1.The influence of exercise intensity on cardiorespiratory function in the sedentary elderly with coronary heart disease

Objective To observe the influence of exercise intensity on cardiorespiratory function in the elderly with coronary heart disease using simplified 24-action Taijiquan and jogging. Methods A total of 115 sedentary elderly patients with heart disease were randomly divided into a control group ( no exercise), a low-intensity exercise group, a medium-intensity exercise group (whose exercise was simplified 24-action Taijiquan at 40-60% of their individual maximum oxygen consumption (VO2max) for 90 to 200 minutes per week) and a high-intensity exercise group ( whose exercise was jogging on a treadmill at > 70% of their maximum oxygen consumption for 200 minutes per week). The regime was maintained for 6 months. All patients accepted a treadmill exercise test before and after the 6 months of exercise to evaluate the changes in VO2max, maximum heart rate ( HRmax), VO2max/HRmax and exercise time (t). Results ( 1 ) After 6 months of exercise, VO2max, HRmax, VO2/HRmax and t had increased sisnificantly in all three exercise groups. Work capacity had increased 18.6% , 33% and 34.4% and HRmax had increased 9.3% , 13.8%, 19.1%. There was no difference between before and after in the control group. (2) Compared with the control group the average VO2max, HRmax and t of all three exercise groups had increased significantly. VO2 max was 9.4% , 26% and 23. 1% better; HRmax was 6.7% , 8.8% and 12% better; t was 3.5,4 and 4.9 minutes longer. (3) Compared with the low-intensity exercise group, VO2max, HRmax and t in the medium- and high-intensity groups increased significantly more. VO2max increased 16.6% and 13.7% more; HRmax increased 2.1% and 3.2% more and average t was 0.5 and 1.4 minutes longer. (4) There was no significant difference between the medium- and high-intensity exercise groups in terms of VO2max. (5) The incidence of self-reported adverse responses to exercise such as dizziness, chest stuffiness and fatigue in the high-intensity exercise group increased significantly compared with the other 3 groups. Conclusions Exercise can gradually improve cardiorespiratory function in the sedentary elderly with heart disease. The effects of medium-intensity exercise are similar to those of high-intensity exercise, but medium intensity is safer.
2.Value of social support rating scale using in patients with vasovagal syncope

Objective:To explore the value of social support rating scale using in patients with vasovagal syncope (VVS) .Methods:Social support rating scale (SSRS) established by XIAO Shui-yuan was used .A total of 200 VVS patients were collected from inpatients and outpatients and they received social support status investigation ,another 100 normal people were collected from our physical examination center were regarded as control (normal control group) .Results:Compared with normal control group ,there were significant reductions in scores of each item and total score of social support [ (37.22 ± 9.15) scores vs .(33.50 ± 8.64) scores] in VVS group ,P<0.05 or <0.01 ;compared with male patients ,there were significant rise in scores of subjective support [ (17.63 ± 6.72) scores vs . (18.62 ± 7.56) scores] ,utilization of support [ (7.51 ± 2.26) scores vs .(8.92 ± 2.82) scores] and total social sup-port [(34.32 ± 10.61) scores vs .(36.79 ± 7.86) scores] ,and significant reduction in objective support score [(9.28 ± 4.15) scores vs .(8.37 ± 3.49) scores] in female patients , P<0.05 or <0.01;compared with middle-aged and aged group ,there were significant rise in scores of subjective support [ (17.08 ± 6.34) scores vs .(19.51 ± 6.73) scores] and total social support [(33.18 ± 10.74) scores vs .(36.87 ± 8.73) scores] ,and significant reduction in uti-lization of support score [ (8.47 ± 2.60) scores vs .(7.53 ± 1.95) scores] in young group ,P<0.05 or <0.01. Con-clusion:Social support assessment is help to perform individualized diagnosis and therapy for patients with vasovagal syncope ,and help to improve their quality of life .
3.PDGF-induced proliferation of smooth muscular cells is related to the regulation of CREB phosphorylation and Nur77 expression.

This study examined the relationship between PDGF-induced proliferation of vascular smooth muscle cells (VSMCs) and Nur77 expression and the effect of atorvastatin on VSMC proliferation and Nur77 in PDGF-treated VSMCs. Rat VSMCs were isolated and cultured. After incubation with atorvastatin or Nur77 siRNA, the cells were stimulated with PDGF and detected for BrdU incorporation to measure the proliferation of the VSMCs. Quantitative PCR and Western blotting were used to determine the Nur77 protein and the CREB phosphorylation level, to observe their relations with PDGF-induced VSMC proliferation. Our results showed that PDGF increased the BrdU incorporation in VSMCs, suggesting that it induced the proliferation of the cells. The VSMC proliferation was associated with increased Nur77 expression and elevated CREB phosphorylation. Atorvastatin inhibited the PDGF-induced VSMC proliferation, suppressed Nur77 expression. After silencing of Nur77 gene, the PDGF-induced VSMC proliferation was decreased. It was concluded that PDGF-induced VSMC proliferation was related to the Nur77 expression and CREB phosphorylation. Atorvastatin reduced the Nur77 expression and, at the same time, inhibited the VSMC proliferation.
4.The construction and exploration of micro-lecture in Pathology

