Purpose To investigate the clinicopathologic features, diagnosis and differential diagnosis of primary gastric inflammatory myofibroblastic tumor ( IMT) . Methods Four cases of gastric IMTs were studied by clinicopathologic analysis, immunohistochemistry and in situ hybridization, and the related literature was reviewed. Results In four cases there are two males and two females, age range from 21 to 51 years old, and tumor size ranged from 1. 5 to 6. 5 cm in the greatest dimension. Histologically, these tumors were composed of varied spindle cells and chronic inflammatory cells, in a myxoid or hyalinized stroma. Occasionally, there were calcifica-tion and ossification areas. Most of the spindle cells had bland appearance and a minority of the tumor cells showed mild atypia. One to two mitotic figures were recognized in 10 high power fields ( HPFs) in 1 to 2 patients. Smooth muscle actin staining was observed in all tumors and ALK staining observed in two tumors. One tumor focally expressed CD34. S-100, desmin, CD68, CD117 and DOG1 was negative in all IMTs. The patients were followed up from 24 to 66 months, and none of them had tumor relapsed or metastasis. Conclu-sions Primary gastric IMTs have an intermediate behavior, and a few cases have malignant potential. It should be distinguished from other spindle cell lesions similar to IMT.

This paper was aimed to explore the relationship between different commercial specification and chemical constituents on Radix Rehmanni, which provided experimental evidences for drawing up the commodity commercial specification and grading criteria of Radix Rehmanni.Several classified indexes, including grade, surface color, section color, section texture were carried to explore the change rules of chemical constituents by setting the contents of iridoid glycoside, sugars, water content, total ashes,acid-insoluble ashes and water extract as the markers. The results showed that the grade, surface color and section texture had less impact on chemical constituents. However, the section color had more influence.Along with the section color gradually being deepened, the contents of iridoid glycoside and oligosaccharides of Radix Rehmanni were lower, and the contents of monosaccharide and polysaccharide were higher. It was concluded that there was correlation between commercial specification and chemical constituents of Radix Rehmanni.

Small interfering RNAs (siRNAs) can efficiently inhibit gene expression by sequence-specific RNA interference (RNAi). A common 5' leader sequence exists in the genomic RNA and all subgenomic RNAs of SARS-CoV, and is well conserved among various SARS-CoV strains, thus providing a preferable target for RNAi of SARS-CoV replication. Here efficient depletion of the SARS-CoV mRNAs by either a synthetic siRNA or DNA vector-derived short hairpin RNAs (shRNAs) targeting the leader sequence in mammalian cell lines were reported. The siRNA or shRNAs efficiently suppressed the expression of an EGFP reporter gene which contains the leader sequence at the 5' end. Both the siRNA and shRNAs efficiently knocked down the levels of leader-containing transcripts of three SARS-CoV genes encoding the spike protein, membrane protein and nucleocapsid protein were demonstrated. The results suggest that RNAi targeting the leader sequence is a potential efficient strategy for anti-SARS-CoV therapy.

Objective To discuss the influent factors and managements of lower urinary tract injury caused by tension-free vaginal tape (TVT) procedure. Methods From Mar.2001 to Feb.2011,609stress urinary incontinence (SUI) patients were trested by TVT.Lower urinary tract injury appeared in 39 cases (6.4%),which age from 39 to 78 years (average age 52.7 ± 18.3).The history of disease was 2 to 12 years.Preoperative SUI types were 12 cases of Ⅱ type SUI,22 cases of Ⅱ/Ⅲ type SUI and 5 cases of Ⅲ type SUI.The patients who had low urinary tract injury were retrospective analyzed to figure out the causes and influent factors, and recorded the treatments and follow-ups. Results 39 patients (6.4%) suffered from low urinary tract injury, including 36 oases (5.9%) of bladder perforation and 3 cases of urethral injury. In these 39 patients,34 (87.2%) patients had history of pelvic surgeries, including 18 (52.9%) cases of total hysterectomy,9 (26.5%) cases of cesarean section delivery,4 (11.8%) cases of hysteromyomectomia and 3 (8.8%) cases of ovarian surgery. All of the 36 bladder peeroration patients were re-punctured and the catheter was kept for 4 - 5 d.Three urethral injury patients were re-punctured after the urethral rupture was sutured and the catheter was kept for 2 weeks. All the 39 patients were cured and discharged. No urinary fistula, infection or other postoperative complications occurred. Conclusions The history of pelvic surgery may be an important risk factor of the lower urinary tract injury, which should pay attention. If the bladder perforation occurred,re-puncture should be taken by adjust the direction. If there was a urethral injury, the re-puncture should be taken after the suturing of the urethral rupture.

ObjectiveTo evaluate the efficacy and safety of domestic taerolimus in the manage-ment of acute rejection in renal transplantation reeipienta.MethodsA multieeneter, open and com-parative study for domestic Taerolimua and Prograf was performed. Eighty reeipienta of first allogenie renal transplantation were randomized into 2 groups: ①Experimental group (accepting Fumeixin ad-ministration, n=58) included 23 males and 35 females with the mean age of(39.1±9.6)years. ②Control group(accepting Prograf administration, n=22) included 12 males and 10 females with the mean age of(41.34±8.5)years. There was no significant difference in the volume, warm and cold is-chemia time of donor renal, age and sex of donor. All of the 80 cases accepted tacrolimus (domestic or foreign made, at the dose of 0. 10~0. 15 mg·kg-1·d-1 , q 12 h) treatment that combined with MMF and prednisone posttransplantation. Tacrolimus CO was aimed to 8-12 ng/ml in the first 60 days and 5-10 ng/ml later. The dose of MMF was according to the rule of each transplantation center based on the following recommendation: 1.5-2.0 g/d for the weight above 70 kg, 1.5 g/d for the weight 50-69 kg, and 1.0 g/d for the weight below 49 kg, in two divided doses. Prednisone was ad- ministrated as per ruler of each center. ResultsObservation termination was 3 months. Morbility of actue rejection was 3.40% (2/58)and 13.6% (3/22) in experimental and control group(P>0.05). Ad-verse events including hypertension, hyperlipemia, hyperglycemia and slight abnormality of liver func-tion occurred in 36. 2%(21/58) and 36. 4% (8/22) cases of experimental and control group(P> 0. 05). The survival rate in the 2 groups was 100%. The dose of tacrolimus in experimental group was significantly lower than that in the control group at 8 and 12 weeks posttransplant, while drug valley concentration in serum was proximal in 2 groups during the whole observation period. Conclusion Domestic tacrolimus capsules can be used effectively and safely in the management of acute rejection in renal transplantation.
