1.Application of multi-slice CT in measurement of morphological parameters of waist muscle and its clinical significance

Objective To investigate the changes of waist muscle density and area in the subjects with spiral CT scan examination,and through the quantitative analysis of different age and gender to get the degree of lumbar muscle degeneration.Methods 240 cases of lumbar CT scan reconstruction images (including the patients suffering from low back pain)were collected.According to the age,20 - 29 years old group,30 - 39 years old group,40 -49 years old group,50-59 years old group,60-69 years old group,and 70-79 years old group were set up.The lumbar transverse psoas muscle,lumbar side muscle,erector spinae cross-sectional areas (CSA)and mean CT values were measured at different levels.Scanning equipment:Toshiba Activion 16-slice spiral CT scanners, acquisition range from the waist 1 to S1.Results ①Compared between the measurement results of the same age groups (except 70-79 years old group),the average CSA of the male at the same level waist was higher than the female (P <0.05).②Compared between the same level groups of 20-29 years,30-39 years and 40-49 years, there were no significant differences (P >0.05).Compared between the same level groups of 50-59 years,60-69 years and 70-79 years,there was no significant difference (P >0.05).③Compared the CSA of man’s erector spinae muscle between 40-49 years group and 50-59 years group,there were significant differences (P <0.05).④Compared the same level CSA of female between 20-29 years,30-39 years,40-49 years,50-59 years,60-69 years,70-79 years groups,and the adjacent groups,there were no significant differences (P >0.05).⑤The same level average density of erector spinae muscle was lowest compared with the psoas major muscle and quadratus lumborum muscle,and falling faster,the psoas major muscle was relatively higher.The same level average density of female’s erector spinae muscle was lower than man’s,specially in 70-79 years group.Conclusion Multi-slice spiral CT can be used to diagnose the lumbar degenerative conditions.
2.Efficacy of methotrexate combined with mifepristone in the treatment of ectopic pregnancy and its effect on serum human chorionic gonadotropin and cancer antigen 125 levels and endometrial thickness

Objective:To investigate the efficacy of methotrexate combined with mifepristone in the treatment of ectopic pregnancy and its effect on serum human chorionic gonadotropin (β-HCG) and cancer antigen 125 (CA125) levels and endometrial thickness.Methods:Eighty-two patients with ectopic pregnancy who received treatment in Ningbo Yinzhou No. 2 Hospital, China from August 2017 to August 2019 were included in this study. They were randomly assigned to receive treatment with methotrexate (control group, n = 41) or methotrexate combined with mifepristone (observation group, n = 41). Clinical efficacy was evaluated, and adverse reactions and clinical recovery indexes were observed in each group. Before and after treatment, serum biochemical indexes, endometrial thickness and mass diameter were compared between the observation and control groups. Results:Clinical efficacy in the observation group was significantly higher than that in the control group [95.12% (39/41) vs. 75.61% (31/41), χ2 = 6.248, P < 0.05]. The incidence of adverse reaction in the observation group was significantly lower than that in the control group [4.88% (2/41) vs. 19.51% (8/41), χ2 = 4.100, P < 0.05]. After treatment, serum β-HCG, CA125, progesterone and vascular endothelial growth factor levels in the control and observation groups were (1 106.26 ± 122.36) U/L and (902.65 ± 61.32) U/L, (32.26 ± 6.86) kU/L and (28.26 ± 5.26) kU/L, (15.33 ± 3.01) nmol/L and (10.33 ± 1.92) nmol/L, (85.36 ± 16.41) ng/L and (65.33 ± 12.22) ng/L, respectively, which were significantly lower than those before treatment ( t = 56.964 and 82.206, 12.380 and 17.642, 9.976 and 19.471, 7.490 and 12.912, all P < 0.05). After treatment, serum β-HCG, CA125, progesterone and vascular endothelial growth factor levels in the observation group were significantly lower than those in the control group ( t = 10.624, 3.304, 8.967, 6.991, all P < 0.05). After treatment, endometrial thickness and mass diameter in the control and observation groups were (8.23 ± 1.81) mm and (6.25 ± 1.25) mm, (2.21 ± 0.52) cm and (1.52 ± 0.35) cm respectively, which were significantly lower than those before treatment ( t = 3.555 and 15.118, 11.631 and 24.167, both P < 0.05). After treatment, endometrial thickness and mass diameter in the observation group were significantly less or smaller than those in the control group ( t = 6.248 and 7.861, both P < 0.05). After treatment, symptom disappearance time, serum β-HCG recovery time, urine β-HCG negative time, pelvic mass disappearance time and hospital stay in the control and observation groups were (7.26 ± 1.92) d and (4.23 ± 0.85) d, (16.25 ± 3.82) d and (12.33 ± 2.21) d, (15.36 ± 3.26) d and (10.66 ± 2.12) d, (24.12 ± 4.12) d and (18.66 ± 3.61) d, (29.58 ± 5.68) d and (23.52 ± 4.36) d, respectively. After treatment, symptom disappearance time, serum β-HCG recovery time, urine β-HCG negative time, pelvic mass disappearance time and hospital stay in the observation group were significantly shorter than those in the control group, respectively ( t = 10.305, 6.343, 8.631, 7.118, 6.044, all P < 0.05). Conclusion:Methotrexate combined with mifepristone is highly effective in the treatment of ectopic pregnancy because it can reduce adverse reactions, decrease serum β-HCG, CA125, progesterone and vascular endothelial growth factor levels, promote the recovery of serum β-HCG to normal level, reduce endometrial thickness, and promote adnexal mass disappearance, which are conducive to the early recovery of patients, with good clinical safety.
3.Protective effects of adenosine on cultured rat hippocampal neurons after oxygen-glucose deprivation

