Objective To explore the therapeutic effects of dexmedetomidine for cesarean shivering after spinal-epidural anesthesia and observe the maternal adverse reaction.Methods 94 patients with cesarean shivering after spinal-epidural anesthesia in our hospital were divided into the two groups according to random number table.The study group(47 cases) was given 0.3μ g/kg of dexmedetomidine by intravenous injection after the baby delivered,while the control group (47 cases) was given 1mg/kg of tramal after the baby delivered.The maternal shivering changes were observed,the Ramsay sedation scores before (T0),5min after treatment (T1),10min after treatment (T2) were recorded,and the maternal adverse reaction was compared between the two groups.Results After the treatment,the shivering effective rate of the study group was 93.6％,and which of the control group was 95.8％,the two groups showed no significant difference (P ＞ 0.05) ; Ramsay sedation score of T1,T2 in the study group was (3.21 ± 0.73) and (3.28 ± 0.65),which were significantly higher than T0 (1.53 ± 0.51),and significantly higher than (1.84 ± 0.71) and (1.92 ± 0.63) in the control group (t =5..43,9.81,all P ＜ 0.05).The incidence rate of adverse reaction of the study group was 6.4％,which was significantly lower than 57.4％ of the control group(x2 =19.20,P ＜ 0.05).Conclusion The therapeutic effects of dexmedetomidine for cesarean shivering after spinal-epidural anesthesia is good and it has good sedative effect,and it is safe,which is worth to be promoted in clinical.

Objective:To explore the immune response in mice fed orally with the conjugated antigen of HpaA-CtxB(HCTB).Methods:A recombinant strain which could express bivalent antigen of HpaA and CtxB subunit was constructed. HpaA and CtxB gene was amplified by PCR. The DNA products of HpaA and CtxB were inserted into a prokaryotic expression vector pQE-30 respectively, and then translated into E.coli strain DH5? to express HCTB fusion protein. Its immunogenicity was analyzed by Western blot. After purification, to fed mice by oral immunization. The change of antigen-specific ASC antibody(IgG and IgA) in the mucosa of the animals were detected by ELISPOT and ELISA assay. Results:HCTB fusion gene was sequenced as 1 161 bp, the fusion protein encoded polypeptides of 387 amino acid residues.The molecular weight was 40 kD analysed by SDS-PAGE. The level of soluble expression product was about 41.67% of total cell protein. After affinity chromatography, the purity of fusion protein was above 92%.Western blot analysis confirmed that fusion protein could be specifically recognized by the serum of anti-HpaA and anti-CT. Antigen-specific ASC and antibody response in animals immunized with HCTB or HpaA was determined by ELISPOT and ELISA. The results showed that the number of sIgA and IgG-ASC increased significantly in PP and gastric mucosa, especially those of the sIgA-ASC by orally immunization with HCTB. The levels of specific antibody were also higher than those of controls. Conclusion:The results indicated that the oral immunization with HCTB induces effective mucosal immune respones and produced higher levels sIgA. The recombinant fusion protein HCTB can be used as an effective oral vaccine for prevention and treatment of infection of Hp.

ObjectiveTo explore the diagnosis and prognosis value of clinical pneumonic infection score (CPIS) in patients with stroke-associated pneumonia (SAP).MethodsOne hundred and fifty stroke patients were evaluated and analyzed by CPIS.SAP was regarded as gold standard,and sensitivity,specificity,diagnose accordance rate,positive predictive value and negative predictive value were calculated.Results Thirty-nine patients had SAP and 111 patients did not have SAP by CPIS,and the incidence of SAP was 22.0％ (33/150).CPIS diagnostic sensitivity was 84.8％ (28/33),specificity was 90.6％ ( 106/117 ),positive predictive value was 71.8％ (28/39),negative predictive value was 95.5％ (106/111),and diagnose accordance rate was 89.3％(134/150).The patients of SAP were divided into good prognosis (28 cases) and bad prognosis (5 cases),and the CPIS was significantly lower in patients of good prognosis 7 days after SAP than that in patients of bad prognosis[(4.21 ± 2.23) scores vs. (6.05 ±2.32) scores,P ＜0.05].ConclusionsNot only CPIS has higher diagnosis rate in SAP incidence,but also has good judgment to prognosis.It is worthy of clinical application.

