1.Surgical jaundice after laparoscopic cholecystectomy:Analysis of 16 cases

Objective To discuss the causes,treatment and prophylaxis of surgical jaundice after laparoscopic cholecystectomy (LC). Methods A retrospective analysis was carried out in 16 cases of surgical jaundice consisting of 14 cases from this hospital out of 3 092 cases of LC from January 1994 to January 2002 and 2 cases transferred from other hospitals. Results Out of the 16 cases,residual bile duct stones were found in 9 cases,and bile duct injuries had taken place in 7 cases (2 cases of cystic duct stump leakage,3 cases of common bile/hepatic duct injuries and 2 cases of recurrent jaundice resulted from aberrant bile duct leakage).All the 16 cases were cured by surgery and no bile duct stricture occurred after surgical procedures. Conclusions Strict following indications of LC and normative surgical performance are the keys to the decrease of surgical jaundice after LC.The patient should be given active surgical intervention once surgical jaundice occurs.
2.Correlation between polymorphism of regulated upon activation normal T-cell expressed and secreted gene promoter and diabetic nephropathy in type 2 diabetes mellitus

BACKGROUND: Hereditary susceptibility is an important role in episode and development of diabetic nephropathy (DN). Aggregation of monocyte in kidney tissue, differentiation to histocyte and reinforcement of signal are possibly closed to episode and development of DN in type 2 diabetes mellitus (DM). Regulated upon activation normal T-cell expressed and secreted (RANTES) is a chemokine of C-C subtribe. It can mediate infiltration and activation of inflammatory cells and cause injuries of renal corpuscle and renal mesenchymal. This may be a key mechanism of episode and development of DM. Thus, RANTES gene may be one of the susceptible gene of DN. OBJECTIVE :To investigate correlation between RANTES gene promoter -28C/G polymorphism and DN in type 2 DM in Chinese. DESIGN: Case-contrast study. SETTING: Department of Endocrine, Affiliated Hospital of Guiyang Medical College; Center of Diabetes Mellitus of Guizhou Province. PARTICIPANTS: A total of 143 patients with type 2 DM were recruited from the Department of Endocrine, Affiliated Hospital of Guiyang Medical College and Department of Endocrine,Guizhou People's Hospital from September 2003 to September 2005. Among them, 53 cases had normoalbuminuria, 54 microalbuminuria and 36 macroalbuminuria. Another 55 healthy subjects who were selected from Center of Health Examination, Affiliated Hospital of Guiyang Medical College were regarded as control group. METHODS: 3-5 mL peripheral venous blood was collected from all subjects. RANTES gene promoter -28C/G genotype was detected with polymerase chain reaction restriction fragment length polymorphism technique. MAIN OUTCOME MEASURES: ① Allele frequency and genotypic frequency of RANTES gene promoter -28C/G; ② risk factor of DN was analyzed with Logistic regression analysis. RESULTS: All 198 subjects were involved in the final analysis. ① Comparison of CG genotypic frequency of -28C/G: There was no significant difference between normal control group and DM group (P> 0.05); however, that in microalbuminuria group and macroalbuminuria group was higher than that in normoalbuminuria group (24.1%, 47.2%, 9.4%, P < 0.05), and that in macroalbuminuria group was also higher than that in microalbuminuria group (P < 0.05). ② Frequency of G allele of -28C/G: There was no significant difference between normal control group and DM group (P > 0.05); however, that in micro albuminuria group and macrealbuminuria group was higher than that in microalbuminuria group (12.0%, 23.6%, 4.7%, P < 0.05), and that in macroalbuminuria group was also higher than that in microalbuminuria group (P< 0.05). ③ Logistic regression analysis showed that positive genotype of -28G was related to DN (95% Cl:1.402-15.032, P =0.012). CONCLUSION: Positive genotype of RANTES gene promoter-28G may be related to DN.
3.Effect of Astragalus Membranaceus Polysaccharide on Myocardial Ischemia-Reperfusion Injury in Rats

Objective To research the cardioprotective effect of Astragalus Membranaceus Polysaccharide (AMP) on hemodynamic and oxygen free radicals in myocardial ischemia followed by reperfusion. Methods By using experimental rats cardiac model, rats were divided into 3 groups:CG (control group), IR (Ischemia-Referpusion) and AMP (Astragalus Membranaceus Polysaccharide). Results AMP could promote the restoration of LVSP、?dp/dtmax、CF、CO、SV、SI、CI, and decrease the rising of LVEDP and T value distinctively. The recovery of left ventricular function in AMP group was better than that of the ischemia-reperfusion group, which was accompanied with the significant reduction of oxygen free radicals and increase of superoxide dismutase. Conclusion AMP could attenuate ischemia- reperfusion injury and ameliorate functional recovery in isolated rat hearts.

5.Role of liver dendritic cell subsets in liver homeostasis and injury repair

Dendritic cells (DC) represent a heterogeneous cell population including many subsets. DC subpopulations with different characteristics and functions have been identified. The liver, as an immunological organ, is important in induction of immune tolerance. The unique anatomical architecture and immune properties of the liver have given DC the ability to maintain liver homeostasis and respond quickly to liver tissue damage. This article reviewed the role of hepatic DC subsets in the maintenance of tissue homeostasis and repair of damaged tissue in the liver.
6.A P-wave detection method based on multi-feature.

