2.Prokaryotic expression and purification of Spink12,a mouse epididymal secretary protein

Objective To study the effect of immunocontraception of Spink12,the fusion protein 6?His-Spink12 was induced to express in E.coli BL21,and purified.MethodsThe total mouse epididymal RNA was extracted,and the Spink 12 protein coding exon without signal peptide sequence was amplified by RT-PCR,digested by restrictive enzyme Nde Ⅰ and EcoR Ⅰ,and then cloned into the prokaryotic expression vector pET28 (a) which contained the 6?His tag in N terminal.The recombinant plasmid pET28a-Spink12,after being confirmed by enzyme digestion and sequencing,was transformed into the competent cells of E.coli BL21.The fusion protein 6?His-Spink12 was expressed by IPTG induction and analyzed by SDS-PAGE and Western blotting.The products were purified by Ni-NTA under native condition with different imidazole gradients and characterized by Western blotting with anti-His monoclonal antibody.ResultsThe results of digestion and sequencing indicated that the recombinant prokaryotic expression vector pET28a-Spink12 was successfully constructed.After E.coli BL21 was transformed with pET28a-Spink12 and induced with IPTG,the recombinant target protein of about 9 kDa was obtained by SDS-PAGE analysis,which was coincident with the theoretical value.The fusion protein was of 23.5% of the total E.coli cell proteins.The Western blotting analysis with anti-His monoclonal antibody showed that the band was fusion protein 6?His-Spink12.After purified with Ni-NTA,a higher quality of fusion protein 6?His-Spink12 was obtained in a purity of 91.1%.ConclusionsThe fusion protein 6?His-Spink12 has been successfully expressed in E.coli BL21 and purified.It may provide a base for the study of the effect of Spink12 on immunocontraception.
3.Risk factors of airway mucous plug caused byMycoplasma pneumoniae pneumonia in children

Objective To analyze the risk factors of mucous plug caused byMycoplasma pneumoniae pneumonia (MPP) in children and guide clinical therapy to decrease the incidence of sequela caused by airway inlfammatory obstruc-tion.Methods We conducted a retrospective study of the clinical data of 116 children with MPP who received ifber broncho-scope from January 2012 to January 2014 admitted to the respiratory department of the Children's Hospital of Hebei provience. Based on the airway mucosal lesions seen through the bronchoscope, the subjects were divided into the mucous plug group (n=67) and the control group (n=49). Gender, age of onset, febrile course, pleural effusion, white blood cell counts, neutrophil ratio, C reactive protein (CRP), serum lactate dehydrogenase level (LDH), initial time of received macrolides and ifber bronchoscope were compared between the two groups. If the Results of single factor analysis showedP<0.05, the indicators were analyzed by the logistic regression analysis. The receiver operator characteristic (ROC) curve was drawn to evaluate the predictability of logistic regression model.Results The Results of single factor analysis showed that age of onset, febrile course, CRP, serum LDH, pleural effusion cases in the mucous plug group were higher than those in control group (P<0.05). The Results of the logis-tic regression analysis showed that older than three years (OR=7.45), febrile course more than 10 days (OR=4.01), the level of CRP and LDH higher than 40 mg/L (OR=5.41) and 350 U/L (OR=3.63) were respectively the risk factors of mucous plug caused by MPP in children. The area under the ROC curve is 0.846 (95%CI: 0.773-0.919,P=0.000).Conclusions MPP children with age older than three years, or with febrile course more than 10 days, or with the level of CRP and LDH higher than 40 mg/L and 350 U/L has the possibility of forming a mucous plug.
4.Study on Quality Standard of Buxin Ruanmai Granules

Objective To establish the quality standard of Buxin Ruanmai granules. Methods TLC was used to identify Ophiopogonis, Corni Fructus, Hirudo, Ginkgo Folium and Polygoni Cuspidati Rhizoma et Radix. HPLC was used to determine the content of salidroside and loganin. Results Ophiopogonis, Corni Fructus, Hirudo, Ginkgo Folium and Polygoni Cuspidati Rhizoma et Radix could be identified by TLC. The linear range of salidroside was within 0.290 2-2.902 4 μg and the average recovery (n=6) was 99.64%(RSD=2.72%). The linear range of loganin was within 0.083 1-0.831 2 μg and the average recovery (n=6) was 100.12% (RSD=2.27%). Conclusion The method is simple, accurate and reproducible, which can effectively control the quality of Buxin Ruanmai granules.
5.Changes of multimodality therapeutic patterns of care study for resectable gastric cancer

