. It was suggested that the changes of [Ca 2+ ] n and [Ca 2+ ] c have different respond to taurine. CONCLUSION It was concluded that there exist the independent regulating mechanisms of Ca 2+ in the nucleus from differences between the nucleus and cytosol Ca 2+ in sensitivity to action of H 2O 2 and taurine.

6.Prognostic Analysis of Skull-base Invasion of Nasopharyngeal Carcinoma Based on Magnetic Resonance Imaging

[Objective]To evaluate the prognostic value of skull-base invasion of nasopharyngeal carcinoma(NPC)based on magnetic resonance imaging(MRI).[Methods]A total of 924 patients who were diagnosed with NPC between 2003 and 2004,had undergone MRI scan and received mdiothempy as their primary treatment,and had no distant metastasis were included in this study.MRI images and medical records were analyzed retrospectively.All the 924 eases.patients who developed skull-base invasions based on MRI,315 patients with T3 disease and 227 patients with T2 disease were selected for analysis.The staging was according to the sixth edition of the American Joint Commission on Cancer(AJCC)staging system.[Results]Incidence of skullbase invasion according to MRI was 55.4%.Of 924 cases.skull-base invasion on MRI was not an independent prognostic factor for overall survival(OS)and distant metastasis-free survival(DMFS),but was a marginally significant independent prognostic factor for local relapse-free survival(LRFS),P=0.068.Grading of MRI-detected skull-base erosion according to the site of invasion was an independent prognostic factor for OS(P=0.002 and P=0.005)and DMFS(P=0.001 for both)in the 512 patients with skull-base invasions and 315 patients with T3 disease.Severe-grade of skull-base invasion on MRI was an independent prognostic factor for OS and DMFS in the 924 patients(P < 0.001 for both).No significant differences were observed on OS,LRFS,and DMFS between T2a patients and T3 patients with low-grade of MRI-deteeted skull-base involvement.[Conclusions]Skull-base invasion based on MRI is not an independent prognostic factor for NPC.However,severe-grade of invasion according to the site of involvement has positive prognostic value.
7.Inhibition of three pentacyclic triterpenoids on calcium-induced liver mitochondrial permeability transition in mice.

OBJECTIVETo study effects of three pentacyclic triterpenoids, oleanolic acid (OA), ursolic acid (UA) and asiatic acid (AA) on Ca(2+)-induced liver mitochondrial permeability transition (MPT).
METHODEffects of three compounds on liver MPT induced by Ca2+ were assessed by measuring the change in mitochondrial swelling, mitochondrial membrane potential and release of matrix Ca2+ in vitro.
RESULTObvious mitochondrial swelling, loss of mitochondrial membrane potential and release of matrix Ca2+ occurred after the addition of 50 micromol x L(-1) Ca2+. However, preincubation with 50 mg x L(-1) OA, UA or AA significantly blocked the above changes. In addition, it was also found that there are differences in the inhibitions of three compounds on liver MPT induced by Ca2+.
CONCLUSIONThree pentacyclic triterpenoids, OA, UA and AA, have significant mitochondrial protection through blocking on liver MPT and the inhibition on liver MPT of AA is stronger than that of UA and OA.

8.Clinical analysis of intraoperative hyperthermic intraperitoneal chemotherapy combined with total laparoscopic D2 radical gastrectomy in treatment of gastric cancer

