1.Effect of governor vessel moxibustion on cardiopulmonary fitness in stable chronic obstructive pulmonary disease with qi deficiency of lung and kidney.

OBJECTIVE:
To observe the effect of governor vessel moxibustion on improving cardiopulmonary fitness in patients with stable chronic obstructive pulmonary disease (COPD) of qi deficiency of lung and kidney.
METHODS:
A total of 40 patients with stable COPD of qi deficiency of lung and kidney were randomized into an observation group (20 cases) and a control group (20 cases). The routine basic treatment and nursing were adopted in the control group. In the observation group, on the basis of the treatment in the control group, governor vessel moxibustion was applied at the area from Dazhui (GV14) to Yaoshu (GV2) of the governor vessel, 1 hour a time, once a week. Both groups were treated for 8 weeks consecutively. The maximal oxygen uptake (VO2max), 6-minute walking test (6MWT) and Borg score, pulmonary function indexes (forced expiratory volume in 1 second [FEV1], forced vital capacity [FVC] and ratio of FEV1 and FVC [FEV1/FVC]), chronic obstructive pulmonary disease assessment test (CAT) score, and TCM syndrome score were observed in the two groups before and after treatment respectively.
RESULTS:
After treatment, the VO2max, 6MWT, FEV1, FVC and FEV1/FVC were increased compared with those before treatment in the two groups (P<0.001, P<0.05), the above indexes in the observation group were higher than those in the control group (P<0.05, P<0.01). After treatment, the Borg, CAT and TCM syndrome scores were decreased compared with those before treatment in the two groups (P<0.05, P<0.001), the above indexes in the observation group were lower than those in the control group (P<0.05).
CONCLUSION
Governor vessel moxibustion can effectively improve the cardiopulmonary fitness, clinical symptoms and the quality of life in patients with stable COPD of qi deficiency of lung and kidney.

2.Development and application of intensive care unit digital intelligence multimodal shift handover system.

OBJECTIVE:
To develop a digital intelligent multimodal shift handover system for the intensive care unit (ICU) and evaluate its application effect in ICU shift handovers.
METHODS:
A research and development team was established, consisting of 1 department director, 1 head nurse, 3 information technology engineers, 3 nurses, and 2 doctors. Team members were assigned responsibilities including overall coordination and planning, platform design and maintenance, pre-application training, collection and organization of clinical feedback, and research investigation respectively. A digital intelligent multimodal shift handover system was developed for ICU based on the Shannon-Weaver linear transmission model. This innovative system integrated automated data collection, intelligent dynamic monitoring, multidimensional condition analysis and visual reporting functions. A cloud platform was used to gather data from multi-parameter vital signs monitors, infusion pumps, ventilators and other devices. Artificial intelligence algorithms were employed to standardize and analyze the data, providing personalized recommendations for healthcare professionals. A self-controlled before-after method was adopted. Before the application of the ICU digital intelligent multimodal shift handover system (from December 2023 to March 2024), the traditional verbal bedside handover was used; from June 2024 to March 2025, the ICU digital intelligent multimodal shift handover system was applied for shift handovers. Questionnaires before the application of the shift handover system were collected in April 2024, and those after the application were collected in April 2025. The shift handover time, handover quality (scored by the nursing handover evaluation scale), satisfaction with doctor-nurse communication (scored by the ICU doctor-nurse scale) before and after the application of the handover system were compared, and nurses' satisfaction with the shift handover system (scored by the clinical nursing information system effectiveness evaluation scale) was investigated.
RESULTS:
After the application of the ICU digital intelligent multimodal shift handover system, the shift handover time was significantly shorter than that before the application [minutes: 20 (15, 25) vs. 30 (22, 40)], the handover quality was significantly higher than that before the application [score: 84.0 (78.0, 88.5) vs. 71.0 (55.0, 79.0)], and the satisfaction with doctor-nurse communication was also significantly higher than that before the application (score: 84.58±6.79 vs. 74.50±11.30). All differences were statistically significant (all P < 0.05). In addition, the nurses' system effectiveness evaluation scale score was 102.30±10.56, which indicated that nurses had a very high level of satisfaction with the ICU digital intelligent multimodal shift handover system.
CONCLUSIONS
The application of the ICU digital intelligent multimodal shift handover system can shorten the shift handover time, improve the handover quality, and enhance the satisfaction with doctor-nurse communication. Nurses have a high level of satisfaction with this system.

