2.Relationships of concentrations of true insulin, proinsulin to coronary heart disease: a preliminary study

Objective To determine serum true insulin (TI) and proinsulin (PI) levels in patients with coronary heart disease (CHD), and to explore the relationships of them to other cardiovascular risk factors. Methods Highly specific ELISA assays for TI and PI and RIA for immunoreactive insulin (IRI) were used. The fasting serum IRI, TI, PI levels in 30 nondiabetic patients with CHD and 30 healthy, age-matched control subjects were investigated. And the fasting and 2h postprandial plasma glucose, serum lipids and blood pressure were also determined in each individuals. Results The levels of serum IRI, TI and PI were significantly higher in patients with CHD than those in controls (P<0.05), while the value of insulin sensitivity index (ISI) was lower in patients with CHD than in controls (P<0.05). There were significant independent correlations between TI, PI and age, BMI, plasma glucose, serum lipids. Conclusion Nondiabetic patients with CHD have hyperinsulinemia and insulin resistance and there is an independent association between proinsulin and dyslipidemia.
3.Comparison of detection rates of women’s subclinical thyroid diseases between iodine-deficient regions with iodine supplying and water-borne iodine excess regions

Objective By comparing the detection rates of subclinical thyroid diseases in women(pregnant,lactating and child-bearing age)between iodine deficient regions (supplied iodized salt)and water-borne iodine excess regions (consumed non-iodized salt),and to find the different prevalence of subclinical thyroid disease between two regions under their different iodine source and iodine levels, and to provide reference for screening susceptible population with iodine-related thyroid diseases.Methods The iodine deficiency but salt iodine surpplying regions were selected from six provinces in our country,the local people who were pregnant women,lactating women and 18 to 45 years old women of child-bearing age,a total of 991 cases were investigated.The iodine nutrition levels of the pregnant women were grouped by <150,150-249,and ≥500μg·L-1;the iodine nutrition levels of the lactating women were grouped by <100 and ≥100μg·L-1 .The high water-iodine regions in Shanxi Province were selected,and according to the water-iodine levels 50-99,100-149,149-299,and more than 300μg·L-1 four regions were selected;20 cases of three kinds of people mentioned above were selected in each region,a total of 241 cases.The blood and urine samples were collected, and the serological thyroid function indexes were detected by chemiluminescence immunoassay method or radioimmunity method, and the urine-iodine was detected with cerium catalytic spectrophotometric method. Results In iodine deficient regions and water-borne iodine excess regions, the concurrence rates of subclinical hypothyroidism(hypothyroidism for short)and thyroid antibody positive of women were 2.32% and 4.98%,respectively;accounting to about 1/3 to 1/2 of those subclinical hypothyroidism population.The prevalence rates of subclinical thyroid diseases in the women population between the two regions were 27.55% and 34.85%,respectively;nearly accounting for 1/3 of the women population.The subclinical hypothyroidism detection rates of the three populations in high water iodine regions were significantly higher than those in iodine deficient regions(P<0.05).The lactating women’s detection rates of thyroid antibody positive and subclinical hypothyroidism with antibody positive in high water iodine regions were significantly higher than those in iodine deficient regions(P<0.05).With different iodine sources,when took appropriate iodine,there was no statistical difference of the detection rates of subclinical thyroid diseases among three kinds of women(P>0.05). With the increase of iodine exposure levels,the prevalence of women who suffered from subclinical hypothyroidism and thyroid antibody positive was increased,the coincidence rate was also increased.The detection rates of low T4 concentration and total subclinical thyroid disease of pregnant women in iodine nutrition<100μg·L-1 group were significantly higher than those in iodine nutrition 250-499μg·L-1 group (P<0.05).The detection rates of low T4 concentration and total subclinical thyroid disease of lactating women in iodine nutrition <100μg· L-1 group were significantly higher than those in iodine nutrition >100μg·L-1 group (P<0.05).Conclusion When the iodine intake is appropriate, iodine intakes from salt or from water have no effect on subclinical thyroid diseases. When the iodine intake increases,the prevalence of subclinical thyroid diseases will increase too.
4.A case of fibrous dysplasia of temporal bone.

Osteofibrous dysplasia is a kind of innocent bone tumor derived from fibrous tissue. Its pathogens has not been identified, it rarely affects the temporal bone, its clinical manifestations mainly is hearing loss. Imageological examination, for example, CT and MRI, is valuable for the diagnosis of this disease,the histopathological evidence is absolutely necessary to make definite diagnosis. Surgery is an effective method to treat the fibrous dysplasia of temporal bone.

