1.Comparison of the effects of breast conservative therapy and radical mastectomy in treatment of patients with early stage breast cancer

Objective To compare the effects of breast conservative therapy and radical mastectomy in treatment of patients with early stage breast cancer.Methods 76 patients with early breast cancer,informed consent by themselves and their families were divided into breast conservative therapy group(BCT group,2 1 cases)and breast radical group(RT group,55 cases),and BCT group was conducted breast conserving surgery,and RT group Was given radical surgery of breast cancer.The two groups were all given conventional postoperative chemotherapy and endocrine therapy.Then the therapeutic effects and complications were observed and compared.Following up for 1 to 6 years,the recurrence was compared.Results All patients were cured.In BCT group,5 cases of subcutaneous fluid,2 cases offat liquefaction of incision,1 ease of wound infection,1 case of upper limb edema,and in RT group,8 cases of subcu-taneous fluid occurred,5 cases of wound fat liquefaction,2 cases of wound infection,3 cases of upper limb edema,and the difference of complication rate between the two groups had no significant difference(P>0.05).The scores of quality life of BCT group were significantly higher than those of RT group(t=3.42,3.18,4.07,3.09,2.87,3.26,all P<0.05).All patients were regularly followed up for 1 to 6 years,local recurrence rate and survival rate of BCT group Was 4.8%(1/21)and 95.2%(20/21),and those of RT group Was 3.6%(2/55)and 96.4%(53/55),and the difference of local recurrence rate and survival rate between the two groups had no statistical significance(all P>0.05).Conclusion Breast conserving surgery for early breast cancer was safe and feasible without increasing the relapse rate,and could get a good quality of life.
2.Clinical imaging characters of gastric lymphoma and diagnostic significance of different examinations

Objective To summarize clinical imaging characters of the gastric lymphoma in gastroscopy and double contrast radiography of upper gastrointestinal tract.Methods We reviewed 24 cases of gastric lymphoma from January 1996 to June 2006 and analyzed their endoscopic and X-ray features,as well as their pathological results.Results The most frequently involved parts were gastric body(75%,18 cases) and antrum(67%,16 cases).The most common changes were: ①ulcer(71%,17 cases),especially multiple shallow ulcers which had different forms;②nodular protuberance(67%,16 cases),sometimes submucous giant protuberance;③giant duplicature(54%,13 cases);and ④involvement spreading along the gastrointestinal tract(21%,5 cases).Conclusions The characters of gastric lymphoma are ulcer,nodular protuberance,giant duplicature,and so on.The combination use of gastroscopy,radiography,endoscopic mucosal resection,endoscopic ultrasound,and laparoscopy is important for definite diagnosis.
3.The effect and their mechanisms of gastric mucosal protective drugs.

Objective To compare the protective effect of colloidal bismuth subcitrate-1 (CBS-1, Lizudele), or colloidal bismuth subcitrate-2 (CBS-2, De-Nol) and sucralfate against gastric mucosal lesion and to investigate their mechanisms. Methods Gastric mucosal injury of rats was induced by ethanol, stress, aspirin and hydrochloric acid. Gastric ulcer was then induced by 50% acetic acid applied to the gastric tunica serosa. We observed the protective effects against gastric mucosal lesion and measured the injury index and the area of ulcer in each group. Statistical t test was used to compare the difference of each group. Results (1)CBS-1, CBS-2, and sucralfate had protective effect against lesions caused by ethanol, stress, aspirin and hydrochloric acid and could promote acetic acid-induced gastric ulcer healing. (2) The mechanisms of protective effect and ulcer healing promotion were that these drugs could increase gastric blood flow and increase activities of QR, GST and GR, and could also promote overexpression of bFGF mRNA and iNOS mRNA. Conclusion Gastric mucosal protective drugs, CBS and sucralfate had effect of resisting injury and promoting ulcer healing. The mechanisms were that they could increase gastric mucosal blood flow and the expression of bFGF mRNA and iNOS mRNA, and reduce oxygen free radical.
4.Evaluation of the Helicobacter pylori stool antigen test for the detection of Helicobacter pylori infection before and after eradication

Objective The aim of this study was to assess the reliability of a newly developed enzymeimmumoassay, the Helicobacter pylori (H. pylori) stool antigen (HpSA) test, for the detection of H.pylori infection before and after eradication. Methods The H.pylori infected patients referred to our department were included. They were divided into two groups. The 331 patients in group A had intact stomach, and 65 patients in group B had history of subtotal gastrectomy. All patients underwent gastroscopy with biopsies for rapid urease test (RUT) and histology, which was viewed as “gold standard”. H.pylori status was defined as positive or negative with both RUT and histology presenting concordant positive or negative results. The results of these reference tests were compared with those obtained by HpSA test and ()~(13)C-urea breath test (()~(13)C-UBT). In addition, Fifty-six-positive patients in group A, constituting group C, were treated with 1-week triple therapy. At the 28 th day after the end of therapy, the patients underwent another ()~(13)C-UBT which was also defined as “gold standard”. The stool specimens were collected on days 1, 7, 14, 21, and 28 after completion of therapy and were used to detect the antigen of H.pylori by HpSA. Results In group A, 175 patients were defined as H.pylori-positive and 156 as negative by the “gold standard”. The sensitivity and specificity of the HpSA test was (95.4%) and 91.0%, respectively. There was no significant difference between HpSA test and ()~(13)C-UBT. In group B, 30 patients were defined as H.pylori-positive and 35 as negative by the “gold standard”. 5.The effects of scopolamine on Na~+-K~+-ATPase activity during cerebral postischemic reperfusion in rabbits

