Objective To compare the clinical effect and safety of children with acute upper respiratory tract infection treated with Yanhuning and ribavirin.Methods 182 cases with acute upper respiratory tract infection treated were selected and divided into observation group and control group randomly.All children were given general symptomatic and supportive treatment.Yanhuning was added in observe group,10mg · kg-1 · d-1,ivgtt,qd,and ribavirin was added in control group,10mg · kg-1 · d-1,ivgtt,qd.Changes and disappearance time of clinical symptoms were observed.Results 89 cases (97.80％) were treated effectively in observation group,while 73 cases (80.22％) in control group,the efficacy of Yanhuning was higher than that of ribavirin (P ＜ 0.05).The time of clinical symptoms disappeared and blood examination recovering were shorter in observation group than that in control group (all P ＜ 0.05).Scores of fever and cough in observation group were (2.90 ± 0.04) and (2.87 ± 0.06),while those in control group were (2.37 ± 0.11) and (2.26 ± 0.13).The scores in observation group were higher than those in control group (all P ＜ 0.05).Conclusion Yanhuning applied in children with acute upper respiratory tract infection treatment is efficacy and safe,and it could adjust systemic immunity of children,which is worthy of clinical application.

Objective To explore the clinical characteristics of multiple primary carcinoma (MPC) in elderly patients and the effects of postoperative adjuvant chemotherapy on the prognosis and advanced cancer genesis. Methods The study population consisted of 220 elderly patients with malignant tumor. Thirty-seven elderly patients with MPC were selected in the study. The characteristics including onset age, carcinoma sites, effects of postoperative adjuvant chemotherapy, interval and survival time between first and advanced cancer were analyzed. Results of the 220 subjects,37(16.8%) patients were MPC, among whom 2 patients were synchronous multiple primary carcinomas (SMPC) and 35 patients were metachronous multiple primary carcinomas(MMPC). The median ages at onset were 70,77 and 77.5 years old for the first,second and third tumor respectively. Among a total of 82 tumor focus, the most predilection site of MPC was gastrointestinal tract, with a 34.2%(28 cases) in colorectum and a 13.4%(11 cases) in stomach. The postoperative patients were divided into two groups: 23 accepted adjuvant chemotherapy (chemotherapy group) and 14 accepted no adjuvant chemotherapy (no chemotherapy group). The survival time of chemotherapy group was significantly longer than no chemotherapy group (x2= 4.63, P = 0.0427 ). The median intervals between the first and the second cancer were 6 years and 4 years in chemotherapy group and no chemotherapy group, respectively(x2=1.63, P = 0.207), while the median intervals between the second and the third cancer were 7 years and 2 years, respectively(x2 = 4.255,P=0.043). The mediansurvival time of the third cancer postoperative patients were 3.5 years and 7 months in chemotherapy group and no chemotherapy group, respectively (x2 = 4.62, P = 0.0316 ) . ConclusionsPostoperative adjuvant chemotherapy may prolong the survival time and delay the advanced cancer genesis in elderly patients with MPC.

In recent years the incidence of acute kidney injury (AKI) has increased, with a high mortality rate and poor prognosis.The key of improving the prognosis of AKI is early diagnosis and intervention.Due to the lack of biomarkers of early diagnosis in AKI, the effective early treatment is generally delayed.Traditional diagnosis biomarkers such as serum creatinine and blood urea are delayed and affected by various factors.Recently, some new potential early diagnostic biomarkers have been found.MicroRNA(hereinafter referred to as the miRNA) is a kind of endogenous non-coding small RNA, and studies have shown that it is related to the development of AKI.

Objective　To observe rat plasma angiotensin Ⅱ (AⅡ) content change after exposed to infrasound. Methods　Infrasound was exposed to rats for 1,7,14,21 d and two hours every time in a day. After half, six, twelve, eighteen and twenty-four hours, AⅡcontent change was observed. Results　After exposed to infrasound for 1 d,AⅡcontent continued to increase in the next twenty-four hours compared to control group (P<0.01).After infrasound exposure for 7 d,AⅡ began to increase after half hour compared to control group(P<0.01), reached maximum at twelve hours, then began to decrease and turned less than that in the control group at twenty-four hours(P<0.05).After infrasound exposure for 14 d, AⅡ increased significantly compared to the control group(P<0.01).At six hours and twelve hours,there was no significant difference(P>0.05),it increased notably at eighteen hours and twenty-four hours(P<0.01).After infrasound exposure for 21 d, AⅡ began to increase at half hour compared to the contol group(P<0.05),reached maximum at twelve hours,then began to decrease and showed no significant difference at twenty-four hours(P>0.05).Conclusion　After infrasound exposure, there was irregular change of rat plasma AⅡ content at different times.

Objective To evaluate effect of preventing ventilator associated pneumoia (VAP) of patients with oral cavity endotracheal intubation by air impulse clearing away secretion drainage. Methods 348 patients with oral trachea cannula and mechanical ventilation (MV) in intensive care unit (ICU) were divided into control group (n=174) and observational group (n=174). The control group adopted conventional airway management methods without the subglottic secretion drainage while the observational group adopted conventional airway management methods on the basis of combining air impact method remove stranded on airbags. Other treatment and care for all of patients were basically the same. The MV time, duration in ICU and incident rate on VAP were compared between the two groups. Results The average of MV time in control group was (5.25±1.18) days, (5.62±3.20) days in ICU and the rate of VAP was 10.34%(18/174). In observational, it was (8.96 ±5.43) days, (10.43 ±4.96 ) days and 34.48%(60/174), respectively. The MV time, duration in ICU and incident rate on VAP were significantly different (P<0.05), with statistical significance. Conclusions The air impact method to remove airbags retentate could shorten time of MV and during time in ICU, and could reduce the rate of VAP.

