1.Short effect and adverse reaction of dog days plaster for allergic rhinitis.

OBJECTIVETo evaluate the short effect and safety of the acupoint application of dog days for allergic rhinitis.
METHODSTwo hundred and forty-nine patients were randomly divided into an application group (166 cases) and a placebo group (83 cases). On the first day of the first dog days, the first day of the second of the three ten-day periods of the hot season and the first day of the last of the three ten-day periods of the hot season, acupoint application was adopted mainly at Dazhui (GV 14), Feishu (BL 13), Pishu (BL 20) and Shenshu (BL 23) in the application group. The plaster was made of Chinese herbs, including Baijiezi (semen brassicae), Xixin (asarum), Yanhusuo (corydalis tuber) and Gansui (euphorbia kansui). Buckwheat plaster without medical ingredient was used in the placebo group. The application was pasted for 2-6 h every time. On the first three days of all the three periods of dog days, treatment was used continuously. The whole period of dog days was made into one course. The changes of symptom and syndrome scores, visual analogue scale (VAS) for symptom before and after treatment and the adverse reaction with the condition disposed were observed in the two groups.
RESULTSAfter treatment, the symptom and syndrome scores and VAS for symptom were improved compared with those before treatment in the two groups (all P < 0.05), and the improvements of the application group were more obvious than those of the placebo group (all P < 0.05). The short effective rate of the application group was 68.1% (113/166), which was better than 48.2% (40/83) of the placebo group (P < 0.05). The adverse reaction rates were 4.2% (7/166), 2.4% (2/83) respectively, with no statistical significance between the two groups (P > 0.05).
CONCLUSIONThe dog days plaster achieves obvious effect with good safety for allergic rhinitis.

3.Intake Fraction: an Evaluation of Vehicle Emission Health Effects

Intake fraction is a new concept applied in assessing human health effect of pollutants. In the present paper, the history and practical use of it were reviewed, and then the calculation of it was also analyzed. After illustrating and discussing previous approaches for estimating intake fraction, the calculation can be classified into two sorts: one is taking the spatial area as the object, and the other is taking the people as the object. The latter is more fitful to estimate the intake fraction of mobile emission, in which it is necessary to investigate the related population and then combine the time-activity patterns with the microenvironment concentrations, in addition, modeling microenvironmental concentrations is critical for the calculation of intake fraction.

5.Exposure Assessment of Vehicle Exhaust Pollution in Wuhan,Hubei

Objective To investigate the exposure level to vehicle exhaust pollution for population by different commuting modes in roadside environment in Wuhan. Methods Eight air monitoring sites were selected in the six main roads in urban area of Wuhan, Hubei province. The concentrations of NO2, CO, total hydro carbon (THC), PM10, benzene, toluene and xylenes (BTX) emitted from the vehicles were monitored and analyzed in winter and summer of 2007, respectively and combined with breathing rate and time spent of the population in traffic environments, exposure doses of different subpopulation were calculated as indicators in order to evaluate their exposure level to traffic exhaust. Results The PM10 was the primary pollutant on the roadside. Compared with the Ambient Air Quality Standard, the rate of exceeding standard limit for PM10 was 8% in summer and 100% in winter. Objective To study kidney transplantation in the aged and its perioperative management. Methods Scrutinized screening of heart,lung and liver were carried out before renal transplantation for 29 patients,aged 65~84.Color Doppler B type ultrasonic inspection was used to determine the status of arteriae iliaca externa and arteriae iliaca interna.Tissue match included HLA and PRA measurement.Warm ischemia time of donor kidney was within 5 minutes and cold ischemia time 10 hours.The dose of MP was under 2.0 g during the period of operation and the immunosuppressive regimen was CsA or FK506+MMF+Pred.The general condition,blood electrolytes,whole blood CsA or FK506 concentration and immune index were monitored. Results There were 4 cases of AR,1 cases of FK506 toxicity and 1 case of lung infection.All these 6 patients were cured.1 kidney graft was removed because of thrombogenesis of renal artery.Up to now,the other 28 have been surviving with the graft functioning well for 1 to 5 years. Conclusions Abide to the indications of kidney transplantation,donor kidney of good quality and well tissue match were important in the elderly patients.With comprehensive monitoring and adequate management,kidney transplant in old patients can achieve sa tisfying outcome.Rational application of immunosuppressants and effective long term follow up are major factors for long term allograft survival.
7.Kidney transplantation in children:report of 26 cases

