1.Effects of "internet +" nursing intervention model on rehabilitation outcomes in stroke patients: a Meta-analysis

Objective:To systematically assess the effects of "internet +" nursing intervention model on rehabilitation outcomes in stroke patients so as to provide an evidence-based basis for clinical nurses to carry out "internet +" nursing service new model in stroke patients.Methods:Randomized controlled trials (RCTs) related on effects of "internet +" nursing intervention model on rehabilitation outcomes in stroke patients were retrieved in China Biological Medicine (CBM) , Chinese National Knowledge Infrastructure (CNKI) , WanFang Data, PubMed, Embase, Cochrane Central Register of Controlled Trials by computer from building database to May 2019. RevMan 5.3 was used to the systematic review for literatures meth the inclusion criteria.Results:A total of six literatures were included involving 637 samples. Systematic review showed that "internet +" nursing intervention model could improve the motor function [ MD=13.87, 95% CI (7.93, 19.81) , Z=4.57, P<0.01]and activity of daily living [ MD=14.43, 95% CI (6.15, 22.71) , Z=3.41, P<0.01]of stroke patients during rehabilitation. Conclusions:"Internet +" nursing intervention model provides a good channel for rehabilitation and health education for stroke patients after going back to family and community, helps stroke patients to improve motor function during rehabilitation and activity of daily living, but that still exhibits the problems needed to be perfected and solved by medical staff exerting themselves.
2.Intervention effects of cognitive behavior therapy on negative emotion, quality of life and quality of sleep in patients with brain neoplasm: a Meta-analysis

Objective:To evaluate intervention effects of cognitive behavioral therapy on reducing negative emotions in patients with intracranial tumors and improving their quality of life and sleep quality by using Meta-analysis.Methods:Cochrane Library, Web of Science, Embase, PubMed, CNKI, VIP Chinese Journal Database, China Biology Medicine disc and Wanfang Database were searched for randomized controlled studies on interventions of cognitive behavioral therapy in anxiety, depression, quality of life and sleep quality of patients with brain neoplasm. After screening, extracting and evaluating data, the two researchers performed a Meta-analysis using RevMan 5.3 statistical software.Results:A total of 22 literatures met the inclusion criteria, with a total sample size of 1 702 cases. The analysis of the results showed that cognitive behavioral therapy could help reduce the anxiety level of patients with brain neoplasm [ SMD=-1.13, 95% CI: (-1.31, -0.96) , P<0.01], depression level [ SMD=-1.09, 95% CI: (-1.27, -0.92) , P<0.01], which could improve quality of life [ SMD=0.29, 95% CI: (0.06, 0.51) , P=0.01] and sleep quality [ MD=-0.32, 95% CI: (-0.36, -0.28) , P<0.01] of patients and improve satisfaction of patients with nursing [ MD=-8.25, 95% CI: (3.54, 19.24) , P<0.01]. Conclusions:Existing evidence shows that the use of cognitive behavioral therapy can help improve the negative emotions of patients with brain neoplasm, improve their quality of life and sleep quality and improve their satisfaction with nursing.
3.Summary of the best evidence for tracheal intubation management in neurological critical ill patients and its application effects

