1.Expression of sirtuin in hepatocellular carcinoma and its clinical significance

Hepatocellular carcinoma (HCC) development is closely related to overexpression of tumor promoters or down-regulation of tumor suppressors.The mammalian sirtuin family was found to be a nicotinamide adenosine dinucleotide (NAD/NADH)-dependent histone deacetylase (HDAC),which implicated in the regulation of critical biological processes through deacetylation modifying on histone and nonhistone.SIRT1 can regulate metabolism,aging,inflammation and cancer progression.In particular,more and more evidence proves that SIRT1 can act as a tumor promot er in hepatocellular carcinoma through deacetylation on tumor suppressors.On the other hand,SIRT1 can strongly suppress metabolic syndrome-associated liver cancer in the mouse model.This review will discuss the expression of sirtuin family member in liver cancer and its clinical significance.
2.Effect of surrounding-acupuncture on scalp on hemiplegia

ObjectiveTo investigate the effect of surrounding-acupuncture on scalp on hemiplegia.Methods90 patients were divided into three groups. First group were treated with surrounding-acupuncture on scalp, second group with traditional scalp-acupuncture, the third group was control group. ADL and Mannal Function Test (MFT) were used to evaluate the function pre- and post treatment. ResultsThere were no significant differences among the three groups on ADL and MFT at beginning, that were improved significantly after treatment. However, the first group improved more significantly than that of the other two groups for ADL and low limb MFT. The first group showed significantly improvement for Hamilton Rating Scale for Depression pre- and post treatment.ConclusionThe surrounding-acupuncture on scalp is effective method for hemiplegia.
3.Experimental Study on Antiviral Effect of Gankeshuangqing Capsule

OBJECTIVE:To investigate the antiviral effect of Gankeshuangqing capsule on virus infected mouse model.METHODS:The virus infected mouse model was established by administering influenza virus through nose,and then Gankes_ huangqing capsule was given intragastrically,the antiviral effect was observed by experiment of viral pneumonia and viral multiplication.RESULTS:Gankeshuangqing capsule could significantly reduce the lung indices,and lessen the pulmonary histopathological change;It also significantly inhibit virus multiplication in lungs of mouse.CONCLUSION:Gankeshuangqing capsule has a strong anti-influenza effect on virus infected mouse model.
4.Methylation status of tumor suppressor gene ppENK in the pathogenesis of pancreatic carcinoma

Objective To detect the methylation status of ppENK and its role in the pathogenesis of pancreatic carcinoma.Methods The ppENK methylation status in human tissues of pancreatic cancer,pancreatic carcinoma cell lines and normal pancreas was detected by methylation-specific RT-PCR(MSP).The association of methylation status of ppENK gene with clinicopathological parameters was analyzed. The expression of ppENK mRNA was detected by RT-PCR.Two pancreatic carcinoma cell lines (PANC1,AsPC1 ) were treated with demethylating agent (5-aza-dC).The cell growth was measured by MTT.Apoptosis and cell cycle was analyzed by flow cytometry.The expression of DNMT3a was measured by Western blot.Results ppENK mRNA was expressed in normal pancreas.And methylation of ppENK was not detected in normal pancreas.Methylation of ppENK was detected in 90.3% (28/31) of pancreatic carcinoma tissue,and there were no correlation between methylated ppENK with clinicopathological features of pancreatic carcinoma.There was no ppENK mRNA expression in SW1990,PANC1,PC3,AsPC1,PuPan-1,and ppENK was methylated.Methylated ppENK was associated with no ppENK mRNA expression.After 5-Aza-dC treatment,PANC1,AsPC1 was demethylated and ppENK mRNA expression was reversed.The proliferation of PANC1 and AsPC1 was inhibited in a dose dependent manner.The apoptotic rates of PANC1 and AsPC1 were increased [ (31.57 ± 6.76)%ts (3.21 ±1.43)%,P =0.002,(16.6 ±8.22)% vs (3.82 ±1.71)%,P=0.058];the expression of DNMT3a protein was decreased; the PANC1 cells of G1 phase significantly increased [ (67.87 ± 2.72 ) % vs (54.57 ± 7.18) %,P =0.040 ],but PANC1 cells of S phase significantly decreased [ ( 22.37 ± 4.31 )% vs (33.73 ± 4.63)%,P =0.036 ].But the percentage of G1,S phase in AsPC1 cell line was not significantly changed ( P =0.236,0.075 ).ConclusionsppENK demethylation is an important molecular event in inducing ppENK expression inhibition,which can inhibit pancreatic cancer proliferation,promote apoptosis,arrest cell cycle at G1 and decrease the expression of DNMT3a protein.
5.The role of four criteria in assessment of the severity and prognosis of hyperlipidemic acute pancreatitis

