1.Role of Toll-like receptor 4 in mitigation of inflammatory responses following myocardial ischemia-reperfusion injury by sufentanil post-conditioning in rats

Objective To evaluate the role of Toll-like receptor 4 (TLR4) in mitigation of inflammatory responses following myocardial ischemia-reperfusion (I/R) injury by sufentanil postconditioning in rats.Methods Thirty Wistar rats were randomly divided into 3 groups (n =10 each):sham operation group (S group); I/R group; sufentanil postconditioning group (SP group).Myocardial I/R was produced by temporary ligation of left anterior descending branch of coronary artery for 30 min followed by 120 min reperfusion.In group SP,sufentanil 0.6 μg/kg was injected intravenously at 5 min before reperfusion.The myocardial specimens and blood samples were taken at the end of 120 min reperfusion for determination of the interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) concentrations and TLR4 content (by ELISA).Results Compared with S group,the levels of TLR4,IL-6 and TNF-α were significantly increased in the other two groups.Comparedwith I/R group,the levels of TLR4,IL-6 and TNF-α were significantly decreased in group SP.Conclusion The mechanism by which sufentanil postconditioning mitigates inflammatory responses following myocardial I/R injury may be related to downregulation of TLR4 expression in myocardial tissues of rats.
2.Observation on the Effect of Acupoint Injection plus Massage Therapy on Primary Dysmenorrhea

Objective: To observe the clinical effects of acupoint injection therapy plus massage on primary dysmenorrhea (PD).
Methods: Ninety patients with PD were randomly divided into a treatment group or a control group, 45 cases in each group. The treatment group was treated by injection of Vitamin K3into Sanyinjiao (SP 6) plus massage on Diji (SP 8). The control group was treated by oral administration of Ibuprofen sustained-release capsule. Before and after the treatment, visual analogue scale (VAS) was adopted to assess pain degree of the patients. The therapeutic effects were observed after continuous treatment of three cycles of menstruation.
Results: After treatment, VAS scores were obviously decreased in both groups and the differences were statistically significant (allP<0.05). The difference in VAS score after treatment was more significantin the treatment group than that in the control group (P<0.01). The total effective rate was significantl higher in the treatment group than that in the control group (P<0.05).
Conclusion: Acupoint injection therapy plus massage for PD is effective and better than simple oral administration of Ibuprofen sustained-release capsule.
3.Correlation between the uncertainty in illnessand social support in elderly patients with prediabetes

Objective To investigate the level of uncertainty of illness and social support state in patients with prediabetes,and to discuss the correlation between the two.Methods 243 cases with prediabetes were analyzed by Uncertainty in Illness Scale and Social Support Rating Scale.Results There was a middle uncertainty in the elderly patients with prediabetes.The objective support,sabjective support,social support utilization degree and social support score were negatively related to the complexity of uncertainty(r--0.419,-0.433,-0.390 and-0.421,respectively,all P< 0.05).Conclusions Medical staff should evaluate the uncertainty in illness in elderly patients with prediabetes and conduct nursing intervention accordingly,in order to reduce the uncertainty,increase social support,and ultimately improve the quality of life.

AIM To study the effects of PDB on blood glucose. METHODS 3 groups of health mice and 3 groups of model mice were administered with high, mean and low doses of PDB compatibility groups (combined with glibenclamide 0.66 mg?kg -1) respectively for 15 days. 5.Impact of invasive treatment strategy on health-related quality of life six months after non-ST-elevation acute coronary syndrome

BackgroundFew studies have compared change in the health-related quality of life (HRQL) following treatment of non-ST-elevation acute coronary syndrome (NSTE-ACS) with either percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). This study is tocompare changes in HRQL six months after hospital discharge between NSTE-ACS pa-tients who underwent either PCI or CABG.Methods HRQL was assessed using the Seattle angina questionnaire at admission and six months after discharge in 1012 consecutive patients with NSTE-ACS. To assess associations of PCI and CABG with HRQL changes, logistic regression models were constructed treating changes in the score of each dimension of the Seattle angina question-naire as dependent variables.Results Although both the PCI and CABG groups experienced angina relief and other improvements at 6-month follow-up (P<0.001), the CABG relative to PCI group showed more significant improvements in angina frequency (P= 0.044) and quality of life (P= 0.028). In multivariable logistic analysis, CABG also was an independent predictor for both im-provement of angina frequency (OR: 1.62, 95%CI: 1.09-4.63,P= 0.042) and quality of life (OR: 2.04, 95%CI: 1.26-6.92,P= 0.038) relative to PCI.Conclusions In patients with NSTE-ACS, both PCI and CABG provide great improvement in disease-specific health status at six months, with that of CABG being more prominent in terms of angina frequency and quality of life.
6.Arterial stiffness and associated factors in non-diabetic pre-dialysis patients with chronic kidney disease

Objective To study the arterial stiffness in non-diabetic pre-dialysis chronic kidney disease (CKD) patients and to explore the associated factors. Methods Automatic pulse wave velocity (PWV) measuring system was used to examine carotid-femoral pulse wave velocity (CFPWV) as the parameters reflecting central elastic large arterial elasticity. Vascular calcification was quantitatively evaluated by plain radiographic film of abdomen, pelvis and hands. Blood pressure, biochemical parameters and intact parathyroid hormone (iPTH) were routinely detected. Stepwise multiple linear regression analysis was used to assess the associated factors of arterial stiffness. Results Ninety-six non-diabetic pre-dialysis CKD patients and 30 healthy people were enrolled in this trial. CFPWV in stage 3, 4 and 5 CKD patients was significantly higher than that in healthy controls [(11.63±2.39) m/s, (11.70±2.80) m/s, (12.88±2.49) m/s vs (9.70±1.66)m/s , all P<0.05]. Stepwise multiple regression analysis demonstrated that age, mean arterial pressure, vascular calcification and iPTH were independent impact factors of CFPWV. Conclusions Arterial stiffness of large artery increases in non-diabetic pre-dialysis CKD patients. Age, mean arterial pressure, vascular calcification and iPTH are independent impact factors of CFPWV.
7.Relationship study on serum lipids in severe heart failure patients