Objective:To explore the efficacy and safety of intraoperative hyperthermic intraperitoneal chemotherapy combined with total laparoscopic D2 radical gastrectomy in the treatment of gastric cancer.Methods:The clinical data of 127 patients with gastric cancer who were admitted to the Central Hospital of Hanzhong in Shaanxi Province from August 2017 to July 2019 were retrospectively analyzed. All patients underwent total laparoscopic D2 radical gastrectomy, of which 58 patients underwent total laparoscopic D2 radical gastrectomy combined with intraoperative hyperthermic intraperitoneal chemotherapy (observation group), and 69 patients underwent total laparoscopic D2 radical gastrectomy (control group). Observation indicators included surgical and postoperative recovery situations and postoperative tumor-related indicators. Follow-up was performed by using outpatient examination and telephone interview, and the content of follow-up included patient's adjuvant chemotherapy, tumor recurrence and metastasis, and surgery-related complications.Results:In the observation group, the intraoperative blood loss was (199±48) ml, the number of lymph node dissection was 35±8, the total hospitalization cost was (53 261±4 316) yuan, alanine aminotransferase was (30±10) U/L, and creatinine was (124±26) μmol/L; in the control group, the intraoperative blood loss was (184±46) ml, the number of lymph node dissection was 34±13, the total hospitalization cost was (52 146±4 817) yuan, alanine aminotransferase was (31±10) U/L, and creatinine was (128±33) μmol/L; there were no significant differences between the two groups ( t values were 1.833, 0.618, 1.363, 0.721, and 0.856, all P > 0.05). In the observation group, the operating time was (352±44) min, carcinoembryonic antigen (CEA) at 1 month after operation was (3.9±2.1) ng/ml,CEA at 6 months after operation was (12.7±7.2) ng/ml, tumor abnormal protein (TAP) at 1 month after operation was (75±36) μm 2,TAP at 6 months after operation was (131±33) μm 2; in the control group, the operating time was (308±58) min,CEA at 1 month after operation was (8.3±4.5) ng/ml, CEA at 6 months after operation was (15.8±4.2) ng/ml, TAP at 1 month after the surgery was (88±24) μm 2, TAP at 6 months after operation was (149±37) μm 2; there were significant differences between the two groups ( t values were 4.792, 7.185, 2.832, 2.284, and 2.984, all P<0.05). One hundred and twenty seven patients were followed up for 12-24 months. Fifty-one and 58 patients in the observation group and control group received postoperative adjuvant chemotherapy, and there was no significant difference between the two groups ( χ2 = 0.389, P = 0.533). Tumor recurrence was respectively detected in 0 and 6 patients in the observation group and control group at 6 months after operation; tumor recurrence was respectively detected in 2 and 11 patients in the observation group and control group at 1 year after operation; the differences in the recurrence rates between the two groups were statistically significant (both P < 0.05). Conclusion:Intraoperative hyperthermic intraperitoneal chemotherapy combined with total laparoscopic surgery for gastric cancer does not increase the patient's perioperative risk and the incidence of postoperative complications, and it can reduce the risk of postoperative recurrence and improve the short-term efficacy.
9.The effects mutant DJ-1 and sumoylation of DJ-1 protein on DJ-1 mitochondrial localization

Objective To investigate the effects of DJ-1 A39S mutation in Parkinson’s disease(PD)and SUMO modification of DJ-1 on DJ-1 mitochondrial localization. Methods Five COS-7 cell lines that can stably express wild type,A39S mutant,K130R mutant,A39S K130R double mutant DJ-1 protein and SUMO-1 protein were constructed. The transfection results were detected by Western blot,double enzyme digestion and PCR sequencing. The expression and subcellular localization of DJ-1 protein were observed by Western blotting,confocal microscope and immunofluorescence.Results Immunofluorescence showed that DJ-1 protein was mainly distributed in the cytoplasm and also in the nucleus.Some wild-type and K130R DJ-1 proteins were distributed in mitochondria,most of A39S and A39S-K130R mutant DJ-1 proteins were transferred to mitochondria,and SUMO-1 was mainly distributed in the nucleus and around the nucleus.Immunofluorescence co localization showed that four DJ-1 proteins were colocalize with SUMO-1 protein. 10.Research advances in brain lactate level and hepatic encephalopathy

Hepatic encephalopathy (HE) is a common complication and an independent risk factor for death in patients with liver cirrhosis. Brain lactate level is associated with the progression and severity of HE, and research on brain lactate level may help to further explain the pathogenesis of HE. This article summarizes the metabolic process of brain lactate, the association between brain lactate level and HE, and the potential therapeutic targets for HE and provides a reference for clinicians to further systematically evaluate the progression, treatment outcome, and prognosis of patients with HE, in order to reduce the medical burden of patients and improve the prognosis of patients with HE.