Objective:To investigate and analyze the current status of multimodality therapy for resectable gastric cancer, aiming to provide reference for optimizing the multimodality treatment strategy for gastric cancer.Methods:Clinical data of patients diagnosed with gastric adenocarcinoma undergoing radical gastrectomy in the Second Affiliated Hospital of Soochow University were retrospectively analyzed. Clinical characteristics, preoperative medical comorbidities, pathological features, surgical and perioperative status and clinical efficacy were recorded. The gap between the diagnosis and treatment procedures and the standard guidelines was analyzed. The changes in the multimodality treatment patterns for gastric cancer were understood.Results:A total of 265 patients were included in this study. All patients were divided into two cohorts: early[2008] and late[2013] cohorts. In the early cohort, 127 patients were assigned, and 138 cases in the late cohort. In the early cohort, 67 patients (52.8%) underwent D 2 lymph node dissection, significantly less than 83 patients (60.1%) in the late cohort ( P<0.01). In the early and late cohorts, the proportion of patients with the number of lymph node dissection of ≥15 was 5.5% and 52.8%( P<0.01). The median number of lymph node dissection was increased from 6 to 16. The proportion of patients receiving neoadjuvant chemotherapy in the early and late cohorts was 2.4% and 3.6%( P=0.55). In the early cohort, the proportion of patients treated with postoperative chemotherapy and postoperative adjuvant chemoradiotherapy was 62.6% and 2.4%, significantly higher compared with 58.0% and 8.0% in the late cohort ( P=0.04). In addition, the proportion of patients receiving postoperative chemotherapy in the early cohort was 62.2%( n=79) and 58.0%( n=80) in the late cohort ( P=0.48). Conclusions:Although the level of radical gastrectomy has been continuously improved and standardized in China, which still lags behind the standard D 2 radical gastrectomy in Japan and South Korea. Adjuvant therapies including postoperative adjuvant radiotherapy can bring clinical benefits. However, the proportion of patients receiving adjuvant therapy is still low, and the multimodality therapy of gastric cancer should be widely applied.
6.A meta-analysis of controlled clinical trials comparing postoperative adjuvant chemoradiotherapy with adjuvant chemotherapy in patients with gastric cancer

Objective To compare postoperative adjuvant chemoradiotherapy with adjuvant chemotherapy in patients with gastric cancer by a meta-analysis.Methods PubMed,EMbase,Cochrane Library,Wanfang,CNKI,VIP,and CBM databases were searched to identify the controlled clinical trials of postoperative adjuvant chemoradiotherapy versus adjuvant chemotherapy for gastric cancer.The obtained data were analyzed using RevMan 5.2.5 and Stata 12.0.The difference between two groups was estimated by calculating the odds ratio (OR) with 95% confidence interval (CI).Results A total of 12 controlled clinical trials involving 1674 gastric cancer patients,which were selected according to inclusion and exclusion criteria,were included in this meta-analysis.The meta-analysis showed that the 3-and 5-year survival rates were significantly higher in the adjuvant chemoradiotherapy group than in the adjuvant chemotherapy group (OR=2.96,95% CI=1.75-5.03,P=0.000; OR=1.45,95% CI=1.06-1.99,P =0.020);the local recurrence rate was significantly lower in the adjuvant chemoradiotherapy group than in the adjuvant chemotherapy group (OR =0.50,95% CI =0.34-0.72,P =0.000) ; there was no significant difference in distant metastasis rate between the two groups (OR =0.79,95% CI =0.58 -1.07,P =0.130).Conclusions The meta-analysis of existing study results shows that compared with adjuvant chemotherapy alone,adjuvant chemoradiotherapy is a relatively safe and effective postoperative treatment for gastric cancer.
7.Comparison of CRP,PCT and IL-6 in patients with two types of ends of HBV infection

