Objective To discuss the clinical observation of diarrhea type irritable bowel syndrome with Shu-gan Jianpi prescription.Methods 82 cases of diarrhea-predominant irritable bowel syndrome were selected using a random number table,the observation group(n=45 )was treated with relieving liver and reinforcing spleen methods;the control group(n=37 )was treated with the universal application of clinical Pinaverium bromide,were observed after treatment of irritable bowel syndrome score,treatment effects as well as the clinical symptoms,the results were processed using SPSS 17.0 analysis.Results The scores of the two groups at 1 month after treatment,the control group was (157.38 ±72.72),the observation group was (101.52 ±60.16)(t=-7.994,P=0.000).The score of the two groups at 3 months after treatment,the control group was (172.71 ±62.91 ),the observation group was (109.92 ±42.38)(t=-8.428,P=0.000),the results had statistical significance;The total efficiency of the two groups at 1 month after treatment,the control group was 75.7%,the observation group was 95.6%(χ2 =6.909,P=0.019),the total efficiency of the two groups at 3 months after treatment,the control group was 86.5%,the observation group was 100%,the comparison was(χ2 =6.476,P=0.016),the results had statistical significance.On the aspect of the relieve of clinical symptoms,On the abdominal pain,the control group controlled 21cases,the observation group controlled 38 cases,compared(χ2 =7.713,P=0.007),On the diarrhea,the control group relieved 15 cases,the observation group relieved 30 cases,compared(χ2 =7.713,P=0.007),both had statistical significance.Conclusion The methods with relieving liver and reinforcing spleen,to the patients with irritable bowel syndrome medicine syndrome differentiation,play each medicine spleen and stomach,benefit lung and kidney tonic win effect,can effectively reduce symptoms and reduce IBS integration,reduce symptoms treatment results.

Objective To investigate the prevalence tendency and risk factors study of hematuria in occupational population of Changzhi city,Shanxi province.Methods Cluster sampling was used to select 29 852 occupational population from January 2007 to December 2010.Questionnaire survey,physical examinations and biochemical assessment were conducted among all subjects.Results A total of 5206 patients were indentified form all subject and enrolled in the final analysis.(1) The number of newly diagnosed cases increased year by year and females was highter than males (P ＜0.01).(2)Mixed hematuria rate increased from 22.5％ in 2007 to 45.3％ in 2010; and homogeneity hematuria decreased from 58.1％ to 14.9％ ;non-uniformity of hematuria increased from 19.3％ to 39.8％.(3) High triglycerides,and age ＞ 50 years were the a major risk factors of hematuria.The main risk factors in young group (＜ 39 years) was a major risk included urinary tract infections,stones,and physical exertion.Conclusion The hematuria rate in female higher than that of male patient.For men hematuria,high triglycerides,prostatic hypertrophy,age ＞ 50 years,high cholesterol were the risk factors.As for women,high triglycerides,gynecological diseases,age ＞ 50 years.High triglycerides and physical exertion were associated with hematuria.

OBJECTIVE:To observe the clinical efficacy and safety of Azithromycin combined with Xiaoer xiaoji zhike oral so-lution in the treatment of mycoplasma pneumonia in children. METHODS:Data of 120 children with mycoplasma pneumonia were retrospectively analyzed and divided into observation group (63 cases) and control group (57 cases) by different treatment meth-ods. All children received conventional treatment(reducing fever,relieving cough and resolving phlegm). Based on it,control group received Azithromycin 10 mg/kg,intravenous infusion,once a day,after continuous infusion of 3 d,it stopped 4 d. Observation group additionally received Xiaoer xiaoji zhike oral solution,10 ml for children younger than 5 years old,15 ml for children in 5 years old or over,3 times a day. 1-week was regarded as 1 treatment course,and it lasted for 2-3 courses. Clinical efficacy in 2 group was observed,recovery time of clinical signs,reduction of the number of inflammatory cells before and after treatment were compared,and the incidence of adverse reactions during treatment was recorded. RESULTS:After treatment,the total effective rate in observation group was significantly higher than control group,fever clearance time,disappearance time of cough,rales and pulmonary lesions were significantly shorter than control group,with statistical significance(P<0.05). Before treatment,there was no significant difference in the number of eosinophils and serum C reactive protein (CRP) level (P>0.05). After 1 week treat-ment,the number of eosinophils and CRP level in 2 groups were significantly lower than before,and observation group was signifi-cantly lower than control group,with statistical significance(P<0.05). And there was no significant difference in the incidence of adverse reactions(P>0.05). CONCLUSIONS:Azithromycin combined with Xiaoer xiaoji zhike oral solution shows good efficacy in the treatment of mycoplasma pneumonia in children,it can shorten the duration of clinical symptoms,improve the level of in-flammatory cells,and does not increase the incidence of adverse reactions.