3.Establishment and application of measurement range of main blood quality indicators in provincial blood stations

Objective To obtain the monitoring measurement range of quality indicators of red blood cells,plasma and derivatives and leukocyte-reduced apheresis platelets provided by blood stations in Hebei province,explore the distribution of monitoring values and the change of monitoring level,so as to further strengthen the homogenization construction of quality control laboratories in blood stations in Hebei.Methods In 2023,the sampling data of 12 blood stations in Hebei from 2015 to 2022 were collected,scatter plots were made and the range markers were set,and the"mean±SD"line was taken as the upper limit and lower limit of the measurement range.In 2024,the monitoring values in 2023 were added,and the changes of two measurement ranges were compared to analyze the stability and overall level.Results Comparison of the measurement range from 2015 to 2022 and the measurement range from 2015 to 2023 showed that the standard deviation of the content of deleukocyte suspension of red blood cells-hemoglobin,washed erythrocyte-hemoglobin,washed erythrocyte-su-pernatant protein,cryoprecipitate coagulation factor-FⅧ,fresh frozen plasma-FⅧ,leukocyte-reduced apheresis platelets-leukocyte residue and leukocyte-reduced apheresis platelet-red blood cell concentration decreased from 8.132 to 7.993,6.252 to 6.104,0.273 to 0.267,57.506 to 56.276,0.920 to 0.892,0.653 to 0.644 and 2.653 to 2.603,respectively.The narrowing of the standard deviation range of the above items led to more concentrated monitoring values and reduced disper-sion.Comparison of the measurement range from 2015 to 2022 and the measurement range from 2015 to 2023 showed that the mean value of leukocyte residue of the deleukocyte suspension of red blood cells,hemoglobin content of the wash eryth-rocyte,protein content of supernatant of the wash erythrocyte,hemolysis rate of the wash erythrocyte,FⅧ content of the cryoprecipitate coagulation factor,plasma protein content of the fresh frozen plasma,FⅧ content of the fresh frozen plasma,platelet content of the leukocyte-reduced apheresis platelets changed from 0.362 to 0.476,44.915 to 44.861,0.280 to 0.283,0.137 to 0.142,133.989 to 133.271,60.262 to 60.208,1.301 to 1.277 and 3.036 to 3.033,respectively,and was closer to the national standard line,which reflects an increase in the number of unqualified monitoring values or values close to the national standard line in 2023.The long-term qualified rate of coagulation items was low,and no improvement has been ob-served.The stability of biochemical items has been enhanced but overall deviation has occurred,with the average value close to the national standard line.The possibility of subsequent testing failure has increased.The counting items showed no obvi-ous common characteristics.Conclusion The use of"mean±SD"in the analysis can visually display the distribution of mo-nitoring values of different items in Hebei,forming an indicator measurement range covering the past nine years.It shows the characteristics of each item,and provides reference for subsequent quality control laboratory data analysis of each blood sta-tions to takes active measures to improve the monitoring level.
4.Clinical advantages of nebivolol in the treatment of hypertension

