1.Experimental research of levodopa dosage and levodopa-induced dyskinesia

Objective To ohaerve the behavior of PD rat models treated with different dosages of levodopa and analyze the relation- ship between levodopa - induced dyskinesia (LID) and dosage of levedopa. Method The successful PD rat models were set up by 6-OHDA injection into right MFB. Two groups were divided according to the different treatment dosages of levodopa, which were given through perito- neal injection twice a day for 21days. One group was given for 10 mg/kg body weight, the other for 50 mg/ kg body weight. The score of AIM and rotational behavior were observed. Results Among altogether 28 female SD rats, 22 were chosen as the successful PD models and the successful rate arrived at 78. 57%. LID could be induced in both groups, but there were great differences in occurrence and duration time of AIM, AIM score, pharmacodynamic action time, and rotational behavior between these two groups( P<0.05). Conclusions The be- havior of LID has great relationship with the given dosage of levodopa.
2.Clinical and risk factors analysis of brain injury in very low birth weight infants

Objective To investigate the incidence of brain injury and analyze the risk factors in very low birth weight infants (VLBWI),in order to explore methods to improve neurodevelopmental outcome.Methods This was a retrospective study in which we reviewed the incidence of brain injury in 326 VLBWI who admitted to our NICU from October 2007 to September 2014.The differences of general characteristics,high-risk perinatal factors,complications of prematurity and respiratory support in brain injury group (n =132) and no brain injury group(n =194) were analyzed,and Logistic regression was used to analyze the related risk factors.Results In 326 VLBWI,premature brain injury was observed in 132 cases(incidence of 40.49%).Hemorrhagic brain injury was observed in 80 cases(incidence of 24.54%,80/326),in whom total 66 cases (20.25%,66/326) with periventricular-intraventricular hemorrhage (PVH-IVH),41 cases with PVH-IVH grade Ⅰ-Ⅱ,25 cases with PVH-IVH grade Ⅲ-ⅣV.Non-hemorrhagic brain injury was observed in 52 cases (incidence of 15.95 %,52/326),in whom 46 cases with periventdcular leukomalacia (14.11%,46/326).There were no statistically significant differences in general characteristics except gender differences between the two groups.Further analysis of the 28 factors including high-risk perinatal factors,premature complications and respiratory support between the two groups,showed that there were significant differences in 16 factors including asphyxia,chorioamnionitis,antenatal steroid use,neonatal respiratory distress syndrome,hypoxemia,apnea,pulmonary hemorrhage,patent ductus artedosus,hypotension,acidosis,hypocapnia,anemia,hyperglycemia,thrombocytopenia,high frequency ventilation and extrautedne growth retardation.Further binary Logistic regression analysis showed that antenatal steroids use was negatively correlated with brain injury,chorioamnionitis,acidosis,and patent ductus arteriosus were positively correlated with brain damage,the relative risks of premature brain injury was 0.108,7.222,3.045 and 4.565 respectively.Conclusion The mortality rate and the incidence of brain injury in VLBWI are still high.Chorioamnionitis,acidosis and patent ductus arteriosus are closely associated with premature brain damage.Antenatal steroids can prevent premature brain injury.We should pay much attention to these risk factors in the perinatal management of VLBWI,in order to decrease the incidence of premature brain injury.
3.The dual-source CT analysis of vascular lesions causing abdominal pain