ObjectiveTo investigate the effect of the levels of serum lipid on the prognosis of patients with severe heart failure.MethodsThe clinical data of 149 patients with severe heart failure was analyzed retrospectively,the levels of serun lipid were compared among different causes and different NYHA cardiac functional grading patients.And the prognosis was compared between low-serum lipid and non-low-serum lipid patients.Results There were significant differences in the levels of total cholesterol(TC),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),albumin (ALB) among different NYHA cardiac functional grading (P <0.05),but there was no significant difference in triacylglycerol (TG) among different NYHA cardiac functional grading(P > 0.05 ).The levels of TC,HDL-C,LDL-C in dilated cardiomyopathy and rheumatic heart disease were lower than those in coronary heart disease combined with hypertension and ischemic cardiomyopathy,there were significant differences (P <0.05).Followed up for 2 years,the mortality,deteriorated frequency,hospitalization frequency more than 2 times a year and hospitalization time more than 1 month in low-serum lipid patients [29.0% (9/31 ),25.8% ( 8/31 ),48.4% ( 15/31 ),61.3% ( 19/31)]were higher than those in non-low-serum lipid patients [9.3 % ( 11/118 ),9.3% ( 11/118 ),17.8% (21/118 ),24.6% (29/118 )],and there were significant differences (P < 0.01 or <0.05).Conclusions For patients with severe heart failure,NYHA cardiac functional grading has significantly negative correlation with their serum lipid levels.Lower serum lipid levels will seriously affect the prognosis of patients.
8.Role of JAK2-STAT3 pathway in reduction of myocardial ischemia-reperfusion injury by sufentanil postconditioning in dogs

Objective To investigate the role of Janus kinese 2-signal transducer and activator of transcription 3 (JAK2-STAT3) pathway in reduction of myocardial ischemia-reperfusion (I/R) injury by sufentanil postconditioning in dogs.Methods Twenty-four healthy dogs of either sex,weighing 10-15 kg,were randomly divided into 4 groups (n =6 each):sham operation group (group S); I/R group; sufentanil postconditioning group (group PO) and sufentanil postconditioning + specific JAK2 inhibitor AG490 group (group AG).In groups I/R,PO and AG,myocardial I/R was produced by occlusion of left anterior descending coronary artery for 30 min followed by 120 min reperfusion.In groups PO and AG,sufentanil 0.6 μg/kg was infused intravenously over 5 min before reperfusion and in addition in group AG,AG490 1 mg/kg was injected intravenously before sufentanil infusion.Myocardial specimens were taken at the end of 120 min reperfusion for microscopic examination and determination of the expression of caspase-3 and p-STAT3 by immuno-histochemistry and myocardial cell apoptosis index (AI) by TUNEL.Results AI and the expression of caspase-3 and p-STAT3 were significantly higher in groups I/R,PO and AG than in group S ( P < 0.05).Compared with group I/R,AI and the expression of caspase-3 were significantly decreased in groups PO and AG,the expression of p-STAT3 was significantly increased in group PO,and the expression of p-STAT3 was significantly decreased in group AG ( P < 0.05).AI and the expression of caspase-3 were significantly higher and the expression of p-STAT3 was significantly lower in group AG than in group PO (P < 0.05).The pathologic changes were significantly attenuated in group PO compared with groups I/R and AG.Conclusion JAK2-STAT3 pathway is involved in reduction of myocardial I/R injury by sufentanil postconditioning in dogs.
9.Study on recombinant human brain natriuretic peptide in the treatment of heart failure patients with coronary heart disease

Objective To explore the efficacy of recombinant human brain natriuretic peptide (rhBNP) in the treatment of heart failure patients with coronary heart disease (CHD).Methods Ninety-six CHD patients with heart failure were divided into observation group (48 cases) and control group (48 cases) by random digits table,and rhBNP was used in observation group and nitroglycerin was used in control group.The efficacy,left ventricular ejection fraction (LVEF) and urine output in two groups were observed.Results The total effective rate in observation group was 93.75% (45/48),and 81.25% (39/48) in control group,there was significant difference between two groups (P < 0.05).LVEF and urine output after treatment were better than those before treatment [observation group:(57.7 ± 3.9)% vs. (32.5 ± 3.1 )%,(70.5 ± 31.1 ) ml/h vs.(34.5 ± 13.7) ml/h;control group:(52.2 ± 3.3)% vs.(33.4 ± 2.9)%,(53.6 ± 23.4) ml/h vs.(33.8 ± 13.4)ml/h,P < 0.05 ].After treatment,LVEF and urine output in observation group were higher than those in control group (P < 0.05 ).Incidence of adverse reaction in observation group was lower than that in control group [ 18.75% (9/48) vs.35.42% ( 17/48),P < 0.05 ].Conclusion rhBNP has curative effect and small adverse reactions on CHD patients with heart failure,and can significantly improve the patients' general condition.