Objective To report our experience in surgical technique of pediatric renal transplantation and administration of immunosuppressive agents for improvement of recipient/graft survival. Methods Twenty six pediatric recipients (less than 18 years) received cadaveric renal allografts between June 1980 and March 2001.A retrospective analysis was made in these 26 cases with respect to the technical consideration of transplant,immunosuppressive regimens and long term survival. Results Two cases (8%)had delayed graft function,11(42%)had acute rejection and 10 (38%)had chronic rejection.One case develo ped recurrent nephritis,which was pathologically confirmed to be focal segment glomerulosclerosis (FSGS).The recipient/graft survival rates of 1,3,5 years were 96%/88%,92%/73% and 88%/62% respectively,with mortality of 12%. Conclusions In pediatric renal transplantations,good tissue matching,appropriate technique of vessel anastomosis,early diagnosis of acute rejection and optimal adjust ment of blood level for immunosuppressants are keys to success.
8.Polycystin is down regulated in the vessel wall of aortic dissection

Objective To study the expression of polycystin1 and polycystin2 in dissection aorta specimens by Real time PCR and immunohistochemisty. Methods Twelve descending dissection aorta specimens from patients of acute Stanford B dissection were taken during surgery; 12 normal descending aorta specimens were taken from multi-organ donors. The G APDH gene was used as control, gene expression of polycystinl and polycystin2 were compared in these two groups with Real time PCR. The expression of polycystinl and polycystin2 protein were showed with immunohistochemisty in the specimen of these two groups. Three high magnify fields were randomly chosen to count the expression of polycystin1 and polycystin2 protein. The counting of polycystin1 and polycystin2 protein were compared between these two groups. Results The gene expression of polycystinl in AD group is 0. 32 fold as that in the control group (P < 0.01 ) and polyeystin2 in AD group is 0.34 fold as that in the control group (P <0.01 ); The protein expression of polyeystinl was 0.47 folds as that in the control group ( P < 0.05 ) and polycystin2 in AD group is 0.35 folds as that in the control group ( P < 0.01 ). Conclusions The expression of polycystin1 and polycystin2 on gene and protein level were down regulated in AD group. The down regulation of polycystinl and polycystin2 may play a pivotal role in the development of AD.
9.Assessment of glomerular filtration rate in renal transplant recipients using serum cystatin C during follow-up

Objective To evaluate whether serum cystatin C (SCys C) could be used as an ideal index to assess renal function of renal transplant recipients during posttransplant follow-up.Methods Seventy patients who were followed up for at least 6 months after renal transplantation in our centre were recruited in the study. SCys C and serum creatinine (SCr) were determined during the follow-up period, and glomerular filtration rate (GFR) was measured using an isotope Tc99m DTPA.The correlation between SCys C, SCr and GFR was analyzed. The performance of SCys C and SCr in diagnosing the mild impairment of renal allgraft function (GFR < 1 ml/s) was evaluated using ROC curve. Results Both SCys C and SCr had a linear negative correlation with GFR (r = -0. 82 and -0. 66 respectively, P<0. 01 ). The sensitivity, specificity and positive predictive values (PPV) of SCys C for diagnosing the mild impairment of renal allgraft function were higher than those of SCr,but the AUC of SCys C did not differ from that of SCr significantly (0. 935 vs. 0. 877, P>0. 05).Conclusion SCys C could be used an ideal index to evaluate the allograft renal function for its better correlation with actual GFR.
10.THE DISTRIBUTION OF PRIMARY AFFERENT FIBERS FROM THE GREATER SPLANCHNIC NERVE TO THE SPINAL GRAY MATTER AND NUCLEUS GRACILIS IN THE CAT

The central distribution of visceral primary afferent fibers from greater splanchnic nerve of the cat have been studied. Horseradish peroxidase (HRP) dissolved in distilled water was injected into the coeliac ganglion or greater splanchnic nerve unilaterally. The HRP was taken up by afferent fibers and transported across the spinal ganglia and then anterogradely as far as central terminals in the spinal cord and medulla. The HRP reaction-product within the central processes of the primary sensory neurons was detected ipsilaterally in the Lissauer's tract, dorsal funiculus and spinal gray matter. After entering the spinal cord via the spinal dorsal roots, the great majority of labeled fibers (viz. the central processes of primary sensory neurons) were found to run longitudinally within the zone of Lissauer, and only sparse fibers to ascend in the dorsal funiculus. The medial projection fibers (MPF) and lateral projection fibers (LPF) arose periodically from the labeled fiber bundle in the Lissauer's tract to form a thin shell surrounding both the medial and lateral sides of the dorsal horn. The terminal areas of the more prominent LPF were found at laminae I, V, VII and the region surrounding the central canal, whereas the MPF ended at the medial part of lamina V and lamina X. Some smaller fiber bundles from LPF were seen to enter the cell clusters of the intermediolateral nucleus, where these fibers divided rostrocaudally and ran parallel with the longitudinal dendritic branches of the intermediolateral cells. The few labeled fibers located at the dorsal funiculus were traced to the lower medulla and the distinct terminal area of them was situated in the ventrolateral part of the nucleus gracilis at the level below the obex.