Objective With the CT analysis of vascular lesions which caused abdominal pain,to improve the radiologist’s understanding of this disease,and to reduce the rate of misdiagnosis.Methods The CT data of 69 cases with vascular lesions which caused abdominal pain were analysis retrospective.Results In the vascular lesions causing abdominal pain,the occlusions of the superior mesenteric artery and vein were the most common,a total of 30 patients (43%),and the most common cause of occlusion was thrombosis (27%);There were the abdominal aorta dissection (Ⅰ+Ⅲ type+limitation)in 1 5 cases,abdominal aorta and/or iliac artery vein multiple ulcers in 9 cases (13%);Type B intramural hematoma in 6 cases (8%);Simply superior mesenteric artery dissection in 5 cases;Other less common splenic arterial thrombotic occlusion in 2 cases,pure splenic artery occlusion in 1 case and renal artery dissection in 1 case.7 cases were missed lesions,the main reason for radiologists to focus placed in the abdominal cavity prone to abdominal pain caused by solid organ or cavity organ,while ignoring the observation in the scanning range of the abdominal aorta and its main branches of the artery,lack of knowledge and experience of the lesions,and therefore not to see the consciousness or habits.Conclusion Vascular causes is an important reason causing abdominal pain.In patients with abdominal pain who underwent CT examination,if the cause of the gut and other abdominal organs can be exclused,the possibility of vascular etiology should be considered,and we should carefully observe intraabdominal artery and vein to exclude the possibility of this disease.
4.Combined examination of 64-slice spiral computed tomography and inflammatory cytokines in the detection of vulnerable plaque of acute coronary syndrome

Objective To evaluate the clinical value of 64-slice spiral computed tomography combined with hs-CRP and MMP-9 to detect the vulnerable plaque of acute coronary syndrome.Methods Seventy-two patients with coronary heart disease were included and among them,43 patients were diagnosed as ACS and the other 29 patients diagnosed as stable angina group(SAP).All patients underwent 64-slice spiral computed tomography coronary artery imaging and examination of plasma hs-CRP and MMP-9 concentration.All patients underwent selective coronary angiography.Another 25 cases who underwent coronary angiography were included in the control group.Results The incidence of soft plaque was higher in the ACS group than in the SAP group(24 cases vs 4 cases,P=0.001).Compared with the control group,levels of hs-CRP and MMP-9 in the ACS group and the SAP group were both significantly higher(all P
5.Establishment of a novel mutant-enriched liquid chip technology for detecting EGFR mutations in plasma of patients with non-small cell lung cancer

Objective To establish a sensitive,specific,simple and high-throughput method for detection of the epidermal growth factor receptor (EGFR) mutation in the plasma samples of patients with non-small cell lung cancer (NSCLS) by the use of mutant-enriched liquid chip (MEL) assay.Methods The specific probes for the EGFR exon19 E746-750 deletion,exon 21 L858R mutation and wild-type sequence were designed and coupled to the microspheres coding with different fluorescent dye.The probe coupling efficiency was verified by crossing hybridization test with biotin-labeled reverse sequence.A blood-based MEL approach which integrates a sensitive mutant-enriched PCR and quantitative high throughput liquid chip assay for assessment of EGFR mutations was developed.The sensitivity and specificity of MEL was further evaluated using the mixture with different copy numbers of mutant and wild-type plasmids as template.The mutations of exon 19 and 21 of EGFR gene in plasma samples from 201 patients with stage ⅢB or Ⅳ NSCLC who enrolled in the First Affiliated Hospital of Guangzhou Medical College from September 2008 to April 2010 were analyzed by both the MEL and the mutant-enriched PCR assay.The result comparison was made between direct sequencing and MEL in 50 cases whose EGFR gene type had been tested by MEL and mutantenriched PCR.The correlation of EGFR gene mutation and the response to the Geftinib treatment was analyzed in 16 patients with lung adenocarcinoma as well.Results The probes were successfully coupled to the microspheres encoding with different fluorescent dye,and could be specifically recognized by the corresponding target sequence.The MEL was capable of detecting as few as 10 copies of EGFR mutants (sensitivity was 0.1%).Among the enrolled 201 cases of advanced NSCLC,the detection rate of the EGFR exon19 E746-750 del and exon 21 L858R was 55.7% (112/201) by MEL assay.Conpared with mutantenriched PCR[58.2% (117/201)],the coincidence rate was 97.5% (196/201).There was no statistically significant difference between the results of mutant-enriched PCR and MEL (x2 =3.20,P > 0.05).The mutations detection rate was 22.0% (11/50) by directing sequencing,which was significantly lower than by MEL[50.0% (25/50),x2 =12.07,P < 0.05].Among the 16 patients treated with Gefitinib,9 cases who had EGFR mutation showed a higher response rate(P =0.041)and prolonged progression-free survival (x2 =6.76,P =0.009) after the treatment compared to those 7 who without EGFR mutation.Conclusions A new method of MEL with accuracy,specificity,fast and high-throughput is established for the detection of EGFR 19 E746-750 deletion and exon 21 L858R mutations in plasma from advanced NSCLC patients.It has the ability to provide the most direct and valuable guidance for clinicians to make decision on EGFR tyrosine kinase inhibitors therapy in the advanced NSCLC paticnts.
6.Study on the obtaining situation of osteoporotic fractures related knowledge and influencing factors of the nurses