[Adstract] Micro-lecture, as the main teaching form of MOOC and SPOC network teaching platform, is deeply welcomed by the majority of students. Pathology is the bridge connecting basic medicine and clin-ical medicine in medical education, which has such characteristics as many knowledge points, dispersion and obscurity, etc. Therefore, the production of micro courses is based on the knowledge points, and according to the characteristics of the knowledge points, it should also be applied reasonably according to the charac-teristics of each chapter. Adopting the teaching of self-study after class and teachers and students' discussion in class for micro courses can not only greatly improve the quality of teaching, but also develop the students' ability of independent thinking and the ability to analyze and solve problems, which transforms the teaching of pathology from pure knowledge to ability.
5.Effects of phenytoin on VEGF and SCF in rat bone marrow mesenchymal stem cells and vascular endothelial cells co-culture system

ABSTRACT:Objective To investigate the effects of phenytoin (PHT)on the secretion of vascular endothelial growth factor (VEGF)and stem cell factor (SCF)based on the establishment of indirect co-culture system of rat bone marrow mesenchymal stem cells (BMSCs)and vascular endothelial cells (VECs).Methods Indirect co-culture model of rat BMSCs and VECs was established.Experimental groups:indirect co-culture groups (PHT concentrations were 0,20 and 40 μg/mL);the control group:BMSCs culture group and VECs culture group (PHT concentrations were 0,20 and 40μg/mL).The contents of VEGF and SCF in the culture supernatant were measured using double antibody sandwich ABC-ELISA method on cultivation days 2,4,6.Results ELISA assay of the rBMSCs and rVECs in indirect co-culture supernatants,collected on culture days 2,4 and 6 showed that:① VEGF:On culture day 2,VEGF level in the co-culture groups was significantly higher than those in BMSCs group (P <0.05)and rVECs group (P <0.001).As culture time prolonged and PHT concentration increased to 20 μg/mL and 40 μg/mL,VEGF level increased too (P <0.001,P <0.05 ).② SCF testing results showed that the secretion of SCF in co-culture groups was higher than that in the control groups.When PHT was 20 μg/mL,the secretion of
SCF increased as the incubation time increased;but as the incubation time increased, PHT concentration of 40 μg/mL made SCF content decrease.Each group did not significantly differ (P > 0.05 ).Conclusion PHT promotes the secretion of VEGF and may reduce the secretion of SCF.
6.Pharmacokinetics Study of Schisandrin in Shengmai Granule

objective To study the pharmacokinetics of lignans components in Shengmai granule in volunteers and in mice. Methods After oral administration of Shengmai granule (3.6 g/person) for the volunteers and ig administration of the drug (4.7 g/kg) for the mice, the plasma was collected at different time points. The lignans components in Shengmai granule and in the plasma were analyzed by HPLC to monitor the changes of plasma concentration of schisandrind. Pharmacokinetic parameters were calculated from the plasma concentration- time data with the 3P97 software package. Results After oral administration of Shengmai granule by volunteers and mice, schisandrin and some new components in plasma were detected. The new components may be the metabolites of schisandrin. The main pharmacokinetic parameters of schisandrin in mice and in volunteers were as follows: T1/2ka was 0.03 and 0.04 hour, T1/2ke 0.88 and 0.86 hour, Vd 19.12 and 1.73 L? kg- 1, CL 15.06 and 1.46 L? h- 1? kg- 1, Cmax 1.196 and 0.098 mg? L- 1, Tpeak0.21 and 0.50 h, AUC0- ∞ 1.096 and 0.137 mg? h? L- 1, respectively. Conclusion Schisandrin in Shengmai granule can be absorbed in the volunteers and mice after oral administration. It can be absorbed and eliminated rapidly, and can be transformed into the metabolite. The pharmacokinetics of plasma Schizandrin complies with linear kinetic course.
7.Research on the relationship between proinflammatory cytokines and cognitive function in patients with mild cognitive impairment