5.Expression of miR-124 in gastric cancer and its clinical signiifcance

Background and purpose:miR-124 is considered to be a tumor suppressor in multiple tumors, including lung cancer, prostate cancer, bladder cancer, and breast cancer. However, its function is unclear in gastric cancer. This study aimed to explore the expression of miR-124 in human gastric mucosal epithelium cells and different gastric cancer cells, as well as gastric cancer tissues and matched para-cancerous tissues. The correlations of miR-124 expression with gender, age, histological grade, T stage, TNM stage, lymph node metastasis and prognosis of gastric cancer patients were analyzed.Methods:A real-time lfuorescent quantitative polymerase chain reaction (RTFQ-PCR) was employed for detecting the expression of miR-124 in human gastric mucosal epithelium cells and gastric cancer cells. The expression of miR-124 in gastric cancer tissues and matched para-cancerous tissues was detected by in situ hybridization.Results:The RTFQ-PCR results indicated that the expression of miR-124 was down-regulated in MKN-74, MKN-28, MKN-45, MGC-803, SGC-7901 and AGS cells compared to GES-1 cells.In situ hybridization showed that miR-124 was strongly expressed in normal gastric mucosa. However, low expression, focal positive expression or lack of miR-124 expression were observed in gastric cancer tissues. Statistical analysis showed that miR-124 was close-ly correlated to histological stage, TNM stage and node metastasis of gastric cancer patients, but not the age, gender and tumor size. The OS and DFS of the patients with low expression of miR-124 were shorter than that of the patients with high expression of miR-124. Multivariate analysis suggested that miR-124 down-regulation was an independent prog-nostic factor for survival in patients with gastric cancer.Conclusion:miR-124 is down-regulated in gastric cancer cells and tissues. The expression of miR-124 is correlated to histological stage, TNM stage, node metastasis and prognosis.
6.Etiology and Drug Resistance of Lower Respiratory Tract Infection in Emergency Ward

OBJECTIVE To analyze the pathogens distribution and the antibiotic resistant characters.METHODS Sputum culture was performed in 267 cases who suffered from lower respiratory tract infection in emergency ward from Jan 2004 to Jun 2007.The results of 183 pathogens and drug sensitivity were analyzed.RESULTS The detected 183 strains of pathogens showed that Gram-negative bacilli accounted for 77.1%(Klebsiella pneumoniae was in the first place) and Gram-positive cocci were for 22.9%(Enterococcus faecium and Staphylococcus aureus were in the first places),the total drug resistance rate of Gram-negative bacilli to gentamicin,cefazolin,SMZ/TMP,piperacillin,ciprofloxacin and aztreonam had highly reached 60.5-70%,however to meropenem,cefepime,amikacin,imipenem and Sulperazone was 32.5-38.6%.Gram-positive cocci appeared generally higher multidrug resistant,the drug resistance rate of Gram-positive cocci to penicillins and erythromycin was all 100%,but the resistantce of Gram-positive cocci to vancomycin hadn′t been found.CONCLUSIONS Gram-negative bacilli are major pathogens in lower respiratory tract infection of emergency ward,K.pneumoniae is in the first place of Gram-negative bacilli and E.faecium and S.aureus are in the first places of Gram-positve cocci,both of them appearing highly multidrug resistant,analysis of pathogens and drug sensitivity are necessary to rational antibiotics usage,and the immunological protection should be enhanced.
7.Expression of thyroid-stimulating hormone receptor in thyroid tissue of lactating rats

