Objective To investigate the relationship between the serum ghrelin level and β-cell function after treatment of patients with newly diagnosed type 2 diabetes mellitus.Methods 34 patients with newly diagnosed type 2 diabetes mellitus were divided into three groups of pre-treatment(DM0),two weeks after treatment(DM1)and six months after treatment(DM2).Intravenous glucose tolerance tests(IVGTT)were performed and blood glucose,HbA1C,ghrelin,insulin were measured during the study.Insulin sensitivity was assessed by homeostasis model assessment(HOMA).Results(1)Compared with DM0 group,beta-cell function and the mean AUC of insulin were dramatically improved in groups of DM1 and DM2(all P<0.05).(2)Compared with DM1 and DM2,ghrelin levels and the mean AUC of ghrelin in DM0 groups were lower(all P<0.05).(3)Only positive correlation between ghrelin and HOMA-β(r=0.446,P<0.05),and negative correlations of ghrelin with glucose(r=-0.274,P<0.05),insulin(r=-0.35,P<0.05)and BMI(r=-0.276,P<0.05)were found after six months of treatment.Conclusions Ghrelin might play an important role in the pathogenesis and progress of type 2 diabetes mellitus.

5.Pharmacokinetics of oral FK506 in patients after renal transplantation

Objective To carry out a pharmacokinetic evaluation of oral FK506 in 13 patients after renal transplant. Methods 13 patients after renal transplantation were given prograf based immunosupressive regimen 24 hours after surgery.Blood samples to determine FK506 levels were drawn in heparinized tube at 0、20、40、60、90 min and 2、3、6、8、10 hours after the first oral dosing.The whole blood concentrations were measured by MEIA and the pharmacokinetic parameters were calculated by 3P87 program.The FK506 doses were recorded in detail for the first month. Results Cmax was (13.6259?4.1117)ng/ml;T(peak) was (1.4866?1.0725)h;t1/2 ? was (0.7749?0.7791)h,t1/2 ? was (10.7267?10.4926)h;AUC was (91.0415?40.7694)ng?ml -1 ?h -1 ,CL was (0.046?0.0036)ng?ml -1 ?h -1 and MRT was (8.1540?4.2937)h.AUC was negative correlateld with prograf dose in the first month posttransplant (r=-0.53, P =0.038). Conclusions The absorption of oral administration of FK506 was rapid in patients after renal transplantation,and can achieve Cmax in (1.5?1.1)h,the mean half life time being 10.7 h.The pharmacokinetic parameters can be the guideline for FK506 application.
8.Mediating effect of self-nursing ability between living condition and anxiety in patients with peptic ulcer caused by helicobacter pylori

Objective:To explore the mediating effect of self-nursing ability between living condition and anxiety in the patients with peptic ulcer caused by helicobacter pylori.Methods:By using questionnaire survey method,300 patients with a peptic ulcer caused by helicobacter pylori were assessed with the exercise of self-care agency (ESCA),Self-rated Health Measurement Scale (SRHMS) and Self-Rating Anxiety Scale (SAS).The result was compared with that of 300 healthy crowd whose general condition was well-matched.The data were processed and analyzed by using SPSS13.0 and AMOS7.0.Results:The total scores of the ESCA,SRHMS and SAS were (101 ± 13),(299 ±35),(56 ±8) respectively.The result of the correlation analysis showed there were varying degree correlation among the disease self-nursing ability,self-rated Health,and anxiety (Ps < 0.01).Bootstrap test showed the size of the mediating effect of self-test health to anxiety was 0.17,the 95% confidence intervals was 0.11-0.26,and the ratio of the mediating effect to the total effect was 0.44.Contusion:It suggests that self-nursing ability may have much strong mediating role between health condition and anxiety.
9.The structure and technical research of medical information interaction and sharing among regions.

The hospital information structure, which is made up of various medical business systems, is suffering from the problems of the "information isolated island". Medical business systems in the hospital are mutually isomerous and difficult to become a whole. How to realize the internal barrier-free interaction of the patients effective medical information in the hospital and further to complete the area sharing of patients longitudinal diagnosis and treatment information has become a question having to be solved urgently in the process of healthcare informatization. Based on the HL7 standard, this paper refers to the IHE technical framework, expounds the overall structure of the interaction in the hospital internal and area sharing of medical information with the medical information exchange platform. The paper also gives the details of the whole process of the complete display of the discrete patient health information using Portal technology, which is saved in the business systems in different hospitals. It interacts internally through the information exchange platform and at last stores the information in the regional clinical data repository (CDR).

10.The prevalence of combined extra-and intra-cranial artery stenosis in patients with extracranial internal carotid artery occlusive disease

Objective The intra-cranial arteries are more affected in Asian ischemic stroke patients. We thought that tandem or diffuse arterial occlusive disease may be more common among patients with extracranial internal carotid artery (ICAex) occlusive disease. In the current study, the frequency of other co-existing extracranial artery ( EA) or intracranial artery ( IA) stenosis in patients with ICAex occlusive disease and its risk factors were studied. Method One hundred and ninety-eight consecutive patients with ischemic stroke or TIA who were admitted to our hospital from Jan 2001 to May 2008 and who had stenosis (70% ) or occlusion of ICAex were enrolled in the study. All the patients were examined with carotid duplex and transcranial Doppler (TCD). Among them, 75 patients were also examined with DSA, 52 patients with intracranial MRA and 25 patients with extra- and intra-cranial CTA. Extracranial artery stenosis or occlusion were diagnosed with DSA/CTA in 93 patients, and with carotid duplex only in 105 patients. Intracranial artery stenosis was diagnosed with DSA/MRA/CTA in 118 patients, and with TCD only in 80 patients. Patients with presumably cardioembolism such as atrial fibrillation were excluded. Results Among the 198 patients, 172 (86. 8% ) had other combined EA or IA stenosis. One hundred and thirty-five patients (135/198, 68.2%) had other combined EA stenosis, which were more frequently found in the contralateral ICAex (71/198, 35.9%). One hundred and twenty-three patients (123/198, 62. 1%) had combined IA stenosis, which were more frequently found in MCA (75/198, 37.9%). 52 (52/198, 26. 3% ) had ipsilateral terminal ICA and/or middle cerebral artery stenosis. Conclusion Tandem or diffuse arterial occlusive disease is common among patients with steno-occlusion of ICAex . Therefore, it is important to evaluate other EA and IA stenosis before CEA or CAS in Chinese patients.
