1.Long-term Toxicity of Jiuxin Fumai Injection in Rats

Objective To observe the long-term toxicity of Jiuxin Fumai Injection and to investigate the safety of clinical medication.Methods The rats were given intramuscular injection with Jiuxin Fumai Injection in large,medium,small dosage(respectively 20,10,5g? kg-1)every day for two weeks,and normal saline group served as the normal control.Two weeks after drug withdrawal,the toxic reaction in rats was observed.Results After two-week continuous administration,all the animals were alive.Some animals were vomiting and getting excited when administered the large dosage and medium dosage injection.The blood sugar elevated,the thoracic gland coefficient lowered,the hepatic cells were cloudily swollen in the animals of large dosage group.Two weeks after drug withdrawal,the above phenomenon vanished.There was no obvious toxic reaction in the small-dosage injection group.Conclusion Long-term administration of Jiuxin Fumai Injection in large dosage shows certain toxic reaction in rats.The safe dosage for intramuscular administration is less than 5 g? kg-1? d-1.
2.Bioinformatics analysis of microRNAs differently expressed in major depression disorder

Objective To predict the target genes and function of has-miR-26b,has-miR-1972,has-miR4485,has-miR-4498,and has-miR-4743 by bioinformatics analysis,and provide the theoretical basis for the further research.Methods The targets of the five microRNAs were predicted by Target Scan,miRBD,and DIANA-microT-CDS,and the result were analyzed by gene ontology and pathway analysis using FunNet.Results 734 predicted targets were obtained by finding the intersected genes of Target Scan,miRBD,and DIANA-microT-CDS.GO analysis showed that biological processes regulated by the differentially expressed microRNAs included diverse terms,among which some terms (e.g.,central nervous system development,neuron differentiation,axonogenesis,synaptic transmission,learning,and memory,etc.) had direct relationship with the central nervous system and brain functions.The pathway analysis showed that a significant enrichment in several pathways related to neuronal brain function,such as axon guidance,glutamatergic synapse,Wnt signaling pathway,mTOR signaling pathway,VEGF signaling pathway,etc.Among the five microRNAs,has-miR-26b,has-miR-1972,has-miR-4498 might have more important regulatory functions.Conclusion Bioinformatic analysis indicates that has-miR-26b,has-miR-1972,has-miR-4485,has-miR-4498,and has-miR-4743 are closely related to the mechanism and pathogenesis of major depressive disorder.
3.Detection of serum amyloid protein and polypeptide growth factor in patients with dementia

AIM and METHODS: To detect the changes of amyloid protein (?-AP) and polypeptide growth factor levels and study their role in the possible pathogenesis of Alzheimers disease (AD) and vascular dementia (VD). We detected serum ?-AP, transforming growth factor- ?(TGF-?) and insulin-like growth factor (IGF-Ⅱ) levels by radio immunoassay in 8 patients with AD,15 patients with VD and 63 patients with ischemic cerebrovascular disease(ICVD) and 38 cases healthy volunteers. RESULTS: In AD and VD groups, serum ?-AP, TGF-? and IGF-Ⅱ were significantly higher than that in ICVD and control group; Serum ?- AP,TGF-? and IGF-Ⅱ contents in ICVD group were obviously higher than that in normal control, the highest increase occurred in sequel of cerebral infarction(SCI) and vertebrobasislar ischemia(VBI) groups. There are positive correlation among ?- AP, TGF-? and IGF-Ⅱ in AD and VD groups.CONCLUSION: ①?- AP is a risk factor in the pathogenesis of AD and VD. ② TGF- ? and IGF-Ⅱ play roles in the neurotoxin effect that lead to dementia. ③ ?-AP plays a important role in formation of senile plaque.
4.Diagnostic value of microRNAs in peripheral venous blood for patients with depressive disorder