To analyze congenital sensorineural hearing loss combined with blepharophimosis-ptosis-epicanthus inversus syndrome (BPES). For the case of cochlear implantation to child with congenital sensorineural deafness combined BPES, accomplish routine examination and assessment, combining with literature to analyze the clinical diagnosis of this disease and its significance. Sensorineural hearing loss is a common congenital diseases with neonatal incidence of 1 per thousand - 3 per thousand, 50%-70% of deafness is associated with genetic factors, the incidence of congenital sensorineural hearing loss combined with eye disease is about 40%-60%, mainly reflected in ametropia and retinopathy. BPES's main clinical manifestations is blepharophimosis, ptosis, epicanthus inversus, and telecanthus. BPES is a rare autosomal dominant disease caused by FOXL 2 gene mutation, sometimes associated with retarded growth, delayed development, congenital heart disease, and microcephaly. Suffering from both sensorineural hearing loss and BPES is rare in reported literature. This case is diagnosed by clinical examination, without visual impairment. Facial nerve dysplasia has been found during the surgery. For congenital deafness patients with eye disease or other diseases, timely and correct diagnosis has important clinical significance, which can improve the diagnostic rate and make it coming true to early intervention, and then, effectively improve the quality of the patients. There are few literature reports, of patients with two kinds of genetic diseases. Our inference is that the cases are rare or the patients has visited different departments and ignored the other systems' signs. Therefore, in such doubtful cases, we should do the professional comprehensive examination in daily clinical work in order to avoid missed diagnosis or delayed treatment and intervention. By analyzing this case, the patient may also suffer from facial nerve dysplasia. Preoperatively viewing CT scan and operatively facial nerve monitor being used can avoid the occurrence of surgical complications.

Aim To investigate the effects of Cariporide on expression of intercellular adhesion molecule-1(ICAM-1) and P-selectin and adhesion of monocytes and endothelial cells induced by lysophophatidylcholine(LPC).Methods The ratio of adhesion of monocytes and endothelial cells was assessed by measuring protein content.The expression of ICAM-1 and P-selectin in endothelial cells was examined by enzyme-linked immunosorbent assay(ELISA).Intracellular pH of endothelial cells was measured with 2',7'-bis(2-carboxyethyl)-5(6)-carboxyfluorescein(BCECF).Results The results showed that LPC enhanced the adhesion of monocytes and endothelial cells in a concentration-dependant and time-related manner.The dosage of 5,10 and 20 ?mol?L-1 Cariporide reduced the adhesive ratio of endothelial cells induced by LPC(5 mg?L-1)from 36% to 23%,18% and 16%(P

Amyotrophic lateral sclerosis is a progressive neurodegenerative disorder involving primarily motor neurons in the cerebral cortex,brainstem and spinal cord.The clinical diagnosis of ALS may be categorized into four levels of certainty according to clinical and electrophysiological examination,depending on the presence of upper and lower motor neuron signs together in the same topographical anatomic region in either the brainstem(bulbar cranial motor neurons),or cerival,or thoracic,or lumbosacral spinal cord(anterior horn motor neurons).Electromyography is of great importance in the diagnosis of ALS.Neruoimaging studies and clinical laboratory studies are helpful to exclude other disorders.

Objective To compare the effects of aero bi c exercise training of two different intensities on circadian blood pressure. Methods Twenty-eight sedentary patients with hyperte nsion were randomly allocated into 2 groups, and were treated with aerobic exerc ise training at 20% or 60% of the maximal intensity for 8 weeks, respectively. Not only were the relevant parameters of exercise capacity measured by use of m aximal bicycle ergometer tests, but also was the circadian blood pressure monito red pre-and post-therapy. Results Each paramet er of blood pressure decreased in both groups with similar circadian pattern. Conclusion The aerobic exercise training at 20% an d 60% of the maximal intensity have similar effects on circadian blood pressure.

Objective To investigate the changes of waist muscle density and area in the subjects with spiral CT scan examination,and through the quantitative analysis of different age and gender to get the degree of lumbar muscle degeneration.Methods 240 cases of lumbar CT scan reconstruction images (including the patients suffering from low back pain)were collected.According to the age,20 - 29 years old group,30 - 39 years old group,40 -49 years old group,50-59 years old group,60-69 years old group,and 70-79 years old group were set up.The lumbar transverse psoas muscle,lumbar side muscle,erector spinae cross-sectional areas (CSA)and mean CT values were measured at different levels.Scanning equipment:Toshiba Activion 16-slice spiral CT scanners, acquisition range from the waist 1 to S1.Results ①Compared between the measurement results of the same age groups (except 70-79 years old group),the average CSA of the male at the same level waist was higher than the female (P <0.05).②Compared between the same level groups of 20-29 years,30-39 years and 40-49 years, there were no significant differences (P >0.05).Compared between the same level groups of 50-59 years,60-69 years and 70-79 years,there was no significant difference (P >0.05).③Compared the CSA of man’s erector spinae muscle between 40-49 years group and 50-59 years group,there were significant differences (P <0.05).④Compared the same level CSA of female between 20-29 years,30-39 years,40-49 years,50-59 years,60-69 years,70-79 years groups,and the adjacent groups,there were no significant differences (P >0.05).⑤The same level average density of erector spinae muscle was lowest compared with the psoas major muscle and quadratus lumborum muscle,and falling faster,the psoas major muscle was relatively higher.The same level average density of female’s erector spinae muscle was lower than man’s,specially in 70-79 years group.Conclusion Multi-slice spiral CT can be used to diagnose the lumbar degenerative conditions.

Objective To evaluate the diagnostic efficiency of fiberoptic ductoscope versus galactography for nipple discharge. Methods Literatures on comparing the diagnostic value of galactography with fiberoptic ductoseope (FDS) on the nipple discharge were searched from the Cochrane library, Evidence based medicine (EBM ),PubMed,China Vip database, and 8 studies were intaked. Then the data was analyzed by software Metadisc1.4 and Review Manager5.1. Results There are 1170 patients in the 8 studies. The pooled estimates for sensitivity of FDS and galactography was 82%, 78%, and the pooled estimates for specificity was 51%, 50%, respectively. The weighted AUC (area under curve) was 0.776, 0.705, Q*values was 0.715, 0.657, respectively. The sensitivities and specificity of the 2 methods were not significantly different. The overall diagnostic efficacy of FDS was better than that of galactography. Conclusion FDS is better than galaetography in the diagnostic value of nipple discharge.
