OBJECTIVE To investigate the antimicrobial application in a hospital of traditional Chinese medicine(TCM).METHODS Data of antimicrobial application in patients who were discharged within a month were investigated.RESULTS The usage rate of antimicrobials was 62.62%,the surgical departments were higher in antibacterial use than departments of internal medicine;the pathogenic detection rate was 17.28%;the usage rate of combinations accounted for 34.14%;the duration of preoperative and postoperative antimicrobial use was long,some patients didn′t receive rationally preoperative prophylactic antimicrobials.CONCLUSIONS Management of clinical antimicrobial application in hospital of TCM should be strengthened,especially on the indication of antimicrobial and pathogenic detection.

Objective To investigate the prevalence of gastric cancer and the relationship between H.pylori infection and the histopathologic changes of gastric mucosa in patients who had their H.pylori eradicated for 8 years in the high incidence region of gastric cancer. Methods One thousand and six adults(age from 35 to 75 years old)were randomly selected in Yantai area, Shandong province in 1996. Five hundreds and fifty two individuals with H.pylori positive were randomly divided into treatment group (n=276) and placebo group(n=276). Treatment group received triple therapy (omeprazol, amoxycillin and clarithromycin) for one week. The placebo group was served as control. One month after completion of treatment, H.pylori status was reassessed by 13C-UBT. Eradication rate of H.pylori in the treatment group was 89%.Follow-up study was carried out with endoscopy in the 2nd, 5th and 8th year, respectively. The endoscopic appearance and histology of the biopsies from the same site obtained at the first and final endoscopy were compared. Results ①At the 8th year′s follow up, 7 patients deve-loped gastric cancer, 1 in the H.pylori negative group(n=246)and 6 in the H.pylori positive group(n=306). The prevalence was 1/1968 person years in the treatment group and 6/2448 person years in placebo group(P0.05). ③ Eight years after eradication of H.pylori , the proportion of gastric atrophy in the body in the H.pylori eradication group was much lower than that in the non-eradication group(P0.05). Conclusions H.pylori infection increased the risk of gastric cancer occurrence, H.pylori eradication is able to reduce such risk. H.pylori eradication results in reduction in the pre-valence of gastric atrophy in the body, while continued H.pylori infection leads to progressive aggrava-tion of gastric atrophy and intestinal metaplasia.

Objective To investigate the gastroscopic manifestations and pathological diagnosis of large gastric folds.MethodsClinical data of 161 cases with large gastric folds,who were diagnosed by gastroscopy from January 2002 to April 2008,were analyzed.Endoscopic mucosal resection or strip-off biopsy by ultrasonic endoscopy was performed if diagnosis could not be identified using routine biopsy by the gastroscopy.ResultsOf the 161 cases,79 patients had chronic inflammation(49.1%),67 had gastric cancer(41.6%),11 showed malignant lymphoma(6.8%),2 suffered from Menetrier's disease(1.2%),1 had acute inflammation(0.6%),and 1 showed no obvious lesion(0.6%);the rate of Helicobacter pylori infection was 42.2%(68/161),and the positive rate for Helicobacter pylori in patients with gastric cancer was 34.3%(23/67).Endoscopic mucosal resection biopsy was performed on 15(9.3%) of the cases,5 of them had chronic inflammation,5 showed gastric cancer,4 were confirmed with malignant lymphoma,and 1 was diagnosed as having Menetrier disease.The lesion of large gastric folds involved the whole stomach in 10 patients,the gastric fundus in 10,gastric corpus in 86,gastric antrum in 5,both the gastric fundus and corpus in 31,and both the gastric corpus and antrum in 19.All of the 10 cases,who had the whole stomach being invaded,were diagnosed as having gastric cancer.Though both gastric cancer and chronic inflammation showed similar manifestations under gastroscope,the levels of the characteristics were significantly different(Dilatancy poor: ?2=10.093,P=0.001;peristalsis poor: ?2=19.116,P=0.000;gastric cavity narrowing: ?2=18.159,P=0.000;texture stiffness: ?2=54.302,P=0.000;texture brittle: ?2=6.132,P=0.013;erosion: ?2=5.619,P=0.018;and ulcer: ?2=22.743,P=0.000).ConclusionsMost of the patients with large gastric folds are diagnosed as having chronic inflammation,gastric cancer,or malignant lymphoma,while Menetrier disease and acute inflammation seldom occur.The lesion usually involves the gastric corpus.For those who have the whole stomach being invaded,gastric cancer can be revealed.Most of the malignant cases show ulcers,dilatancy poor,and peristalsis poor under a gastroscope.Endoscopic mucosal resection biopsy is effective for identifying the diagnosis of malignant lesions.