Objective To investigate the relationship between proinflammatory cytokines and cognitive function in patients with mild cognitive impairment (MCI).Methods From May 2007 to May 2009,70 patients (aged ≥ 60 years) with MCI were collected.Among them,50 cases were amnestic MCI,and 19 cases developed into AD.The cognitive function was assessed,and all patients were followed up.The venous blood samples were obtained and proinflammatory cytokines (IL-1α,IL 1β,IL-6,TNF-α) were detected by using enzyme linked immunosorbent assay.Results There were differences in the levels of proinflammatory cytokines between patients with aMCI and patients with Alzheimer's disease (AD) [IL-1β,(40.5 ± 7.7) μg/L vs.(38.6 ± 7.3) μg/L ; IL 6,(70.4 ±24.3) μg/'L vs.(53.6±20.5) μg/L;TNF-α,(58.6±13.5) μg/'L vs.(50.3±-17.1) μg/'L;t=3.537,2.229,2.226,P=0.002,0.039,0.039,respectively].Conclusions MCI is a preclinical state of AD.The cognitive function damage of MCI patients are different from that of AD patients,and the immune status of MCI patients is also changed.
8.Effects of cannulation time on post-endoscopic retrograde cholangiopancreatography pancreatitis

Objective To analyze the risk factors of post-ERCP pancreatitis (PEP) and to evaluate the relationship between cannulation time and PEP.Methods The data of cannulation time in 1 625 patients who underwent ERCP from 2010 to 2012 were retrospectively studied.The risk factors associated with PEP were analyzed by univariate and multivariate Logistic regression analysis.The effect of different cannulation time on PEP was evaluated.Results The incidence of overall PEP was 4.6% (75/1 625) including 4.1% (67/1 625)of mild and 0.5% (8/1 625)of moderate-to-severe.Univariate analysis revealed that diabetes mellitus (P =0.02),choledocholithiasis (P =0.02),malignant biliary stenosis (P =0.007),duodenal stenosis (P =0.029),precut (P<0.01),cannulation time ≥ 8 min (P<0.01),blood platelet count ≥ 180× 109/L(P =0.089),alkaline phosphatase ≥ 120 U/L (P =0.083) and total bilirubin ≥ 17.1 μmol/L (P =0.094)were associated with PEP.Multivariate analysis revealed that precut (OR=1.93,95%CI:1.10-3.39,P=0.022),cannulation time ≥8 min (OR =3.50,95%CI:2.00-6.13,P<0.01) and duodenum stenosis (OR=2.92,95%CI:1.08-7.86,P=0.034) were independent risk factors of PEP.Within 30 min of cannulation,longer cannulation time was accompanied with higher PEP rate.Conclusion The cannulation time is an independent risk factor of PEP.Overall PEP is increased when cannulation time is more than 8 min.
9.Radioimmunoimaging of ~(99m)Tc labeled anti-mouse uterine cervical cancer monoclonal antibody Au_(14-1) in tumor-bearing mice

AIM and METHODS: A study of radioimmunoimaging was carried out on Kcnming mice - uterine cervical cancer (U14) using 99mTc labeled monoclonal Au14-1 with a modified Schwartaz method. RESULTS: The bio - distribution showed that radioactivity accumulated in tumor tissue at 12h after 99mTc - Au14- 1 injection in tail vein. The uptake by tumor was 4. 12 % ID/g at 2h and 8. 79 % ID/g at 24h respectively. The tumor/non - tumor (T/NT) radiocativity ratios for organs except kidneys were ranged from 2.02 to 6.71 at 24h post - injection. The image of tumor showed at 12h and clearer at 24h after injection. CONCLUSION: The quality of tumor image was relevant to the T/NT radioactivity ratios. It was demonstrated that 99mTc- Au14-1 has a good capability of localization for tumor.
10.Clinical study of minimally invasive internal fixation treatment of talus fracture

Objective To study and summary the treatment of talus fracture. Methods Fifteen patients with talus fracture were treated by limited cut off reduction by leverage and hollow lag screw under C arm X-Ray machine January 2008 to November 2014, of whom there were 11 males and 4 females, aged 15-61 years old. Seven patients sufferred from talus fracture because of traffic accidents, 4 patients because of falls, 2 patients because of crush injury, and 2 patients because of sprains. Hawkins typing:Ⅰtype in 1 patient, Ⅱ type in 6 patients, Ⅲ type in 6 patients of Ⅲ type, and Ⅳ type in 2 patients. Results Followed up for 6 months to 4 years (mean 1.5 years), fracture healed better. Hawkins grading standard:9 patients showed excellent (1 patient ofⅠtype, 5 patients ofⅡtype, 3 patients ofⅢtype), and two patients showed good (Ⅲtype). The excellent and good rate was 11/15. Necrosis of talus occurred in one patient, and degenerative joint disease occured in one patient. Conclusions Minimally invasive internal fixation with hollow lag screw under C arm X-Ray machine in the treatment of talus fracture has a small injury to the periosteum and the blood supply of small soft tissue. It can fix reliably and can pressurize the fracture fragments, and is conducive to fracture healing.