AIM:To investigate the protective effects of adenosine on cultured rat hippocampal neurons after oxygen-glucose deprivation. METHODS:The control and adenosine-treated hippocampal neurons cultured for 12 d were exposed to oxygen-glucose deprivation environment for 0.5-4 h and then cultured with original medium in normoxia for 24 h . The soma area,survival rate, effluxes of lactate dehydrogenase (LDH)and apoptosis of neurons were observed. RESULTS:The soma area, effluxes of lactate dehydrogenase from neurons and apoptosis were increased while survival rate of neurons was decreased after oxygen-glucose deprivation compared with those pre-oxygen-glucose deprivation. Compared with the control, after oxygen-glucose deprivation the soma area, effluxes of lactate dehydrogenase from neurons and apoptosis were decreased, however, the survival rate of neurons was increased in the adenosine group.CONCLUSION: Oxygen-glucose deprivation can lead to the severe damage of cultured hippocampal neurons, and adenosine can reduce neuronal injury induced by oxygen-glucose deprivation.
4.Development of the cross sign of olivepontocerebellar atrophy and its diagnostic value

Objective To explore the development of the cross sign of olivepontocerebellar atrophy(OPCA) and its diagnostic value.Methods The clinical and imaging data of 8 patients with OPCA were analyzed retrospec-tively.MRI imaging used SE sequence,plain T1 WI,T2 WI,axial,coronal,sagittal imaging.Results All patients were hide onset.The major clinical presentation was cerebellar atsxia and autonomic nerve dysfunction.Positive radiographic findings were consistent with clinical presentation,mainly for the brain stem atrophy,8 patients of the group had this performance,followed by cerebellar atrophy,6 patients had symmetrical atrophy and 2 patients had asymmetric atro-phy.6 patients after onset of 1.5 -3 years showed a typical cross.Conclusion The cross sign gradually appears at a certain stage of development,and it has high specificity.So the combination of clinical and MRI is valuable for the diagnosis and differential diagnosis of OPCA.