Objective To assess the clinical significance of detecting the immune markers in idiopathic thrombocytopenic purpura (ITP). Methods The frequencies of circulating B cells secreting platelet-specific antibody, platelet-specific antibody, the percentage of T lymphocyte subsets, the percentage of reticulated platelet and the level of thrombopoietin in 64 ITP patients and 31 healthy controls were measured with enzyme-linked immunospot assay (ELISPOT),modified monoclonal antibody immunobilization of platelet antigens assay (MAIPA), flow cytometry and sandwich enzyme-linked innnunosorbent assay respectively. Results Compared with the controls[1.3 ± 0. 5/105 peripheral blood mononuclear cell (PBMC), (0.33±0.06,0.41±0.03), (22.08±4.54)% and (8.19±2.46)%], the frequencies of circulating B cells secreting platelet-specific antibody (7.6±4.6/105 PBMC in acute ITP group, 5.3±3.0/105 PBMC in chronic ITP group), platelet-specific antibody (including the anti-GP Ⅱ b/Ⅲa antibody, anti-GP Ⅰ b/X antibody) (0.51 ±0.11, 0.48±0.06 in acute ITP group; 0.49±0.10,0.46±0.09 in chronic ITP group), the percentage of CD8+ T Lymphocyte (27.09±9.86 ) %, the percentage of reticulated platelet in ITP patients[the megakaryocyte cytosis group (24. 85 ± 19. 18)%, the normal megakaryocyte group (23.89±18.90)%]were significantly increased ( all P＜0.05).The frequencies of circulating B cells secreting platelet-specific antibody in acute ITP patients were notably increased (P＜0.05) compared to the chronic ITP patients. In T lymphocyte subsets, the percentage of CD3+T lymphocyte and CD4+ T lymphocyte and the ratio of CD4+/CD8+ in the patients with ITP[(60.88±14.59)%, (28.41±10.55)%, 1.18±0.59]were notably decreased than those in the healthy controls [(69.89±6.43)%, (35.38±5.05) %, 1.64±0.29, P＜0.05]. There was no apparent difference of the level of thrombopoietin between ITP patients with megakaryocyte cytosis (72. 09 ± 41.64 ) and health controls (75.37± 26. 32, P ＞ 0. 05 ), however, the level of thrombopoietin of ITP patients with normal megakaryocyte apparently increased (118.60±70.72, P＜0.05). Conclusion Detecting the frequencies of circulating B cells secreting platelet-specific antibody, platelet-specific antibody, the percentage of T lymphocyte subsets, the percentage of reticulated platelet and the level of thrombepoietin in the patients with ITP may improve the diagnosis and guide clinical therapy.

METHODS Totally 100 cases of preoperative patients needed for indwelling urethral catheters were randomly grouped, 50 of 100 cases as test group, and the others 50 cases as the control group. AIM To investigate whether ATP sensitive pota ss ium channels are involved in the protective effects of ischemic preconditioning on spinal cord in rabbits. METHODS Twenty seven male New Zealands white rabbits were randomly assigned to 3 groups (9 in each group):ischemia gro up(IC)、ischemic preconditioning group (IPC) and glibenclamide + ischemic precon ditioning group(G+I). In IC group, spinal cord ischemia was induced by an infrar enal aorta clamping for 20 min; IPC group underwent a 6 min ischemic preconditio ning followed by 30 min of reperfusion before the 20 min clamping; G+I group wer e administered glibenclamide (an ATP sensitive potassium channel blocker, 2 mg? kg -1 )intravenously 20 min before the ischemic preconditioning. Objective To explore the relationship between the Chinese Version Wechsler intelligence scale for children(C-WISC) and the Das-Naglieri cognitive assessment system(DN:CAS) in children with attention deficit hyperactivity disorder (ADHD).Methods Thirty-two children with ADHD were evaluated with C-WISC and the digital remote assessment system——DN:CAS,and both results were administered for correlation analysis.Results The results showed that there was a strong correlation between the full intelligence quotient (FIQ) of C-WISC and the full score of DN:CAS,also the scores in Plan,Simultaneous,and Successive of DN-CAS,and the correlation coefficient was between 0.6 to 0.8.There was a strong correlation between the full score of DN:CAS and the verbal comprehension IQ (VCIQ),perceptual organization IQ (POIQ),memory/concentration IQ (M/CIQ),verbal IQ (VIQ),and performance IQ(PIQ) of C-WISC,as the correlation coefficient ranges from 0.6 to 0.8.There was also a strong correlation between the score in Planning and VCIQ,POIQ,M/CIQ,VIQ of C-WISC.As to the score in Simultaneous of DN:CAS and POIQ,VIQ,PIQ of C-WISC,the correlation was also strong with the correlation coefficient ranging from 0.6 to 0.8.For the rest of the sub-tests of C-WISC and DN:CAS,there was moderate correlation between them,the correlation coefficient was from 0.4 to 0.6.Conclusions These results show that the correlation between C-WISC and DN:CAS in children with ADHD is significant.The correlation coefficient shows diversity in the sub-tests of C-WISC and DN:CAS.