Background and purpose:Paclitaxel and cisplatin were demonstrated to be effective and well-tolerated for advanced gastric cancer.The clinical efficacy of hyperthermic intraperitoneal perfusion(CHPP) has been established in numerous studies in recent years.The purpose of the study was to evaluate the efficacy and toxicities of paclitaxel combined with CHPP with cisplatin in the treatment of patients with advanced gastric cancer.Methods:51 patients with advanced gastric cancer were randomized into systemic chemotherapy group(25 patients) who received paclitaxel 70 mg/m2,iv,1/wk?8wk,DDP 25 mg/m2,iv,d1-3,reapeated every 3 weeks for 4 cycles and systematic plus CHPP group(26 patients)who received paclitaxel 70 mg/m2,iv,1/wk?8wk,DDP 60 mg/m2,CHPP,d1,8,22,29,and abdomen hyperthermia treatment twice a week in the whole chemotherapy period.Results:The response rate(CR+PR),the clinical benefitial response(CBR) in systematic plus CHPP group were 65.4% and 92.3%,and those in systemic chemotherapy group were 36.0% and 68.0%.There was obviously a statistical difference between two groups(P0.05).The major toxicities were myelo-suppression and nausea/vomiting.There was no significant difference in side-effect between the two groups.Conclusions:Paclitaxel administered by weekly infusion combined with hyperthermic intraperitoneal perfusion chemotheraphy with cisplatin in the treatment of advanced gastric cancer is effective and tolerable.It is worth studying in the future.

Objective To investigate the early post-operative nutritional support for kidney transplant recipients.Methods Totally 67 patients who received early post-operative nutrition supports were randomly divided into three groups:group A (n = 24,relatively high nitrogen intake),group B (n = 25,intermediate nitrogen intake) and group C (n = 18,relatively low nitrogen intake).Two weeks after operation,albumin,hemoglobin,the nutrition indieators,serum lipids,and renal function 1 day before operation and 2 weeks after operation were evaluated. Results Body weight was significantly different among three groups;the nitrogen balance in group A was significantly better than those in groups B and C (P ＜0.05);albumin and hemoglobin were not significantly different between group A and group B (P ＞ 0.05),but were significantly higher than those in group C (P ＜ 0.05).The renal function in group C was significantly better than those in group A and group B (P ＜ 0.05).The triglyceride level was not significantly different among three groups,while the cholesterol level in group A was significantly higher than those in group B and group C (P ＜ 0.05),but not between group B and group C.Conclusion Proper nitrogen intake in early post-operative nutrition support can improve the recovery of renal function and nutritional status in kidney transplant recipients.

Objective To investigate the invasion ability and collagenolytic activity of human glioma cells in vitro.Methods Boyden chamber invasion assay was employed to evaluate the cell migration ability of human malignant glioma cell line U87MG in vitro and the effect of conditioned medium of U87MG cells on matrix collagenolytic activity was tested by agar-gelatin gel.Results The number of U87MG glioma cells migrating through the Matrigel-coated membrane was more than that of addition of EDTA or anti-MMP-2 antibody in U87MG glioma cells(P0.05).EDTA or anti-MMP-2 antibody markedly inhibited the number of the migrated cells,the inhibitory rates were 79.2% and 77.1%,respectively. The conditioned medium collected from the U87MG cells showed an increase in the transparent ring area on agar-gelatin gel.Inhibition of MMP-2 enzymatic activity by EDTA or anti-MMP-2 antibody reduced the transparent ring area on agar-gelatin gel.Conclusion Both invasion ability and collagenolytic activity of U87MG cells are depended on MMP-2,suggesting that MMP-2 plays an important role in glioma invasiveness.

Objective To investigate the urinary iodine level and the thyroid function of early pregnant women in Qingdao,to explore the relationship between the iodine nutritional status and hypothyroidism.Methods 326 early pregnant women (<12 gestational weeks)were collected.The urinary iodine concentration and serum thyroid stimulating hormone (TSH),free thyroxine (FT4 ),thyroid peroxidase antibody (TPOAb)were detected. Results The median of urine iodine of pregnant women was 176.57μg/L,and the ratios of iodine deficient,iodine adequate,iodine more than adequate and iodine excessive groups were 3 8.65%,41.10%,15.95% and 4.30%.The total ratio of innormal urine iodine was 58.9%(192 /326).The differences between the ratios of the iodine deficient group,iodine excessive group and iodine adequate group both had statistically significant(χ2 =4.308,P <0.05;χ2 =4.826,P <0.05 ).There was no effect of iodine intake levels on serum thyroid peroxidase antibody.Conclusion There is no iodine deficiency among early pregnant women in Qingdao,but still individuals expose iodine deficiency.Iodine deficiency and iodine excessive are the risk factors of hypothyroidism during early pregnancy women,so we commend to detect the urinary iodine level.