Objective To explore the four criteria,including bedside index for severity in acute pancreatitis(BISAP),Ranson score,modified CT severity index(MCTSI) and acute physiology and chronic health evaluation scoring system Ⅱ (APACHE Ⅱ) in assessment of severity and prognosis of hyperlipidemic acute pancreatitis.Methods A total of 326 patients with hyperlipidemic acute pancreatitis were studied retrospectively from August 2006 to July 2015.The discrepancy of the four criteria in assessment of severity and prognosis of hyperlipidemic acute pancreatitis was compared with chi-square test and receiver operating characteristic curve.Results The incidences of moderately severe acute pancreatitis and severe acute pancreatitis,local complications and mortality of patients with BISAP score ≥3,Ranson score ≥3,APACHE Ⅱ score≥8 and MCTSI score≥4 were significantly higher than BISAP score < 3,Ranson score < 3,APACHE Ⅱ score < 8 and MCTSI < 4 respectively (all P < 0.05).As far as severity was concerned,the sensitivity and AUC of APACHE Ⅱ were 57% and 0.814,which were higher than the other systems.The second most sensitive criterion was BISAP.In assessment of local complications,the sensitivity and AUC of MCTSI were 68% and 0.791,which were higher than the other three.The most sensitive criterion to predict mortality was BISAP with sensitivity 89% and AUC 0.867,which was followed by APACHE Ⅱ.Conclusions All four criteria can be used to determine the severity,local complications and mortality.Generally,BISAP is simple and easy to practice,and better than the other three.

The paper presented the thoughts and steps taken by the National Center for Cardiovascular Diseaes in biobank quality management system.By means ofprocess approach,the organizational structure,identification and analysis process were established,along with the management mechanism and normalized documentation.Centering onPlan,Do,Check and Act(PDCA),a complete set of quality management system was established.This system enables normalized management of biobanks in China,and provides practice guidelines for development industry standards of the country as well.
7.Effects of lung protective ventilation on the inflammatory transmitters in peripheral blood and bronchoalveolar lavage fluid of dogs with acute respiratory distress syndrome caused by pulmonary and extrapulmonary insults

BACKGROUND: Lung protective ventilation strategies and positive endexpiratory pressure (PEEP) have been widely used as an effective ventilation pattern in clinical practice of administration of acute respiratory distress syndrome (ARDS) in recent years, but there arestill great arguments on the therapeutic effects.OBJECTIVE: To observe the changes of oxygenation index and inflammatory transmitters in peripheral blood and bronchoalveolar lavage fluid(BALF) of different lung areas [superior area of lung (upper lobe), ventral side of inferior lung (heart lobe), and dorsalis inferior lung (diaphragm lobe)] of ARDS dogs caused by pulmonary and extrapulmonary insults under lung protective ventilation treatment.DESIGN: A randomized control animal study.SETTING: Department of Respiratory Medicine, the General Hospital of Chinese PLA.MATERIALS: Twenty-four adult healthy male mongrel dogs were randomly divided into four groups with 6 dogs in each: pulmonary ARDS experimental group, pulmonary ARDS control group, extrapulmonary ARDS experimental group and extrapulmonary ARDS control group.METHODS: Models of extrapulmonary ARDS were induced by intravenous injection of oleic acid (0.1-0.15 mg/kg), and the pulmonary ARDS models were established by intratracheal administration of diocty sulfosuccinate sodium salt. After lung injury, the experimental groups received lung protective ventilation treatment (tidal volume: 8 mL/kg, PEEP: 0.981 kPa)for 3 hours, and the control groups also received ventilation of large tidal volume (tidal volume: 14-17 mL/kg, PEEP: 0 kPa).MAIN OUTCOME MEASURES: ① changes of oxygenation index in each group; ② dynamically observed the changes of the inflammatory transmitters [tumor necrosis factor alpha (TNF-α), interleukin-1 beta (IL1β) and interleukin-6 (IL-6)] in pe. Ripheral blood and BALF of different lung areas (upper lobe, heart lobe and diaphragm lobe) of ARDS dogs under lung protective ventilation treatment.RESULTS: All the 24 dogs were involved in the analysis of results. ①After lung injury, the oxygenation indexes were significantly decreased in all the groups, and the oxygenation indexes after lung protective ventilation in the experimental groups were obviously higher than those in the control groups (P < 0.05). At 2 and 3 hours after lung protective ventilation, the oxygenation indexes in the extrapulmonary ARDS experimental group were markedly higher than those in the pulmonary ARDS experimental group (P< 0.05). ② After lung injury, the levels of the inflammatory transmitters in peripheral blood were all obviously increased in all the groups, which were decreased to different extent after the lung protective ventilation treatment,but the therapeutic effect in the pulmonary ARDS experimental group was not as good as that in the extrapulmonary ARDS experimental group. ③The levels of inflammatory transmitters in BALF of lung upper lobe and heart lobe were obviously higher in pulmonary ARDS dogs than in extrapulmonary ARDS dogs.CONCLUSION: The ameliorations of the release of inflammatory transmitters and oxygenation index at different areas are obviously different between ARDS dogs caused by pulmonary and extrapulmonary insults, and lung protective ventilation has good effect on extrapulmonary ARDS dogs,but has bad effect on pulmonary ARDS ones.
8.Paracrine action of bone marrow mesenchymal stem cells and cell apoptosis following cerebral ischemia