Objective To summarize the characteristics of regional lymph node metastasis in patients with early gastric cancer and analyze the risk factors for lymphatic metastasis. Methods 103 cases surgically treated for early gastric cancer in the Third Hospital of Peking University between March, 1988 and March, 2009 were analyzed retrospectively. Several clinicalpathologic variables including patients' age, gender, size of tumor, tumor location, macroscopic type, histological type, invasion depth were investigated by using chi-square test and logistic regression analysis for the possible relationship to lymphatic metastasis. Results The rate of lymph node metastasis in early gastric cancer was 17.5% (18/103), which in mucosal cancer was 4. 1% (2/49). Submucosal cancer had a lymph node metastatic rate of 29. 6% (16/54). Logistic regression indicated that invasion to submucosa and tumor size > 2 em were independent risk factors for lymph node metastasis of early gastric cancer. Metastatic cases of mucosal cancer were all signet ring cell cancer with diameters more than 2 cm. Lymph node metastatic rate in submueosal cancers within 2 cm was 16. 1% (5/31), that in > 2 cm submucosal cancers was 47. 8% (11/23) (P = 0. 012). Rate of lymph node metastasis in well-differentiated cancers was 0 (0/13), that in moderately-differentiated, poorly differentiated and signet ring cell cancers were 18. 2% (4/22), 16. 7% (5/30) and 23.7% (9/38) respectively (P = 0. 294). Patients' age, gender, tumor location and macroscopic type showed no relationship with lymph node state. Conclusion The tumor size and invasion depth are related with lymph node metastasis in early gastric cancer, considering these factors and assessing lymph node state is essential to appropriate therapeutic options for early gastric cancer.

Objective To evaluate the risk factors for prognosis in liver cirrhotic patients with acute esophageal variceal bleeding (EVB). Methods Retrospective analysis was carried out in patients with liver cirrhosis,who were admitted to the hospital for EVB between January 2000 and December 2006 . All patients were follow-up to August 31st, 2009. The death of the patients was identified as the end of the study. The vital signs and results of laboratory tests were recorded when patients were admitted to the hospital. Child-Pugh score/grade, model for end stage liver disease (MELD) score and Rockall score were calculated. Results Ninty-five cases (66 males and 29 females) were enrolled in the study. Among them, 60 cases died and 35 cases survived for (42. 61±33.21) months (ranged from 2 days to 114 months). Univariate Cox regression analysis showed that age, male, Child-Pugh score/grade, MELD score, Rockall score, white blood cell counts and active hepatitis B were risk factors for death in liver cirrhotic patients with EVB (P＜0.05), while high level of hemoglobin and high hematocrit, endoscopic variceal sclerosis therapy or surgical treatment were protective factors (P＜0.05). Multivariate Cox regression analysis revealed that Child-Pugh grade (RR= 4. 997,95%CI:2. 787～8. 960, P＜0.01),Rockall score (RR= 1. 284,95%CI:1. 062～1. 553,P=0. 010), high counts of white blood cells (RR= 1. 072,95%CI: 1. 001 ～ 1. 148, P= 0. 046) were risk factors for prognosis of liver cirrhotic patients with EVB. Conclusion It is demonstrated that Child-Pugh grade,Rockall score and high counts of white blood cells are risk factors for prognosis of liver cirrhotic patients with EVB.

Objective To compare eradication rates,safety and compliance of minocycline quadruple and tailored therapies in patients retreated for Helicobacter pylori (H.pylori) infection.Methods Between January 2014 and June 2014,135 patients with dyspepsia (18-70 years) and H.pylori infection after at least one previous eradication treatment at a tertiary hospital were randomly assigned to a 10-day treatment with minocycline quadruple therapy versus tailored triple regimen of PPI,amoxicillin and a third antibiotic.In the group of tailored therapy,medications were adjusted based on clarithromycin sensitivity and cytochrome P450 isoenzyme 2C19 genotype.Eradication status was assessed 4-12 weeks after treatment.Results Although H.pylori eradication rates were higher in the minocycline therapy group than that in the tailored therapy group in intention-to-treat [84.1％ (95％ CI 75.0％-93.2％) vs 75.8％ (95％ CI 65.1％-86.5％),P =0.245] and per-protocol [88.3％ (95％ CI 80.3％-96.3％) vs 79.7％ (95％CI 69.7％-89.7％),P =0.197] analyses,the differences between the two groups were not statistically significant.The incidence of adverse effects and compliance between the two groups were also comparable.Conclusions The tailored therapy in this study had a poor eradication efficacy in the retreated patients with H.pylori infection.Minocycline quadruple therapy achieved a relatively satisfactory eradication efficacy and may be an alternative choice for the retreatment of H.pylori infection.Clinical trial registration Chinese Clinical Trial Registry,ChiCTR-TRC-13003975.

