1.The effect of Danhong injection on hemodynamic parameters, Cys-c, and Hcy in patients with transient cerebral ischemia

Objective To detect the effect of Danhong injection on cerebral vascular hemodynamic parameters, Cys-c and Hcy in patients with transient cerebral ischemia(TIA), and analyze its clinical effect. Methods 80 TIA patients were selected. The patients were divided into a control group and a Danhong injection observation group with 40 cases each group. The control group was given conventional treatment, and the observation group was given conventional treatment and Danhong injection. The treatment course was 14 d. The hemodynamic parameters, Cys-c and Hcy expresssion were observed. Clinical effect was analyzed. Results After treatment, average blood flow speed (20.07 ± 4.28 cm/s vs. 16.17 ± 2.46 cm/s, t=5.230), average blood flow (11.14 ± 2.24 ml/s vs. 9.54 ± 1.65 ml/s, t=3.637), and cerebral vascular resistance (1 602.4 ± 98.3 kPa/s·m-1 vs. 1 738.5 ± 104.3 kPa/s·m-1, t=6.024) was significantly improved in the observation group than those in the control group (P<0.05). Cys-c (0.48 ± 0.11 mg/L vs. 0.71 ± 0.14 mg/L, t=8.170) and Hcy (17.45 ± 3.26 μmol/L vs. 23.62 ± 4.12 μmol/L, t=7.428) were significantly decreased in the observation group than those in the control group (P<0.05). The recurrence rate of TIA and cerebral infarction were 7.5% and 5% in observation group, which were significantly lower than that of 22.5% and 15% in control group (χ2=2.451, P<0.05;χ2=2.630, P<0.05).Conclusion Danhong injection can reduce the expression of Cys-c and Hcy and recurrence rate of TIA and cerebral infarction.
2.Mechanism of decimeter wave in the prevention of flexor tendon adhesion

BACKGROUND: Tendon injury is a common injury in the department of hand surgery. The postoperative recovery of hand function is always affected by tendon adhesion after tendon repair. To prevent tendon adhesion, especially the adhesion after flexor tendon repair is always the key in the rehabilitation of hand surgery.OBJECTIVE: To investigate the impact of decimeter wave therapy on tendon adhesion and healing after flexor tendon repair.DESIGN: Randomized controlled study based on experimental animal SETTING: Provincial Institute of Orthopaedics.MATERIALS: The study was conducted in Hebei Provincial Institute of Orthopaedics from January 2001 to June 2003. Totally 28 Leghorn chickens were randomly divided into decimeter wave therapy group and operation control group.METHODS: The flexor digitorum profundus tendons of Leghorn chickens were transected and repaired. Decimeter wave therapy was applied to the toes on chickens of decimeter wave therapy group. Animals were executed at week 3 or 6 after operation for macroscopical observation and histological observation under optical and electron microscopes, and biomechanical analysis.MAIN OUTCOME MEASURES: Principle index: results of macroscopical observation and the observatory results under optical and electron microscope, and the results of biomechanics. Secondary index: results of the classification of tendon adhesion and healing.RESULTS: It could be seen under macroscopical and histological observation that the adhesion significantly reduced in decimeter wave therapy group. The protein synthesis of fibroblast was significantly more than that of the control group under electron microscope. As indicated by biomechanical analysis, the tendon gliding distance[ (5.37 ± 1.06) mm at week 3, (6.76 ± 1.52) mm at week 6]and the rehabilitative compliance( 1. 04 ± 0.65 at week 3)of decimeter wave therapy group were bigger than those of the control group respectively [ (4.43 ±1.03) mm, (5.33±1.27)mmand0.63±0.31](P <0.05), and the anti-tension strength of decimeter wave in therapy group (N, 26. 93 ± 4. 80,47. 12 ± 7.76) was significantly bigger than that of the control group respectively(21.29 ±4. 88 and 38.96 ±7.52) (P <0. 01).CONCLUSION: Decimeter wave therapy can effectively promote tendon healing and reduce tendon adhesions and provide prerequisites for early rehabilitative training after flexor tendon repair. Hence, it is an ideal assistance in the prevention of tendon adhesion.
3.Long-term effect of surgical treatment in male infertility patients with varicocele