BACKGROUND: One of mechanisms involved in treating cerebral ischemia with bone marrow mesenchymal stem cells (BMMSCs) implantation is paracrine action. However, few studies have reported this mechanism.OBJECTIVE: To observe the inhibitory effect of BMMSCs paracrine action on apoptosis and its mechanism after cerebral ischemia. METHODS: BMMSCs were isolated from rats with adherent culture. Rat cerebral ischemia model was established by the middle cerebral artery occlusion. A total of 24 rats were divided into 4 groups, with 6 animals in each group. Cell implantation medication group: rats were received U0126 medication after BMMSCs implantation; Non-implantation medication group: rats were received U0126 medication after PBS injection; Cell implantation control group: received solvent medication after BMMSCs implantation; Non-implantation control group: received solvent medication after PBS injection. At 7 days after operation, the expressions of vascular endothelial cell growth factor (VEGF) and p-ERK1/2 protein were measured by Western blot analysis, and the apoptosis cells in the area of ischemic penumbra and cortex were examined by TUNEL. RESULTS AND CONCLUSION: The VEGF protein content in the brain tissue was significantly greater in the cell implantation groups than that of the non-implantation group, with increased p-ERK1/2 and decreased apoptosis cells. The expression of p-ERK1/2 was down-regulated in rats which were administrated U0126 while the number of the apoptosis cells was increased, but the VEGF protein expression had no statistical difference. It suggested that BMMSCs can paracrine VEGF in the striatum of brain and play an inhibitory effect on apoptosis in the ischemia area via activating ERK1/2.
9.One side polycystic kidney removal and homonymy kidney transplantation simultaneously for giga-polycystic kidney disease of terminal stage

Objective To investigate the security and result of operation of one side polycystic kidney removal and homonymy kidney transplantation simultaneously for giga-polycystic kidney di-sease of terminal stage. Methods Forty-five patients with polycystic kidney of transplantation were retrospectively analyzed. The patients were divided into 2 groups. Patients of group A (n=23) under-went resection of the cystic kidney by extraperitoneum and the other 22 patients(group B) didn't re-move the cystic kidney. The data including average length of hospital stay, variance of blood pressure, lessen of abdominal circumference, lung capacity, total lung capacity, FEV1.0/FVC, incidence rate of delayed graft function (DGF) and 1 year patient/kidney survival rate of the 2 groups were compared. Results The average length of post-operative hospital stay of group A was (14.5±2.6)d,lessen of blood pressure was (30.0±0.7/13.34±8.4)mm Hg, lessen of abdominal circumference was (11.0+ 6. 3) cm, lung capacity increased (1.4±0.3)L, total lung capacity increased (2.0±1.0)L, FEV1.0/ FVC increased (5.3±1.0) %, the incidence rate of DGF was 8.7% (2/23), 1 year patient/ kidney survival rate was 100.0%/95.7%. The average length of post-operative hospital stay of group B was (28.45±7.9)d,lessen of blood pressure was (3.9±11.2/2.9±12. 0)ram Hg, lessen of abdominal circumference was (3.3±2.2)cm, lung capacity increased (0.44±0.3)L, total lung capacity increased (0.8±0.2) L, FEV1.0/FVC increased (2.0±0.9)%, the incidence rate of DGF was 9.1%(2/22), 1 year patient/kidney survival rate was 100.0%/95.5%. There were no significant differences of the incidence rate of DGF and 1 year patient/kidney survival rate between the 2 groups. While there were significant differences of the other data between the 2 groups(P<0.05). Conclusions It is safe and convenient for one side polycystic kidney removal and homonyrny kidney transplantation simultaneous-ly for giga-polycystic kidney disease of terminal stage. The procedure could be applied to the patients of graveness complication or giga-polycystie kidney hampering operation of transplantation.
10.Effects of HIF-1α on human gastric cancer cell's apoptosis in CO2 pneumoperitoneum environment

Objective To study the effect and mechanism of human gastric cancer cell's apoptosis in simulated CO2 pneumoperitoneum environment by HIF-1α. Methods We used a closed box to simulate CO2 pneumoperitoneum environment, a standard surgical insufflator maintained pressure of the box at 0,5,10 and 15 mm Hg respectively. MKN-45 cell's HIF-1α mRNA and protein expression were observed before and after silencing cell's HIF-1α by real time RT-PCR and Western blot. Cell's bcl-2/bax was studied by immunohistochemistry before and after silencing HIF-1α. Cell's apoptosis ratio was measured using Annexin V-FITC/PI double labelled staining. Results In 15 mm Hg group, MKN-45 cell's HIF-1α mRNA and protein expression were significantly higher than control group (P<0.01 ), while the difference was not significant among 0,5,10 mm Hg group and control group (P>0.05). In 15 mm Hg CO2 pressure, cell's bcl-2/bax was obviously lower than in control group (P<0.05) and apoptosis ratio was more than control group(P<0.01). When HIF-1α was silenced, cell's bel-2/bax and apoptosis ratio weren't significantly different between 15 mm Hg group and control group (P>0.05 ). Conclusions MKN-45 cell's apoptosis did not experience any alterations under 0,5,10 mm Hg.15 mm Hg CO2 pneumoperitoneum environment enhanced cell's apoptosis probably by way of HIF-1α.