6.Clinicopathologic analysis of solid papillary carcinoma of breast

Purpose To investigate the clinical pathological features, histologic features, and prognosis of solid papillary carcinoma ( SPC) of breast. Methods Nine cases of SPC of breast were analyzed by morphological observation, AB-PAS staining and immuno-histochemical MaxVision staining. The follow-up and analysis of the clinical datas of these nine cases were performed and relevant liter-atures were also reviewed. Results The age of 9 cases of SPC of breast ranged from 32 to 83 years old (mean age is 69. 2 years old). Three cases presented with nipple discharge. Nuclear levels of 9 cases were between low to mid-level. Seven cases were associated with intracellular and extracellular mucus secretion, two cases with calcification, and 3 cases with comedonecrosis. All of these nine cases were ER, PR positive and the positive rates were high ( >70%) , HER-2 was all negative. Syn expression in these nine cases were diffused positive except for one case showing focal positive, 4 with CgA diffused positive, 1 with partial positive and 4 were negative of CgA. Six patients had partial loss of the expression of myoepithelial markers, and 3 cases with complete myoepithelial expression. Ki-67 proliferation index was between 3% and 20%. Except for only one case with small area of invasive ductal carcinoma (IDC) (2 mm in great diameter) , all other cases were not associated with other types of breast cancer, sentinel lymph nodes and/or axillary/supracla-vicular lymph nodes metastasis. 8 patients accepted breast-conserving surgey, one case did not undergo radical mastectomy for small le-sion. All of these cases were accepted further endocrine drugs treatment and only one case accepted chemical therapy simultaneously because of the accompanied IDC. None of them was found to relapse for 3 to 61 months (mean 19. 6 months) of follow-up. Conclusion Nine cases of SPC of breast showed some characteristics such as relatively low nuclear level, often accompanied by intracellular and extracellular mucus secretion, positive for neuroendocrine marker, low lymph node metastasis rate, and no recurrence, which suggest that SPC may have a better clinical prognosis.
7.THE FIELD TRIAL ON RAPID DETFCTION OF SCHISTOSOMA CIRCULATING ANTIGENS WITH DOT IMMUNOGOLD FILTRATION ASSAY (DIGFA)

A total number of 2221sera collected from different kinds of subjects , i.e.patients with various stages of schistosomiasis. normal individuals, persons with other parasitic diseases or non-parasitic diseases, were detected with Dot Immunogold Filtration Assay (DIGFA) using anti-SVLBP IgG as capturig antibody. Positive rate of 70 sera from patients with acute schistosomiasis was 100%, and that of 307 sera with chronic schistosomiasis was 68.4% .None of 200 sera from normal individuals showed false positive reaction. No obvious cross reaction was found in sera from other parasitic/non-parasitic diseases except sera from patients with paragonimiasis.2 batches of sera were detected with DIGFA by single-blindness method. Results showed that sensitivities to chronic schistosomiasis were 69.4% and 68.9%, and 100% to acute schistosomiasis while specificities were 98.9%-99% .508 samples from residents in endemic areas were tested with DIGFA and Kato-Katz. Positive rate of circulating arrtigen was 20.9% with DIGFA while 65 samples were positive with stool examination.46 samples were positive both by DIGFA and by Kato-katz.The coincidental rate was 70.8% .These results indicated that DIGFA showed sensitive and specific in the detection of circulating antigen. It will be useful in mass application and vahuable in epidemiological stuvey.
8.The relationship between number of peripheral artery stenosis and cardio-cerebrovascular events in the elderly

Objective To investigate the association between the number of periphery artery atherosclerotic stenosis and cardio-cerebrovascular events in elderly people.Methods Totally 163 subjects aged 60 years and over (median age 83 years) in special outpatient service of PUMC Hospital were retrospectively reviewed and underwent Doppler ultrasound of carotid,lower extremity,and renal artery.General conditions,cardio-cerebrovascular events,risk factors and results of periphery artery ultrasound were assessed.Results Age (F =13.413,P < 0.001 ),hypertension,hyperlipidemia and the statin users(x2 =24.961、13.592、16.207,all P<0.001) significantly enhanced along with increasing number of peripheral artery stenosis (P<0.001).The more the peripheral arteries stenosis,the more cardio-cerebrovascular events occurred (x2 =15.258,P < 0.001).Symptomatic peripheral artery obstructive disease increased the prevalence of cardio-cerebrovascular events.Conclusions Multiple periphery artery atherosclerotic stenosis is associated with cardiocerebrouascular events and Doppler ultrasound is effective to detect high-risk patients.
9.Morphologic changes in the upper airway in patients with acute cerebral infarction