Objective To know the situation of nurses obtaining osteoporotic fractures related knowledge ,and provid guiding basis for further education and relevant training on target people .Methods Questionnaire survey and semi -structured interviews methods were used to investigate 595 nurses ,9 clinical nurse workers and nurse managers who come from a tertiary comprehensive teaching hospital were interviewed personally .Results The scores of the related knowledge in nurses were 35 .70 ± 6 .36 ,score rate was 67 .36% ;Interview data analysis showed that lack of professional training ,training was not systematic ,the approach obtain knowledge was simple were result in lack of the related knowledge in nurses ;By the multiple liner stepwise regression ,The educa‐tion level and professional title were main factors which affect nurses acquired osteoporosis knowledge .Conclusion It is necessary to focus on improving the strengthen of related knowledge education for the nurses with low education background and low profes‐sional title ,At the same time ,related departments of the hospital should strengthen the training of professional knowledge .
7.Effects of valproic acid on rats with experimental autoimmune encephalomyelitis and its immuno-modulatory mechanism

Objective To investigate the effects of valproic acid ( VPA ) on SD rats with experimental autoimmune encephalomyelitis ( EAE) and its possible immunomodulatory mechanism .Meth-ods Fifty female Sprague-Dawley ( SD) rats were randomly divided into five groups by random digit table , including control group (n=10), EAE group(n=10), low dose VPA treated group (100 mg/kg, n=10), median dose VPA treated group (300 mg/kg, n=10) and high dose VPA treated group (600 mg/kg, n=10).The SD rat model of EAE was induced by immunizing with a guinea pigs′spinal cord homogenate (GPSCH).Normal saline and various doses of VPA were given to rats in according groups twice a day from day 0 to day 19 ( close to the peak stage of EAE ) .The severity of EAE was scored according to the signs and symptoms.Pathological changes were observed through Hematoxylin-Eosin staining, and then the degrees of inflammatory infiltration were evaluated .The numbers of activated neuroglia that expressed Iba-1 in cerebral and lumber cords were counted by immunohistochemistry .The expression of IFN-γ, IL-17 and IL-10 in cer-ebral and lumber cords were measured by ELISA .Results Compared with EAE group , rats in the low, me-dian and high dose VPA treated groups had lower incidence of EAE and prolonged latency , but only the me-dian dose treated group showed significant alleviation in clinical symptoms (P<0.05).Both the median and the high dose treated group showed decreased inflammatory cell infiltration in CNS (P<0.05).Immunohisto-chemistry results showed that the numbers of activated microglia were significantly inhibited in rats treated with median and high dose of VPA in comparison with those in EAE group (P<0.05).Results of ELISA demonstrated that the expression of IFN-γand IL-17 in both median and high dose VPA treated groups were significantly decreased compared with those in EAE group (P<0.05), but only the median dose treated group showed a remarkably increased expression of IL-10 (P<0.05).Conclusion VPA, especially medi-um dose of VPA ( 300 mg/kg ) , had neuroprotective effects on rats with EAE .The possible mechanism might be associated with the inhibited activation of microglia and the increased percentage of anti -inflammato-ry cytokines .
8.Correlation between the uncertainty in illnessand social support in elderly patients with prediabetes