Objective:To summarize the best evidence for tracheal intubation management in neurological critical ill patients and explore its application effect.Methods:The Clinical Evidence Practice Application System of Joanna Briggs Institute in Australia was adopted to retrieve and summarize the best evidence, develop appropriate review indicators based on clinical scenarios and professional judgement, identify obstacles to the application of evidence, and construct the best action strategy. Using the convenient sampling method, a total of 68 nurses from the Neurosurgical Care Unit of Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School, 98 tracheal intubation patients during baseline review from January to April 2022, and 99 tracheal intubation patients after evidence application from September to December 2022 were selected as the research objects. Before and after evidence-based practice, the implementation rate of review indicators, nurses' knowledge of tracheal intubation management in neurocritical critical ill patients, and the incidence of patients' airway complications (oral mucosal ulcer, airway mucosal hemorrhage, tracheoesophageal fistula, pulmonary infection) were compared.Results:A total of 28 best evidences were summarized and corresponding review indicators were developed. Except for six review indicators (indicators 11, 13, 19, 20, 27, 28), which increased from 97.96%-100.00% before evidence-based practice to 100.00%, the differences were not statistically significant ( P>0.05), and the implementation rates of the remaining 22 review indicators were improved after evidence-based practice. Among them, the implementation rates of eight indicators (indicators 5, 6, 9, 14, 18, 21, 25, 26) increased from 0 to 89.90%-100.00%, and the differences were statistically significant ( P<0.05). After evidence-based practice, the scores of nurses' knowledge of tracheal intubation management in neurological critical ill patients increased from (62.41±6.07) to (93.82±2.74), and the difference was statistically significant ( P<0.05). Except for tracheoesophageal fistula, the incidence of other airway complications were decreased in all patients, and the differences were statistically significant ( P<0.05) . Conclusions:An evidence-based programme for the management of endotracheal intubation in neurocritical care patients can improve nurses' knowledge of tracheal intubation management for neurological critical ill patients, reduce the incidence of airway complications and improve the clinical outcome of them.
4.Application of objective structured clinical examination assessment mode in neurosurgery specialist nurse training

Objective:To analyze the application effect of objective structured clinical examination (OSCE) assessment mode in the training of neurosurgery specialist nurses.Methods:Using convenience sampling method, 21 trainees who were trained in neurosurgery specialist nurse training base of Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School in 2018 were selected as the control group, and the conventional training methods of theory and clinical practice were adopted; the 22 trainees in 2019 were selected as the experimental group, and the OSCE training and assessment mode was adopted. After the training, the core competence, satisfaction, learning and growth of the trainees were compared.Results:There was no significant difference in the score of the core competence before the training between the two groups( P>0.05). After the training, the scores of professional skills, critical and difficult medical record management and professional quality were (4.55 ± 0.51), (4.14 ± 0.77), (4.36 ± 0.66) points in the experimental group and (3.90 ± 0.62), (3.62 ± 0.80), (3.76 ± 0.83) points in the control group, the differences were statistically significant ( t=3.69, 2.15, 2.64, all P<0.05). The total score of satisfaction was (66.82 ± 2.26) points in the experimental group and (62.05 ± 2.29) points in the control group, the difference was statistically significant ( t=6. 87, P<0.05). There was no significant difference in the learning and growth of the trainees before and after the training between the two groups ( P>0.05). Conclusions:Using OSCE assessment mode to carry out neurosurgery specialist nurse training is helpful to improve students′ professional skills, critical and difficult medical record management, professional quality, and improve students′ satisfaction with the training.
5.Experience and feelings of informal caregivers of patients with intracranial tumors: a systematic review

Objective:To systematically integrate the experience and feelings of informal caregivers of patients with intracranial tumors.Methods:Qualitative researches on the real experience and inner needs of informal family caregivers of patients with intracranial tumors were retrieved in the PubMed, Web of Science, China National Knowledge Infrastructure (CNKI) , WanFang Data, China Biomedical Database and VIP Chinese Journal Database by computer. The search time was from the establishment of the database to May 31, 2021. After screening and analysis, the critical appraisal tool for qualitative research (2017) of the Joanna Briggs Institute (JBI) Evidence-based Health Center in Australia were used to evaluate the articles and the results were Meta-integrated.Results:A total of 14 articles meeting the criteria were included. A total of 48 complete research themes were refined, and they were summarized and combined to form 3 comprehensive Meta-integration results and 4 new categories. The 3 Meta-integration results were that the caregiver experienced the shock, sadness, disbelief and hope in the early stage of diagnosis, and began to assume responsibility and learn to become a caregiver's psychological and emotional changes; the caregiver's family role and living environment changed, affecting the quality of life of caregivers; caregivers lacked information, care experience and social support.Conclusions:The quality of life of informal caregivers of patients with intracranial tumors and the changes in family functions have attracted attention. Social and medical workers should combine our country's cultural background to formulate effective intervention strategies and related health policies for the difficulties and challenges faced by this special population.