Objective To observe the expressions of thyroid-stimulating hormone receptor (TSHR) protein and mRNA in thyroid gland of lactating rats.Methods Eighty adult Wistar rats (60 females and 20 males),weighting 210-250 g were selected.The 60 female Wistar rats were divided into 6 groups according to their body weight by means of random number table:normal non-pregnant group,lactating for 5,10,15 and 20 days groups and weaning for 5 days group,10 rats in each group.All rats were fed with conventional fodder and tap water freely,and the rats of lactating groups except the normal non-pregnant group cohabited with male rats (3 ∶ 1).Then all rats were killed on the 5th,10th,15th and 20th day after lactation and on the 5th day after weaning to get thyroid tissues.The expressions of TSHR protein and mRNA were determined by immunohistochemical staining and realtime quantitative PCR.Results TSHR protein was expressed in cytoplasm and membrane of rat thyroid follicular cells.The expression of TSHR protein in thyroid gland was significant different statistically between groups (x2 =11.227,P < 0.05); the staining intensity of rat thyroid tissues in the normal non-pregnant gruop (weak,n =2; moderate,n =5; strong,n =3) was stronger than that of rats lactating for 5 days (weak,n =7; moderate,n =3; x2 =5.895,P < 0.05).But the expression of TSHR protein in thyroid tissues in the normal non-pregnant group was not significantly different statistically compared with the expression of TSHR protein in other groups (lactating for 10,15and 20 days) and weaning for 5 days group (all P > 0.05).The expression of TSHR mRNA in thyroid gland was significantly different statistically between groups (F =2.970,P < 0.05); the expression of TSHR mRNA in lactating for 5 days group (0.74 ± 0.13) was lower than that of the non-pregnant group (1.02 ± 0.24,P < 0.05); and the expression of TSHR mRNA in the normal non-pregnant group was not significantly different statistically compared with those of other groups (lactating for 10,15 and 20 days) and weaning for 5 days group (all P > 0.05).Conclusion TSHR is widely expressed in thyroid gland of lactating rats,but relatively lower in early lactation period.
8.Effect of anisodamine on myocardial connexin 43 expression in pig after resuscitation from cardiac arrest

ObjectiveTo investigate the effect of anisodamine on the expression of connexin 43 (Cx43) in swine ventricular myocardium after resuscitation from cardiac arrest.Methods The experiment was conducted on healthy pigs, and they were randomly divided into three groups, namely sham group, epinephrine group (control group) and anisodamine group (experimental group, animals were resuscitated combined with injection of 0.4 mg/kg of anisodamine), with 5 pigs in each group. Model of ventricular fibrillation was reproduced by alternating current challenge, and cardiopulmonary resuscitation (CPR) was performed 8 minutes after cardiac arrest. Left ventricular myocardium was harvested at 24 hours after restoration of spontaneous circulation. The expression and distribution of Cx43 were observed by immunofluorescence, Cx43 mRNA expression was assessed with reverse transcription-polymerase chain reaction (RT-PCR), and the protein expressions of Cx43 and phosphorylation of Cx43 (p-Cx43) were analyzed by Western Blot.Results The positive expression of Cx43 in ventricular muscle was distributed uniformly, mostly at the end-to-end linkage of myocardial cells, with a few side-to-side linkage in sham group. The positive expression of Cx43 in control group was significantly weaker than that in the sham group, and the signal intensity was significantly declined (4.35±2.10 vs. 10.02±3.66,P< 0.01). The positive expression of Cx43 at the end-to-end linkage and side-to-side linkage was irregular in experimental group, and the signal intensity was obviously higher than that in the control group (7.91±2.54 vs. 4.35±2.10,P< 0.05), but it was significantly weaker than that in the sham group (7.91±2.54 vs. 10.02±3.66,P< 0.05). For control group and experimental group, the Cx43 mRNA and protein expressions were significantly lower than those of the sham group [Cx43 mRNA (A value): 0.32±0.05, 0.32±0.03 vs. 0.48±0.07; Cx43 protein (A value): 0.43±0.03, 0.50±0.07 vs. 0.65±0.04, allP< 0.01], and there were no significant differences between experimental group and control group (allP> 0.05). The p-Cx43 protein expression of control group was significantly lower than that of the sham group (A value: 0.22±0.03 vs. 0.37±0.06,P< 0.01), and it was significantly higher in the experiment group than that in the control group (A value: 0.29±0.07 vs. 0.22±0.03, P< 0.01), but there was no significant difference with the sham group (P> 0.05). No significant difference in p-Cx43/Cx43 was found among sham, control, and experiment groups (0.57±0.09, 0.51±0.05, 0.58±0.06, all P> 0.05).ConclusionAnisodamine can improve the abnormal expression of Cx43 in ventricular muscle of pigs with cardiac arrest, which may be related to the protection effect of anisodamine on cardiac conduction.
9.Establishment of a flow cytometry-based test for the detection of pyroptosis of BMDM