β-blocker is one of the commonly used anti-hypertensive drugs, and there are obvious differences in the selection of this class of drugs. Nebivolol is a third-generation β-blocker with a unique mechanism of action. This article summarizes the clinical application of nebivolol in anti-hypertensive treatment in recent years, and it is found that compared with other β-blockers, nebivolol has certain clinical treatment advantages. In addition to having a significant antihypertensive effect, it also has little impact on sexual function and heart rate of patients, and does not affect the blood glucose and lipid metabolism, so the drug is more suitable for some special groups of patients, including sexually active male hypertensive patients, hypertensive patients with complications such as type 2 diabetes mellitus and metabolic syndrome.
5.External quality assessment of laboratories in blood stations of Hebei Province in 2022: a retrospective analysis of HeBEQA project

【Objective】 To conduct the laboratory quality assessment between 12 blood stations in Hebei province, analyze the results and explore the accuracy and comparability of testing, so as to improve the level of testing ability and quality management. 【Methods】 With reference to the external quality assessment rules of National Center for Clinical Laboratories and combined with the instructions of quality assessment samples, daily testing process of the laboratories were assessed. The quality indicators include blood cell count (WBC, RBC, Hb, HCT, MCV, MCH, MCHC and PLT), biochemical items (TP) and coagulation parameters (FIB and FⅧ). 【Results】 There are still problems in laboratories in terms of personnel operation, instrument maintenance and the impact of different reagent batches, especially in biochemical items and coagulation parameters. The pass rate of biochemical items was the lowest, only 72.75%, and that of blood cell count was the highest, reaching 98.75%. 【Conclusion】 With the progress of the project, the quality monitoring level of daily blood sampling tests in the quality control laboratory of each blood station has been improved. However, it is still necessary for each laboratory to improve the testing ability and quality management to a higher level in Hebei.

Objective:To analyze the risk factors of bronchopulmonary dysplasia(BPD)in very preterm infants(VPI), and to provide scientific basis for the prevention and treatment of BPD in VPI.Methods:A prospective multicenter study was designed to collect the clinical data of VPI in department of neonatology of 28 hospitals in 7 regions from September 2019 to December 2020.According to the continuous oxygen dependence at 28 days after birth, VPI were divided into non BPD group and BPD group, and the risk factors of BPD in VPI were analyzed.Results:A total of 2 514 cases of VPI including 1 364 cases without BPD and 1 150 cases with BPD were enrolled.The incidence of BPD was 45.7%.The smaller the gestational age and weight, the higher the incidence of BPD( P<0.001). Compared with non BPD group, the average birth age, weight and cesarean section rate in BPD group were lower, and the incidence of male infants, small for gestational age and 5-minute apgar score≤7 were higher( P<0.01). In BPD group, the incidences of neonatal respiratory distress syndrome(NRDS), hemodynamically significant patent ductus arteriosus, retinopathy of prematurity, feeding intolerance, extrauterine growth restriction, grade Ⅲ~Ⅳ intracranial hemorrhage, anemia, early-onset and late-onset sepsis, nosocomial infection, parenteral nutrition-associated cholestasis were higher( P<0.05), the use of pulmonary surfactant(PS), postnatal hormone exposure, anemia and blood transfusion were also higher, and the time of invasive and non-invasive mechanical ventilation, oxygen use and total hospital stay were longer( P<0.001). The time of starting enteral nutrition, cumulative fasting days, days of reaching total enteral nutrition, days of continuous parenteral nutrition, days of reaching 110 kcal/(kg·d) total calorie, days of reaching 110 kcal/(kg·d) oral calorie were longer and the breastfeeding rate was lower in BPD group than those in non BPD group( P<0.001). The cumulative doses of amino acid and fat emulsion during the first week of hospitalization were higher in BPD group( P<0.001). Multivariate Logistic regression analysis showed that NRDS, invasive mechanical ventilation, age of reaching total enteral nutrition, anemia and blood transfusion were the independent risk factors for BPD in VPI, and older gestational age was the protective factor for BPD. Conclusion:Strengthening perinatal management, avoiding premature delivery and severe NRDS, shortening the time of invasive mechanical ventilation, paying attention to enteral nutrition management, reaching whole intestinal feeding as soon as possible, and strictly mastering the indications of blood transfusion are very important to reduce the incidence of BPD in VPI.
7.Effects of mobile health management model in patients with acute myocardial infarction after PCI under the background of "Internet +"