Objective To investigate the diagnostic value of microRNA (miRNA) in peripheral venous blood for depressive disorder.Methods Real-time fluorescence quantitative PCR (RT-qPCR) was used to verify expression level of miRNAs in peripheral blood of non-specific mental retardation children,which were aberrantly expressed in depressive disorder patients,and then Receiver Operating Characteristics (ROC)curve was employed to confirm the sensitivity and specificity of abnormal miRNA expression in depressive disorder and non-specific mental retardation.Results MiR-1972,miR-26b,miR-4485,miR-4498 and miR-4743 were upregulated significantly in case group of depressive disorder(P<0.05),meanwhile miR-4485 and miR-4743 in aforementioned 5 miRNAs also upregulated significandy in patients of non-specific mental retardation(P<0.05),but miR-26b showed no significant difference between case group of non-specific mental retardation and the control group (P>0.05).The ROC curve of miR-26b in depressive disorder patients and their control group showed that the sensitivity and specificity were 0.609,0.664 respectively,and the area square under the curve was 0.614(P=0.021).The ROC curve of miR-26b in patients of depressive disorder and non-specific mental retardation indicated that the sensitivity and specificity were 0.784 and 0.471,and area square under the curve was 0.643 (P=0.003).Conclusion miR-26b probably have diagnostic value for depressive disorder,which may comorbidity with non-specific mental retardation.But genetic and psychosocial mechanism of comorbidity still needs further exploration.
5.Clinical characteristics and prognosis of peritoneal dialysis treatment in children with acute kidney injury

Objective:To investigate the efficiency and safety of peritoneal dialysis (PD) in pediatric patients with acute kidney injury (AKI).Method:A retrospective study of children who underwent PD for AKI in the First Affiliated Hospital of Xi’an Jiaotong University from 2003 to 2013 was performed, and the laboratory examinations, the causes, the complication, the prognosis and the risk factors were evaluated.Results:The study included 48 children, with the age of (67.6±51.7) months (ranging from 3 months to 15 years old), including 31 males (64.6%) and 34 co-infections (70.8%). Primary glomerulonephritis (27.1%) was the most common cause of AKI, followed by the hemolytic uremic syndrome (18.7%) and drug induced AKI (18.7%). Peritoneal dialysis was performed manually using percutaneous or adapted catheters. The duration of PD during hospitalization was 11(7,14) days. PD treatment was highly effective in attenuation of toxics retention and correction of electrolyte disturbances (all P<0.05). There were 3 cases of PD-related complications, including 1 case of peritonitis, 1 case of catheter outflow obstruction, 1 case of catheter exit site hematoma, and no child patient died of PD complications. Among the AKI children, 37 cases (77.1%) recovered with the PD treatment and had the catheter successfully removed till discharge, 7 cases (14.6%) needed further peritoneal dialysis and 4 cases (8.3%) died. The serum albumin level was significantly higher in patients who got recovered with PD treatment than other unrecovered cases [(32.6±6.7) g/L vs (23.2±4.3) g/L, t=-3.994, P<0.001]. Conclusions:PD can be safely and efficiently performed for the treatment of pediatric AKI. Low albumin level may be related to poor prognosis of AKI.
6.Application of electronic rotation registration manual for residency training based on hospital information system docking: Practice and exploration

The contents recorded in the rotation registration manual is not only the quantitative indicators for evaluating the quality of residency training, but also the important basis for training assessment and issuance of training certificates. In order to solve the problems of data authenticity, information delay, and repeated entry in the rotation registration manual for residency training, Shanghai East Hospital, Tongji University, launched a project to dock the electronic rotation registration manual with the hospital information system. Through the establishment of the project team, the development of working mechanisms, and the implementation of the project, data analysis was used for process reformation and system optimization, so as to continuously improve management efficiency and medical safety while solving problems and form a set of implementation system with reference significance in practice.
7.Relationships of ultrasound vascular index quantification and elastic modulus with biological characteristics of breast cancer and its clinical significance