Objective To observe the long-term effect of surgical treatment in infertile varicocele patients. Methods 180 infertile patients with varicocele accepted modified Palomo operation (retroperitoneal high ligation of internal spermatic cord vein) from 2004 to 2007, and the long-term effect of thesurgery were analyzed retrospectively, such as semen parameters and spouse pregnancy. Results The operations were performed successfully in 180 cases, and semen parameters improved significantly after the surgery (Ps < 0. 05 ).Among all patients, 10 cases had spouse pregnancy within 3 months after operation;47 cases had spouse pregnancy from 3 to 6 months after operation; 25 cases had spouse pregnancy from 6 to 12 months after operation;13 cases had spouse pregnancy from 12 to 24 months after operation. The fertility rate was 52. 8% (95/180) in total. Conclusion Surgical treatment of infertile male patients with varicocele has good long-term effects, made it the first choice in these patients.
4.Study on the therapeutical effect of chitosan nano-particles used as a Bla g 7 polypeptide antigen in the sensitized mice

To investigate the therapeutical effect of the chitosan (CS) nana-paritclessingle administration of Bla g 7 polypeptide - CS nanoparticles used as a BLa g7 polypeptide antigen in the sensitized mice and to explore its immune mechanism, the polypeptide Bla g 7 was enclosed into CS to develop the Bla g 7 polypeptide entrapped CS nano-particles. In the present experiment, 25 BALB/c mice were divided randomly into the Bla g 7 polypeptide treated group(group A , n= 5) , Bla g 7 polypeptide plus CS treated group(group B , n= 5) , CS=control group (group C , n= 5),model group (group D , n= 5) and negative control group (group E, n= 5), After sensitization by intraperitoneal route and challenged by intranasal instillation with crude extracts of German cockroach , the inflammatory changes in the mouse lung tissues were observed after the lung tissues were fixed and stained with haematoxylin and eosin(H&E). The total cell number and the cellular composition of bronchoalveolar lavage fluid (BALF) were detected; and the changes of the mouse airway hyper-reactivity were determined by the whole body plethysmograohy pre-and post-treatments. In these ways, the Bla g7 peptide CS nano-particel vaccine was successfully developed. It was found that the pathological changes in mouse lungs in group D were not so prominent in comparison with those of group A of mice sensitized with crude extract of German cockroach. in which the development of eosinophil infiltration in the airway of mice in D group could be demonstrated. The lung inflammatory reactions and the mucus secretion in lungs of D group were significantly alleviated than those of the B group. but there was no therapeutical effect for the mice fed with the isodoses of Bla g 7 polypeptide or CS. It was also shown that the airway hyper-reactivity of mice was depressed after treatment (P<0.05). It is evident that CS nano-particles show definite therapeutical effect and may serve as a powerful vehicle to improve the tolerance effect of the Bla g 7 polypeptide-CS nanoparticle vaccine, and a single administration of Bla g 7 polypeptide-CS nanoparticle vaccine may hold promise as a new strategy to desensitize the Bla g 7 sensitized disease.

6.Myocardial protective effects of“integrated strategy”with Na~+/H~+ exchange inhibitor (HOE-642) plus modified K/Mg cold blood cardioplegia on canine heart grafts preserved

Objective To investigate the myocardial protective effects of“integrated strategy”with HOE-642 plus modified K/Mg on canine heart grafts.Methods Sixteen donors-recipient pairs of adult canines were divided into the“integrated strategy”group,i.e.,donor hearts were protected with HOE-642 plus K/Mg(IS,n=8)and the routine method(RM,n=8)group.Both groups un- derwent hypothermia stored for 4 h,followed by orthotopic transplantation.Cardiac heamodynamics was measured and myocardial ultrastructure and myocardial water content were evaluated.Results One of 8 animal died of aortic anastomoses leakage in each group,and the rest could wean from car- diopulmonary bypass successfully.Preservation of left ventricular contractility,as well as left ventric- ular compliance was significantly better in the IS group than in the RM group(P<0.05),also,per- centage of myocardial water content was significantly lower in the IS group than in the RM group(P<0.05).Myocardial ultrastructures were not damaged significantly in both groups,however,better myocardial preservation were seen in the IS group.Conclusion Both the clinically used“routine meth- od”and“integrated strategy”might provide safe donor heart preservation for 4 h,but the later scheme conferred more optimal effects with alleviating the myocardial swelling,preserving myocardial ultra- structure integrity and enhancing postischemic-reperfusion cardiac functional recovery.
7.Effects of HIF-1α on human gastric cancer cell's apoptosis in CO2 pneumoperitoneum environment