0 05).CONCLUSION:Na +-K +-ATPase is not sensitive to ischemic damage,but sensitive to reperfusion damage.Like hypothermia,scopolamine can protect Na +-K +-ATPase activity in the cerebral cortex during postischemic reperfusion,but hypothermia has no cooperating effect on scopolamine.
6.Cardioprotection of orientin against myocardial ischemia reperfusion injury through induction of autophagy

Aim To investigate the role of autophagy in the cardioprotection by orientin and the relative molec-ular mechanisms in cell apoptosis and autophay. Meth-ods The isolated ischemia reperfusion ( I/R ) heart model was built firstly. The Experiments were divided into seven groups:control group, I/R group, different concentrations of orientin-treated group ( 1. 0 mg · kg-1 ,2. 0 mg· kg-1 ,4. 0 mg · kg-1 ) , autophagy in-hibitor group and resveratrol group. Hemodynamic in-dex were recorded by PowerLab, the activities of myo-cardial enzymes were detected through Biochemical an-alyzer, the ultrastructure changes and autophagosomes in myocardial cells were detected by electron microsco-py, the apoptosis was detected by TUNEL, and LC3 and Beclin1 protein levels of left ventricle were meas-ured by Western blot. Results Orientin at middle and high concentrations(2 and 4 mg·kg-1 ) induced auto-phagy shown by increased the number of autophago-somes, LC3-Ⅱ/LC3-Ⅰ ratio and Beclin 1 expression after ischemia-reperfusion. The induction of autophagy by orientin was correlated with enhanced cardiac func-tion and decreased apoptosis. But wortmannin, a kind of autophagy inhibitor, significantly attenuated the ori-entin-induced autophagy and increased apoptosis. Con-clusion The cardioprotection of orientin against myo-cardial ischemia reperfusion injury may be mediated through the inhibition of apoptosis and induction of au-tophagy.
7.Dielectric response of erythrocyte in patients with Kaschin-Beck disease

Objective To measure the erythrocyte dielectric response variation of patients with KaschinBeck disease,to explore the pathogenesis of Kaschin-Beck disease and seek more effective means for evaluating the effectiveness of early diagnosis and control measures against Kaschin-Beck disease.Methods According to the principle of dielectrophoresis and cell dielectric response phenomena,dielectrophoretic pool was manufactured and dielectrophoretic detection and separation system was established.Patients with Kaschin-Beck disease (438) were selected in Kaschin-Beck disease areas of Aba County,Sichuan.At the same time,healthy persons (480) in KaschinBeck disease areas and non-Kaschin-Beck disease areas in Aba County were selected.Anticoagulant blood samples from patients with Kaschin-Beck disease and healthy persons were collected,erythrocytes were separated,cell suspension (10s cells/ml) was prepared and 200 μl cell suspension was added to the dielectrophoretic pool to measure erythrocyte dielectric response rate.Results Erythrocyte dielectric response rates were significantly different between patients living in Kaschin-Beck disease areas with Kaschin-Beck disease [(75.87 ± 5.89)%] and healthy persons living in Kaschin-Beck disease areas and non-Kaschin-Beck disease areas [(92.43 ± 4.45)%,(92.81 ±5.01)%,F =1.843,P < 0.01).Erythrocyte dielectric response rate was significantly reduced in patients with Kaschin-Beck disease compared with that of healthy persons (all P < 0.01);erythrocyte dielectric response rate of adult patients with Kaschin-Beck disease was lower [(69.57 ± 6.87)%] than that of pediatric patients with Kaschin-Beck disease [(82.17 ± 4.91)%,P < 0.01].Erythrocyte dielectric response rates were significantly different in patients with different degrees of Kaschin-Beck disease (F =1.647,P < 0.01).Erythrocyte dielectric response rate of patients with Kaschin-Beck disease was negatively correlated with prevalence of the disease (r =-0.87,P < 0.01).Conclusions Erythrocytes of patients with Kaschin-Beck disease have some degree of pathological damage.The more severe the disease,the more serious the damage of red blood cell.The change of erythrocyte dielectric response properties may be used as an index to judge the prevalence of Kaschin-Beck disease and for early diagnosis.

9.Analysis of etiology and prognosis of neonatal gastric perforation

Objective To investigate the etiology,clinical features and prognosis of neonatal gastric perforation.Methods The medical records of 18 patients with neonatal gastric perforation with respect to sex,age,birth-weight,course of disease,clinical presentations,and prognosis were retrospectively analyzed.Results There were 13 boys and 5 girls,8 of whom were full term infants and 10 preterm infants.The most common initial manifestations were poor activity,abdominal distension,and respiratory distress.All neonates were treated by surgical operation,the overall survival rate was 11/18.Prematurity,birth-weight,course of disease,were the statistically significant risk factors (P < 0.05).Conclusions Neonatal gastric perfora tion is mainly caused by congenital gastric wall defect,and associated with high mortality,particularly in premature infants.It is necessary to early diagnosis,surgical operation in time,nurse intensively,and observe the condition closely.
10.Analysis of laboratory proficiency testing results of fluoride in drinking water

Objective To find out the situation of laboratory testing capacity of fluoride in drinking water in Guizhou Province.Method Assessment of fluoride detection capabilities in nine cities (states) and 31 countylevel laboratories of the province in 2010 was carried out,the test results of individual indicators were evaluated using a single Z-score method,and the determination was based on the fluoride ion selective electrode method of Drinking Water Saritary Standard Test Method (GB/T 5750.5-2006) or fluorine reagent spectrophotometry.Results The feedback rate of fluoride measurement results was 100%,the satisfactory rate of laboratory results was 82.5%,the suspicious rate of results was 7.5%(33/40),and the unsatisfied rate was 10.0%(3/40).Conclusion The fluoride detection capability of most laboratories that participated in the proficiency test has met the requirements,and 10 percent of the laboratory's capacity needs to be improved.