Objective To establish a flow cytometry-based test for the detection of pyroptosis of murine bone marrow-derived macrophages (BMDM). Methods Bone marrow cells were isolated from wild type (WT) C57BL/ 6 mice and/ or caspase-1-/ - C57BL/ 6 mice and then stimulated with macrophage colony-stimulating factor (M-CSF) to differentiate into murine BMDM. PBS, LPS and LPS+adenosine triphosphate (ATP) were respectively used to stimulate the BMDM. Western blot assay was performed to detect the cleav-age of IL-1β and caspase-1. The levels of IL-1β in the supernatants of cell culture were measured by ELISA. Lactate dehydrogenase (LDH) released in the culture media was detected by using LDH kit. The pyroptosis of murine BMDM was detected by using flow cytometry analysis after double-staining with TMR red+caspase-1, AnnexinⅤ+caspase-1 and propidium iodide (PI)+caspase-1. Results IL-1β was detected in the culture medium of BMDM treated with LPS+ATP and the cleavage of IL-1β and caspase-1 was confirmed by Western blot assay, which indicated that the NLRP3 inflammasome was activated by LPS+ATP treatment. Compared with the caspase-1-/ - mice group, higher levels of LDH were detected in the culture medium of BMDM isolated form the WT mice. Results of the flow cytometry analysis after staining BMDM with caspase-1 plus AnnexinⅤ or PI showed that more cells undergoing pyroptosis were detected in the LPS+ATP treat-ment group than that in LPS or PBS treatment group, which were consistent with the results of the reported flow cytometry with caspase-1+TMR red staining. Conclusion The flow cytometry-based test with double-staining of caspase-1 plus AnnexinⅤ or PI could be used for the detection of pyroptosis of murine BMDM.

Objective To observe the effect of cardiac massage by subdiaphragmatic compression (D-CPR) on the length of time required from cardiac arrest (CA) to restoration of spontaneous circulation (ROSC),hemodynamics,rate of ROSC,survival rate of 6 h,level of Caspase3 in myocardial cells and apoptosis index (AI) of myocardial cells and compare the effect of standard cardiac massage by chest compression (S-CPR) on those variables in order to choose the more effective resuscitation method for the patient with CA during abdomen operations.Methods A total of 32 healthy New Zealand rabbit were randomly (random number) divided into two groups,namely S-CPR group and D-CPR group (n =16 in each group).All of rabbits were anesthetized with ketamine and Shumianxin (a kind of hypnotics) by intraperitoneal injection,subsequently tracheotomy was made for endotracheal intubation,and right internal jugular vein was catheterized for monitoring central venous pressure (CVP) and left common carotid artery was for indwelling cannula to monitor arterial blood pressure.Lead-2 of ECG was placed.After laparotomy and vital signs of rabbits stabilized for 5 minutes,the endotracheal tube was clamped at the end expiration for 8 minutes to make asphyxial cardiac arrest model.The effects of two different methods were observed and compared in respects of changes in hemodynamics、length of time elapsed from CA to ROSC、ROSC rate and the survival rate in 6 h.The level of Caspase3 in myocardial cells and AI of myocardial cells were detected by using immunohistochemistry staining method and TUNEL,respectively 6 hours after successful resuscitation.Results ①The length of time consumed fiom CA to ROSC in D-CPR group was shorter than that in S-CPR group (P <0.05) ②Coronary perfusion pressure (CPP) and MAP 15 minutes after CPR were higher in D-CPR group than those in S-CPR group (P < 0.05).③SBP and DBP after ROSC were higher in D-CPR group than those in S-CPR group.④ROSC rate in D-CPR group was significantly higher than that in S-CPR group (81% vs.43%,P <0.05).⑤Survival rate in 6h in D-CPR group was substantially higher than that in S-CPR group (75% vs.25%,P < 0.05).⑥HE staining showed that severe myocardial damage manifesting in edema of myocardial cell,indistinguishable cell boundary,and patchy necrosis with infiltration of scanty inflammatory cells were found in S-CPR group.While in D-CPR group,mild myocardial damage in form of slight cellular edema and distinctive cell boundary was observed.⑦Level of Caspase3 in myocardial cells in terms of integrated optical density (IOD) of postive Caspase3 cells was substantially lower in D-CPR group than that in S-CPR group (P < 0.05).⑧Apoptosis index (AI) of cells was lower in D-CPR group than that in S-CPR group (P < 0.05).Conclusions ①The hemodynamics in D-CPR group was more stable than that in S-CPR.group,and D-CPR increased CPP,MAP,ROSC rate and survival rate in 6h,improving achievement of successful resuscitation.②D-CPR was more effective in terms of shortening the length of time for restoration of spontaneous circulation、decreasing level of Caspase3 in myocardial cells、decreasing apoptosis index of myocardial cells and ameliotating myocardial damage from ischemic repeffusion injury.