Objective To study the effect and mechanism of human gastric cancer cell's apoptosis in simulated CO2 pneumoperitoneum environment by HIF-1α. Methods We used a closed box to simulate CO2 pneumoperitoneum environment, a standard surgical insufflator maintained pressure of the box at 0,5,10 and 15 mm Hg respectively. MKN-45 cell's HIF-1α mRNA and protein expression were observed before and after silencing cell's HIF-1α by real time RT-PCR and Western blot. Cell's bcl-2/bax was studied by immunohistochemistry before and after silencing HIF-1α. Cell's apoptosis ratio was measured using Annexin V-FITC/PI double labelled staining. Results In 15 mm Hg group, MKN-45 cell's HIF-1α mRNA and protein expression were significantly higher than control group (P<0.01 ), while the difference was not significant among 0,5,10 mm Hg group and control group (P>0.05). In 15 mm Hg CO2 pressure, cell's bcl-2/bax was obviously lower than in control group (P<0.05) and apoptosis ratio was more than control group(P<0.01). When HIF-1α was silenced, cell's bel-2/bax and apoptosis ratio weren't significantly different between 15 mm Hg group and control group (P>0.05 ). Conclusions MKN-45 cell's apoptosis did not experience any alterations under 0,5,10 mm Hg.15 mm Hg CO2 pneumoperitoneum environment enhanced cell's apoptosis probably by way of HIF-1α.
8.Expression of the thermostable carboxypeptidase Taq gene in Pichia pastoris GS115.

To express recombinant carboxypeptidase from Thermus aquaticus (Cpase Taq) in Pichia pastosis, the open reading frame coding thermostable Cpase Taq was optimized based on the preference of P. pastoris codon usage and synthesized in vitro. The novel gene was cloned into P. pastoris expression vector pHBM905A and the sequence coding 6xHis tag was fused with the ORF of Cpase Taq gene. The recombinant plasmid was named pHBM905A-Cpase Taq and transformed into P. pastoris GS 115. Transformants were induced with 1% methanol for 72 h until the enzyme yield reached 0.1 mg/ml. The enzyme was purified and its enzymatic properties were analyzed. The results showed that the specific enzyme activity reached maximum at 75 °C and pH 7.5, which was about 80 U/mg. It was the first report about the secretory expression of Cpase Taq in P. pastoris GS115. Because of its large-scale preparation, this enzyme may be applied in industrial hydrolysis of peptides into amino acids in the future.

9.Glycated hemoglobin levels in non-diabetic adults residing at different altitudes and influencing factors

In the present study,healthy adults who had been residing at the average altitudes of 6,2260,and 2800 m for 10 years were recruited,and the number of red blood cells,the levels of hemoglobin,and HbA1C were tested.The results showed that the level of HbA1C in altitude 2800 m group was higher compared with the other 2 groups(5.80% ± 0.50% vs 5.39% ± 0.47%,P<0.05 ).Therefore,this difference should be considered when diagnosis and treatment for diabetes mellitus were made in the region.
10.Safety of metformin in diabetic patients at high altitude area

Sixty-one newly-diagnosed diabetic patients from high altitude area were treated individually plus metformin 1.5 g/d for 14 days. The difference of oxygen partial pressure was 3.5 mm Hg (1 mm Hg=0. 133kPa) between altitude 2 260 m and 2 780 m districts. In these patients, their serum lactic acid was increased before treatment. At altitude 2 780 m district, serum lactic acid in middle-aged and elder groups were even higher [(3.90±0.85 vs 3.65±0.70), (4. 67±0.80 vs 3.69±0.78) mmol/L, both P<0. 05=. After 14-day treatment, serum lactic acid in middle-aged diabetic patients remained statistically unchanged [(4.50±0.50 and 3.79±0.62 ) mmol/L, P>0.05], while that in elder patients was increased significantly [(5.59±0.55 and 5.27±0.43 ) mmol/L,P<0. 05]. The results suggested that metformin should not be used or be used carefully in elder diabetic patients at high altitude area.