Objective To explore the relationships of ultrasound vascular index quantification and elastic modulus with biological characteristics of breast cancer and its clinical significance.Meth-ods A total of 103 patients with breast cancer who received neoadjuvant chemotherapy(NAC)were selected as study subjects.The relationships of the quantification of vascular index(Ⅵ)and maxi-mum elastic modulus(Emax)of shear wave elastography(SWE)with the biological characteristics of breast cancer before NAC were analyzed.According to the pathological response of NAC,the patients were divided into significant response group and non-significant response group.The change rates ofⅥ and Emax at the end of different cycles of NAC were compared between the two groups.Receiver operating characteristic(ROC)curve was used to analyze the change rates of Ⅵ and Emax at the end of different cycles to predict the pathological response of NAC in breast cancer.Results Before NAC,Ⅵ and Emax were related to the degree of differentiation,pathological stage and HER-2 expres-sion in breast cancer tissues(P<0.05).The significant response group had higher change rates ofⅥ and Emax at the end of 1,2,and 4 week of NAC compared to the non-significant response group(P<0.01).At the end of the second cycle,AUG of change rates of Ⅵ combined with Emax predicted the pathological response of NAC in breast cancer was 0.918,with the sensitivity of 0.874,and specificity of 0.905,which were higher than 0.832 of AUC,0.78 of sensitivity,and 0.827 of specificity at the end of 1 week of NAC,and were higher than 0.853 of AUC,0.827 of sensitivity,and 0.849 of spe-cificity at the end of the fourth week.Conclusion Ⅵ and Emax are correlated with the degree of dif-ferentiation,pathological stage and HER-2 expression in breast cancer.At the end of the second cy-cle of chemotherapy,the change rates of the Ⅵ combined with Emax have the best efficacy in predic-ting the pathological reaction of NAC in breast cancer.

9.Current status of radiological Kashin-Beck disease among school-aged children in Chamdo City, Tibet

Objective:This study aimed to explore the status of radiological Kashin-Beck disease (KBD) among school-aged children in Chamdo City, Tibet, through a 3-year monitoring survey, providing epidemiological evidence for prevention and control strategies.Methods:The target areas for this study were Luolong, Bianba, and Basu counties in Chamdo City, Tibet Autonomous Region, identified as having the most severe historical cases of KBD. Children aged 7-12 years attending school were enrolled as study subjects. Anteroposterior X-ray films of the right-hand were taken, and radiological diagnoses were made based on the "Diagnosis of Kashin-Beck Disease" criteria (WS/T 207-2010). Two experienced researchers independently reviewed the X-rays, and intra- and inter-group consistency were assessed using weighted Kappa values and percentage agreement. Cross-sectional surveys were conducted in 2017 and 2020 to describe the X-ray detection rates of KBD, and logistic regression analysis was employed to construct a predictive model of risk factors for radiological KBD cases.Results:In 2017, a total of 5,711 children aged 7-12 years in Chamdo City, Tibet, participated in the baseline cross-sectional survey (average age 9.2 years, 48.0% female), with 28 cases of radiological KBD. The age- and gender-standardized prevalence rate was 0.527%. In 2020, 6,771 participants (average age 9.3 years, 49.5% female) underwent a second cross-sectional survey, with 9 cases of radiological KBD and a standardized prevalence rate of 0.134%. Logistic regression analysis indicated that older age [ OR=2.439, 95% CI(1.299, 4.580), P=0.006] and female gender [ OR=8.157, 95% CI(1.016, 65.528), P=0.048] were independent risk factors for radiological KBD cases. Conversely, higher residential altitude, under the premise of Tibet's high altitude, was a protective factor [ OR=0.995, 95% CI(0.990, 0.999), P=0.032). Conclusion:The radiographically positive detection rate of KBD among school-aged children in Chamdo City, Tibet Autonomous Region, is at an extremely low level and showing a declining trend, reaching the historical standard in 2020. Considering the absence of positive signs in affected children, it suggests that local KBD has been effectively eliminated.