Objective:To explore the effect of mobile health management model in patients with acute myocardial infarction after percutaneous coronary intervention (PCI) under the background of "Internet +".Methods:From July 2018 to March 2020, convenience sampling was used to select 124 patients with acute myocardial infarction who underwent PCI successfully in the First Affiliated Hospital of Zhengzhou University as the research object. According to the random number table method, the patients were divided into the observation group and the control group, 62 cases in each group. The control group received routine nursing after surgery, and the observation group was given the mobile health management model under the background of "Internet +" on the basis of the control group. Self-management Scale for Patients Undergone Coronary Artery Stent Implantation, Hamilton Depression Scale (HAMD) , Hamilton Anxiety Scale (HAMA) , and Nursing Satisfaction Questionnaire were used to evaluate the effect of the intervention.Results:After six months of intervention, the scores of each dimension of the Self-management Scale for Patients Undergone Coronary Artery Stent Implantation in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . After six months of intervention, the HAMD and HAMA scores of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . After six months of intervention, the nursing satisfaction of the observation group was higher than that of the control group, and the difference was statistically significant ( P<0.05) . Conclusions:The mobile health management model under the background of "Internet +" is conducive to improving the self-management ability of patients with acute myocardial infarction after PCI, reducing patients' depression and anxiety, and increasing patients' satisfaction with nursing.
8.Expression of serum galectin-3, cystatin-C and endothelin-1 in elderly patients with hypertension and their correlation with left ventricular hypertrophy

Objective:To investigate the expression of serum galectin-3 (Gal-3), cystatin-C (Cys-C) and endothelin-1 (ET-1) in elderly patients with hypertension and analyze their correlation with left ventricular hypertrophy(LVH).Methods:A total of over 65 years old 110 patients with essential hypertension in Tieling Central Hospital from July 2018 to May 2019 were selected as hypertension group, according to the LVH, patients with essential hypertension were divided into (non LVH group, 66 patients) and LVH group (44 patients). At the same time, a total of 110 healthy people in the same period were selected as control group. The expressions of serum Gal-3, Cys-C and ET-1 in each group were compared, and the correlation between left ventricular mass index (LVMI) and Gal-3, Cys-C in patients with essential hypertension were analyzed.Results:The levels of Gal-3, Cys-C and ET-3 in the hypertension group were higher than those in the control group: (132.28 ± 38.18) μg/L vs. (110.60 ± 17.51) μg/L, (1.19 ± 0.12) mg/L vs. (0.81 ± 0.06) mg/L, (21.28 ± 0.72) ng/L vs. (16.50 ± 0.68) ng/L, and the differences were statistically significant ( P<0.05). The levels of Gal-3, Cys-C and ET-1 in the LVH group were significantly higher than those in the non LVH group: (159.75 ± 36.37) μg/L vs. (113.96 ± 26.79) μg/L, (1.24 ±) mg/L vs.(1.16 ± 0.08) mg/L, (21.84 ± 0.71) ng/L vs.(20.90 ± 0.41) ng/L, and the differences were statistically significant ( P<0.05). Pearson correlation analysis showed that LVMI was positively correlated with Gal-3( r=0.650, P<0.01), Cys-C( r=0.659, P<0.01) and ET-1( r=0.858, P<0.01). The receiver operating characteristic(ROC) curve showed that the area under the curve (AUC) of Gal-3 in diagnosing hypertensive LVH was 0.821, 95% CI 0.743-0.899, the cutoff value was 148.55 μg/L, and the sensitivity and specificity were 59.10% and 89.40%. The AUC of Cys-C in diagnosing hypertensive LVH was 0.672, 95% CI 0.558-0.786, the cutoff value was 1.320 mg/L, and the sensitivity and specificity were 43.20% and 98.50%. The AUC of ET-1 in diagnosing hypertensive LVH was 0.876, 95% CI 0.811-0.941, the cutoff value was 22.275 ng/L, and the sensitivity and specificity were 65.90% and 95.50%. The AUC of combined detection was 0.948, 95% CI 0.855-0.976, and the sensitivity and specificity were 84.10%, 98.50%. Conclusions:The serum levels of Gal-3, Cys-C and ET-1 in elderly patients with hypertension are significantly increased, and there are a good correlation between them and LVH, and combined detection can be used as a sensitive index for the diagnosis of hypertensive LVH.