Generally, P-wave is the wave of low-frequency and low-amplitude, and it could be affected by baseline drift, electromyography (EMG) interference and other noises easily. Not every heart beat contains the P-wave, and it is also a major problem to determine the P-wave exist or not in a heart beat. In order to solve the limitation of suiting the diverse morphological P-wave using wavelet-amplitude-transform algorithm and the limitation of selecting the pseudo-P-wave sample using the wavelet transform and neural network, we presented new P-wave detecting method based on wave-amplitude threshold and using the multi-feature as the input of neural networks. Firstly, we removed the noise of ECG through the wavelet transform, then determined the position of the candidate P-wave by calculating modulus maxima of the wavelet transform, and then determine the P-wave exist or not by wave-amplitude threshold method initially. Finally we determined whether the P-wave existed or not by the neural networks. The method is validated based on the QT database which is supplied with manual labels made by physicians. We compared the detection effect of ECG P-waves, which was obtained with the method developed in the study, with the algorithm of wavelet threshold value and the method based on "wavelet-amplitude-slope", and verified the feasibility of the proposed algorithm. The detected ECG signal, which is recorded in the hospital ECG division, was consistent with the doctor's labels. Furthermore, after detecting the 13 sets of ECG which were 15 min long, the detection rate for the correct P-wave is 99.911%.

7.Effects of curcumin on mRNA expression of cytokines related to Tou-like receptor 4 signaling in THP-1 cells

Objective To investigate the effects of curcumin on mRNA expression of cytokines related to Toll-like receptor (TLR) 4 signaling in THP-1 cells.Methods After pretreatment with different concentrations (50,25,12.5 mg/L) of curcumin or dexamethasone for 12 hours,THP-1 cells were stimulated by lipopolysaccharide (LPS.1 mg/L) for 4 hours followed by the collection of cells.Then total RNA was isolated from these cells and subjected to reverse transcription-polymerase chain reaction (RT-PCR) for the detection of mRNA expression of tumor necrosis factor receptor-associated factor 6 (TRAF6),interleukin-1 receptorassociated kinase (IRAK1) and nuclear factor (NF)-κB.THP-1 cells without pretreatment or stimulation served as negative control,and those only stimulated with LPS served as LPS group.Results After stimulation with LPS (1 mg/L) for 4 hours,the mRNA expressions of TRAF6,IRAK1 and NF-κB were significantly upregulated in THP-1 cells compared with negative control cells (f=38.69,39.13,23.99,all P<0.01).Curcumin of 50 mg/L and 25 mg/L significantly inhibited the mRNA expressions of TRAF6.IRAK 1 and NF-κB upregulated by LPS with an inhibition rate of more than 50% (all P<0.0 1).Conclusions Certain concentrations of curcumin can inhibit the mRNA expressions of TRAF6.IRAK1 and NF-κB.which demonstrates the regulatory effect of curcumin on the mRNA expressions of TLR4 signaling pathway-associated cytokines.
8.Correlation of Spiral CT Features and Angiogenesis,Expression of TP,TGF-?_1 in Hepatocellular Carcinoma

Objective To evaluate the correlation between the contrast enhanced features on spiral computed tomography(SCT) and microvessel density(MVD),TP,TGF-?_1 expression.Methods Forty seven cases with hepatocellular carcinoma(HCC) proved pathologically were examined with enhanced dual-phase SCT scanning at the arterial phase and the portal vein phase.The SCT features including the size of HCC lesions,enhanced patterns,capsule patterns,the diameter of tumor,intrahepatic daughter foci and tumor-emboli in portal veins were studied.The expression of TP,TGF-?_1 and MVD were detected by immunohistochemical SP method.The enhanced features of SCT were compared with the immunohistochemical results and clinicopathological characteristics of HCC.Results TP and MVD were correlated with metastasis and capsule patterns(P0.05).Conclusion The expression of TP,TGF-?1 and angiogenesis of HCC can be evaluated by some SCT features in a certain degree.

10.Simultaneous determination of four kinds of components in Tongda Granule by HPLC-DAD

AIM:To determine four bioactive components in Tongda Granule ( Radix Paeoniae alba,Flos Carthami,Radix Scutellariale,etc. ),namely hydroxy safflower yellow A,peoniflorin,ferulic acid,and baicalin. METHODS:The isolation was performed on an Alltech Apollo C18 (250 mm ? 4. 6 mm,5 ?m) with the mobile phase consisting of methanol-0. 2% (w)phosphoric acid for gradient elution. The flow rate was 1. 0 mL/min and the column temperature was set at 30 ℃. RESULTS:This method was successfully applied to determining four bio-active components in Tongda Granule. The standard curves were linear over the ranges of 5. 20 -104 mg/L for hydroxy safflower yellow A(r =0. 999 1),25. 6 -512 mg/L for peoniflorin (r =0. 999 6),7. 72 -154. 4 mg/L for ferulic acid (r =0. 999 5),32. 5 -650 mg/L for baicalin (r = 0. 999 5),respectively. The average recoveries were 101. 1% ,99. 6% ,98. 9% and 101. 9% ,RSD were 2. 3% ,1. 8% ,2. 6% and 1. 3% ,respectively. CONCLUSION:The method is simple and can be used as a quality control method for Tongda Granule.