Objective To investigate the level of uncertainty of illness and social support state in patients with prediabetes,and to discuss the correlation between the two.Methods 243 cases with prediabetes were analyzed by Uncertainty in Illness Scale and Social Support Rating Scale.Results There was a middle uncertainty in the elderly patients with prediabetes.The objective support,sabjective support,social support utilization degree and social support score were negatively related to the complexity of uncertainty(r--0.419,-0.433,-0.390 and-0.421,respectively,all P< 0.05).Conclusions Medical staff should evaluate the uncertainty in illness in elderly patients with prediabetes and conduct nursing intervention accordingly,in order to reduce the uncertainty,increase social support,and ultimately improve the quality of life.
9.Effects of antidepressants on osteoporosis related protein OPG, RANKL and RANK in ovariectomized rat model with depressive disorder

Objective To investigate the effects of antidepressant therapy on whole regulating function of OPG/RANKL/RANK system in ovariectomized (OVX) rat model with depressive disorder.Methods Forty sexually mature female SD rats were used as subjects.The rats were randomly divided into 8 groups as following:sham group(Sn),depression group(Dn),the OVX group (On),the OVX with depression group(ODn),OVX with depression +sertraline group (ODs),and OVX with depression + citalopram (ODx),OVX with depression + reboxetine group(ODr),OVX with depression + venlafaxine group (ODw).Bilateral oophorectomy was applied to establish the ovariectomized rats model.Chronic unpredictable mild stress(CUMS) was used to estalish depression model.Open-field test and sugar consumption experiment were applied before the experiment and the 28th day of the experiment.Immunohistochemistry and Quantitative Real-time PCR were used to observe the expression of OPG,RANKL and RANK.Results After chronic unpredictable mild stress the scores of horizontal motion,vertical motion and percentage of sugar consumption experiment in Dn group were decreased contrast to the in control Sn group (P<0.05)and the scores in ODn group were decreased in contrast to the On group (P<0.05).The protein and gene expression of RANK in Dn group and On group were increased in contrast to the Sn group (P<0.05).The protein and gene expression of RANKL in Dn group and On group were increased in contrast to the Sn group (P<0.05).The gene expression of OPG in ODs group(5.94±0.79),ODw group(5.99± 1.08) and ODr group(6.59±0.69) was decreased in contrast to the ODn group (4.96±0.51) (P<0.05).The gene expression of RANK in ODs group (2.82±0.23),ODx group(3.03±0.04) was increased in contrast to the ODn group(3.44±0.18) (P<0.05).The gene expression of RANKL in ODs group(4.82±0.94),ODx group(5.00±0.82) and ODw group(5.23±0.88)was decreased in contrast to the ODn group (3.69± 0.69) (P<0.05).Conclusion The expression of RANK and RANKL in femoral neck is increased both in depressive disorder model rats and ovariectomized model rats.The antidepressant can up-regulate the expression of RANK,and down-regulate the expression of OPG and RANKL in the OVX with depression group.Of the effect,sertraline is the strongest,however reboxetine mesylate is the poorest.
10.Nursing about 131 children with closed abdominal injury

Objective To explore and observe the nursing of the children with the closed abdominal injury.Methods Using a retrospective analysis in 131 children with closed abdominal injury in our hospital from January 1993 to January 2005,sumarize the nursing points Results 109 got nonoperative treatment and the rest had an operation.No complications and death had happened in the duration of hospital stay.All 131 patients were full recovered.Conclusion The points of nursing the children with the closed abdominal injury are to developed observe continuously,to make sure whether the injury was combined with other damages of the organs or out of the abdominal cavity.Furthermore,use the indication of nonoperative treatment and operation therapy into the attending process,which can provide effective information and index to the doctor,and then make doctor use corresponding methods to gain satisfactory results.