Objective:To summarize and analyze the clinical data of Chinese patients with colony-stimulating factor 1 receptor (CSF1R)-related leukoencephalopathy, and clarify the phenotypic and genetic characteristics of Chinese patients.Methods:Medical history of patients with CSF1R-related leukoencephalopathy diagnosed from April 1, 2018 to January 31, 2021 in the department of neurology of 22 hospitals in China was collected, and scores of Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment Scale (MoCA), magnetic resonance severity scale were evaluated. Group comparison was performed between male and female patients.Results:A total of 62 patients were included, and the male-female ratio was 1∶1.95. The age of onset was (40.35±8.42) years. Cognitive impairment (82.3%, 51/62) and motor symptoms (77.4%,48/62) were the most common symptoms. The MMSE and MoCA scores were 18.79±7.16 and 13.96±7.23, respectively, and the scores of two scales in male patients (22.06±5.31 and 18.08±5.60) were significantly higher than those in females (15.53±7.41 , t=2.954, P=0.006; 10.15±6.26, t=3.328 , P=0.003). The most common radiographic feature was bilateral asymmetric white matter changes (100.0%), and the magnetic resonance imaging severity scale score was 27.42±11.40, while the white matter lesion score of females (22.94±8.39) was significantly higher than that of males (17.62±8.74 , t=-2.221, P<0.05). A total of 36 CSF1R gene mutations were found in this study, among which c.2381T>C/p.I794T was the hotspot mutation that carried by 17.9% (10/56) of the probands. Conclusions:The core phenotypic characteristics of CSF1R-related leukoencephalopathy in China are progressive motor and cognitive impairment, with bilateral asymmetrical white matter changes. In addition, there exist gender differences clinically, with severer cognitive impairment and imaging changes in female patients. Thirty-six CSF1R gene mutations were found in this study, and c.2381T>C/p. I794T was the hotspot mutation.
10.Correlation analysis of early exercise capacity, mental state and quality of life in patients with acute myocardial infarction after percutaneous coronary intervention

Objective:To explore the correlation between early exercise capacity, mental state and quality of life in patients with acute myocardial infarction after percutaneous coronary intervention (PCI) .Methods:Using the convenient sampling method, a total of 120 patients with acute myocardial infarction treated by PCI who were admitted to the First Affiliated Hospital of Zhengzhou University from January 2020 to January 2021 as the research objects. Exercise cardiopulmonary test system, Positive and Negative Emotion scale and SF-36 were used to investigate.Results:In 120 patients with acute myocardial infarction, oxygen uptake at anaerobic threshold after PCI was (8.11±0.95) ml/ (kg·min) , peak metabolic capacity was (3.04±0.12) METs, peak oxygen uptake was (10.97±1.25) ml/ (kg·min) and the metabolic equivalent at the anaerobic threshold was (2.25±0.12) METs. The early exercise capacity of patients with acute myocardial infarction after PCI was positively correlated with positive emotions and quality of life ( P<0.01) , and negatively correlated with negative emotions ( P<0.01) . Conclusions:There is a positive correlation between early exercise ability and positive emotions and quality of life in patients with acute myocardial infarction after PCI.