Objective To summarize the clinical features, diagnosis and treatment of Ménétrier disease in China. Methods A systematic review of Ménétrier disease in China was performed. Four electronic databases were searched from 1993 to 2008 including CNKI and Wanfang seeking studies about pathologically performed Ménétrier disease. Results Data of 95 patients with Ménétrier disease from 62 studies was included. There were 74 males and 21 females. The mean age of onset in adults was (47.4± 5.3) years . The process of the disease was longer in male individuals that in female individals (47.5 months vs. 13.7 months, P<0.05). The main symptoms included abdominal pain (76.8%), weight loss (42.1%), anorexia (40.0%) and edema of lower extremities (36.8%), etc.Hypoproteinemia was accounted for 71.6 % and most of the patients had hypochlorhydria. The typical giant hyperplastic gastric fold was found in 88.4 % of the patients under endoscopy. The diffuse type was in predominance and mainly involved the greater curve of the stomach (55.3 %). Whereas 5.3 patients were accompanied with gastric cancer. The misdiagnostic rate of the disease was 31. 6%,which accounted for 50% in surgically treated patients. Conclusions In China, the Ménétrier disease is predominant in middle-aged males, and the process is longer in males than in females. Misdiagnosis is related to existence of weight loss and hypoproteinemia. Most of the patients with Ménétrier disease have a good prognosis even the disease process is long.

Conclusion The morbidity of erosive reflux esophagitis rate is increasing parallel with the increase of age and severity of the lesion, and prevalence is more in males than females. The morbidity rate in males was higher than that in females. The incidence of e-rosive reflux esophagitis is not affected by H. pylori infection.

Objective To explore the clinical significance of typical reflux symptoms in the diagnosis of gastroesophageal reflux disease (GERD).Methods Consecutive patients older than 16 years,who initially visited department of gastroenterology at clinic of Peking University Third Hospital from May 9,2012 to Dec 31,2012,were required to complete a self-reported GERD questionnaire.Upper endoscopy was performed in some selected patients.Results A total of 18 987 patients were enrolled with a response rate of 91.5％.The prevalence of symptom-defined GERD was 13.6％ (2 579/18 987).A total of 4 357 (22.9％) patients underwent the upper endoscopy,and the diagnostic rates of reflux esophagitis,Barrett's esophagus,peptic ulcer disease,and upper gastrointestinal malignancy were 13.1％ (572/4 357),1.8％ (78/4 357),10.5％ (456/4 357),and 1.7％ (75/4 357),respectively.The incidence of reflux esophagitis was 22.7％ (216/951) in patients with reflux symptoms and 10.5％ (356/3 406) (P ＜0.001) in patients without reflux symptoms,2.7％ (26/951) and 1.5 ％ (52/3 406),respectively (P =0.013) for Barrett's esophagus; 6.8％ (65/951) and 11.5％ (391/3 406),respectively (P＜0.001) for peptic ulcer disease; 1.7％ (16/951) and 1.7％ (59/3 406),respectively (P =0.917) for upper gastrointestinal malignancy.Conclusions GERD is one of the major diseases at gastroenterology clinic.Typical reflux symptoms suggest a diagnosis of GERD.But some patients with peptic ulcer disease or upper gastrointestinal malignancy can also present typical reflux symptoms.Upper endoscopy is valuable to avoid the misdiagnosis of other disorders.

Objective By analysing the clinical features of Indigo Naturalis-associated ischemic lesion of colon mucosa to improve the precautionary and therapeutic level of the disease.Methods Thirteen patients diagnosed as Indigo Naturalis-associated ischemic lesion of colon mucosa in Peking University Third Hospital from 2005 to 2010 were reviewed.The endoscopic and clinical features were analysed.Results The 13 patients with an average age of(60.6±14.1)years old were prescribed Chinese traditional medicine containing Indigo Naturalis for psoriasis or idiopathic thrombocytopenic purpura(ITP).The ratio of males to females was 1:1.6.The typical manifestations were abdominal pain and bloody stool with watering diarrhea before bloody stool in 61.5%patients.Endoscopic and pathological characteristics were coincident with ischemic lesion and more like a chronic index.Vasodilatic medicine was effective and the average hemostatic time was(1.7±0.8)days.The prognosis was well and no recurrence was found during 3 months follow-up.Conclusions Patients having psoriasis or ITP treated with Chinese traditional medicine containing Indigo Naturalis have an inclination to colon mucosa lesions, even ischemic lesion. Careful assessment and observation before prescribing arc necessary in these patients.