Objective To investigate the feature of the morphology changes in the upper airway in patients with acute cerebral infarction and to find a new method to prevent and cure cerebral infarction.Methods Sixty-six patients with cerebral infarction confirmed by brain MRI or CT scan(within 3 weeks of onset) were recruited.The patients were examined by upper airway MRI scan and polysomnography (PSG).Then the patients were divided into obstructive sleep apnea hypopnca syndrome(OSAHS)group and non-OSAHS group.In addition.16 patients showing OSAHS but without stroke history(OSAHS nonstroke group)were included in the study.The sagittal and horizontal lengths of the nasopharynx,palatopharynx,glossopharynx and hypopharynx were measured and their closs-sectional areas were calculated.The length,thickness and cross-sectional area of the palate were also measured.Statistic analysis of each data among the groups was performed using SPSS software.Results Among 66 cases with acute cerebral infarction,75.8 % (50/66)were diagnosed with OSAHS.The anteropesterior diameer,left and right diameters and smallest section area in upper airway were all smaller in the OSAHS group with acute cerebral infaretion than those in the non-OSAHS group and OSAHS non-stroke group.The narrowest segments in upper airway were nasopharynx and ompharynx.which are caused by shortened left and right diameters.The area of the soft palate in the OSAHS-stroke group was significant bigger((452.2±99.6)mm2)than that in non-OSAHS group((350.0±69.4)mm2,t:4.575,P<0.05).The lowest SO2 in OSAHS-stroke group(68.9 % ±10.5 % )was the lowest among three groups.The more severe the airway constriction was.the higher the apnea-hypopnea index(AHI)was and the lower the lowest SO2 was.Conclusion Patients withl stroke show higher incidence of OSAHS and present more severe multilevel upper airway constriction.Upper airway constriction may be the new target of early treatment for better prognosis of cerebral infarction.
10.A combined study of diffusion tensor magnetic resonance imaging and magnetic resonance spectroscopy on amyotrophic lateral sclerosis

Objective To investigate the diffusion tensor imaging(DTI)and magnetic resonance spectroscopy(MRS)changes in amyotrophic lateral sclerosis(ALS)and to study on the physiopathology of ALS.Methods Thirty-eight AIS patients,8 patients with pure lower motor neuron involvement and 5 patients with mixed-type cervical spondylosis and 34 healthy controls were enrolled and examined with the conventional brain MRI,DTI and 1H-MRS.Results T2-weighted and T2 FLAIR images revealed abnormal high signals in bilateral pyramidal tract positions in 21%(8/38)of ALS patients,which was higher than that in the coaex When compared with the control group,the values of FA in the precentral gyms(0.492±0.059 vs 0.552±0.045,F=17.150,P<0.01),the posterior limb of the internal capsule(0.679±0.048 vs 0.727 4-0.031.F=19.481.P<0.01)and the cerebral peduncle(1eft:0.740 4-0.038 vs 0.761 4±0.024.F=4.290;right:0.720 ±0.044 vs 0.746 ±0.034,F=3.264,both P<0.01)of the ALS group were decreased significantly.so were the values of NAA/Cr.And the FA reduction rate was 10.9%,6.6% and 2.8%-3.5% respectively.There was also a significant decrease in FA in the anterior limb of the internal capsule and the occipital lobe in the ALS group.Conclusion Not only does the application of DTI and MRS help to demonstrate upper motor neuron involvement in the brain and facilitate the diagnosis and differential diagnosis of ALS,but also increases the understanding of the lesion distribution and physiopathology in ALS.