Objective We investigate the possible effect of inflammative indicator C reactive protein(CRP) ,interleukin‐6(IL‐6) and procalcitonin(PCT) on the outcome of HBV infection .Methods The clinical data and peripheral serum of 48 patients with HBV related hepatocellular carcinoma and 48 patients with decompensated cirrhosis were collected ,HBV copy numbers ,PCT ,IL‐6 , CRP ,ALT ,AST ,prothrombin time(PT ) and other indicators were tested for comparison and correlative analysis .Results HBV copy numbers ,PCT ,CRP ,lymphocytes and mononuclear cells showed no significant difference between primary hepatocellular car‐cinoma and decompensated cirrhosis group;however ,IL‐6 level was significantly higher in decompensated liver cirrhosis than in hep‐atocellular carcinoma ,as opposed to the absolute number of neutrophils .Conclusion IL‐6 increase may be related to final outcome of HBV infection ,dynamic monitoring IL‐6 is helpful to evaluate the prognosis of HBV infection .
8.Current research on molecular mechanism of ionizing radiation-induced neurogenesis disorders

With the development of modern radiotherapy techniques,radiotherapy has been widely used in the multimodality therapy for various malignant tumors,including head and neck cancers such as nasopharyngeal cancer and laryngeal cancer.A combination of surgery and radiochemotherapy significantly improves patients' cure rate and survival time;however,with the increase in survival time,some patients receiving radiotherapy develop marked cognitive impairment.Ionizing radiation-induced cognitive impairment mainly nanifests as hippocampus-dependent cognitive impairment,which is associated with inhibited hippocampal neurogenesis due to ionizing radiation.Therefore,it is necessary to investigate the mechanisms of the inhibition of hippocampal neurogenesis by ionizing radiation.This article reviews the molecular mechanism of neurogenesis disorders induced by ionizing radiation.
9. The effect of cardiac autonomic function on skeletal function and weakness in elderly diabetic patients

Objective:
To study the effect of cardiac autonomic nerve function on body function and weakness in elderly diabetic patients.
Methods:
A total of 100 patients diagnosed as diabetic autonomic neuropathy in the endocrine ward of Dalian Friendship Hospital from August 2017 to August 2018 were enrolled. The demographics, clinical data, and Activities of Daily Living (ADL) scale of each patient were recorded, and the Instrument Activities of Daily Living (IADL) and the weak score were used to record heart rate variability in each patient. Aweak score of 5 was used as boundaries. Patients with weak score greater than or equal to 5 were divided into weak group, and those less than 5 were divided into non-weak groups. Each indicators were compared between the two groups. The statistically significant variables were used as covariates, and the weak scores were used as dependent variables to perform direct and indirect Logistic regression analysis of covariates and dependent variables and the receiver operation characteristic (ROC) curve.
Results:

10.Relationship between monocytopenia and neutropenia during concurrent chemoradiotherapy in patients with nasopharyngeal carcinoma and cervical cancer

Objective:To evaluate whether the decrease in peripheral blood monocyte count was a potential predictor for neutropenia in patients with nasopharyngeal carcinoma and cervical cancer.Methods:The medical records of 95 patients with nasopharyngeal carcinoma and cervical carcinoma who received intensity-modulated radiation therapy (IMRT) combined with paclitaxel liposomes and platinum (TP) synchronous chemotherapy and presented with neutropenia in the Second Affiliated Hospital of Soochow University from January 2017 to December 2018 were retrospectively analyzed. Paired sample t-test was used to assess whether the number of days when the monocytes initially dropped/decreased to lowest level/eventually increased to normal value was significantly less than those of the neutrophils. In addition, the chi-square test was performed to determine the correlation between the degree of reduction in the absolute neutrophil count (ANC) and baseline absolute monocyte count (AMC). Results:The change trend of AMC was consistent with that of ANC in the two cycles of concurrent chemotherapy. The number of days when AMC initially decreased/decreased to the lowest level/finally increased to normal value was significantly less than that of ANC (4 d vs. 6 d, 4 d vs. 10 d, P<0.001; 5 d vs. 6 d, 6 d vs. 9 d, 7 d vs. 12 d, P<0.001). However, no correlation was found between the baseline level of monocytes and the degree of subsequent neutropenia [(AMC<0.4×10 9) vs.( AMC≥0.4×10 9)=32 vs. 63, P=0.172]. Conclusions:Decreased monocyte count is an important potential predictor for neutropenia and a significant indicator for guiding the next monitoring of neutrophil count and treatment with granulocyte colony-stimulating factor.
